Metabolic Syndrome (MetS) represents a group of cardiovascular risk factors. This article aims to evaluate the accuracy of the tools of MetS diagnosis in Nursing professionals from Primary Health Care (PHC) in Bahia, Brazil. A cross-sectional study with a random sample selected according to essential health information for the diagnostic of MetS. For MetS diagnostic, we used EGIR, NCEP-ATPIII, AACE, IDF, Barbosa et al. (2006), and IDF/AHA/NHLBI (defined as gold standard) definition. Sensitivity, specificity, predictive values, and likelihood ratio were estimated for each diagnostic tool and compared with the gold standard. Kappa statistic was used to determine the agreement between the diagnostic methods. One thousand one hundred and eleven nursing professionals were included in this study. Sensitivity varied from 15% to 95.1%, and specificity varied between 99.5% and 100%. IDF and Barbosa et al. (2006) definitions were more sensitive (95.1% and 92.8%, respectively), and EGIR, NCEP, ATP III, and IDF showed 100% specificity. IDF and Barbosa et al. (2006) use suitable metabolic syndrome identification and confirmation criteria. The highest agreement was found in the definition of the IDF, Barbosa et al. (2006) and the NCEP ATP III. Defining metabolic syndrome with a higher diagnostic accuracy could contribute to the screening and the early identification of nursing professionals with cardiovascular disease risk factors, which provide opportunities for appropriate prevention and treatment.

Evolocumab and empagliflozin yield a modest rise in plasma high-density lipoprotein cholesterol (HDL-C) through unknown mechanisms. This study aims to assess the effect of evolocumab plus empagliflozin vs. empagliflozin alone on HDL subspecies isolated from individuals with type 2 diabetes mellitus (T2D). This post hoc prespecified analysis of the EXCEED-BHS3 trial compared the effects of a 16-week therapy with empagliflozin (E) alone or in combination with evolocumab (EE) on the lipid profile and cholesterol content in HDL subspecies in individuals with T2D divided equally into two groups of 55 patients. Both treatments modestly increased HDL-C. The cholesterol content in HDL subspecies 2a (7.3%), 3a (7.2%) and 3c (15%) increased from baseline in the E group, while the EE group presented an increase from baseline in 3a (9.3%), 3b (16%) and 3c (25%). The increase in HDL 3b and 3c was higher in the EE group when compared to the E group (p < 0.05). No significant interactive association was observed between changes in hematocrit and HDL-C levels after treatment. Over a 16-week period, empagliflozin with or without the addition of evolocumab led to a modest but significant increase in HDL-C. The rise in smaller-sized HDL particles was heterogeneous amongst the treatment combinations.

OBJECTIVES: This study aims to identify the two-dimensional and three-dimensional analyses and evaluate the loss of tooth structure in the tooth's different constituent elements and the ceramic antagonist's surface. METHODS: In this study, three groups (n = 10) represented by different ceramic systems (lithium disilicate [GDis], lithium silicate reinforced with zirconia [GSil], and monolithic zirconia [GZir]) were evaluated. Each group obtained ten ceramic blocks and submitted them to the sintering/crystallization process. To carry out the wear test, healthy mandibular premolars were used as the specimens and the ceramic blocks as antagonists. The premolars were submitted to two-dimensional and three-dimensional analyses using a computerized microtomography (µTC) before and after the wear test. The wear test was performed with 30 N and 300,000 cycles load, with occlusion, laterality, and disocclusion movements. RESULTS: The statistical analysis comparing the loss of two-dimensional tooth structure showed a statistically significant difference among all groups (p < 0.05). Statistical analysis comparing the percentage of loss of three-dimensional tooth structure showed a statistically significant difference between groups GDis and GSil and between groups GDis and GZir. However, when comparing GSil with GZir, no statistically significant difference was found. The qualitative analysis of the teeth showed that GDis showed considerable enamel loss and dentin exposure, GSil showed enamel wear with flattening the cusp without dentin exposure, and GZir showed minimal enamel wear without dentin exposure. In the qualitative analysis of ceramic antagonists, more significant wear of the ceramic material for GDis was observed, followed by the GSil and GZir groups, respectively. SIGNIFICANCE: The use of the lithium disilicate should be cautious, restricting it to areas with lower masticatory forces. Areas of higher masticatory forces showed a large amount of antagonist wear, with dentin involvement. This can lead to dentinal hypersensitivity, risk of compromising patients' occlusion, by harming group disocclusion guides, causing pain and temporomandibular disorders.

BACKGROUND: In humans, allergic conjunctivitis is a well described disease. In contrast, allergic conjunctivitis has not received much attention from the veterinary community so far. Canine allergic conjunctivitis (cAC) is one of the possible manifestations associated with canine atopic dermatitis (cAD), being often underdiagnosed and undertreated. Our aim is to contribute to disease characterization and clinical stagingfor cAC severity. RESULTS: A retrospective observational study including 122 dogs that underwent a complete ophthalmological and dermatological examinations and diagnosed with allergic conjunctivitis was conducted. A total of six ophthalmic clinical signs were considered for disease characterization and clinical staging: conjunctival hyperemia, chemosis, ocular pruritus, epiphora, seromucoid to mucopurulent discharge and keratitis, classified from 0 (absent) to 3 (severe). Scores comprised between 1-5 were considered mild, 6-10 moderate and 11-18 severe. The majority of dogs (64%) presented with moderate cAC followed by 24% of mild stages and only 12% of severe presentations. The severity of allergic conjunctivitis was not correlated to sex or age at the time of diagnosis and all presented with a bilateral form of the disease. Chemosis (84%), hyperemia (83%) and ocular pruritus (79%) was observed in 55% of the cases. Seromucoid to mucopurulent discharge (62%) and epiphora (69%) were less frequent, with keratitis being the least encountered clinical sign (15%). The degree of keratitis showed a positive correlation with both severity and chronicity of cAC (rho = 0.21-0.29, p ≤ 0.02)). Severity of cAD and cAD were not significantly correlated (p-value = 0.4). DISCUSSION AND CONCLUSION: The triad hyperemia, chemosis and ocular pruritus, already known in human medicine to be a reliable way of diagnosing human allergic conjunctivitis, also proved to be important in cAC Mild forms of the disease may pass unnoticed, ocular pruritus being hard to assess in canine patients.The proposed standardized diagnostic approach and novel grading scheme for cAC may be of value for both veterinary ophthalmologists and dermatologists, as well as general practitioners.

OBJECTIVE: To explore cultural and immigration-related factors, among Central American unaccompanied immigrant youth (UIY), affecting the safety and acceptability of Cognitive Behavioral Intervention for Trauma in Schools (CBITS), a group intervention designed to treat trauma symptoms. METHOD: Thematic analysis of data from grounded theory study of group interventions to support resilience in UIY, consisting of interviews with 10 key stakeholders (5 CBITS facilitators) and 16 UIY (6 CBITS participants) from El Salvador, Guatemala, or Honduras, interviews conducted in Spanish, English, or Mam. RESULTS: Five themes emerged from interviews with CBITS facilitators and UIY: (a) Todo está bién: self-protective silence about trauma and symptom denial, (b) Chisme goes around: personal risks of disclosure, (c) marginalizing the language and world view of indigenous youth, (d) "CBITS didn't really quite land for them": adapting the curriculum and delivery, and (e) "I learn to appreciate things": benefits of the CBITS group. CBITS facilitators endorsed skill-building aspects of the groups and expressed concerns about a curriculum relying on written homework and parental support for youth with limited home country schooling, currently living with distant relatives. CBITS participants endorsed hearing about other youths' stories and learning coping skills. CONCLUSIONS: Models for group intervention that emphasize coping skill development and group support, while de-emphasizing the trauma narrative, should be explored and tested. Group intervention leaders should consider the impact of differences in gender, country of origin, and native language on group dynamics. Resumo Fundamento No microambiente da placa aterosclerótica, os fosfolipídios oxidados expressos na superfície de lipoproteína de baixa densidade oxidada (oxLDL) se ligam a receptores scavenger em macrófagos provocando a formação de células espumosas e a progressão da placa. Autoanticorpos contra oxLDL (oxLDL-Ab) interagem com epítopos oxidativos levando à formação de imunocomplexos que são incapazes de interagir com receptores de macrófagos, assim suprimindo a aterogênese. A liberação de oxLDL-Ab pelas células B envolve a resposta da interleucina 5 e Th2, que por sua vez são potencializadas pela HDL. Assim, levantamos a hipótese de que indivíduos com níveis mais altos de HDL-C podem apresentar níveis elevados de oxLDL-Ab. Objetivo Avaliar a relação entre os níveis de HDL-C e oxLDL-Ab. Métodos Indivíduos assintomáticos (n = 193) foram agrupados de acordo com sua concentração de HDL-C para uma das três categorias seguintes: baixa (< 68 mg/dL), intermediária (de 68 a 80 mg/dL) ou alta (> 80 mg/dL). Os valores p < 0,05 foram considerados estatisticamente significativos. Resultados Nossa análise incluiu 193 indivíduos (média etária: 47 anos; masculino: 26,3%). Em comparação com os indivíduos no menor tercil de HDL-C, os mais elevados foram mais velhos (36 versus 53 anos; p = 0,001) e, menos frequentemente, masculinos (42,6% versus 20,9%; p = 0,001). Os valores médios de oxLDL-Ab aumentaram à medida que o grupo HDL-C aumentou (0,31, 0,33 e 0,43 unidades, respectivamente; p = 0,001 para tendência). A regressão linear simples encontrou uma relação significativa e positiva entre a variável independente, HDL-C, e a variável dependente, oxLDL-Ab (R = 0,293; p = 0,009). Essa relação manteve-se significativa (R = 0,30; p = 0,044), após ajuste por covariáveis. Os níveis de apolipoproteína AI também estiveram relacionados a oxLDL-Ab nos modelos de regressão linear simples e ajustada. Conclusões HDL-C e oxLDL-Ab estão independentemente relacionados.

STATEMENT OF PROBLEM: Preheated composite resin and adhesive coating have been recommended as luting agents for ceramic veneers; however, information about the mechanical behavior of feldspathic porcelain veneers after using these methods is still limited. PURPOSE: The purpose of this in vitro study was to evaluate the influence of the use of preheated composite resin or an adhesive resin layer on the strengthening of a feldspathic porcelain. MATERIAL AND METHODS: Feldspathic porcelain disks were fabricated (N=210), etched (10% hydrofluoric acid for 90 seconds), and bonded according to resin-based luting agents (30 per group) for the biaxial flexural strength tests: photopolymerized resin cement, flowable composite resin, preheated composite resin, with or without previous adhesive coating. Characteristic strength and Weibull modulus were calculated. The viscosity of the luting agents was measured by using isothermal analyses. Finite element analysis simulated the flexural test. Fractographic and bonded interfaces were observed. RESULTS: The use of preheated composite resin led to the lowest stress levels in the feldspathic porcelain veneers. However, it did not increase the ceramic strengthening compared with other luting agents, as higher values were observed with flowable composite resin (P<.05). The use of an adhesive resin layer had no significant effect on ceramic strengthening when associated with preheated composite resin or resin cement (P>.05). Preheated composite resin presented the highest viscosity and led to incomplete filling of the porcelain surface irregularities. Failures originated on the ceramic surface subjected to tensile stress and were typically associated with pores at the ceramic-luting agent interface. CONCLUSIONS: The use of preheated composite resin or an adhesive resin layer did not have a positive effect on strengthening feldspathic porcelain.

BACKGROUND: Increased cholesterol absorption and reduced synthesis are processes that have been associated with cardiovascular disease risk in a controversial way. However, most of the studies involving markers of cholesterol synthesis and absorption include conditions, such as obesity, diabetes, dyslipidemia, which can be confounding factors. The present study aimed at investigating the relationships of plasma cholesterol synthesis and absorption markers with cardiovascular disease (CVD) risk factors, cIMT (carotid intima-media thickness), and the presence of carotid plaques in asymptomatic subjects. METHODS: A cross-sectional study was carried out in 270 asymptomatic individuals and anthropometrical parameters, fasting plasma lipids, glucometabolic profiles, high-sensitivity C-reactive protein (hs-CRP), markers of cholesterol synthesis (desmosterol and lathosterol), absorption (campesterol and sitosterol), cIMT, and the presence of atherosclerotic plaques were analyzed. RESULTS: Among the selected subjects aged between 19 and 75 years, 51% were females. Age, body mass index, systolic and diastolic blood pressure, total cholesterol, non-HDL-C, triglycerides, glucose, and lathosterol/sitosterol ratios correlated positively with cIMT (p ≤ 0.05). Atherosclerotic plaques were present in 19% of the subjects. A direct association of carotid plaques with campesterol, OR = 1.71 (95% CI = 1.04-2.82, p ≤ 0.05) and inverse associations with both ratios lathosterol/campesterol, OR = 0.29 (CI = 0.11-0.80, p ≤ 0.05) and lathosterol/sitosterol, OR = 0.45 (CI = 0.22-0.95, p ≤ 0.05) were observed in univariate logistic regression analysis. CONCLUSIONS: The findings suggested that campesterol may be associated with atherosclerotic plaques and the lathosterol/campesterol or sitosterol ratios suggested an inverse association. Furthermore, synthesis and absorption of cholesterol are inverse processes, and the absorption marker, campesterol, may reflect changes in body cholesterol homeostasis with atherogenic potential.

BACKGROUND: This study aimed to evaluate the associations between Lipoprotein (a) ‒ Lp(a) levels and carotid Intima-Media Thickness (cIMT) and with carotid plaques in healthy subjects because of previous contradictory data. METHODS: A total of 317 healthy normolipidemic subjects (20‒77 years old) were selected. The cIMT and atherosclerotic plaques were determined by B-mode ultrasonography. Mann-Whitney tests were performed to compare the groups according to Lp(a) levels and to explore the associations between Lp(a), carotid plaques, and cIMT, logistic and linear regression analyses were performed. RESULTS: Studied population (51% females, median age 43 years old) presented carotid plaques and cIMT ≥ 0.9 mm in 23% and 18% of the participants, respectively. The group with Lp(a) levels > 30 mg/dL presented significantly higher age and atherosclerotic plaques. Indeed, multivariate linear regression analysis showed a significant association between Lp(a), age, and race. On the other hand, logistic regression analysis demonstrated that the subjects with Lp(a) > 30 mg/dL have a significantly high risk of carotid plaques. CONCLUSION: The data from the present study indicate that Lp(a) levels above 30 mg/dL contribute to the development of carotid plaques even in apparently healthy participants.

ABSTRACT: The objective of this in vitro study was to evaluate the effects of finishing protocols and oral hygiene procedures on the surface and mechanical properties of CAD/CAM ceramics. Specimens (n = 96, (15x10x2mm) of the leucite glass-ceramic (LGC) and feldspathic ceramic (FP) received mechanical polishing or glazing. Surface roughness (Ra) and microhardness (VHN) were obtained with a profilometer and a hardness tester, respectively, before and after each oral hygiene procedure (TB: toothbrushing; PB: prophylaxis with paste and rubber cup; and BJ: prophylaxis with bicarbonate jet). ANOVA, paired t and Tukey's tests were applied (a=0.05). For both materials, only BJ affected Ra's values when mechanical polishing was performed. Moreover, no significant variations in VHN were observed only for LGC glazed with TB. Significant Ra and VHN changes in both materials were observed with BJ. Besides, BJ exhibited similar effects to TB on VHN, regardless of the material and finishing protocol used. Changes in mechanical and surface properties varied with the finishing protocol and the oral hygiene method. In general, prophylaxis with bicarbonate jet was the most harmful method for both materials and the type of finishing protocol did not have a protective effect against changes produced by oral hygiene methods.


RESUMEN: El objetivo de este estudio in vitro fue evaluar los efectos de los protocolos de acabado y los procedimientos de higiene oral sobre la superficie y las propiedades mecánicas de las cerámicas CAD/CAM. Los especímenes (n = 96, (15x10x2mm) de vitrocerámica de leucita (LGC) y cerámica feldespática (FP) recibieron pulido mecánico o glaseado. La rugosidad de la superficie (Ra) y la microdureza (VHN) se obtuvieron con un perfilómetro y un probador de dureza, respectivamente, antes y después de cada procedimiento de higiene oral (TB: cepillado dental; PB: profilaxis con pasta y vaso de hule; y BJ: profilaxis con chorro de bicarbonato). Se aplicaron ANOVA, t pareada y prueba de Tukey (a=0.05). Para ambos materiales, solo BJ afectó los valores de Ra cuando se realizó el pulido mecánico. Además, no se observaron variaciones significativas en VHN solo para LGC vidriado con TB. Se observaron cambios significativos de Ra y VHN en ambos materiales con BJ. Además, BJ exhibió efectos similares a TB en VHN, independientemente del material y protocolo de acabado utilizado Los cambios en las propiedades mecánicas y superficiales variaron con el protocolo de acabado y el método de higiene oral. En general, la profilaxis con chorro de bicarbonato fue el método más perjudicial, tanto los materiales como el tipo de protocolo de acabado no tuvieron un efecto protector frente a los cambios producidos por los métodos de higiene oral.

Metabolic syndrome (MS) is a clinical condition and a relevant risk factor in the development of cardiovascular diseases; it occurs as a result of lifestyle factors, e.g., work. The aim of this research was to estimate the interaction between work and MS among primary health care (PHC) nursing professionals in the state of Bahia, Brazil. A sectional multicentered study carried out in 43 municipalities in Bahia, whose study population consisted of nursing professionals. The exposure variables were occupation, professional exhaustion, and working time, and the outcome variable was MS. Interaction measures based on the additivity criteria were verified by calculating the excess risks due to the interactions and according to the proportion of cases attributed to the interactions and the synergy index. The global MS prevalence is 24.4%. There was a greater magnitude in the exposure group regarding the three investigated factors (average level occupation, professional exhaustion, and working time in PHC for more than 5 years), reaching an occurrence of 44.9% when compared to the prevalence of 13.1% in the non-exposure group (academic education, without professional burnout, and working time in PHC for up to 5 years). The study's findings showed a synergistic interaction of work aspects for MS occurrence among PHC nursing professionals.

OBJECTIVES: The purpose of this study was to evaluate the dimensional stability of prototyped surgical guides after autoclave and 2% glutaraldehyde sterilization processes. MATERIAL AND METHODS: Twenty prototyped surgical guides were prepared and submitted to two sterilization processes (n = 10): Physics - sterilization by autoclave (saturated water vapor under pressure, temperature of 126 to 130 °C, pressure of 1,7 at 1.9 kgf/cm2, 16 min); Chemistry - sterilization by 2% glutaraldehyde for 10 h. Six pre-established points were measured in the prototyped surgical guides, before and after sterilization, using a digital caliper rule. The comparisons were made using orthogonal contrasts using the linear model of mixed effects (random and fixed). RESULTS: there are no significant differences between autoclave and glutaraldehyde 2% (p>0.05) there are significant differences after autoclave sterilization (p<0.05) and there are not significant differences after 2% glutaraldehyde sterilization (p>0.05). CONCLUSION: autoclave promoted dimensional alteration of the prototyped surgical guides, and the chemical sterilization by glutaraldehyde 2% did not cause dimensional alteration of the prototype surgical guides, being a favorable choice for sterilization. CLINICAL SIGNIFICANCE: the sterilization of surgical guides can be performed through the chemical process with 2% glutaraldehyde without changing the linear precision of the prototype surgical guides.

Abstract Background: This study aimed to evaluate the associations between Lipoprotein (a) ‒ Lp(a) levels and carotid Intima-Media Thickness (cIMT) and with carotid plaques in healthy subjects because of previous contradictory data. Methods: A total of 317 healthy normolipidemic subjects (20‒77 years old) were selected. The cIMT and atherosclerotic plaques were determined by B-mode ultrasonography. Mann-Whitney tests were performed to compare the groups according to Lp(a) levels and to explore the associations between Lp(a), carotid plaques, and cIMT, logistic and linear regression analyses were performed. Results: Studied population (51% females, median age 43 years old) presented carotid plaques and cIMT ≥ 0.9 mm in 23% and 18% of the participants, respectively. The group with Lp(a) levels > 30 mg/dL presented significantly higher age and atherosclerotic plaques. Indeed, multivariate linear regression analysis showed a significant association between Lp(a), age, and race. On the other hand, logistic regression analysis demonstrated that the subjects with Lp(a) > 30 mg/dL have a significantly high risk of carotid plaques. Conclusion: The data from the present study indicate that Lp(a) levels above 30 mg/dL contribute to the development of carotid plaques even in apparently healthy participants.

Cancer cell plasticity due to the dynamic architecture of interactome networks provides a vexing outlet for therapy evasion. Here, through chemical biology approaches for systems level exploration of protein connectivity changes applied to pancreatic cancer cell lines, patient biospecimens, and cell- and patient-derived xenografts in mice, we demonstrate interactomes can be re-engineered for vulnerability. By manipulating epichaperomes pharmacologically, we control and anticipate how thousands of proteins interact in real-time within tumours. Further, we can essentially force tumours into interactome hyperconnectivity and maximal protein-protein interaction capacity, a state whereby no rebound pathways can be deployed and where alternative signalling is supressed. This approach therefore primes interactomes to enhance vulnerability and improve treatment efficacy, enabling therapeutics with traditionally poor performance to become highly efficacious. These findings provide proof-of-principle for a paradigm to overcome drug resistance through pharmacologic manipulation of proteome-wide protein-protein interaction networks.

Abstract This study evaluated the surface characteristics and bond strength of Y-TZP treated with Er:YAG laser at different focal distances. 120 Y-TZP blocks were divided into five groups (n=24), according to the surface treatment: no treatment (C-); sandblasting with silica-coated aluminum oxide particles (C+); and Er:YAG laser application at focal distances of 1mm (Er:YAG-1), 4mm (Er:YAG-4), and 7mm (Er:YAG-7). Surface characteristics were analyzed using Vickers microhardness, confocal laser microscope, scanning electron microscopy (SEM), and X-ray diffractometer (XRD). For the bond strength test, 100 Y-TZP blocks were subdivided into two subgroups (n=10), according to the resin cement used: with (MDP+) or without 10-methacryloyloxydecyl dihydrogen phosphate (MDP-). The Vickers microhardness and surface roughness were analyzed by one-way ANOVA and bond strength by two-way ANOVA and Tukey's test for both (α=0.05). Vickers microhardness differences were not observed between the groups (p>0.05); C+ showed higher surface roughness values. SEM images showed micromorphological differences between the groups. The XRD data detected tetragonal crystals for C- and, for the other groups, tetragonal and monoclinic peaks. For bond strength, no statistically difference significance were observed among the cements with or without MDP (p>0.05) but showed significant difference between the surface treatments (C+ > C- = Er:YAG1 > Er:YAG4 = Er:YAG7) (p<0.05). Suggested that the Er:YAG laser cannot replace conventional treatment with aluminum oxide particles and the presence of MDP in the resin cement had no influence on the bond strength.


Resumo Este estudo avaliou as características de superfície e resistência de união de Y-TZP tratado com laser de Er: YAG em diferentes distâncias focais. Cento e vinte blocos de Y-TZP foram divididos em cinco grupos (n=24), de acordo com a superfície de tratamento: sem tratamento (C-); asperização com partículas de óxido de alumínio revestidas por sílica (C +); e aplicação de laser Er: YAG a distâncias focais de 1 mm (Er: YAG-1), 4 mm (Er: YAG-4) e 7 mm (Er: YAG-7). As características de superfície foram analisadas por meio de microdureza Vickers, microscópio confocal a laser, microscopia eletrônica de varredura (MEV) e difratômetro de raios-X (DRX). Para o ensaio de resistência de união, cem blocos de Y-TZP foram subdivididos em 2 subgrupos (n=10) de acordo com o cimento resinoso utilizado (n=12): com 10-metacriloiloxidecil dihidrogenofosfato (MDP+) ou sem (MDP-). Microdureza Vickers e rugosidade de superfície foram analisadas por ANOVA a 1 fator e a resistência de união por ANOVA a 2 fatores e ambos seguidos de teste complementar de Tukey (α=0,05). Não foram observadas diferenças de microdureza Vickers entre os grupos; C+ apresentou maiores valores de rugosidade superficial. Imagens de MEV mostraram diferenças micromorfológicas entre os grupos. Os dados de DRX detectaram apenas cristais tetragonais para C- e, para os outros grupos, picos de zircônia nas fases tetragonal e monoclínica. Para a resistência de união, não foram observadas diferenças estatisticamente significante entre os cimentos com e sem MDP (p>0.05), mas foram observadas diferenças significativas entre os tratamentos de superfície (C+ > C- = Er:YAG1 > Er:YAG4 = Er:YAG7) (p<0.05). Sugere-se que o laser de Er: YAG não pode substituir o tratamento convencional com partículas de óxido de alumínio e a presença de MDP no cimento resinoso não influenciou na resistência de união.

INTRODUCTION: The purpose of this study was to evaluate the impact of conservative endodontic cavities (CECs) on root canal preparation, restoration, and biomechanical behavior of teeth prepared using different shaping systems and restorative materials. METHODS: Ninety upper premolars with a bifurcated root were matched based on morphology and randomly assigned to a control group (n = 10) or 1 of the following experimental groups (n = 40): traditional endodontic cavity and CEC. Teeth were subdivided according to instrumentation (n = 10) as follows: ProTaper Universal (Dentsply Maillefer, Ballaigues, Switzerland), Reciproc (VDW GmbH, Munich, Germany), Reciproc Blue (VDW GmbH), and Hyflex EDM (Coltene/Whaledent, Altstätten, Switzerland). After canal obturation, teeth were restored using temporary material, conventional composite, regular bulk fill composite, or bulk fill flow combined with conventional composite. Before and after preparation and after obturation, the teeth were scanned using micro-computed tomographic imaging. Canal transportation (CT), the percentage of untouched canal surfaces (UCSs), voids in restoration (VRs), and residual filling material in the pulp chamber were evaluated. Finite element analysis, fracture resistance, and the failure pattern were recorded. The data were analyzed using analysis of variance and the Tukey and chi-square tests. RESULTS: CECs had greater CT, percentage of filling material, and VRs compared with traditional endodontic cavities (P < .0001). The highest CT and UCSs were observed in CEC with ProTaper Universal. Bulk fill flow combined with conventional composite showed a lower percentage of VRs compared with other restorative materials (P < .05). Finite element analysis, fracture resistance, and failure pattern revealed similar behaviors in all groups (P > .05) . CONCLUSIONS: CECs had a negative impact on root canal centralization, UCSs, cleaning of the pulp chamber, and percentage of VRs. Controlled memory instruments were the most adequate for the root canal preparation of CECs. The endodontic cavity did not influence the biomechanical behavior of restored teeth.

STATEMENT OF PROBLEM: The mechanical behavior of ceramic endocrowns is unclear. PURPOSE: The purpose of this in vitro and 3-dimensional finite element analysis (3D-FEA) study was to evaluate the mechanical behavior of endodontically treated teeth restored with ceramic endocrowns made by using different computer-aided design and computer-aided manufacturing (CAD-CAM) systems. MATERIAL AND METHODS: Sixty mandibular human molars were endodontically treated, prepared for endocrowns, and divided into 4 groups (n=15) according to the following various ceramic systems: leucite-based glass-ceramic (LC group), lithium disilicate-based glass-ceramic (LD group), glass-ceramic based on zirconia-reinforced lithium silicate (LSZ group), and monolithic zirconia (ZR group). After adhesive bonding, the specimens were subjected to thermomechanical loading and then to fracture resistance testing in a universal testing machine. The failure mode of the specimens was qualitatively evaluated. Three-dimensional FEA was performed to evaluate the stress distribution in each group. Data were analyzed by using a 1-way ANOVA and the Tukey HSD test (α=.05). RESULTS: Statistically significant differences among the groups were observed (P<.05). The outcomes of the LC, LD, and LSZ groups were similar (1178 N, 1935 N, and 1859 N) but different from those of the ZR group (6333 N). The LC and LD groups had a higher ratio of restorable failures, while LSZ and ZR had more nonrestorable failures. Fractographic analysis indicated a regular failure pattern in the ZR group and irregular failure patterns in the other groups. Three-dimensional FEA revealed similar values and stress pattern distributions among the groups. CONCLUSIONS: The mechanical performance of monolithic zirconia was better than that of the other ceramic endocrowns considered in this research; however, monolithic zirconia presented a higher rate of catastrophic tooth structure failure.

This study evaluated the effect of endocrown and ferrule restorative treatment on the mechanical behavior of anterior endodontically treated teeth. Human canines were treated endodontically and distributed in five groups (n = 10): sound teeth (S), crown associated with glass fiber post (GFP) and ferrule (GFPf+) and that without ferrule (GFPf-), and endocrown with ferrule (Ef+) and that without ferrule (Ef-). The crowns were obtained in lithium disilicate-based glass ceramic and cemented with resin. Thermomechanical loading (TL) was performed with progressive load of 80, 120, 160, 200, 240, 280, and 320 N with 20,000 cycles each, 140,000 cycles in total, frequency of 5 Hz, and temperature variation of 5°C-55 °C, followed by fracture resistance testing with load cell of 1000 Kgf and crosshead speed of 0.5 mm/min in a universal testing machine. Failure mode has been evaluated. Data were compared by Kaplan-Meier survival analysis (p < 0.05) for TL and one-way ANOVA and Tukey's test (p < 0.05) for fracture resistance. TL showed higher number of fractures for groups without ferrule (GFPf- = 7, GFPf+ = 3, Ef- = 10, and Ef+ = 2) and lower mean fracture load probability (GFPf+ = 300 N, GFPf- = 280 N, Ef+ = 320 N, Ef- = 188 N) than those with ferrule. Regarding fracture, teeth rehabilitated with GFP were more resistant to endocrowns with ferrule (p < 0.05). A higher percentage of type III failures after fatigue and type II failures after fracture were found. In conclusion, rehabilitation using GFP with ferrule is more favorable for anterior teeth, but the use of endocrown with ferrule proved feasible.
