RESUMO

Two patients are described with large intracranial arteriovenous angioma with minimal neurological symptoms. The first patient suffered from repeated right fronto-temporal headache. Migraine was diagnosed and he was treated with analgesic drugs. We carried out scintigraphic investigation and computerized tomography of the brain. The results of these examinations suggested the presence of large brain lesion (tumour or vascular malformation). Bilateral carotid angiography and aortic arch angiography were done in this patient. A large arteriovenous angioma was discovered in the right occipital area. The second patient complained of throbbing headaches. A bruit could be heard over the left eye which was exophthalmic. Angiograms showed a carotid-cavernous fistula on the left side and a large angioma in the fronto-parietal area. The angioma was clinically "silent". It diminished the symptoms of the carotid-cavernous fistula through the uptake of blood. Both patients were regarded as unsuitable for surgery. The reasons were the great dimension of these angiomas and a large number of supplying arteries.

Using contrast and Doppler ultrasonic diagnostic methods the authors examined 78 internal carotid arteries in 67 patients with cerebral circulatory disturbances. Ultrasonographic assessment of the blood flow in the internal carotid arteries was done in direct study of these arteries, and indirect study through the periorbital branches of the ipsilateral ophthalmic arteries. The results of USG-D examinations of internal carotid arteries were compared with their angiograms showing in 15 cases uneven outlines of the lumen, in 24 cases arterial loops, in 29 minor narrowing (21-40%), in 10 - moderate narrowing (41-70%). In the group of arteries with uneven lumen outlines USG-D examination showed impaired blood flow in 86.7% of cases, in the group of arterial loops--in 91.7% of cases, in the group of minor narrowing--in 86.2%, in the group of moderate narrowing--in 100% of cases. In all, out of 78 internal carotid arteries with angiographically confirmed abnormalities in 70 cases (89.7%) Doppler study suggested presence of lesions. In 24 cases (30.7%) disturbances of blood flow dynamics in the internal carotid artery caused an atypical result of the carotid compression test carried out during examination of opthalmic arteries. The obtained results indicated a high usefulness of Doppler test in the diagnosis of minor and moderate abnormalities of the internal carotid arteries.

RESUMO

The authors describe a method of investigations of the extracranial cerebral arteries--angioultrasonography based on the Doppler effect. The study was done with a Polish apparatus designed and produced by the Experimental Laboratory of the Institute of Basic Technical Problems, Polish Academy of Sciences. The study confirmed the usefulness of this method in the diagnosis of arterial diseases involving the extracranial arteries supplying the brain. A high agreement was demonstrated between the results of angioultrasonography and classical angiography. Owing to the use of a grey scale it was possible to localize accurately the site of stenosis or occlusion of the studied artery.

After an analysis of anatomical preparations of cerebral hemispheres, postmortem angiograms and normal intravital angiograms the morphological pattern of the branches of the middle cerebral artery forming the system of the ascending frontal artery and the most frequent varieties of the origin, course and shape of these branches were evaluated.

RESUMO

Visualization in angiographic investigations of the common carotid artery of certain collateral circulation routes can sometimes facilitate evaluation of the site of occlusion and its extent. Collateral circulation via the arterial network of the nape of the neck indicates stenosis or occlusion of the subclavian artery. Collateral circulation through the muscular branches of the occipital artery to the vertebral and basal arteries suggests lesions in the vertebral artery. Collateral circulation via the anterior communicating artery suggests stenosis or occlusion of the contralateral internal carotid artery. Visualization in the angiograms of the common carotid artery of numerous channels of collateral circulation indicates presence of disseminated changes in the extracranial brain-supplying arteries.

RESUMO

The authors report a comparative analysis of the results of angiographic investigations and computer tomography in patients with clinical manifestations of stroke. Attention is called to the accuracy of the information obtained in computer tomography with regard to the extent of focal encephalomalacia and stage of evolution. Signs are described making possible differentiation between vasogenic lesions and other pathological processes of identical or similar features in computer tomography.

RESUMO

This report concerns a case of dolichoectasia of the anterior cerebral artery. The authors present the angiographic and scintigraphic pictures. The clinical symptoms were connected with ischaemia in the region supplying by the anterior cerebral artery and seemed to indicate expanding process.
