The dynamics of spiroveloergometric parameters and perfusion scintigraphy of the lungs with a postural test in 129 patients with congenital heart diseases were studied before and at different terms after surgical correction of the disease. It was established that main spiroergometric parameters were improved within the period from 3 to 6 months. However, normalization of physical working ability in the group of women took place about one year later than in men.

An examination of 33 patients has shown that complicated course of Echinococcosis of the lungs and their multiple lesions results in certain exhaustion of reserves of the external ventilation system. A conclusion is made on preference of performing organ-preserving and organ-sparing operations, on necessity of dispensary observations and physical rehabilitation of patients operated upon for complicated forms and multiple lesions of the lungs.

The method of spiroveloergometry was used in 97 patients after operation of mitral commissurotomy for studying reserves of pulmonary ventilation and its relation to the level of physical working capacity. The results obtained have shown that in patients of worse functional classes the ventilation potency of the lungs deteriorated which resulted in the restricted physical working capacity. It should be necessarily taken into consideration when making programs for the rehabilitation of cardiosurgical patients.

A functional classification of the physical condition of patients operated on for heart diseases is proposed on the basis of exercise testing results. It is based on the assessment of maximum individual capacity to increase oxygen consumption over the resting level. The identification of 4 functional classes is supported by hemodynamic, ventilation and pulmonary gas exchange data obtained at the peak of exercise. The proposed classification meets the requirements of an objective and quantitative assessment of the patients' functional capacity as may be needed in the assessment of the efficiency of cardiosurgical correction of the defect, the rationing of therapeutic and domestic activity, working capacity prognosis and occupational recommendations.

During rehabilitation of 40 patients after correction of mitral stenosis, a study was made of ventilation and gas exchange in the lungs under spirometric bicycle ergometry according to the "standard protocol" until the maximal oxygen consumption (MOC) was attained. The patients were examined 20-30 days, 3 and 6 months after operation. In accordance with the magnitude of the MOC expressed in metabolic equivalents (MET), all the patients were divided into 3 functional classes which corresponded roughly to the NYHA classification. It was revealed that with the same magnitude of the MOC the patients of the worst functional class had a lower production of CO2, a greater volume of the dead space and a higher level of minute ventilation. It is supposed that the described respiratory abnormalities are connected with enhanced ventilation of the poorly perfused alveoli, apparently because of venous congestion in the pulmonary capillaries. The characteristics of gas exchange and ventilation in the lungs investigated under the conditions of the "standard protocol" of spirometric bicycle ergometry objectively mark pump function of the heart and respiratory potentialities of the cardiorespiratory system, which might be the basis for the functional classification of the patients after cardiosurgical interventions.
