RESUMO

PURPOSE: Peripherally inserted central venous catheters (PICC) in the onco-hematological patients may be associated with thrombosis or infections that may have short- to medium-term repercussions. MATERIAL AND METHODS: Single-centre retrospective analysis of a prospectively collected cohort. Primary objective was to establish the PICC-thrombosis and infections incidence. Secondary objectives were to analyze profile of patients suffering from these complications and variables associated with an increased likelihood of developing these events. RESULTS: 549 patients were recruited. 58.5% (n = 321) were oncology patients and 41.5% (n = 228) hematology patients. The incidence of PICC-associated thrombosis was 3.5% (n = 19). Thrombosis was associated with progression of the underlying malignant pathology in 10.6% (n = 2) of cases. No association was found between clinical variables analysed and development of thrombosis. Incidence of PICC-associated infections was 7.65% (n = 42). In the 30 days prior to PICC infection, 57.1% (n = 24) had a febrile syndrome of another focus, 73.8% (n = 11) had been hospitalized, 49.5% (n = 25) had a neutrophil count of 0-500 cells/mm3 and 47.6% (n = 20) had an episode of neutropenic fever. Variables significantly associated with the development of infection were hematological patients, high-flow PICC, 3-lm PICC or PICC insertion because of administration of vesicant therapy. CONCLUSIONS: Incidence of PICC-associated thrombosis is low and apparently less prognostically aggressive than other forms of thrombosis associated with cancer, without identify predictive factors. Infection was more prevalent and the identification of risk factors in our series could facilitate its prevention.

Seagrasses, which are marine flowering plants, provide numerous ecological services and goods. Zostera marina is the most widely distributed seagrass in temperate regions of the northern hemisphere, tolerant of a wide range of environmental conditions. This study aimed to (i) examine seasonal trends and correlations between key seagrass traits such as biomass production and biochemical composition, and (ii) compare seasonal adaptation of two ecotypes of Z. marina exposed to similar environmental conditions on the west coast of Ireland. During summer, plants accumulated higher levels of energetic compounds and levels of unsaturated fatty acids (FAs) decreased. Conversely, the opposite trend was observed during colder months. These findings indicate a positive seasonal correlation between the production of non-structural carbohydrates and saturated fatty acids (SFAs), suggesting that seagrasses accumulate and utilize both energetic compounds simultaneously during favorable and unfavorable environmental conditions. The two ecotypes displayed differential seasonal responses by adjusting plant morphology and production, the utilization of energetic reserves, and modulating unsaturation levels of fatty acids in seagrass leaves. These results underscore the correlated seasonal responses of key compounds, capturing ecotype-specific environmental adaptations and ecological strategies, emphasizing the robust utility of these traits as a valuable eco-physiological tool.

Silicon is a promising alternative to graphite as an anode material in lithium-ion batteries, thanks to its high theoretical lithium storage capacity. Despite these high expectations, silicon anodes still face significant challenges, such as premature battery failure caused by huge volume changes during charge-discharge processes. To solve this drawback, using amorphous silicon as a thin film offers several advantages: its amorphous nature allows for better stress mitigation and it can be directly grown on current collectors for material savings and improved Li-ion diffusion. Furthermore, its conductivity is easily increased through doping during its growth. In this work, we focused on a comprehensive study of the influence of both electrical and structural properties of intrinsic and doped hydrogenated amorphous silicon (aSi:H) thin-film anodes on the specific capacity and stability of lithium-ion batteries. This study allows us to establish that hydrogen distribution in the aSi:H material plays a pivotal role in enhancing battery capacity and longevity, possibly masking the significance of the conductivity in the case of doped electrodes. Our findings show that we were able to achieve high initial specific capacities (3070 mAhg-1 at the 10th cycle), which can be retained at values higher than those of graphite for a significant number of cycles (>120 cycles), depending on the structural properties of the aSi:H films. To our knowledge, this is the first comprehensive study of the influence of these properties of thin films with different doping levels and hydrogen distributions on their optimization and use as anodes in lithium-ion batteries.

Echinoderms with high levels of magnesium (Mg) in their skeletons may be especially sensitive to ocean acidification, as the solubility of calcite increases with its Mg content. However, other structural characteristics and environmental/biological factors may affect skeletal solubility. To better understand which factors can influence skeletal mineralogy, we analyzed the Mg content of Antarctic echinoderms from Deception Island, an active volcano with reduced pH and relatively warm water temperatures, and Livingston Island. We found significant interclass and inter- and intraspecific differences in the Mg content, with asteroids exhibiting the highest levels, followed by ophiuroids and echinoids. Specimens exposed to hydrothermal fluids showed lower Mg levels, which may indicate local environmental effects. These patterns suggest that environmental factors such as seawater Mg2+/Ca2+ ratio and temperature may influence the Mg content of some echinoderms and affect their susceptibility to future environmental changes.

Purpose: To evaluate the refractive and visual outcomes of eyes implanted with monofocal, optimized, aspheric, hydrophobic acrylic intraocular lenses (IOL) following cataract surgery. Methods: The study assessed 55 eyes implanted with CT LUCIA® 621P IOLs (Carl Zeiss Meditec AG, Jena, Germany) at 3-months postsurgery. The main outcome measures were refractive error, monocular corrected distance visual acuity (CDVA), monocular uncorrected distance visual acuity (UDVA), photopic and mesopic contrast sensitivity, and wavefront aberrations. Patients were asked to complete the Catquest-9SF questionnaire. Results: At 3 months, the cumulative Snellen visual acuity was 20/20 in 94.55% of eyes and 20/25 or better in 100%. The difference between the UDVA and CDVA was either the same (70.91%) or better (29.09%) in all eyes. The mean Snellen decimal UDVA and DCVA were 1.07 ± 0.15 and 1.13 ± 0.11, respectively. The safety and efficacy indexes were 1.48 and 1.40, respectively. 47.27% of eyes showed a spherical equivalent ±0.13 D, with 92.73% of the eyes were within ±0.50 D and all eyes were within ±1.00 D. The mean spherical equivalent was -0.03 ± 0.30 D and the mean refractive cylinder -0.36 ± 0.34 D. The log photopic and mesopic contrast sensitivity functions were good and similar for all spatial frequencies. All patients reported being either fairly satisfied (score 3) or very satisfied (score 4) with their vision (mean = 3.64 ± 0.49). No intra- and postoperative complications were reported during the 3 months of follow-up. Conclusion: The current study demonstrates that excellent visual and refractive outcomes, as well as patient satisfaction, can be achieved after cataract surgery with optimized, aspheric CT LUCIA 621P monofocal IOLs.

The anesthetic technique for calcaneal surgery has been reported to include peripheral nerve blocks, such as a sciatic block in the popliteal fossa, followed by intraoperative sedation. Sciatic nerve blocks are associated with limb weakness and fall risk. We present a case of a patient presenting for outpatient calcaneal surgery. The anesthetic plan consisted of a proximal, ultrasound-guided, single-injection selective posterior tibial nerve block followed by intraoperative sedation. The nerve block was performed, surgery concluded, and the patient received six hours of postoperative analgesia. Once the nerve block effects receded, the postoperative pain was managed with only over-the-counter analgesics while the patient was at home. We recommend an ultrasound-guided proximal posterior tibial nerve block for outpatient surgery involving the calcaneus to preserve lower extremity motor strength and provide postoperative analgesia.

The aims of our study were to analyse compliance with the 2014 GELTAMO SMZL Guidelines, in patients with splenic marginal zone lymphoma (SMZL), and to evaluate the outcome according to the HPLLs/ABC-adapted therapeutic strategy. Observational prospective multicenter study of 181 SMZL patients diagnosed between 2014 and 2020. Lymphoma-specific survival (LSS), composite event-free survival (CEFS) and response rates were assessed. 57% of the 168 patients included in the analysis followed the Guidelines. The overall response rate was higher in the rituximab chemotherapy and in the rituximab arms compared with the splenectomy arm (p < 0.001). The 5-year overall survival was 77% and the 5-year LSS of 93%. There were no differences in the 5-year LSS according to the treatment received (p = 0.68). The 5-year CEFS in the overall series was 45%, and there were significant differences between scores A and B (p = 0.036). There were no significant differences when comparing LSS and progression-free survival in patients treated with rituximab or rituximab chemotherapy at diagnosis or after observation. Our data support HPLLs/ABC score as a practical tool for the management of SMZL, observation as the best approach for patients in group A and rituximab as the best treatment for group B.

Electrocardiographic (ECG) artifacts may resemble ventricular tachycardia (VT), leading to inappropriate therapies. Despite extensive training, electrophysiologists have still been shown to misinterpret artifacts. The literature is scant regarding the intraoperative identification by anesthesia providers of ECG artifacts resembling VT. We present two cases of the intraoperative occurrence of ECG artifacts resembling VT. The first case involved a patient undergoing extremity surgery after receiving a peripheral nerve block. The patient was treated with a lipid emulsion for a presumptive local anesthetic systemic toxicity diagnosis. The second case was a patient with an implantable cardiac defibrillator (ICD) with suspended anti-tachycardia functionality due to the location of the surgery in the region of the ICD generator. The second case's ECG was identified as an artifact, and no treatment was initiated. Misinterpretation of intraoperative ECG artifacts continues to lead clinicians to institute unnecessary therapies. Our first case occurred in the context of a peripheral nerve block leading to the misdiagnosis of local anesthetic toxicity. The second case occurred during the physical manipulation of the patient during liposuction.

Spontaneous retroperitoneal hematomas are a rare yet potentially devastating occurrence associated with antiplatelet and anticoagulant therapies. We present a case of a spontaneous retroperitoneal hematoma post-operatively after a total hip arthroplasty surgery performed under a midline approach spinal anesthetic. A 79-year-old male with a BMI of 25.72 kg/m2 presented for anterior total hip arthroplasty. A midline approach with an uncomplicated spinal anesthetic was performed. On the night of postoperative day 0, the patient received a prophylactic dose of dalteparin. The patient reported back pain, contralateral leg numbness, and weakness that began overnight on postoperative day 0. A computed tomography (CT) scan confirmed a 10 cm, contralateral retroperitoneal hematoma. The patient underwent interventional radiology embolization followed by surgical evacuation and demonstrated improvement in the neurologic function of his affected leg. Despite the rarity of a spontaneous retroperitoneal hematoma formation in the perioperative period, it could be simultaneously evaluated when performing an MRI to rule out spinal hematoma if a patient suffers a post-op neurologic deficit after a neuraxial technique. Understanding the evaluation and timely treatment of patients at risk for a perioperative retroperitoneal hematoma could help clinicians prevent a permanent neurologic deficit.

The synthesis of a ditopic interlocked building-block and its self-assembly into a cyclic dimer is reported herein. Starting from a thread with two recognition sites, a three-component clipping reaction was carried out to construct a bistable [2]rotaxane. A subsequent Suzuki cross-coupling reaction allowed the connection of a second ring to that of the rotaxane, affording a self-complementary ditopic system. NMR studies were carried out to identify a cyclic hetero[4]pseudorotaxane as the main supramolecular structure in solution. Its assembly is the result of a positive cooperativity operating in the hydrogen-bonding-driven assembly of this mechanically interlocked supramolecule, as revealed by computational studies. The increase of the polarity of the solvent allows the disruption of the intercomponent interactions and the disassembly of the hetero[4]pseudorotaxane into the two interlocked units. The disassembly of the cyclic dimer was also achieved through a Diels-Alder reaction over the fumaramide binding site of the thread, triggering the translational motion of the entwined macrocycle to an adjacent glycylglycine-based station and precluding the supramolecular dimerization. The competitive molecular recognition of a guest molecule by one of the self-templating counterparts of the dimer also led to the controlled disassembly of the hetero[4]pseudorotaxane.

Tire pyrolysis oil (TPO) is one of the most interesting products derived from the pyrolysis of end-of-life tires. Among others, it contains valuable chemicals, such as benzene, toluene, ethylbenzene, and xylene (BTEX), as well as limonene. In order to recover these chemicals, a pilot-scale distillation plant has been designed, erected, and operated using TPO derived from an industrial-scale pyrolysis plant. The distillation facility consists of a packed column (20 kg/h) and is within the fifth technological readiness level. This work describes for the first time the fractioning of the TPO in a continuous operational mode under industrially relevant conditions. For this purpose, different reboiler temperatures (250-290 °C) and reflux ratios (up to 2.4) were preliminarily assessed on the yields and properties of the resulting products: light fraction (LF) and heavy fraction (HF). Thus, the distillation plant is capable of producing 27.0-36.7 and 63.3-73.0 wt % of LF and HF, respectively. The highest BTEX concentration in the LF (55.2 wt %) was found using a reboiler temperature of 250 °C and a reflux ratio of 2.4. Contrarily, the highest limonene concentration (4.9 wt %) in the LF was obtained at 290 °C in the reboiler without reflux. In this sense, the lower the reboiler temperature, the higher the BTEX, and the lower the limonene concentration in the LF. The main results herein obtained serve to gain key insights to operate packed distillation columns using complex and promising hydrocarbons as TPO in order to recover valuable products. In addition, this work provides significant information for optimizing the recovery efficiencies of both BTEX and limonene, as well as their potential applications including that for the resulting HF.

Physicochemical properties, kinetic pyrolysis and thermodynamic study of spent green tea, pure spent coffee grounds, spent coffee grounds blended with 50% torrefied barley and coffee husk were experimentally investigated using thermogravimetric analysis under an inert atmosphere to evaluate their thermochemical application. Five isoconversional methods were applied to determine effective activation energy (E a) of the pyrolysis processes. All methods showed good agreement by determining fluctuating E a values (150-500 kJ mol-1). Complex E a profiles with conversion were divided into four stages corresponding to thermal degradation of main biomass constituents (extractives, hemicellulose, cellulose and lignin), indicating that extractives decomposition was the least demanding reaction while lignin decomposition was the most demanding. The kinetic process was verified by reconstruction according to the Friedman parameters. The thermodynamic parameters were evaluated to determine the energy demand and efficiency throughout the process. The values obtained for physicochemical properties such as volatile matter (> 68%) and higher heating value (> 17 MJ kg-1), average E a (223-319 kJ mol-1) and significant energy efficiency implied that these types of biomass waste have significant reactivity and consequently the highest potential for the production of bioenergy and a range of high-value chemicals and materials. Nanocars are carbon-based single-molecules with a precise design that facilitates their atomic-scale control on a surface. The rational design of these molecules is important in atomic and molecular-scale manipulation to advance the development of molecular machines, as well as for a better understanding of self-assembly, diffusion and desorption processes. Here, we introduce the molecular design and construction of a collection of minimalistic nanocars. They feature an anthracene chassis and four benzene derivatives as wheels. After sublimation and adsorption on an Au(111) surface, we show controlled and fast manipulation of the nanocars along the surface using the tip of a scanning tunneling microscope (STM). The mechanism behind the successful displacement is the induced dipole created over the nanocar by the STM tip. We utilized carbon monoxide functionalized tips both to avoid decomposition and accidentally picking the nanocars up during the manipulation. This strategy allowed thousands of maneuvers to successfully win the Nanocar Race II championship.

Background and Objectives: To determine the most commonly used patient-reported outcome measures (PROMs) in exercise-induced leg pain (EILP) and to identify specific PROMs for EILP in order to evaluate their psychometric properties and methodological quality. Materials and Methods: A strategic search was performed in different databases to identify and extract the characteristics of studies based on the use of PROMs in patients with EILP. Specific PROMs were evaluated according to the Terwee et al. and COSMIN criteria. Results: Fifty-six studies were included in the review. The Medial Tibial Stress Syndrome Score (MTSSS), Lower Extremity Functional Scale (LEFS) and Exercise-Induced Leg Pain Questionnaire (EILP-Q) were identified as specific PROMs for EILP. The Visual Analog Scale (VAS) was the most widely used instrument in the assessment of EILP. The methodological quality assessment showed six positive values for the LEFS, four for the MTSSS and three for the EILP-Q for the eight psychometric properties analyzed according to the COSMIN criteria. The evaluation of the nine psychometric properties according to Terwee showed five positive values for the LEFS and MTSSS, and three for the EILP-Q. Conclusions: The overall methodological quality of the PROMs used was low. The VAS was the most widely used instrument in the assessment of EILP, and the LEFS was the highest quality PROM available for EILP, followed by the MTSSS and EILP-Q, respectively.

Pyrolysis is a chemical recycling process of interest as a means to achieve a sustainable circular economy for end-of-life tyres (ELTs). In the pyrolysis process, ELTs are converted into tyre pyrolysis gas (TPG), tyre pyrolysis oil (TPO) and raw recovered carbon black (RRCB). This work investigates for the first time the effect of different temperature profiles by using a single-auger pyrolysis reactor in an industrially relevant scale (TRL-5). Since the development of this process at this representative scale is quite limited and the temperature profile has not been previously studied, the results achieved in this work can provide a useful database for the development of this process at industrial scale. For this purpose, two different sources of ELTs, commercial truck tyres (CTTs) and passenger car tyres (PCTs), were used. Accordingly, the experimental campaign was conducted using two different incremental temperature profiles (425-550-775 °C and 600-700-800 °C) based on those that can be replicated in an industrial-scale auger pyrolysis plant. For the sake of comparison, two isothermal heating conditions (500-500-500 °C and 600-600-600 °C) were also tested. The results confirmed the remarkable influence of temperature profile on both the distribution and properties of products. The 425-550-775 °C temperature profile was found to enhance limonene production, which is associated with the minimisation of secondary reactions in the first heating zone of the reactor. Additionally, there were very low carbonaceous deposits found in the RRCB because of the high severity of devolatilisation conditions in the third heating zone of the reactor. On the other hand, when the temperature profile was raised, the production of single-ring aromatics, particularly benzene, toluene, ethylbenzene and xylenes (BTEX) significantly increased in the TPO at the expense of limonene. Thus, from this strategy, it is possible to tune the properties of the products depending on the requirements of the application in a single step, getting closer for circular economy in the ELT recycling domain.

OBJECTIVES: Invasive meningococcal disease (IMD) is an urgent notifiable disease and its early notification is essential to prevent cases. The objective of the study was to assess the sensitivity of two independent surveillance systems and to estimate the incidence of IMD. DESIGN: We used capture-recapture model based on two independent surveillance systems, the statutory disease reporting (SDR) system and the microbiological reporting system (MRS) of the Public Health Agency of Catalonia, between 2011 and 2015. The capture-recapture analysis and 95% CIs were calculated using the Chapman formula. Multivariate vector generalised linear model was performed for adjusted estimation. MEASURES: The variables collected were age, sex, year of report, size of municipality (<10 000 and ≥10 000), clinical form, death, serogroup, country of birth and type of reporting centre (private and public). RESULTS: The sensitivity of the two combined surveillance systems was 88.5% (85.0-92.0). SDR had greater sensitivity than the MRS (67.9%; 62.7-73.1 vs 64.7%; 59.4-70.0). In 2014-2015, the sensitivity of both systems was higher (80.6%; 73.2-87.9 vs 73.4%; 65.2-81.6) than in 2011-2013 (59.3%; 52.6-66.0 vs 58.3%; 51.6-65.1). In private centres, the sensitivity was higher for SDR than for MRS (100%; 100-100 vs 4.8%; -4.4-13.9). The adjusted estimate of IMD cases was lower than that obtained using the Chapman formula (279; 266-296 vs 313; 295-330). The estimated adjusted incidence of IMD was 0.7/100 000 persons-year. CONCLUSIONS: The sensitivity of enhanced surveillance through the combination of two complementary sources was higher than for the sources individually. Factors associated with under-reporting in different systems should be analysed to improve IMD surveillance.

BACKGROUND: Halophila stipulacea seagrass meadows are an ecologically important and threatened component of the ecosystem in the Gulf of Aqaba. Recent studies have demonstrated correlated geographic patterns for leaf epiphytic community composition and leaf morphology, also coinciding with different levels of water turbidity and nutrient concentrations. Based on these observations, workers have suggested an environmental microbial fingerprint, which may reflect various environmental stress factors seagrasses have experienced, and may add a holobiont level of plasticity to seagrasses, assisting their acclimation to changing environments and through range expansion. However, it is difficult to tease apart environmental effects from host-diversity dependent effects, which have covaried in field studies, although this is required in order to establish that differences in microbial community compositions among sites are driven by environmental conditions rather than by features governed by the host. RESULTS: In this study we carried out a mesocosm experiment, in which we studied the effects of warming and nutrient stress on the composition of epiphytic bacterial communities and on some phenological traits. We studied H. stipulacea collected from two different meadows in the Gulf of Aqaba, representing differences in the host and the environment alike. We found that the source site from which seagrasses were collected was the major factor governing seagrass phenology, although heat increased shoot mortality and nutrient loading delayed new shoot emergence. Bacterial diversity, however, mostly depended on the environmental conditions. The most prominent pattern was the increase in Rhodobacteraceae under nutrient stress without heat stress, along with an increase in Microtrichaceae. Together, the two taxa have the potential to maintain nitrate reduction followed by an anammox process, which can together buffer the increase in nutrient concentrations across the leaf surface. CONCLUSIONS: Our results thus corroborate the existence of environmental microbial fingerprints, which are independent from the host diversity, and support the notion of a holobiont level plasticity, both important to understand and monitor H. stipulacea ecology under the changing climate.

The Atrial Fibrillation Better Care (ABC) pathway was proposed for a more integrated atrial fibrillation (AF) care. We investigated if adherence to the ABC pathway was associated to the quality of anticoagulation control in a cohort of AF outpatients starting vitamin K antagonists (VKAs) between July 2016 and June 2018. Patients were considered adherent to the ABC pathway if they met all of its components. The time in therapeutic range (TTR) was estimated at one year. In total, 1045 patients (51.6% female; median age 77 years; 63% ABC pathway adherent) were included. At one year, 474 (51.6%) of 919 patients with international normalized ratio (INR) data for TTR estimation had a TTR < 65%. Among ABC pathway non-adherent patients, a greater proportion had TRT < 65% (56.4% vs. 43.6%, p = 0.025), and TTR < 70% (64.9% vs. 35.1%, p = 0.033), with lower mean TTR in non-adherent patients (59.4 ± 22.3% vs. 63.9 ± 21.1%; p = 0.004). Logistic regression models demonstrated that the ABC pathway adherence in its continuous (aOR: 0.75, 95% CI 0.59−0.96) and categorical (aOR: 0.75, 95% CI 0.57−0.98) forms was independently associated with TTR ≥ 65%. In this 'real-world' cohort of AF patients starting VKAs, the ABC pathway adherent patients had better TTR, and more ABC criteria fulfilled increased the probability of achieving good TTR.
