RESUMO

OBJECTIVE: To analyze the premorbid features, clinical manifestations, dynamics and outcomes of ARFID in adolescent girls, as well as to clarify the relationship of ARFID with other eating disorders (EDs) and comorbid mental pathology. MATERIAL AND METHODS: The study included 52 female adolescents, aged 10.2-16.7 years (mean age 13.1±1.9 years) who, at the time of admission, met the DSM-5 ARFID criteria. The average follow-up period was 5.6±1.6 years, the average age at the time of the last follow-up examination was 19.1±2.7 years. RESULTS: Patients with ARFID accounted for 8.4% of the sample of female adolescents with EDs. The stability of the clinical manifestations of ARFID indicates the validity of considering ARFID as an independent diagnostic category. The clinical picture of ARFID is determined by specific symptom complexes amalgamated with constitutional features. The latter are manifested from early childhood by neuroticism, anxiety, a weakened and somewhat perverted food instinct, metabolic features, partly determining the low need for food, immaturity and weakness of autonomic regulation that may form a predisposition for the development of psychosomatic/somatopsychic disorders. CONCLUSION: The study allows us to talk about a special predisposition, in the form of psychosomatic diathesis, which predisposes to ARFID and is a significant component of the response to psychogenic factors. It is assumed that diathesis as an abnormal constitution that determines the regulation of mental, autonomic and metabolic processes is a significant pathogenetic mechanism for other EDs. A certain commonality of the etiology and pathogenesis of ARFID and the entire group of EDs is confirmed by the high burden of family history of patients with various forms of EDs (13.5%) and a significant frequency of transformation of ARFID into other EDs.

RESUMO

Multifocal arterial injury is common in patients with atherosclerotic cardiovascular diseases and is associated with increased risk of cardiovascular complications and death. Administration of more intensive antithrombotic therapy, particularly combinations of acetylsalicylic acid and a "vascular" dose of rivaroxaban, in patients with multifocal arterial injury is characterized by a beneficial ratio of efficiency and safety due to a pronounced decrease in the risk of cardiovascular complications. Detection of peripheral artery diseases in patients with ischemic heart disease and atherosclerotic cerebrovascular pathology makes it possible to improve the risk stratification, optimize the diagnostic tactics and clarify indications for more intensive antithrombotic therapy.

RESUMO

We studied the sensitivity of domestic proprietary human and animal cell lines from the collection of M. P. Chumakov Federal Scientific Center for Research and Development of Immuneand-Biological Products to infection with different enterovirus 71 strains. A cell system based on domestic proprietary permanent cell line 4647 was for the first time used for reproduction of four enterovirus 71 strains (BrCr, 42266, 42934, and 43374). It was shown that strain 4647 is the optimal cell substrate for enterovirus 71 reproduction. The titers of enterovirus 71 for all four strains considerably (by 2 lgTCID50/ml and more) increased during sequential passages in permanent cell line 4647. The prospects of using permanent cell line 4647 for creation of diagnostic and preventive preparations against 71 was demonstrated.

RESUMO

AIM: To compare the in vivo accuracy of CBCT for the detection of fracture lines versus the diagnosis of vertical root fractures (VRFs) according to characteristic patterns of associated bone resorption. METHODOLOGY: Eighty-eight patients with symptoms typical of VRFs in root filled teeth, who underwent a CBCT examination and later had the teeth extracted, were divided into two groups: the fracture group (n = 65) and the control group (n = 23). Five blinded observers assessed the CBCT images in two sessions. During the first session, they were asked to state the diagnosis according to the CBCT and clinical data. During the second session after 2 weeks, they assessed only axial slices and were asked to detect a fracture line. The mean CBCT specificity, sensitivity, accuracy values and area under the receiver operating characteristic (AUROC) curve were calculated and compared using the Wilcoxon signed-rank test. RESULTS: The average sensitivity of CBCT for the diagnosis of VRFs was 0.84 ± 0.2. The accuracy and AUC values were 0.81 ± 0.08 and 0.84 ± 0.17, respectively. The sensitivity, accuracy and AUC values for the detection of VRFs were significantly lower: 0.17 ± 0.24 (P = 0.042), 0.54 ± 0.07 (P = 0.043), and 0.52 ± 0.09 (P = 0.043), respectively. The specificity of CBCT for the detection and diagnosis of VRFs did not differ significantly (P = 0.50). CONCLUSION: Cone-beam computed tomography was helpful in VRF diagnosis even when it was not possible to visualize the fracture line.

RESUMO

AIM: To perform a phenomenological/clinical/psychopathological analysis of non-suicidal self-injury behavior (NSSI) in adolescents. MATERIAL AND METHODS: The authors studied 155 adolescents, 140 girls and 15 boys, aged 12-17 years, including 111 with self-cutting, 5 with self-burning, 4 with self-beating. Other cases included combined, multiple and other variants of NSSI. RESULTS AND CONCLUSION: NSSI develop more often in female teenagers. NSSI may be a component of suicidal behavior or represent a relatively independent behavioral model existing out of a suicidal context. NSSI behavior can be considered as psychological defense as well as a coping strategy in response to nonspecific stress.

RESUMO

AIM: To investigate affective pathology in patients with adolescent bulimia nervosa (BN) and to analyze comorbid mutual influences of these clinical entities. MATERIAL AND METHODS: A sample consisted of 52 young female patients, aged from 13.9 to 17.4 years, who simultaneously meet ICD-10 criteria of F50.2 - typical BN and F30-F39 - mood disorders. The Eating Attitudes Test (EAT-26) and the Beck Depression Inventory were used for psychometric assessment. Duration of follow-up ranged from 1 to 7 years. RESULTS AND CONCLUSION: The common forms of affective disorders comorbid with adolescent BN were dysthymia and bipolar affective disorder (BAD) type II. Less often BN manifested in association with psychogenic endoform depressions. And the less typical was comorbidity of BN with endogenous recurrent unipolar depression. The normalization of mood and transition depression to hypomania contributed to the remission of eating disorders. Worst clinical and social outcome was detected in patients with long-term persistent hypothymia caused by dysthymia or protracted depressive episodes of recurrent depressive disorder. A more favorable outcome was seen in patients with alternation of depression and hypomania in the structure of the BAD type II and the best outcome in patients with single episode of psychogenic endoform depression.

RESUMO

The aim of the study was to assess the feasibility and precision of vertical root fracture diagnostics by cone beam CT (CBCT). Vertical root fractures were simulated in 25 sound teeth extracted because of orthodontic and periodontal reasons and examined by CBCT. The roots were then sectioned in corresponding planes and fractures were assessed microscopically. CBCT sensitivity and precision was significantly higher for visualization of vertical fractures >150 µm when compares to fractures <50 µm with no difference in specificity. CBCT is an effective tool for vertical root fractures detection.

RESUMO

AIMS: To compare the diagnostic accuracy of cone beam computed tomography (CBCT) for the detection of artificially induced vertical root fractures (VRFs) of different widths in vitro and in vivo. METHODOLOGY: Vertical root fractures were induced in 25 extracted nonendodontically treated single-rooted human teeth (maxillary first premolars, maxillary canines and mandibular incisors). Twenty teeth without VRFs served as a control group. CBCT scanning (3D Accuitomo 170) was performed in vitro and in vivo. For the in vivo scanning, teeth were autoclaved, embedded into bite plates, placed in sterile plastic bags and then inserted into the mouths of volunteers. Teeth with VRFs were sectioned into axial slices and examined using a stereomicroscope to measure the widths of the VRFs. Five observers assessed the presence of VRFs using axial CBCT. Values for sensitivity, specificity, accuracy and interexaminer agreement were calculated. RESULTS: The accuracy, specificity and sensitivity of CBCT were significantly higher in vitro than in vivo for VRFs with widths 50-150 µm (P < 0.05). The sensitivity and accuracy of CBCT were significantly higher for the detection of VRFs with widths greater than 150 µm in vivo and in vitro (P < 0.05). The accuracy of CBCT in vivo was 0.29 and 0.8 for fracture widths ranging from 50 to 150 µm and wider than 150 µm, respectively. No significant differences in CBCT specificity were found between VRF widths both in vitro and in vivo. The interexaminer reliability of the raters revealed a kappa value of 0.72, demonstrating substantial agreement. CONCLUSION: The detectability of VRFs by CBCT in vitro and in vivo was dependent upon fracture width. The accuracy of CBCT in detecting VRFs of 50-300 µm width in vivo was significantly lower compared to the in vitro accuracy.

RESUMO

A new species of bacteria oxidizing ammonium with nitrite under anoxic conditions was isolated from the activated sludge of a semi-industrial bioreactor treating digested sludge of the Kuryanovo wastewater treatment plant (Moscow, Russia). Physiological, morphological, and molecular genetic characterization of the isolate was carried out. The cells were ovoid (-0.5 x 0.8 µm), with the intracellular membrane structures characteristic of anammox bacteria (anammoxosome and paryphoplasm); unlike other anammox bacteria, it possessed extensive intracellular membrane structures located in layers parallel to the cytoplasmic membrane, but never close to the anammoxosome. The cells formed aggregates 5-28 µm in diameter and readily attached to solid surfaces. The cells were morphologically labile, easily plasmolyzed, and lost their content. Doubling time was 28 days, µ(max) = 0.025 day(-1); optimal temperature and pH for growth were 20-45 degrees C and 8.0, respectively. Phylogenetic analysis of the 16S rRNA gene sequences suggested its classification as a new species of the candidate genus Jettenia (order Planctomycetales). The name Candidatus "Jettenia moscovienalis" sp. nov. was proposed for the new bacterium.

RESUMO

Photomultiplier tubes (PMTs) are widely used in physical experiments as well as in applied devices. PMTs are sensitive to magnetic field, so creation of effective magnetic shields for their protection is very important. In this paper, the results of measurements of shielding effectiveness of multilayer film magnetic shields on PMT-85 are presented. Shields were formed by alternating layers of a material with high magnetic permeability (Ni-Fe) and high electric conductivity-Cu. The maximum number of bilayers reached 45. It is shown that in weak magnetic fields up to 0.5 mT, the output signal amplitude from PMT-85 does not change for all used multilayer shields. In strong magnetic field of 2-4 mT, the output signal amplitude decrease with 10%-40% depending from the number of layers in the shield. The Pulse distribution of PMT-85 in magnetic field 0.2-4 mT slightly changed in the range 1.1%-1.3% for the case when the number of layers do not exceed 10 and practically did not change for a shield with 45 double layers.

RESUMO

OBJECTIVE: To specify psychopathological features and dynamics of depression developed at the stage of broad clinical presentations of anorexia nervosa (AN) in adolescent patients. MATERIALS AND METHODS: Authors studied 61 young women, aged from 14 to 18 years, mean age 16.6 ± .9 years. Depressive states manifested during the first year of AN in all patients. RESULTS: Two variants of depressive episodes were described. The first variant was characterized by asthenic/depressive, somatic and autonomic presentations, with the leading role of exogenous/organic and somatogenic factors. In the second variant, affective disorders were characterized by the anxiety affect concomitant with adynamia, dysphoria and melancholia that most probably may be caused by endogenous constitutional/biological factors. The follow-up data (0,5-6 years) indicated that most of the patients with the second variant of depressive episodes had recurrent depressive states. During the course of disease, eating disorders acquired an atypical character and gradually decreased while the affective pathology became dominating. CONCLUSION: Depressive symptomatology in AN patients of pubertal age corresponded to main mechanisms of development of the clinical picture and dynamics of adolescent depression as atypical depressive triad, predominance of anxiety and asthenic/adynamic affect, a trend towards protracted course and substantial frequency of dysmorphophobic symptoms and over-valued ideas.

RESUMO

The contribution of the major technologically important microbial groups (ammonium- and nitrite-oxidizing, phosphate-accumulating, foam-inducing, and anammox bacteria, as well as planctomycetes and methanogenic archaea) was characterized for the aeration tanks of the Moscow wastewater treatment facilities. FISH investigation revealed that aerobic sludges were eubacterial communities; the metabolically active archaea contributed insignificantly. Stage II nitrifying microorganisms and planctomycetes were significant constituents of the bacterial component of activated sludge, with Nitrobacter spp. being the dominant nitrifier. No metabolically active anammox bacteria were revealed in the sludge from aeration tanks. The sludge from the aeration tanks using different wastewater treatment technologies were found to differ in characteristics. Abundance of the nitrifying and phosphate-accumulating bacteria in the sludges generally correlated with microbial activity, in microcosms and with efficiency of nitrogen and phosphorus removal from wastewater. The highest microbial numbers and activity were found in the sludges of the tanks operating according to the technologies developed in the universities of Hanover and Cape Town. The activated sludge from the Novokur yanovo facilities, where abundant growth of filamentous bacteria resulted in foam formation, exhibited the lowest activity The group of foaming bacteria included Gordonia spp. and Acinetobacter spp., utilizing petroleum and motor oils, Sphaerotilus spp. utilizing unsaturated fatty acids, and Candidatus 'Microthrix parvicella'. Thus, the data on abundance and composition of metabolically active microorganisms obtained by FISH may be used for the technological control of wastewater treatment.

RESUMO

Objective of the study were: to specify the features of clinical presentations and dynamics of anorexia nervosa (AN) developed in adolescence during the current depression; to study affective disorders preceding the onset of AN and to analyze comorbidity of these two pathologies. We examined 21 female patients, aged from 15 to 17 years old (mean age 16.7 ± 0.8 years). It was shown that depressive symptoms developed 4-11 months (mean 7.9 ± 2.5 months) before the onset of AN. The development of AN was associated with depression and might be considered as one of mechanisms of formation of eating disorders that might be termed as affective mechanism. Thus, we can specify a variant of adolescent/juvenile depression, or depression with eating disorders. The variant represents a protracted depressive state with overvalued ideas to improve body image and to restrict calories which is combined with adynamic affect associated with anxiety, melancholy and dysphoria.

RESUMO

The lipid composition of the microbial community inhabiting activated sludge in a pilot reactor for the anaerobic oxidation of ammonium (anammox) at the Kur'yanovo Treatment Plant (Moscow) has been studied. The fatty acid composition is mostly based on common fatty acids C14-C18 (95%) with both normal and isomeric structures. The biomass of activated sludge was found to contain lipids with the so-called ladderane substances (ladder alcohols and fatty acids) that are common for anammox bacteria: C20-[3]-ladderane and C20-[5]-ladderane alcohols and C18-and C20-[3]-ladderane and C18- and C20-[5]-ladderane acids. In addition, the native extract contained both simple and compound ethers of the above-mentioned substances with residues of phosphocholine, phosphoethanolamine, and phosphoglycerine. The spectra of the electron impact and tandem mass spectrometry of certain substances have been obtained and published for the first time.

RESUMO

A new genus and species of bacteria capable of ammonium oxidation under anaerobic conditions in the presence of nitrite is described. The enrichment culture was obtained from the Moscow River silt by sequential cultivation in reactors with selective conditions for anaerobic ammonium oxidation. Bacterial cells were coccoid, -0.4 x 0.7 mm, with the intracellular membrane structures typical of bacteria capable of anaerobic ammonium oxidation (anammoxosome and paryphoplasm). The cells formed aggregates 5-25 µm in diameter (10 µm on average). They were readily adhered to solid surfaces. The cells were morphologically labile, they easily lost their content and changed their morphology during fixation for electron microscopy. The organism was capable of ammonium oxidation with nitrite. The semisaturation constants Ks for nitrite and ammonium were 0.38 mg N-NO2/L and 0.41 mg N-NH4/L, respectively. The maximal nitrite concentrations for growth were 90 and 75 mg N-NO2/L for single and continuous application, respectively. The doubling time was 32 days, µ(max) = 0.022 day(-1), the optimal temperature and pH were 20 degrees C and 7.8-8.3, respectively. According to the 16S rRNA gene sequencing, the bacterium was assigned to a new genus and species within the phylum Planctomycetes. The proposed name for the new bacterium is Candidatus Anammoximicrobium moscowii gen. nov., sp. nov. (a microorganisms carrying out anaerobia ammonium oxidation, isolated in the Moscow region).

RESUMO

We examined temporal dynamics of EEG phase-locked alpha oscillations during perception of illusory (Kanizsa square) and non-illusory images in boys with autism and age-matched typically developing boys. In typically developing boys the illusory contour (IC) as compared to the control stimulus provoked an increased alpha response at the parietal scalp areas. This IC effect demonstrated continuity within the time window of 133-267 ms after the stimulus onset. Although boys with autism did not display this effect at the group level, part of the sample showed an atypical two-stage pattern of illusory contour effect. The first early stage of IC effect (50-133 ms) was pronounced at the midline occipital electrode localized in the vicinity of the primary visual cortex. The localization and the early onset time suggest that this early IC effect is related to abnormally enhanced "low-level" locally-oriented processes of contour completion in autism. The second stage of IC effect (267-400 ms) was observed at the left parietal region only, and was delayed comparatively to that in healthy boys, suggesting the deficit of "intermediate" processes of perceptual grouping linked to the higher-order visual areas.

RESUMO

An aim of the study was to specify peculiarities of the clinical dynamics of organic psychic infantilism and psychopathological presentations of this disorder in adolescence. A significant prevalence of psychic infantilism caused by organic CNS damage in patients seeking psychiatric help in adolescence and a negative effect of this disorder on social adaptation in this period of life were found. Sixty-seven patients, aged 15-18 years, (ICD-10 diagnosis F06) with signs of psychic infantilism were studied. In these cases, psychic infantilism encompassed all aspects of mental activity and manifested itself as a constant personality trait that caused the infantile type of reaction to the most of life situations. The clinical heterogeneity of organic infantilism determined by the different degree of psychic immaturity in some areas of psychic activity and concomitant encephalopathy symptoms was noted. This allowed to single out 3 groups of patients: with domination of psychic infantilism in volition (28 patients), in emotional sphere (21 patients) and immaturity of cognitive functions (20 patients). The clinical heterogeneity of psychic infantilism likely reflects the character of CNS damage (localization and time of the damage) and specifics of related organic dysontogenesis.
