RESUMO

Autoimmune tissues may contain ectopic germinal centers (EGCs). However, these structures have never been described in the liver of patients suffering from autoimmune hepatitis (AIH). We retrospectively reviewed histological features of 120 definite AIH cases, and found 10 cases harboring markers of EGCs. In these cases, CD21+ follicular dendritic cells were intermixed with CD3+ T and CD20+ B lymphocytes. The latter expressed the GC-specific marker bcl6, and some were proliferative as assessed by Ki67 expression. Antibody-secreting cells (ASCs) defined by expression of the mum-1 transcription factor and presence of cytoplasmic IgMs were usually present in the periphery of these structures, but some were also present within the EGCs. Notably, some ASCs were IgG-switched. Common treatment applied to AIH patients achieved biochemical normalization as efficiently as in patients without EGCs. In the present study, we provide the proof for the occurrence of functional EGCs enabling differentiation of B cells into ASCs and occurrence of immunoglobulin switch in AIH livers.

INTRODUCTION: Immune-checkpoint inhibitors have been approved for first and second line treatments of metastatic non-small cell lung cancer based on the results of several phase III trials. Patients with organ transplantation were excluded from these studies because checkpoint inhibitors could activate allo-reactive T cells leading to acute graft rejection. CASE REPORT: A 71-year-old Caucasian-male was diagnosed with stage IV pulmonary adenocarcinoma with multiple metastases, without molecular alteration and negative PD-L1 status. He had a left kidney transplant, and his immunosuppressive regimen consisted of sirolimus and mycophenolate mofetil. After failure of two therapeutic lines (carboplatin-paclitaxel and erlotinib) a multidisciplinary oncology meeting with the nephrologist started third line treatment with nivolumab 3mg/kg every 15 days, with no modification of the immunosuppressive treatment. The patient received a total of 14 injections of nivolumab with stable disease but treatment was discontinued due to acute rejection of the transplanted kidney 6 months later, without need for dialysis. The patient died of a chylothorax related to progression of the tumour 12 months after initiation of nivolumab. CONCLUSION: Immune checkpoint inhibitors are a potential treatment for solid organ transplant patients despite the risk of graft rejection.

Different studies described the antibacterial properties of Lavandula angustifolia (Mill.) essential oil and its anti-inflammatory effects. Besides, no data exist on its ability to activate human macrophages during the innate response against Staphylococcus aureus. The discovery of promising regulators of macrophage-mediated inflammatory response, without side effects, could be useful for the prevention of, or as therapeutic remedy for, various inflammation-mediated diseases. This study investigated, by transcriptional analysis, how a L. angustifolia essential oil treatment influences the macrophage response to Staphylococcus aureus infection. The results showed that the treatment increases the phagocytic rate and stimulates the containment of intracellular bacterial replication by macrophages. Our data showed that this stimulation is coupled with expression of genes involved in reactive oxygen species production (i.e., CYBB and NCF4). Moreover, the essential oil treatment balanced the inflammatory signaling induced by S. aureus by repressing the principal pro-inflammatory cytokines and their receptors and inducing the heme oxygenase-1 gene transcription. These data showed that the L. angustifolia essential oil can stimulate the human innate macrophage response to a bacterium which is responsible for one of the most important nosocomial infection and might suggest the potential development of this plant extract as an anti-inflammatory and immune regulatory coadjutant drug.

Mutually unbiased bases (MUBs) play a key role in many protocols in quantum science, such as quantum key distribution. However, defining MUBs for arbitrary high-dimensional systems is theoretically difficult, and measurements in such bases can be hard to implement. We show experimentally that efficient quantum state reconstruction of a high-dimensional multipartite quantum system can be performed by considering only the MUBs of the individual parts. The state spaces of the individual subsystems are always smaller than the state space of the composite system. Thus, the benefit of this method is that MUBs need to be defined for the small Hilbert spaces of the subsystems rather than for the large space of the overall system. This becomes especially relevant where the definition or measurement of MUBs for the overall system is challenging. We illustrate this approach by implementing measurements for a high-dimensional system consisting of two photons entangled in the orbital angular momentum degree of freedom, and we reconstruct the state of this system for dimensions of the individual photons from d = 2 to 5.

Optimal quantum cloning is the process of making one or more copies of an arbitrary unknown input quantum state with the highest possible fidelity. All reported demonstrations of quantum cloning have so far been limited to copying two-dimensional quantum states, or qubits. We report the experimental realization of the optimal quantum cloning of four-dimensional quantum states, or ququarts, encoded in the polarization and orbital angular momentum degrees of freedom of photons. Our procedure, based on the symmetrization method, is also shown to be generally applicable to quantum states of arbitrarily high dimension-or qudits-and to be scalable to an arbitrary number of copies, in all cases remaining optimal. Furthermore, we report the bosonic coalescence of two single-particle entangled states.

RESUMO

In four wards of a large specialized orthopedic hospital in Bologna a self-administered questionnaire has been distributed to 34 nurses and 70 patients to investigate on one side, the representation which nurses have of patients' needs and the difficulties met in their work, on the other side the representation which patients have of the care they receive and of the difficulties they have in their relationship with nurses. Nurses give principal importance to those activities which contribute to the development of a good relationship with the patients. There is however a clear gap between the degree of awareness of the principles and their implementation, possibly as a consequence of the work organization. Some model situations are examined and discussed (e.g. the time allowed for dialogue, which is confined to ten minutes).
