RESUMO

Although it is well known that human bone tissues have obvious orthotropic material properties, most works in the physical modeling field adopted oversimplified isotropic or approximated transversely isotropic elasticity due to the simplicity. This paper presents a convenient methodology based on harmonic fields, to construct volumetric finite element mesh integrated with complete orthotropic material. The basic idea is taking advantage of the fact that the longitudinal axis direction indicated by the shape configuration of most bone tissues is compatible with the trajectory of the maximum material stiffness. First, surface harmonic fields of the longitudinal axis direction for individual bone models were generated, whose scalar distribution pattern tends to conform very well to the object shape. The scalar iso-contours were extracted and sampled adaptively to construct volumetric meshes of high quality. Following, the surface harmonic fields were expanded over the whole volumetric domain to create longitudinal and radial volumetric harmonic fields, from which the gradient vector fields were calculated and employed as the orthotropic principal axes vector fields. Contrastive finite element analyses demonstrated that elastic orthotropy has significant effect on simulating stresses and strains, including the value as well as distribution pattern, which underlines the relevance of our orthotropic modeling scheme.

AIM: To find out whether a newly designed big cup nitinol stent is suitable for treatment of patients with gastric outlet obstruction resulting from gastric cancer. METHODS: The new stent is composed of a proximal big cup segment (20 mm in length and 48-55 mm in diameter), a middle part (60 mm in length and 20 mm in diameter) covered by a polyethylene membrane and a distal sphericity (20 mm in length and 28 mm in diameter). Half of the proximal big cup segment is also covered by a polyethylene membrane, which is adjacent to the middle part of the stent. The stent is preloaded in a 6.0-mm-diameter introducer system. Thirteen patients with gastric outlet obstruction resulting from gastric cancer received the new stents under endoscopic and fluoroscopic guidance. RESULTS: Technical success was achieved in 12 of 13 (92.3%) patients. Among the 12 patients in whom endoscopic stent was placed successfully, the clinical success rate was 91.7% during a follow-up of average 6.5 mo. During the first month follow-up, the migration rate was 0%, recurrent obstruction 0% and gastric bleeding 8.3%. During the follow-up between 2-12 mo, no migration, recurrent obstruction and gastric bleeding occurred. CONCLUSION: The proximal big cup segment seems to be effective and promising for technical efficacy, clinical outcome, and preventing migration and tumor ingrowth and increasing the emptying rate of sinus ventriculi.

RESUMO

PURPOSE: To understand the present condition of statistical methods used in the Journal of Prosthetic Dentistry. METHODS: The statistical methods used in 448 treatises (from 2001 to 2003) in the journals were reviewed, classified and analysed. RESULTS: The utilization rates of statistical methods were 28.06%, 48.34% and 51.02% from 2001 to 2003,respectively. The utilization rate of statistical methods was increasing significantly with year (chi2=29.453,P<0.001). The main statistical methods were analysis of variance (ANOVA), multiple comparison, nonparametric test, t-test, linear regression and correlation and chi2 test. CONCLUSION: The utilization rates of sophisticated statistical methods and the proportion of using more than two kinds of statistical tests were all higher than that in the medical journals published in China.
