RESUMO

A curious association of three rare tumours was described by Carney in 1977. 'Carney's triad' characteristically includes multifocal pulmonary chondroma, gastric stromal sarcoma and extra-adrenal paraganglioma. Patients may exhibit complete or incomplete expression of the triad. Carney acknowledged that, of 79 patients, only 17 possessed all three tumours. We report here two patients with incomplete expression of Carney's triad.

BACKGROUND: Computed tomography (CT) fluoroscopy nowadays is the most preferred method of guidance to perform percutaneous lung biopsy of pulmonary masses. Conventional fluoroscopy is an increasingly forgotten technique that still can be used to perform lung biopsies, with many advantages. PURPOSE: To compare the accuracy, safety, and effective dose (ED) of conventional fluoroscopy-guided needle lung biopsy (FNLB) with CT-guided needle lung biopsy procedures (CTNLB) reported in the literature. MATERIAL AND METHODS: 100 consecutive patients who underwent FNLB were reviewed retrospectively. Using the final histological diagnoses and the clinical and radiological course of the disease as references, the accuracy and sensitivity of FNLB were calculated. The complication rates of FNLB were assessed. Using computer software (XDOSE), the ED was calculated. The accuracy, complication rates, and the ED of FNLB were compared with CTNLB reported in the literature. RESULTS: The overall accuracy rate and sensitivity of FNLB were both 87%, which are comparable to the range of accuracies reported in the literature for CTNLB (74-97%). The complication rates of FNLB were also comparable to the complication rates reported for CTNLB. The commonest complication was pneumothorax, at a rate of 25%. The ED of FNLB was small, significantly lower than reported in the literature for CT-guided procedures. The mean ED of FNLB was 0.029 mSv, which is approximately equivalent to one chest X-ray. CONCLUSION: Conventional fluoroscopy is an accurate, safe, and low-dose alternative modality to CT to obtain an image-guided histological diagnosis of pulmonary lesions.

RESUMO

Idiopathic pulmonary fibrosis (IPF) is a chronic lung condition of uncertain aetiology that should be considered in the differential diagnosis of patients who experience breathlessness, cough and reduced exercise tolerance. IPF is characterized histologically by the presence of usual interstitial pneumonia, and often has typical radiological appearances. Long-term successful management options are limited and frequently unsuccessful; as the disease progresses, palliation of symptoms becomes the mainstay of treatment. In a minority of patients, lung transplantation provides the only hope of long-term survival. The median survival of patients with IPF is approximately 3 years, which in turn emphasizes the need for further investigation into its pathogenesis and potential disease-modifying pharmacological therapies.

RESUMO

Cardiac tamponade secondary to perforation of a hepatic amoebic abscess developed six years after the patient had visited an area where Entamoeba histolytica is endemic. He was treated with metronidazole and imipenem, emergency percutaneous catheter drainage, and open surgical drainage.

The main purpose of this study was to compare subjective estimates of vertebral ostepenia with bone mineral density (BMD) measurements of the same region in 200 (107 male, 93 female) subjects, aged 52-90 years, invited randomly from the community. Standardized plain thoracolumbar radiographs were examined by three senior radiologists, and given a semiquantitative osteopenia score using the method of Saville. The BMD of the anteroposterior (AP) lumbar spine and femoral neck was measured by dual energy X-ray absorptiometry. In addition BMD measurements were compared in subjects without significant vertebral deformity, and those with mild (20-25%) or definite (> 25%) reductions of vertebral height. For the Saville score, intraobserver agreement was moderate to good (kappa 0.46-0.57), and interobserver agreement was fair to moderate (kappa 0.25-0.41). Although the overlap between gradings was considerable, BMD was significantly related to visually estimated osteopenia. Subjects without apparent radiographic osteopenia (Saville grade 0) had a low risk (9-15%), compared to those with definite osteopenia (64-86%), of falling below the lowest quartile of BMD at either the femoral neck or the AP spine. In women (but not men), BMD measured at the hip and spine was related to vertebral deformity.

There has been controversy within the recent literature as to the value of computed tomography (CT) of the chest in patients with T1NoMo non-small cell bronchial carcinoma, as staged by plain radiography and bronchoscopy. This paper reports on 63 such patients, nine (14.2%) of whom were shown correctly by CT of the chest to have metastatic spread which rendered them inoperable. Of these nine, four had stage N3 mediastinal disease; the other five had metastatic disease, either pulmonary or bony. Patients with lesions less than 2 cm in diameter were also looked at separately: 4/20 had inoperable disease on CT of the chest. In both groups the detection rate of inoperability by CT of the chest justifies its use pre-operatively in all T1NoMo non-small cell bronchial carcinomas, in terms of clinical morbidity, mortality and cost.

Bronchogenic carcinoma remains the commonest fatal malignancy worldwide. In this paper we discuss the various imaging modalities available to aid diagnosis and staging, presenting these techniques in a clinical sequence. We also consider some of their limitations.

RESUMO

Mesenchymal chondrosarcomas are rare tumours, the minority arising in an extraskeletal site. We describe a case of extraskeletal mesenchymal chondrosarcoma occurring in the parapharyngeal space. This was demonstrated by computed tomography (CT), magnetic resonance imaging (MRI) and angiography.
