RESUMO

OBJECTIVE: To summarize and analyze the clinical characteristics of patients diagnosed with hepatic portal venous gas (HPVG). METHODS: This was a single center retrospective observational study. All of the patients were diagnosed with HPVG. The patients were admitted to Peking University Third Hospital from January 2017 to January 2021. Demographic characteristics, clinical manifestations, laboratory tests, abdominal imaging, treatment of the primary disease, and clinical outcomes of the patients were collected via electronic medical records. The study was approved by institutional review board and the information of all the patients was kept de-identified. RESULTS: A total of seven cases were included in the study. The median age of the patients was 67 (63, 81) years. Six of the patients were male. The seven patients all presented with sudden onset of severe abdominal pain, which was the most common symptom. Six patients developed septic shock after admission. The signs of HPVG were detected by CT scans in all the patients, showing gas embolization. It might also be found as unique "aquarium sign" in abdominal ultrosonography. Four cases were caused by intestinal lesions, including acute volvulus, intestinal obstruction, and rectal abscess. Two were caused by ischemic bowel disease and the other one was caused by severe acute pancreatitis. The gas accumulation could disappear after effective anti-shock therapy and surgery (Cases 1, 2, and 6). Two patients had good postoperative outcomes, and one patient was discharged after non-surgical treatment. However, the prognosis was poor in the patients with intestinal ischemia necrosis accompanied by shock and multiple organ dysfunction (Cases 3, 4, 5, and 7 all died). CONCLUSION: The HPVG patients generally have acute abdominal pain and show up at Emergency Department. The prognosis depends on the potential cause of HPVG. The mechanism and clinical management for the appearance of gas in the portal vein is not well understood. Patients complicated with shock, ascites, and peritonitis may have intestinal necrosis, which indicates surgical intervention and higher mortality. CT is the preferred diagnostic method in standard clinical practice. Physicians need to have a comprehensive understanding of the proactive diagnostic strategy, and active treatment for the primary disease.

RESUMO

A direct band gap of 1.6 eV has been identified in wurtzite CoO thin films, which matches the required value to achieve a theoretically high conversion efficiency solar cell. Its p-type conduction has been determined and an intense sub-gap absorption between 0.7 and 1.1 eV has been observed.

OBJECTIVE: We aimed to investigate the effect of Embosphere microsphere artery embolization on the serum level of vascular endothelial growth factor (VEGF) in patients with uterine fibroid. PATIENTS AND METHODS: From March 2014 to December 2015, 128 women in child-bearing age with uterine intramural fibroids were enrolled in the patient group. At the same time, 128 healthy cases in child-bearing age were randomly selected and enrolled in the control group. Enzyme-linked immunosorbent assay was used to measure the serum level of VEGF, and immunohistochemical staining method was used to study the expression of VEGF in the uterine fibroids. Embosphere microsphere artery embolization surgery was performed on cases in the patient group. RESULTS: The serum level of VEGF in the patient group was significantly higher than that of the control group. Immunohistochemical staining results showed that in the control group, VEGF expression level in uterine fibroid tissue was significantly higher. Compared with before the treatment, tumor diameter in the patient group reduced significantly 3 months after the treatment. Erythrocyte count, hemoglobin, and menstrual blood volume increased significantly 6 months after treatment. CONCLUSIONS: Serum VEGF level can be considered as a marker for uterine fibroid, and by using VEGF as a marker we can increase the probability of early diagnosis. We showed that, compared with hysterectomy, Embosphere microsphere embolization had an evident advantage and might be an excellent candidate to replace hysterectomy.

RESUMO

Therapeutic hypothermia is an important treatment for cerebral resuscitation in patients after cardiac arrest. But it is rarely used for comatose survivor post-cardiac arrest in China. The patient was the first case who was in coma post cardiac arrest caused by acute myocardial infarction and given hypothermia therapy in our hospital. After coronary reperfusion and therapeutic hypothermia, the patient's sneurologic function was recovered to normal. The paper discussed the indications, contraindications, cooling methods and complications of therapeutic hypothermia.

Two lattice traffic models are proposed by incorporating a cooperative driving system. The lattice versions of the hydrodynamic model of traffic flow are described by the differential-difference equation and difference-difference equation, respectively. The stability conditions for the two models are obtained using the linear stability theory. The results show that considering more than one site ahead in vehicle motion leads to the stabilization of the system. The modified Korteweg-de Vries equation (the mKdV equation, for short) near the critical point is derived by using the reductive perturbation method to show the traffic jam which is proved to be described by kink-anti-kink soliton solutions obtained from the mKdV equations.

RESUMO

An extended car following model is proposed by incorporating an intelligent transportation system in traffic. The stability condition of this model is obtained by using the linear stability theory. The results show that anticipating the behavior of more vehicles ahead leads to the stabilization of traffic systems. The modified Korteweg-de Vries equation (the mKdV equation, for short) near the critical point is derived by applying the reductive perturbation method. The traffic jam could be thus described by the kink-antikink soliton solution for the mKdV equation. From the simulation of space-time evolution of the vehicle headway, it is shown that the traffic jam is suppressed efficiently with taking into account the information about the motion of more vehicles in front, and the analytical result is consonant with the simulation one.
