RESUMO

OBJECTIVE: Paratesticular tumors are very rare and mostly bening. Wa aport a new case of adenomatoid tumor of epididymis METHOD: Male of 40 years old with solid lesion in epidididymis tale treated with mass exéresis RESULTS: Adenoamotid tumor of the epididymis confirmed with hystopathologic technique CONCLUSION: Adenomatoid tumor of epididymis is the most frequent paratesticular tumors and when is suspected, conservative surgery must be performed.

Objetivo: Los tumores paratesticulares son poco habituales siendo en la mayoría de los casos benignos. Aportamos un nuevo caso de tumor adenomatoide de epidídimo. Método: Varón de 40 años con masa sólida en cola epidídimo tratada mediante exéresis de la misma Resultados: Tumor adenomatoide confirmado mediante inmunohistoquímica Conclusión: El tumor adenomatoide es el más frecuente de los tumores paratesticulares y su sospecha permite realizar cirugía conservadora

RESUMO

Presentation of results of treatment using Biofeedback in two conditions which quite frequently associate vesical instability and incontinence: 65 cases of enuresis and 39 of urinary stress incontinence in females. The method was effective and achieved disappearance of detrusor instability in 78.2% of enuretic cases and 58.9% of urinary stress female incontinence. Outcome for incontinence was less favourable since disappearance was only achieved in 70.5% of enuretics and 20.5% cases of female incontinence. A brief description of the method and instrumentation used is done, which can be easily conveyed to any electromiographic device with real time output. The paper substantiates the authors' views relative to the likely pathogeny of the instability in both processes and the mechanism of action of Biofeedback techniques.

In contemporary urology it is increasingly frequent the indication of intermittent catheterization. Patients undergoing this type of therapy use several non-standardized methods to make catheterization as clean as possible. Microwave sterilization has been successfully used for certain materials and biological residues. For this reason and because it is a rather inexpensive system, we tried to demonstrate that sterilization of catheters used by our patients for intermittent catheterization using domestic microwave ovens at home is a convenient, inexpensive and clean alternative that should be taken into account. The catheters used were those employed in our hospitalized patients for a minimum of 4 days, which involves a high percentage of contamination. In 15 cases, the catheter was treated in a 800 W domestic microwave oven after having washed it with water so as to eliminate macroscopic organic residues, and in another 15 cases they were thoroughly washed before microwave treatment. In either case, the treatment was done for over 6 minutes at maximum power with the precautions described in the text. In all instances, the catheter's sterility was confirmed except for one case where contamination occurred during post-treatment handling.

RESUMO

Sleep apnea syndrome is a clinical entity characterized by respiratory pauses during spontaneous sleep during which desaturation exists. A reduced group of patients suffering this pathology present chronic alveolar hypoventilation. The application of continuous positive pressure through the nose (CPAPn) is an effective therapeutic measure in the treatment of sleep apnea syndrome in its obstructive form. Its long term application normalizes, in some patients with chronic alveolar hypoventilation, daytime hypercapnia. We describe two patients presenting sleep apnea syndrome in whom the application of CPAPn reverted chronic hypercapnia after three and five months of treatment respectively. We agree with other authors that such normalization can be conditioned by a better performance of inspiratory musculature after the continuous application of CPAPn, which decreases the pressure which has to be generated by respiratory muscles in order to maintain inspiratory flux.

RESUMO

The modifications in the ventilation pattern when a continuous positive pressure is applied through the nose (CPAPn) in an acute form or by increasing pressures is evaluated in 13 normal subjects and 8 individuals presenting obstructive sleep apnea syndrome (OSAS). No significant modifications are observed in breathing frequency or breathing time when 5 and 10 cm of H2 are applied. It is concluded that this mechanism does not seem to be involved in the disappearance of hypercapnia which occurs in some patients with OSAS.
