Resumen Introducción: La parálisis facial es una patología muy común. La escala "Sunnybrook Facial Grading System" (SFGS) se ha posicionado como una herramienta útil y confiable para su evaluación y evolución. Objetivo: Homologar lingüísticamente desde el idioma inglés al español la escala SFGS en una muestra de población chilena. Material y Método: Tres kinesiólogos chilenos con dominio comprobado del idioma inglés tradujeron la escala SFGS al español. Un comité creó una primera versión de la SFGS en español. Posteriormente, un profesional del Instituto Chileno-Británico tradujo la primera versión nuevamente al inglés (retrotraducción). El comité definió la segunda versión de la SFGS. Finalmente, los investigadores llevaron a cabo los pilotajes. Resultados: En dos pruebas piloto, veinte sujetos respondieron correctamente el total de las expresiones solicitadas. Conclusión: Esta versión de la escala SFGS homologada lingüísticamente al español puede ser aplicada a la población chilena.


Abstract Introduction: Facial paralysis is a very common pathology. The Sunnybrook Facial Grading System (SFGS) scale has positioned itself as a useful and reliable tool for its evaluation and follow up. Aim: To linguistically homologate the SFGS scale in a sample of the Chilean population from English to Spanish. Material and Method: Three Chilean kinesiologists with English proficiency translated the SFGS scale into Spanish. A committee developed a first version of the SFGS in Spanish. Subsequently, a professional from the Chilean-British Institute translated the first version back into English (back-translation). The committee defined the second version of the SFGS. Finally, the investigators carried out the pilots. Results: In two pilot tests, twenty subjects correctly answered the total of the expressions requested. Conclusion: This version of the SFGS scale linguistically homologated to Spanish can be applied to the Chilean population.

Resumen Históricamente, los estudios de campo observacionales han sido el punto de partida para el desarrollo de grandes avances en el entendimiento de las enfermedades autoinmunes. En el caso de la artritis reumatoide (AR), se han descrito varias asociaciones clínicas hasta la actualidad que tienen en común modelos inmunológicos transversales a la historia natural de la misma, lo que ha permitido avanzar en el desarrollo de nuevos objetivos terapéuticos. Se pretende hacer una breve descripción del compromiso nodular en AR, partiendo de las observaciones clínicas del Dr. Anthony Caplan en pacientes con riesgo de neumoconiosis, con el fin de reconocer el valor de este tipo de asociaciones en el ejercicio médico profesional.


Historically, observational field studies have been the starting point for the development of great advances in the understanding of autoimmune diseases. In the case of rheumatoid arthritis (RA), several clinical associations have been described to date which have in common immunological models transverse to its natural history, which has allowed progress in the development of new therapeutic objectives. Our aim is to make a brief description of nodular involvement in RA, based on the clinical observations of Dr. Anthony Caplan in patients at risk of pneumoconiosis, in order to recognize the value of this type of association in professional medical practice.

Uno de los mecanismos de transmisión de protozoarios intestinales, es el consumo de agua contaminada con quistes y ooquistes, cuya eliminación por cloración o filtración no resulta eficaz. En una comunidad de Caracas, se evaluó la posible contaminación del agua de consumo, con Blastocystis spp, Giardia spp y Cryptosporidium spp. El sedimento obtenido mediante filtración y separación inmunomagnética de 15 muestras de agua, se examinó microscópicamente al fresco, con coloraciones, inmunofluorescencia y cultivo en medio de Boeck- Drbohlav modificado (BDM). Se recopiló información sobre las condiciones de suministro, almacenamiento y consumo del agua, además del procedimiento utilizado para el lavado de frutas y vegetales. El único parásito observado fue Blastocystis spp (60%), mediante examen directo/cultivo (33%). Se observó un mayor consumo de agua filtrada que hervida (p= 0,001). Predominó el uso del agua de chorro para el lavado de vegetales y frutas, más que con agua y vinagre (p= 0,011). Se observó una mayor proporción de averías en los sistemas de recolección de aguas servidas (78,6%), más que en los sistemas de aguas blancas (28,6%, p= 0,011). El hallazgo de Blastocystis spp en el agua, se correlaciona con la prevalencia del parásito en habitantes de este sector. Destaca el papel del agua en la transmisión de Blastocystis spp, por lo cual se recomienda filtrarla y hervirla para prevenir la infección con este parásito.


Many intestinal protozoa are transmitted by contaminated water with cysts and oocysts and methods for their elimination as filtration or chlorination are not completely effective. We evaluated a possible consumption water contamination with Blastocystis spp, Giardia spp and Cryptosporidium spp in a community located in Caracas, Venezuela. The pellet obtained by immunomagnetic separation filtration of 15 water samples were examined by microscopic observation (direct examination), stain techniques, immunofluorescence and culture in Drbohlav Boeck-modified medium (BDM). We also collected information about consuming habits, water supply, storage and washing procedures of vegetables and fruits at assessed homes. The only parasite detected was Blastocystis spp (60%), by direct examination/culture (33%). A higher consumption of boiled filtered water (p = 0.001) was observed. The use of tub water for washing vegetables and fruits was predominant, instead of using water and vinegar (p = 0.011). We observed a higher proportion of nonfunctioning sewage collection (78.6%), rather than white water systems (28.6%, p = 0.011). Finding Blastocystis spp in water samples correlates with prevalence of this parasite in residents of this sector. The role of water in Blastocystis spp transmission is significant, so we recommend filtering and boiling it to prevent infection with this parasite.

El presente estudio se realizó con el objetivo de evaluar el efecto de la Albahaca (Ocimum basilicum) sobre dolor pélvico en dismenorrea primaria (menstruación dolorosa) en mujeres en edad fértil, durante los meses de julio y agosto del 2010. Para tal fin esta investigación fue aleatoria estratificada y doble ciego controlada con placebo (cápsulas color blanco con azul). Se seleccionaron 44 pacientes en el rango de edad fértil, con ciclos regulares y que presentaban dismenorrea primaria, las cuales fueron distribuidas en dos grupos de tratamiento: Grupo 1: cápsulas de Albahaca, y Grupo 2: cápsulas de placebo, las cuales estuvieron sometidas a dos meses de tratamiento con dosis específica; a las pacientes inicialmente se les fue socializado el consentimiento informado. Los síntomas fueron registrados por medio de encuestas que tenían en cuenta 3 variables principales las cuales fueron: “escala análoga del dolor” (escala numérica: 0 - ausencia del dolor; 10 - dolor insoportable), “intensidad máxima del dolor” y “total del dolor” (tiempo en minutos). Adicional a esto se tuvieron en cuenta comportamientos de “disminución”, “constante” y “aumento” en cuanto al dolor y al aumento o disminución del consumo de medicamentos de síntesis química Los resultados obtenidos en la investigación y los revisados con los estadísticos revelan que la Albahaca presentó un efecto en cuanto a disminución del dolor en las pacientes y, al compararse con el tratamiento con cápsula de placebo, se evidenciaron diferencias significativas en las variables tomadas en cuenta.

Las amibas de vida libre (AVL) del género Acanthamoeba son parásitos facultativos ampliamente distribuidos en el ambiente, por lo cual el contacto del humano con ellos es frecuente, sin embargo, los reportes de enfermedades producidas por AVL son escasos. La gravedad de éstas justifica su búsqueda, sobre todo cuando la infección es del sistema nervioso central (SNC) y cuando hay compromiso in munológico. En este trabajo se reporta el hallazgo de Acanthamoeba spp en una biopsia cerebral, tomada a un paciente de 44 años de edad con adenocarcinoma. Se exa minó la muestra bajo el protocolo del Laboratorio de Amibiasis, de la Cátedra de Parasitología, Escuela de Bioanálisis, Facultad de Medicina, Universidad Central de Venezuela, para la investigación de AVL y Entamoeba histolytica. Se realizó examen directo y coloraciones especiales, inducción de la flagelación y cultivos a diferentes temperaturas. Se observó crecimiento de Acanthamoeba spp en el medio de Page modificado por Chinchilla y col. (1979), incubado a 37 °C a las 96 horas. La descripción morfológica coincide con la morfología del grupo 3 de Acanthamoeba, según Visvesvara (1991). Se destaca la importancia de la realización del cultivo para el crecimiento e identificación de estos organismos en aquellos laboratorios donde los métodos basados en biología molecular aun no se hayan implementado como parte del diagnóstico y así aplicar el tratamiento oportuno y específico al paciente.


Free-living amoebas (FLA) of genus Acanthamoeba are parasitic facultative organisms whose are widely distributed in the environment, thus the human contact with them is frequent; nevertheless, there are few reports of diseases produced by FLA. The severity of these diseases justifies its re search, mainly when the infection is at central nervous system (CNS) and when there is an immunocompromised host. This work reports the finding of Acanthamoeba spp in a cerebral biopsy from a 44 year-old patient with adenocarcinoma. We followed the protocol established by the Amibiasis Laboratory, Parasitology Chair, School of Bioanalisis, Medicine Faculty, Central University of Venezuela, for the investigation of FLA and Entamoeba histolytica. It wasmade by direct examination of the sample and special stains, flagellation induction and cultures incubated at different temperatures. We observed Acanthamoeba spp growth in biphasic Page media modified by Chinchilla and col. (1979), incubated for 96 hours at 37 °C. The morphologic des cription agrees with the morphology of group 3 of Acanthamoeba, according to Visvesvara (1991). We emphasize the importance of using growth culture for isolation and identification of these organisms in those laboratories where molecular methods has not been implemented yet as part of the diagnosis scheme and thus for applying the opportune and specific treatment to the patient.

Cyclospora cayetanensis, es un protozoario intestinal humano, productor de diarrea en individuos inmunocompetentes e inmunocomprometidos. La transmisión ocurre por ingestión de ooquistes que han esporulado en el ambiente. La transmisión del parásito ocurre a través de un vehículo ambiental. En los países en vías de desarrollo, la ciclosporiosis se ha relacionado con el agua y los alimentos contaminados, contacto con la tierra o animales, y variables asociadas con bajas condiciones socioeconómicas. En junio de 2004 se realizó un estudio parasitológico en un centro pe nitenciario en Venezuela, donde se presentaron varios casos con diarrea. A 7 personas del sexo masculino, con edades entre 19-39 años y con diarrea de 2 a 10 días de du ración, se les realizó examen de heces con S.S. 0,85%, lugol, Quesel, Sudán III, ziehl Neelsen, Micrometría y Esporulación en K2Cr2O4 2,5%. Se encontró ooquistes de C. cayetanensis en 5 (71, 42%) y huevos de Ancylos tomídeos en 1 (14,3%). El número de casos, en los cuales se observó C. cayetanensis, en individuos con diarrea, en este centro penitenciario en Venezuela, resulta muy llamativo en virtud de ser este un microorganismo de baja prevalencia a escala mundial y nacional. No obstante, las condiciones de hacinamiento y probablemente medidas higiénicas inadecuadas, tanto en las áreas físicas como en la conservación y manipulación del agua y alimentos, favorecen la transmisión de parásitos que ocasionan diarrea. Lo cual demuestra, la necesidad de establecer medidas de control y educación sanitaria a los manipuladores de alimentos, para evitar la transmisión y aparición de diarrea por diversos agentes infecciosos entre estas poblaciones.


Cyclospora cayetanensis is a human intestinal protozoan, causing diarrhea in immunocompetent and immunocompromised individuals. Transmission occurs by ingestion of oocysts that have sporulated in the environment. In developing countries, ciclosporosis has been associated with contaminated food and water, contact with soil or animals and low socioeconomic conditions. In June 2004, a parasitological survey in a venezuelan prison in, where several diarrhea cases appeared. All seven (7) people were male, age ranging 19-39 year-old with diarrhea during between 2 and 10 days; they were evaluated by stool examination with ss 0,85%, iodine, quensel, sudan III, ziehl Neelsen stain/mi - crometry and sporulation in K2Cr2O4 2,5%. C. cayetanensis oocysts were found in 5 (71,42%) and hookworm eggs in one (14,3%). The number of diarrhea cases at this prison in Venezuela in which C. cayetanensis was observed, is very striking, knowing the premise for this microorganism to be global and regional low prevalent protozoan. However, overcrowding and poor hygienic measures, probably both, in the physical areas and in the storage and food/water handling, contribute to diarrhea-causing parasites transmission. This shows the need to implement control measures and health education for food handlers to prevent transmission and diarrhea outbreaks caused by infectious agents among this population.

This is an observational retrospective study. Our goal is to describe the local dermonecrotic reaction occurring after a spider bite in eleven pediatric patients. In seven (63.7%), the spider was identified as Loxosceles reclusa, and in four, bites were presumptive. The main symptoms and signs were pain, erythema, swelling, blisters, and vasculitis in five patients. There was a significant relationship between the time of onset before the treatment and the severity of the lesions (63.4 hours in the severe cases vs 14.4 hours in the mild cases, p < 0.05), as well the time spent in the emergency room (50 hours vs 10 hours respectively, p < 0.05). Treatment with dapsone, 1 mg/kg/24 h, and/or paracetamol and dicloxacillin was successful; one case required surgical treatment. No systemic loxoscelism occurred in this patient.

STUDY OBJECTIVE: To evaluate the efficacy of N-acetylcysteine (N-AC) alone or combined with multiple-dose activated charcoal (AC) in the treatment of acetaminophen (ACT) overdose. DESIGN: Prospective observational case series of 14 consecutive pediatric patients. Group A (n = 7) were treated only with N-AC and group B (n = 7) with N-AC combined with AC. Plasma ACT concentrations were measured at 0.0, 24 and 48 h. As a measure of ACT disappearance, half-life of elimination (t1/2 beta) and exogenous body clearance (ClB) were calculated. RESULTS: Group A, Initial and final mean ACT plasmatic levels were 27 micrograms/mL and 4 micrograms/mL; t1/2 beta of 17 h and ClB 0.640 mL.kg.min. Group B, 27 micrograms/mL and 0.66 microgram/mL; t1/2 beta of 10 h and ClB 1.092 mL.kg.min. For both t1/2 beta and ClB differences, p < 0.05 (SS). CONCLUSION: N-AC significantly decreased the plasma ACT levels in both treatments; however, there were several advantages with the combined therapy: AC enhanced the efficacy of N-AC according with the higher eliminatión of the overdosed drug (97.6% vs. 85.2%), the t1/2 beta decreased 42%, and the ClB increased 70% in relation to the group A. Data of this study suggested that N-AC plus AC is more effective than N-AC alone in enhancing ACT elimination in overdosed patients and that it provided additional hepatoprotective benefit.

RESUMO

A 14-year-old female attempted suicide by ingesting the organophosphate methyl parathion. A severe acute poisoning developed with the characteristic symptomatology: muscarinic, nicotinic and neurologic, as well serum cholinesterase activity decreased 88%. An extrapyramidal syndrome appeared suddenly nine days after the onset with ocular and buccal crisis, neck and trunk dystonic movements, and hypertonia and tremors. The patient improved with the administration of i.v. diphenhydramine. Other causes of toxic extrapyramidalism and organophosphate intermediate syndrome were discarded. Although an absolute causal relationship of the transient extrapyramidal symptomatology to the organophosphate exposure cannot be clearly established in this case, we speculate a possible delayed inhibition of the dopaminergic receptors in the substantia nigra and the basal ganglia.

Yucuyahui (zoapatle-Montanoa tomentosa), is a wild herb used as a folk oxytocic-remedy. Eight pregnant women drank infusions of the herb during the labor and their newborns showed cardiorespiratory depression that needs basic or advanced life support to recover. They improved during the first minutes with an average Apgar score at one minute of 4.5, and 7.4 at ten minutes (p < 0.05). A negative correlation was found between the number of ingested infusions and the Apgar score at the one minute (r = -0.65). Simultaneous administration of alcoholic beverages in three of the pregnant woman did not contribute to deep depression in their neonates. The mechanism of M Tomentosa action is still unknown, but it is possible that it is similar to other oxytocic-drugs, as oxytocin or ergot alkaloids, with which Montanoa shares uterine and systemic effects.

The aim of the study was to evaluate tympanic thermometry when compared with conventional glass-mercury thermometry, in 186 consecutive pediatric patients. In patients of less than 6 years of age (n = 120), there were no differences between tympanic and rectal measurements, in febrile and afebrile ranges, and significantly different when compared with axillary range. Similar data were reported in children of more than than 6 years of age (n = 65), in whom oral temperatures replace the rectal one. Using the tympanic thermometer as the standard measurement device, the accuracy of rectal, oral and axillary thermometers in determining a febrile state was examined. When tympanic temperature was 38 degrees C, a febrile state was considered, in this condition sensitivity for rectal measurement was of 73%, for oral 64%, and 23 and 29% for the axillary, according to the age group: specificity was of 100% in all the them. According to our data, tympanic measurement was consistent with glass-mercurial, rectal and oral, temperature in a pediatric population. Advantages of tympanic thermometry are its good correlation with central temperature, substantial time reduction of measurement (1 second), easy and non-invasive procedure, improved patient comfort, and lack of mercurial thermometry disadvantages. The conclusion is that tympanic thermometry becomes an acceptable option for pediatric temperature measurement.

We present a case of acute lethal poisoning by oil of "epazote" (oil of chenopodium), in a 2 y 9 m female. The volatile oil was administered according to the advice of a "curandera" (female healer), in a total quantity of 40 ml. Clinical features of the poisoning were: vomiting, deep coma, seizures, mydriasis, apnea, metabolic acidosis, neurogenic shock and death. The EEG suggested a diffuse encephalopathy, the CT scan with an image of severe brain edema and ventricular collapse. Relevant postmortem findings were brain edema and neuronal necrosis, pneumonia, enteritis, pericholangitis, mild pancreatitis and tubular necrosis. The phytochemical analysis of volatile oil identified ascaridol, the main active compound of the chenopodium herbs, in a quantity of 39 mg/ml (1,560 mg in the dose administered), and Chenopodium graveolens as the plant employed to prepare it. According to the age of the patient, 60 mg of ascaridol would be the recommended dose formerly used in the treatment of parasitic disease. Thus 1,560 mg was 26 times higher than the recommended dose, and exceeded by 56% the dose of 1,000 mg reported as lethal in humans.

Twelve pediatric patients with acute poisonings caused by carbamazepine, digoxin and acetylsalicylic acid were treated with multiple doses of activated charcoal combined with a saline cathartic (adsorption surface of activated charcoal nearly 950 m2/g). This procedure was effective to shorten the plasmatic levels of the drugs, besides the clinical improvement of the poisoned patients. The average initial and final levels of the drugs were: carbamazepine 21.64 and 0.9 micrograms/ml (lowering 95.81%, p < 0.05), digoxin 5.14 and 1.1 ng/ml (lowering 78.6%, P < 0.05) and acetylsalicylic acid 418.5 and 57.5 micrograms/ml, respectively, (lowering 86.3%, p < 0.05). These results suggest the usefulness of activated charcoal in the clearance of the four overdosed drugs.

RESUMO

The authors studied for two years the role of the chicks of aquatic birds in the arboviral cycles in coastal lagoons in central Panama in order to determine the relation between Culex (Melanoconion) ocossa and Mansonia (Mansonia) dyari mosquitoes in the transmission and dissemination of the viruses of Saint Louis Encephalitis (SLE) and Venezuelan Equine Encephalitis (VEE). Mosquitoes were captured every fifteen days on two consecutive nights to isolate the virus, using light traps (CDC) and baited traps. The attempts to isolate the virus were made using Vero cell cultures and the determination of antibodies was performed. The results of the serologic tests seem to indicate that four bird species: the ex (?) heron (Bubulcus ibis), the American heron (Casmerodius albus), the spoon-billed duck (Cochlearius cochlearius) and the needle crow (Anhinga anhinga) could function as intermediate hosts in the transmission cycle of SLE. Two species, the ibis (Endocimus albus) and the spoon-billed duck (Cochlearius cochlearius) could also be intermediate hosts of VEE in the coastal lagoons of Panama. The presence of antibodies in chicks could indicate an infection acquired recently, after their birth, in this area. The VEE virus was recovered from blood filled mosquitoes which had fed on a spoon-billed duck probably infected and exposed in a Trinidad #10 trap. No SLE virus was isolated. Other unknown viruses were isolated from mosquitoes selected for these studies, such as C. ocossa and M. dyari. The results obtained with these studies indicate the need for more studies utilizing new field techniques in order to establish a link between SLE and VEE, the vector mosquitoes and the aquatic birds in the coastal lagoons of the area under investigation.

RESUMO

We report the case of a preschool boy who, without knowledge of his relatives, ingested thallium sulfate in a dose calculated in 30 mg/kg. He presented a systemic lupus erythematosus-like syndrome and only further alopecia oriented the diagnosis of thallium toxicosis; thallium blood levels were; 37.2 micrograms/dl and in urine: 2330 micrograms/L. Treatment with the chelating agent D. penicillamine was effective, the clinical picture disappeared and the decrease of the thallium levels was observed. Thallium intoxication should be considered in the differential diagnosis of connective tissue disease as the above mentioned.
