RESUMO

AIM: Pre-eruptive intra-coronal resorption (PEIR) is a rare condition that can affect children's dentition. Showing the same aspect of dental caries, these lesions are diagnosed in non-erupted teeth. The aetiology is not yet defined and no consensus on their treatment is available. Thus, the aim of the present scoping review of the literature was to try to establish a protocol for treatment and management of PEIR defects. METHODS: The search was performed on Medline via PubMed, Science Direct and EBSCOhost' databases using the appropriate Medical Subject Headings (MESH) terms. Studies that described the PEIR were considered eligible and the data from the selected papers were extracted and analysed independently by two authors. RESULTS: Out of 172 articles identified in initial research, 15 articles were selected for reviewing. Interventions varied from preventive treatment to extraction, depending on the lesion severity and its proximity to the pulp. CONCLUSION: Non-operative procedures, conservative approaches and extractions were recommended for the management of PEIR, depending on the extent of the lesions. Overall, further researches should be conducted to explore the effectiveness of the approaches of PEIR management.

RESUMO

Mucinous cystadenomas are the most frequent cystic benign tumors of the pancreas. They are characterized by a malignant potential. Acute pancreatitis is their most common complication. This manuscript reports a unique complication of mucinous cystadenomas of the pancreas: retroperitoneal rupture associated to acute pancreatitis. After resuscitation, the patient underwent surgery. A cystic mass of the pancreas tail with retroperitoneal rupture was found. Splenopancreatectomy was performed. Outcomes were straightforward. After 3 years of follow-up, no signs of recurrence are highlighted.

RESUMO

Winslow's hiatus hernia is the rarest of the internal hernias. Its diagnosis is difficult and requires an urgent laparotomy to avoid necrosis of the incarcerated loop or even the death of the patient. We report the case of a patient operated urgently for acute intestinal obstruction caused by a strangled hernia through the Winslow's hiatus. A reduction by traction of the hernia was undertaken. Rapid management of this pathology is important using clinical and radiographical elements.

RESUMO

Trigger finger is a very common disease. The vast majority of trigger fingers are primary idiopathic trigger fingers. We report the case of a bilateral and symmetric triggering of the fifth finger secondary to an anatomical variation of the lumbrical muscle. This lumbrical muscle arose from the flexor digitorum superficialis instead of the flexor digitorum profundus as is usual. To our knowledge, this is the third published case of trigger finger caused by an anatomical abnormality of a lumbrical muscle.

RESUMO

The intraosseous capillary haemangioma is classified as a benign vascular bone tumor. It is extremely rare in the hand. We present the case of a carpometacarpal capillary haemangioma involving the capitate and the bases of third and fourth metacarpals. To our knowledge it is the first case reported at this site.

The median nerve entrapment after elbow-dislocation is rare. We report two cases of fracture dislocation elbow completed by entrapment of median nerve occurred after reduction in two children. For both cases, the diagnosis of entrapment was made some months later after injury. Median nerve graft was performed after nerve resection. The nerve recovery was satisfactory at the final follow-up. This nerve complication should be suspected systematically before and after reduction of all dislocation of the elbow in the children. The indication of nerve release or nerve graft depends on the entrapment and the delay of the palsy.

We aimed to evaluate the attitudes of teachers to tobacco smoking in Kalaa Kebira (a semi-urban region in the Tunisian Sahel). Data from 358 of 402 teachers surveyed were obtained using a self-completed questionnaire. The mean age of the teachers was 35.7 (SD 7.9) years. The prevalence of smoking was 29.3% (51.6% of men and 3.6% of women), and 79% of smokers were addicted to nicotine according to the Fagerström test. About 50% of the teachers were badly-informed about the dangers of smoking and 75.2% of smoking teachers did not refrain from smoking in front of their pupils. The lack of knowledge of teachers limits their role as a model and information source in the anti-tobacco struggle. It is essential to draw up an education programme for the teaching staff.

INTRODUCTION: The aim of this study was to evaluate the scale of tobacco use among teachers in the district of Kalaa Kebira (a semi-urban region in the Tunisian Sahel). METHODS: The study design was descriptive and cross sectional, employing a self administered questionnaire given to 402 teachers. RESULTS: The response rate was 89%. The average age of respondents was 35.7 7.9 years. The global prevalence of smoking among teachers was 29.3% (men: 51.6%; women: 3.6%). 79% of smokers were addicted to nicotine according to the Fagerstrôm test. Half of the teachers were poorly informed about the hazards of smoking and 77.9% of smoking teachers didn't refrain from smoking in front of their pupils. CONCLUSIONS: The teachers' lack of knowledge limits their effectiveness both as a role model and information relay in the war against tobacco.

OBJECTIVE: To assess the effectiveness of a patient education program on diabetes control. METHODS: A pre-test post-test pre experimental study was performed with 87 insulin treated diabetic patients followed in the chronic disease unit of Kelibia district (Nabeul, Tunisia). Subjects received education in 6 monthly sessions. Outcome measures included glycated haemoglobin (HbA1c), anxiety level according to the Hamilton scale and Body Mass Index. Changes were assessed after the 6 month program. RESULTS: HbA1c decreased from 8.8% +/- 1.23 to 7,6 +/- 1.43 (P<10-6) with an increase in the prevalence of satisfactory HbA1c (< or = 8%) from 33% to 61.2% (P<0,001). The prevalence of severe anxiety decreased by about a third (P < 0,001). CONCLUSION: Education led to an improvement in diabetes control in insulin treated diabetic patients. More attention should be paid to such strategies in general practice.

Adolescent health poses new kinds of problems which are difficult to handle and solve through traditional school health centres and structures. In order to identify the expectations of adolescents with respect to their health centres, an epidemiological survey was carried out by means of a self-administered questionnaire to a representative sample of 625 high school students in Sfax, Tunisia. The principle results demonstrated that 54% of those questioned were not satisfied with the available services offered by their school health centres. Their expectations concerned physician availability, the quality level of the exchange, in particular with regard to being listened to (85%), dialogue (82%), a clear understanding of the real problems and issues (82%), as well as information (51%), discussion and consultation in the event of health problems (61%) and the quality of bedside manner (50%). Such results indicate the need for an orientation of the work of school health care staff towards a psycho-social approach centred on the individual students and which dedicates more interest to their needs, perceptions and participation.

The therapeutic knowledge of physicians is the corner stone to the rational use of medicines; however information about medicines is generally obtained from the pharmaceutical industry via their sales representatives (reps). We aimed to identify general practitioners' (GPs) attitudes to pharmaceutical reps and the information they provide. We surveyed 140 GPs using a self-administered questionnaire. The response rate was 78% (72 GPs from the public sector and 68 from the private sector). About 10% of the GPs said they received daily visits from pharmaceutical reps; 84% of GPs considered them an efficient source of information and 31% said they might change their therapeutic prescribing following visits from these reps. Because of their positive perception of pharmaceutical reps, GPs are susceptible to the information they provide. Controlling the validity of the therapeutic information imparted by the pharmaceutical industry is thus a fundamental component of the programme for the rational use of medicines.
