RESUMO

The COVID-19 pandemic first affected Lebanon on February 21st, 2020, and one month later it reached Bcharri, a small remote town in northern Lebanon. When similar rural areas with under-equipped facilities and financial limitations are affected, outcomes could be catastrophic, raising the need for meticulous preparation and rapid response. In our study, we describe the different measures taken to prepare this town for the COVID-19 outbreak, as well as our rapid response after the first case was confirmed. We emphasize the distinctions and the needs of rural areas when facing such threats, and the importance of a proactive community and local initiatives. We also detail our contact tracing strategy and massive testing campaign, as well as our early management of patients infected with COVID-19. We hope that our experience can be reproducible in areas with similar rural settings, during the COVID-19 pandemic and future outbreaks.

The objective was to uncover demographical and clinical factors associated with migraine and irritable bowel syndrome (IBS) in a group of patients experiencing both painful disorders and to identify their triggering factors. A prospective study was performed between January 2016 and June 2017. Clinical characteristics and potential generating factors for both pathologies were then assessed using validated questionnaires. Our study showed that the percentage of patients diagnosed with migraine and having IBS was 34.3%. Compared with patients with migraine alone, patients having both diseases were identified with higher prevalence of concomitant chronic/psychiatric diseases, lower frequency of migraine attacks, and lower prevalence of throbbing/distressing/enervating pain and concentration difficulties than patients with migraine alone. Moreover, patients in this subgroup had moderate abdominal pain intensity, and constipation was the predominant stool type. Finally, some dietary and environmental factors seem to be significantly important triggering factors of migraine/IBS pain.

Gastrointestinal duplications are rare congenital malformations, which may occur anywhere in the gastrointestinal tract, more commonly in the ileum. Most cases appear during the first two years of life. Usually they are located at the paramesenteric side of the bowel and they share with the latter the same blood supply. Vague abdominal pain and distention are common presentations of duplications in adults, although many of them remain asymptomatic. We report a rare case of a jejunal duplication cyst in an elderly female patient, with atypical anatomic features.

PURPOSE OF REVIEW: The most common cause of esophageal injury is gastroesophageal reflux disease, however other less recognized causes may affect the esophagus. In this review we will focus on pill-induced esophageal injury, infections of the esophagus and eosinophilic esophagitis. RECENT FINDINGS: Esophageal infections are less frequent since HIV infection has become better controlled with antiviral therapies. The most emergent 'allergic' disease of the esophagus is eosinophilic esophagitis, which has become increasingly recognized in children and adults over the last decade. Eosinophilic esophagitis is a clinicopathologic disorder characterized by a dense esophageal eosinophilia generally occurring in association with upper gastrointestinal symptoms, primarily intermittent dysphagia, and refractory to proton pump inhibitor therapy. SUMMARY: Drug-induced esophageal injury remains underdiagnosed and should be better recognized.

BACKGROUND: We conducted a quality improvement project in a cervical cancer screening programme in Helsinki in order to see if detection of precancerous lesions could be influenced by external (participation rate) and internal (laboratory praxis) quality measures. METHODS: In order to increase the participation rate, a second personal invitation to Pap-test was mailed to nonparticipants of the first call. In order to improve the quality of screening, the cytotechnicians monitored their performance longitudinally by recording the number of slides reviewed per day, the pick-up rate of abnormal smears, the report of the consulting cytopathologist, and the number of histologically verified lesions detected from the cases that they had screened. Regular sessions were held to compare the histological findings with the cytological findings of all cases referred for colposcopy. No pressure was applied on the cytotechnicians to ensure that they felt comfortable with their daily workload. RESULTS: A total of 110 000 smears were screened for cervical cancer at the Helsinki City Hospital during 1996-99. Initially, the overall participation rate increased from 62% to 71%. The number of histologically confirmed precancerous lesions (CIN 1-3) more than doubled and their detection rate increased from 0.32% to 0.72%. CONCLUSIONS: Continuous education and feedback from daily work performance were important, yet rather inexpensive means in increasing laboratory performance. Additional measures are needed to further increase the participation rate. Impact of the quality measures on cancer incidence needs to be assessed later on.
