RESUMO

In 80 patients with complicated clinical course of carcinoma of the left half of the colon the primary restoration of the intestinal continuity was fulfilled (in 47 patients left-side hemicolectomy, in 15 patients subtotal colectomy, in 15 patients-Hartman operations). Postoperative complications were observed in 18 patients (22.5%), 7 patients died (8.8%). Functional long-term results were favorable in most patients. The clinical experience obtained suggests that primary restoration of the intestinal continuity deserves special attention in a number of cases.

RESUMO

Treatment of patients with a severe form of nonspecific ulcer colitis with subtotal alteration of the colon is described. The disease was complicated by intestinal bleeding which could not be arrested by conservative therapy, so the Hartman operation was performed. Not traditional methods of treatment of ulcer colitis were used such as drainage of the thoracic lymph duct, lymphocytapheresis, lymphosorption, plasmapheresis which allowed to stabilize the patient's state and to perform radical proctectomy, subtotal colectomy with blind gut descended to the anal canal.

RESUMO

The article presents an analysis of results of treatment of 41 patients with different forms of nonspecific ulcer colitis, in 18 of them drainage of thoracic lymphatic duct was used with the following lymphocytapheresis with the help of a home blood fractionator RK-0.5, SKN activated charcoal sorbents and carbohydrate fiber absorbent, lymphosorption and lymph reinfusion. The method of treatment is highly effective for critical and mild forms of ulcer colitis in patients with total and subtotal injury of the large intestine. The results were followed up during 12-30 months and demonstrated stable remission of the disease.

RESUMO

A comparative assessment of different methods of angiography for diagnostics of echinococcosis of the liver is given. Conditions of prolonged contrast of vessels and parenchyma of the organ were created by splenoportography and arteriography of the liver with maiodyl which allows to determine the presence and degree of morphofunctional changes and spread of the pathological process in echinococcosis of the liver.

RESUMO

The intraabdominal anastomoses are thought to be expedient in performing restorative operations when the excluded rectum is longer than 10 cm. When the rectum stump is shorter than 10 cm the extra-abdominal colorectal and coloanal anastomoses are expedient with the descending of the colon on the lateral pelvic wall without a mobilization of the rectum stump. The reconstructive-restorative operations should be performed at proctological and surgical departments.

RESUMO

An experience with using the apparatus plasmacytapheresis in 16 patients with nonspecific ulcer colitis has revealed its high effectiveness mainly in patients with mild and middle-severity forms of the disease. Convincing data were obtained on the improvement of clinical, biochemical and immunological processes. The method can be used in ambulatory conditions for supporting therapy of the patients.

RESUMO

Patients with mild forms of inflammatory complications of carcinoma of the colon should be operated upon after completing the complex conservative treatment with the application of antibiotics and in subsided acute inflammatory process. Radical operations must be performed at the first stage of the surgical treatment of the patients. Such complications as perifocal abscesses, perforation of the tumor, diastatic perforations are considered to be contraindications to a one-step reestablishment of the colonic continuity after left-side resections. Lethality among radically operated patients with carcinoma of the colon complicated by an inflammation or perforation made up 25%.

During histological, quantitative histoenzymological studies of the preparations of the liver, lungs, spleen and gallbladder carried out in experiments on cats after maiodil arteriography of the liver no significant morphofunctional changes in these organs were revealed.

RESUMO

The microbiological investigation was performed in 53 patients with carcinoma of the colon, 32 of them having inflammatory complications of the tumor. High incidence of the infected state of the tumor surface, infected regional lymph nodes and regional venous blood was noted especially by enterobacteria. The bacteria isolated were found to be highly resistant to most widely used antibiotics. For the antibiotics to have a bacteriostatic or bactericidal effect, they must be used in great doses which must be often higher than therapeutically admitted ones.

RESUMO

A method for the descending of the mobilized colon along the lateral wall of the pelvis into the anal sphincter is proposed as the most optimal one. The left lateral length is 7,5 cm shorter than that behind the rectum. The restorative operations for the descending of the colon along the left lateral wall were fulfilled in 31 patients. No complications resulting from the technique of the operation were noted.

RESUMO

Results of the treatment of 152 patients with cardiospasm are described, 46 of them were operated upon, including 12 patients operated by the method proposed by the authors. Good results were obtained in 11 out of 12 patients.

RESUMO

Clinical observations in 25 patients with cancer of the gallbladder are described. Tumor of this localization, as a rule, is characterized by a latent course with symptoms of cholelithiasis, chronic cholecystitis, manifested by jaundice and signs of neoplasia only ar later stages. Special attention is given to a frequent association of gallbladder cancer with an acute inflammatory process. The prophylaxis against gallbladder cancer is early surgery for cholelithiasis.

RESUMO

25 cases were operated upon at the climax of a massive gastrointestinal bleeding from acute ulcers and erosions. Among them 20 patients underwent stomach resection. One patient died from pancreonecrosis within the early postoperative period. There were no cases of the recurrence of bleeding. Excellent late results were found in 12 of 17 cases, in other 5 cases the results were good.

RESUMO

In the paper the diagnostic and therapeutic policy employed in 11 patients with internal biliary fistulas, most frequently caused by cholelithiasis, is analysed. Some aspects of the preoperative diagnosis are eluciadated, and it is concluded that patients with internal biliary fistulas are to be treated surgically: cholecystectomy, suturing of defects in the gastric and intestinal wall, the drainage of bile tracts.

RESUMO

Based on the experimental data obtained, it was concluded that the danger of surgery at the height of the primary hemorrhage, although massive it may be, is somewhat exaggerated by surgical specialists. Recurrent in a short period of time hemorrhages are believed to be more inherent in danger than an operation in primary bleeding. The results of the experiments are supported by observations over 70 patients, operated upon for ulcerous bleeding, and in 11 patients who were subjected to surgery for profuse hemorrhage from cancerous gastric ulcers.
