RESUMO

OBJECTIVES: To investigate plasma and bone moxifloxacin concentrations following oral administration of a single or double dose of the drug, in order to consider its potential role in the treatment of osteomyelitis. PATIENTS AND METHODS: Thirty consecutive patients undergoing total knee arthroplasty were recruited. Three groups, of ten patients each, were formed: group A received moxifloxacin 400 mg orally 2 h (range 1.5-2.5) preoperatively, group B received moxifloxacin 400 mg orally 4 h (range 3.5-4.5) preoperatively and group C received moxifloxacin 400 mg orally 14 h preoperatively, followed by a second dose 2 h (range 1.5-2.5) preoperatively. During surgery, at the time of bone removal, a blood sample and aliquots of cortical and cancellous bone were collected and moxifloxacin concentrations were measured by HPLC. RESULTS: Mean plasma, cancellous bone and cortical bone concentrations were, respectively: 3.45, 1.89 and 1.43 mg/L for group A; 3.73, 1.81 and 1.56 mg/L for group B; and 6.26, 2.97 and 2.54 mg/L for group C. CONCLUSIONS: These data show a good penetration of moxifloxacin into both cancellous and cortical bone, with concentrations, after double dosing, exceeding the MIC90 for most pathogens involved in osteomyelitis and the clinic susceptibility breakpoint for Mycobacterium tuberculosis.

Variations of serum and nasal specific IgE to Dermatophagoides pteronyssinus (Der p) during alternate periods of antigen avoidance-exposure have been evaluated with an open design in a group of allergic children with asthma and rhinitis at the residential house Istituto Pio XII (Misurina, BL, Italy), at 1756 m, in the Italian Dolomites. A method based on direct incubation of allergen coupled substrate on the nasal mucosa has been employed to evaluate the levels of nasal IgE. Serum specific IgE decreased respectively from (median) 117-89.3 kU/l (P < 0.001) during an initial period of 3 months of allergen avoidance and from 88.2 to 78.4 kU/l (P < 0.0002) during a subsequent period of allergen avoidance. No significant increase in serum specific IgE was, in contrast, observed during two periods, 22 and 9 days, of antigen exposure, changing respectively from 89.3 to 88.2 and from 78.4 to 89.1 kU/l. In contrast, nasal IgE has been significantly influenced by the alternate periods of antigen exposure-avoidance, showing a decrease from 19.75 to 4.01 kU/l (P < 0.0001) after the initial period of avoidance, followed by an increase to 9.95 kU/l (P < 0.0001) after 22 days of exposure. A significant decrease to a value of 2.37 kU/l (P < 0.0001) was also observed during the subsequent period of avoidance, followed again by an increase to 7.87 kU/l (P < 0.002) after 9 days of exposure. The evaluation of the kinetics of changes in nasal specific IgE revealed a significant decrease (P < 0.01) as soon as antigen avoidance was implemented for 3 days.(ABSTRACT TRUNCATED AT 250 WORDS)

A modified procedure for hemoglobin A2 determination by microchromatography was developed simplifying an easily commercially available column kit method. Blood samples were analyzed simultaneously by means of DE-52 microchromatography described by Huisman et al. (reference 12) to check the reliability of the new modified method. In the Pediatric Clinic of Cagliari University where the original Huisman method was used the values of HbA2 obtained were as follows: Normal subjects (35 cases) 2.470 +/- 0.351, beta-Thalassemia heterozygous subjects (38 cases) 5.142 +/- 0.342. The modified method gave respectively the following results: Normal subjects 2.578 +/- 0.303, beta-Thalassemia heterozygous subjects 5.465 +/- 0.400. No overlap was found between normal and beta-thalassemia trait samples. It is suggested that the simplified method is very reliable and very simple to organize even in laboratories with few facilities and when needs for only few tests per week exist.

RESUMO

A patient is described who presented at an early age with failure to thrive and vomiting, and had a gross excretion of formimino glutamic acid. She had normal concentration of serum folate and vitamin B12, and no haematological abnormalities, and is not mentally retarded. The Michaelis constant for erythrocyte formimino glutamate transferase was in the normal range, but the enzyme behaves differently from that from reference subjects with respect to inhibitors and activators.

A high-performance liquid column chromatographic technique is reported for the analysis of some tryptophan and phenylalanine acid metabolites in the urine. An acidified and NaCl-saturated urine sample is loaded on to a C18-bonded silica microcolumn. After washing the microcolumn with clean and deionized water, the metabolites of interest are selectively extracted by successive elutions with organic solvents of variable polarity. Acids are eluted first and the neutral compounds with the next fraction. Basic compounds and other neutral substances of higher polarities were eliminated during the washing procedure. The chromatography was performed in the straight-phase isocratic elution mode utilizing 5-micrometers silica-gel columns loaded with a triethanolammonium perchlorate--perchloric acid aqueous solution. The separation achieved have permitted the application of the chromatographic technique to the analysis of urinary metabolites with acceptable accuracy.

A method is described for the utilization of pre-coated, non-impregnated silical gel thin layers in one- and two-dimensional separations of carbodhyrates and related compounds. Boric and phenylboronic acids were added to the organic elution systems in different concentrations and their interactions with the sugar molecules during the chromatographic process were studied. A comparison was made between solvent systems containing boric or phenylboronic acid and systems devoid of both acids as eluents. With boric acid-containing solvents the migration of some sugars was considerably inhibited, whereas phenylboronic acid produced an increase in the RF values of certain sugars. The combination of these two types of solvent in two dimensional development resulted in the clear separation of a group of mono- and disaccharides of bioclinical interest.

RESUMO

A simple method for the electrophoretic separation of glycosaminoglycans from untreated urine is presented. It is shown that this rapid electrophoresis on specially shaped acetate cellulose strips gives more information than other screening tests used in laboratory for discriminating between normal and pathological samples. This technique is useful particularly with low levels of glycosaminoglycans which may give false-negative responses.

RESUMO

The therapeutic effect of orally administered zinc in 3 patients affected with Acrodermatitis enteropathica is reported. Single daily doses of zinc resulted in rapid improvement of the general condition of these patients and in clinical remission within a week. Serum, urine and hair zinc levels as well as alkaline phosphatase, which were very low before treatment, returned to normal after therapy. This brief report confirms the efficacy of oral zinc in the treatment of Acrodermatitis enteropathica.
