RESUMO

Of 103 patients with acute coronary syndrome (mean age 51.6 +/- 0.9 years) 47 patients participated in 5 group psychotherapeutic sessions added to conversional rehabilitation program. Psychotherapy included progressive muscular relaxation, neurolinguistic programming, eriksonian hypnosis, therapeutic metaphora. Psychotherapy decreased the hear rate, number of ventricular extrasystoles, stimulated tonicity of the parasympathetic nervous system. Compared to the controls, the test patients developed higher exercise tolerance and lower reactivity of the central hemodynamics in all the exercise tests.

RESUMO

120 patients with mild and moderate arterial hypertension were treated outpatiently for 6 weeks with dilren (group 1), norvask (group 2), invoril (group 3) and caposide (group 4). Each group consisted of 30 patients. The drugs were given in doses: 300 mg/day, 5-10 mg/day, 10-20 mg/day and 1 tablet a day (50 mg capoten + 25 mg hydrochlortiaside), respectively. Arterial pressure was measured by patients in the morning and in the evening. A complete hypotensive response (AP < 140/90 mm Hg) to dilren was achieved in 25(83.3) patients, norvask in 22(73.3%), invoril in 18(60%), caposide in 13(43.3%) patients. The other 45(37.5%) patients responded partially. Side effects occurred in 31(25.8%) of 120 patients. In caposide, norvask, invoril and dilren treatment they were recorded in 9(30.3%), 8(26.7%), 8(26.7%) and 6(20%) patients, respectively. 11 patients withdrew because of side effects. Thus, dilren (300 mg/day), norvask (5-10 mg/day) and invoril (10-20 mg/day) are effective and safe in mild and moderate arterial hypertension. Caposide (1 tablet a day) failed to provide an adequate fall in arterial pressure throughout 24 hours.

RESUMO

AIM: To test electric stimulation of the skeletal muscles in rehabilitation of patients with complicated myocardial infarction. MATERIALS AND METHODS: 27 patients with macrofocal myocardial infarction (age 67 +/- 2.4 years) with persisting left ventricular insufficiency in the beginning of the subacute period. From day 3-4 all the patients were subjected to electrostimulation of large skeletal muscles (ESSM). The procedures were made daily for 2 hours using the Mioritm-040 unit. In the course of the first session the study was made of the changes in the intracardiac hemodynamics and doppler parameters of circulation in the popliteal artery. RESULTS: In the course of ESSM of 10-15 sessions none of the patients had anginal attacks, aggravation of dyspnea, arrhythmia episodes. After the course there was a marked increase in left ventricular ejection fraction and stroke volume. CONCLUSION: ESSM is safe in complicated myocardial infarction, prevent unwanted effects of long-term stay in bed, improves intracardiac hemodynamics.

AIM: To assess effectiveness of blood hypertension treatment with inhibitors of angiotensin converting enzyme and calcium channels blocker. MATERIALS AND METHODS: 83 outpatients with mild and moderate blood hypertension received monotherapy with prestarium, anap and lomir for 12 months. The trends in parameters of blood pressure, total peripheral vascular resistance, thickness of the interventricular septum and left ventricular posterior wall were estimated. RESULTS: Long-term effective monotherapy with prestarium, anap and lomir was possible in 47, 62 and 45% of the patients, respectively. Diastolic pressure lowered by 25, 24 and 25%, total peripheral vascular resistance reduced by 20, 19 and 22%, respectively. The three drugs produced similar positive effects on left ventricular thickness, had no negative effects on bioelectric activity of the heart, clinical and biochemical blood counts. CONCLUSION: Prestarium, anap and lomir are effective against hypertension and diminish left ventricular hypertrophy in hypertensive patients.

RESUMO

An antihypertensive action and tolerance of prestarium and lomir were studied in 31 and 33 hypertensive patients, respectively. They had hypertension stage I-II and took the drug for 12 months. Monotherapy with prestarium in a dose 8 mg/day and lomir (10 mg/day) given twice a day provides a complete hypotensive effect for a year in 56 and 62% of patients, respectively. Combinations prestarium + ariphon, lomir + anapriline or verapamil increased the above number to 100%. The occurrence of side effects of long-term lomir monotherapy was 4 times higher than due to prestarium therapy (39.4 versus 9.7%, respectively).

RESUMO

Exercise platelet aggregation, blood coagulability, and fibrinolysis were examined in patients prior myocardial infarction 2-3 months following the disease. Seventy patients of whom 35 had received placebo and 35 finoptin prior to exercise were studied. The platelet aggregation, blood coagulability, and fibrinolysis were explored before and after bicycle ergometric test. Following exercise, the patients on placebo showed higher platelet aggregation, blood coagulability and lower fibrinolysis, whereas those on finoption exhibited a lower increase in platelet aggregation and blood coagulability and activated fibrinolysis.

RESUMO

A study was made of hemostasis, fibrinolysis and platelet aggregation at rest and exercise in 32 healthy persons and 35 patients suffering from coronary heart disease (CHD) with a history of myocardial infarction that had occurred 2-3 months before. It has been discovered as a result that in healthy persons at exercise, the blood coagulation system and fibrinolysis are activated, the level of antithrombin-III rises and initial platelet aggregation decreases. In patients with CHD at exercise, hypercoagulation, deceleration of fibrinolysis, and the decline of the level of AT-III are detectable, with platelet aggregation being enhanced.

Altogether 105 patients were investigated 1.5-2 mos after myocardial infarction. They were divided into 2 groups: 1) 57 patients on nitrates or anapriline for one year; 2) 48 patients on acetylsalicylic acid (0.25 g/day) and nitrates by strict indications. Patients of the both groups performed physical exercises for one year by the methods developed at the All-Union Cardiology Research Center. The differences between Groups 1 and 2 were statistically insignificant in deaths (3.5 and 4.2%, respectively), recurrent myocardial infarctions (8.8 and 6.3%), CHD exacerbations (12.2 and 20.8%), higher tolerance, performance volume, double product, and threshold oxygen consumption. It was concluded that during a year therapy in patients with myocardial infarction who performed physical exercises, the long-term antianginal therapy chosen on an individual basis had no advantages over low-dose acetylsalicylic acid and a single use of nitrates (according to strict indications).

Daily exercise drills producing a heart rate increment of about 31.5% of the one recorded at first stress tolerance test and lasting for 105 min, in combination with daily 75 minutes' walks were not sufficient to increase stress tolerance. More intensive drills, producing a 50.6% heart rate increment, while lasting less than 1 hour per day, in combination with 2.5 hours' daily walks, resulted in a significant stress tolerance increase. In neither of the two groups was it associated with any significant rise of the double product, implicating extracardiac factors as the principal contributors.

RESUMO

Proceeding from clinicomorphological correlations in 63 patients with combined right and left ventricular myocardial infarction and in 92 patients with isolated left ventricular myocardial infarction some features of heart coronary pathology that contributed to the formation of a process of different site in the right and left ventricles were revealed. A hypothesis of variants of the pathogenesis of myocardial infarction of the anterior and posterior free walls of the right ventricle was discussed.
