RESUMO

To determine the acute effects on respiratory function of children exposed to sulphur dioxide (SO2), we conducted two population-based longitudinal investigations near a major oil refinery. We enrolled 233 children, age 8⁻14, in Sarroch (Italy). The first study entailed five monthly spirometric visits (Panel 5). In a subgroup, children positive for history of respiratory symptoms were tested weekly (20 times) with spirometry and fractional exhaled nitric oxide (FeNO) measurement (Panel 20). Baseline questionnaires and daily diaries were recorded. SO2, NO2, PM10 and O3 were measured by monitoring stations. Multiple regression models were fitted. Using a multipollutant model, we found that a 10 µg/m³ SO2 increase at lag0⁻2 days determined a percent variation (PV) of -3.37 (90% confidence interval, CI: -5.39; -1.30) for forced expiratory volume after one second (FEV1) in Panel 5 and a PV = -3.51 (90% CI: -4.77; -2.23) in Panel 20. We found a strong dose-response relation: 1-h SO2 peaks >200 µg/m³ at lag2 days = FEV1 PV -2.49. For FeNO, we found a PV = 38.12 (90% CI: 12.88; 69.01) for each 10 µg/m³ SO2 increase at 8-h time lag and a strong dose-response relation. Exposure to SO2 is strongly associated with reduction of lung function and an increase in airway inflammation. This new evidence of harmful effects of SO2 peaks should induce regulatory intervention.

On March 2015, the Environmental Protection Agency of Tuscany Region (Central Italy) and the Laboratory of monitoring and environmental modelling published a Report on spatial representativeness of monitoring stations for Tuscan air quality, where they supported the decommissioning of modelling stations located in the Florentine Plain. The stations of Signa, Scandicci, and Firenze-Bassi, located in a further South area, were considered representative Believing that air quality of the Plain could be evaluated by these stations is a stretch. In this text the author show the inconsistency of the conclusion of the Report through correlation graphs comparing daily means of PM10 detected in the disposed stations and in the active ones, showing relevant differences between the reported values and the days when the limits are exceeded. The discrepancy is due to the fact that uncertainty of theoretical estimates is greater than the differences recorded by the stations considered as a reference and the areas they may represent. The area of the Plain has a population of 150,000 individuals and it is subject to a heavy environmental pression, which will change for the urban works planned for the coming years. The population's legitimate request for the analytical monitoring of air pollution could be met through the organization of participated monitoring based on the use of low-cost innovative tools.

On September 2014, a fire began within an oil refinery involving a storage tank containing several hundreds of thousands cubic meters of virgin naphtha. Mayors of neighbouring municipalities asked the Epidemiology and Prevention Society "Giulio A. Maccacaro" to carry out an environmental survey in order to evaluate what was the nature and how dangerous was suspended dust deposited by the fumes. In the following days, after fire had been extinguished we conducted a sample survey on the presence of polycyclic aromatic hydrocarbons (PAH) and metals in particulate deposited on the soil on a radius of five kilometres from the refinery and we engaged the exposed population. The Milazzo-Valle del Mela (Sicily Region, Southern Italy) high-risk area includes several industrial plants; among them, an oil refinery and a fuel powered energy plant. As reference area we selected the Sarroch municipality (Sardinia Region, Southern Italy), in the coast of the Tyrrhenian Sea, which is geographically comparable, where a large oil refinery is located and where an environmental campaign with measurement of PAH and metals in particulate matter was ongoing. Qualitatively, metal composition of particulate matter resulted similar in the Sarroch and Milazzo samples. Instead, a large excess of PAH was documented in the Milazzo samples as compared to the Sarroch ones. In conclusion, the results of the analysis of the samples of particulate matter deposited in the Milazzo area in the days immediately following the oil refinery fire showed a high quantity of PAH, carcinogenic substances which pose major hazard to population health. The greater fall-out was registered in the proximity of the burnt storage tank and the West neighbourhood, and at lesser extent in the Southern neighbourhood. As a consequence, there was a population exposure to carcinogenic substances which could have reached the food chain.

In 71 adults residing in Florence, Italy, enrolled in a prospective study, we investigated the correlation between individual levels of oxidative DNA damage detected by the Comet assay in circulating lymphocytes, and a specific ozone exposure score calculated in 10 different time-windows (0-5 to 0-90 days) before blood drawing, based on daily measurements provided by the local environmental monitoring system. Overall, statistically significant positive correlations between average ozone concentrations and DNA damage emerged in almost all time-windows considered; correlations were more evident among males, non-smokers, and traffic-exposed workers. Multivariate regression analyses taking into account selected individual characteristics, showed an independent effect on DNA damage of average ozone concentrations in the last 60-90 days before blood drawing. Local residents showed a divergent pattern with correlations restricted to shorter time-windows. Our results suggest that ozone concentrations at ground levels modulate oxidative DNA damage in circulating lymphocytes of residents of polluted areas.

OBJECTIVE: to produce environmental indicators suitable for an epidemiological surveillance in 10 Italian cities part of the EpiAir Project (2001-2005). METHODS: the environmental parameters that correlate to relevant health effects are the particles with diameters less than or equal to 10 micrometers (PM10), the nitrogen dioxide (NO2) and the ozone (O3). The necessary meteorological data are: temperature, relative humidity, barometric pressure and apparent temperature.We have identified some criteria to select monitoring stations and have taken standard methods of calculation to produce environmental indicators starting from the daily data available after closely evaluating the completeness of the existing data. Furthermore, we have checked the homogeneity of the selected data to ensure that it represents the population's exposure. RESULTS: close examination of descriptive statistics shows a critical situation of the considered pollutants. The analysis of the yearly state underlines for PM10 values higher than 40 microg/m3 in the area of Mestre-Venice and in Milan, Turin, Bologna e Taranto. For NO2, values are consistently above 40 microg/m3 in Milan, Turin, Bologna, Florence, Rome and Palermo. For ozone, the concentrations were stable, with the exception of Summer 2003 when we recorded, on average, an increase of 13% compared to the mean value estimated for the ten cities during the study period, especially in Mestre-Venice, Turin and Palermo. CONCLUSIONS: it is important to ensure the consistency of the methods and instruments in environmental monitoring. To evaluate health effects and perform interventions over the longterm, it is therefore fundamental that the data be homogenous, especially during the periodic reorganizations and rationalizations of air quality management. It is also necessary to include daily meteorological data that influence pollutant dispersion and population health status.

Air pollution and particulate matter in urban areas have been associated with increased mortality from cardiovascular and respiratory diseases and increased cancer risk. Carcinogenic effects of particulate matter have been related to the contents of specific compounds, such as polycyclic aromatic hydrocarbons. The latter may form bulky DNA adducts, that may be considered as candidate markers of cancer risk. We have recently shown that traffic-exposed workers and the general population in Florence have divergent levels of DNA adducts, possibly related to different levels of exposure to genotoxic agents from vehicle emissions. In the current study, in a series of 214 Florence City healthy adults enrolled in a prospective study in the period 1993-1998 (152 residents / 62 traffic-exposed workers), we investigated the correlation between individual levels of DNA bulky adducts and PM(10) exposure scores based on daily environmental measures provided by the local Environmental Protection Agency for the whole study period, by two types of urban monitoring stations (high- and low-traffic). We found that PM(10) cumulative scores from high-traffic stations over the last 1-2 weeks prior to blood drawing significantly correlated (r=0.58, p=0.02) with DNA adduct levels among non-smoking traffic-exposed workers (but not among residents with no occupational exposure to vehicle emissions). A multivariate regression analysis adjusted for possible confounders confirmed these findings. PM(10) scores from low-traffic stations did not show any correlation. These results show that DNA adduct levels in non-smoking workers reflect the average levels of exposure to PM(10) in high-traffic urban areas experienced over a time period of 1-2 weeks. Since DNA adduct levels have been found predictive of lung cancer risk, our findings provide clues relevant to the reduction of genotoxic damage and possibly cancer risk among traffic-exposed urban workers.

Prior to 2000 a network of conventional ozone (O3) analysers existed in the Province of Firenze (Tuscany, central Italy). Between 2000 and 2004 the network was extended to incorporate a newly designed bioindicator network of tobacco plants (Nicotiana tabacum Bel W3). The objective was to set-up an integrated monitoring system to obtain estimates of ground-level O3 concentrations over the whole study area (3513 km2) in order to fill data gaps and cover reporting requirements. The existing conventional monitors were purposefully located mainly in urban areas. A total of 45 biomonitoring sites were selected using a systematic design to cover the target area. Two to five additional biomonitoring sites were co-located with conventional O3 analysers for calibration purposes, and five more sites for independent validation of modelled O3 concentrations. Visible Leaf Injury Index (LII) on the tobacco plants was significantly correlated (P: 0.018/0.0014) with a series of O3 exposure variables (mean of weekly 1-hour maxima, M1; mean of 7-hour means, M7; 24-hour mean, M24; and weekly AOT40). LII was found to be a significant predictor of weekly means of the O3 exposure variables with a standard error of estimates between 13.6 and 24.3 microg m(-3) (absolute values). LII was mapped with an ad-hoc spatial model over the study area at a 22 km grid resolution, and mapped values were used to predict O3 concentrations by means of a first order linear model. Results showed that high estimates of O3 (up to 188 microg m(-3) as mean of weekly maxima, M1) occurred more frequently in hilly and mountainous areas, with a spatial pattern changing on an annual basis. Predicted O3 concentrations were not significantly different from the measured concentrations (P: 0.34), although marked differences were observed for individual sites and years. The study provided evidence that integration of monitoring networks using different methods can be a viable option to obtain estimates of O3 concentrations over large areas.

OBJECTIVE: Personal exposure to airborne benzene is influenced by various outdoor and indoor sources. The first aim of this study was to assess the benzene exposure of a sample of urban inhabitants living in an inner-city neighborhood of Florence, Italy, excluding exposure from active smoking. The secondary objective was to differentiate the personal exposures according to personal usage patterns of the vehicles. METHODS: A sample of 67 healthy non-smokers was monitored by passive samplers during two 4-weekday campaigns in winter and late spring. Simultaneously, benzene measurements were also taken for a subset of participants, inside and outside their houses. A 4-day time microenvironment activity diary was completed by each subject during each sampling period. Other relevant exposure data were collected by a questionnaire before the sampling. Additional data on urban ambient air benzene levels were also available from the public air quality network. The passive samplers, after automated thermal desorption, were analyzed by GC-FID. RESULTS: Benzene personal exposure levels averaged 6.9 (SD=2.1) and 2.3 (SD=0.7) microg/m(3) in winter and spring, respectively. Outdoor and indoor levels showed high correlation in winter and poor in spring. In winter the highest benzene personal exposure levels were for people traveling by more public transport, followed by users of only car and by users of only bus respectively. CONCLUSIONS: The time spent in-transport for work or leisure makes a major contribution to benzene exposure among Florentine non-smoking citizens. Indoor pollution and transportation means contribute significantly to individual exposure levels especially in winter season.

Ozone (O(3)), the major oxidant component in photochemical smog, mostly derives from photolysis of nitrogen dioxide. O(3) may have biologic effects directly and/or via free radicals reacting with other primary pollutants and has been reported to influence daily mortality and to increase lung cancer risk. Although DNA damage may be caused by ozone itself, only other photochemical reaction products (as oxidised polycyclic aromatic hydrocarbons) may form bulky DNA adducts, a reliable biomarker of genotoxic damage and cancer risk, showing a seasonal trend. In a large series consisting of 320 residents in the metropolitan area of Florence, Italy, enrolled in a prospective study for the period 1993-1998 (206 randomly sampled volunteers, 114 traffic-exposed workers), we investigated the correlation between individual levels of DNA bulky adducts and a cumulative O(3) exposure score. The average O(3) concentrations were calculated for different time windows (0-5 to 0-90 days) prior to blood drawing for each participant, based on daily measurements provided by the local monitoring system. Significant correlations between DNA adduct levels and O3 cumulative exposure scores in the last 2-8 weeks before enrollment emerged in never smokers. Correlations were highest in the subgroup of never smokers residing in the urban area and not occupationally exposed to vehicle traffic pollution, with peak values for average concentrations 4-6 weeks before enrollment (r = 0.34). Our current findings indicate that DNA adduct formation may be modulated by individual characteristics and by the cumulative exposure to environmental levels of ozone in the last 4-6 weeks, possibly through ozone-associated reactive pollutants.

RESUMO

Meta-analyses of short-term effect of air pollution use environmental exposure measurements defined as the concentration average between selected monitors. A simple quality index, Pearson's correlation coefficient between each pair of monitors, has been used in sensitivity analyses and meta-regression. To better characterize the degree of homogeneity in population exposure we propose to use Lin's concordance coefficient and the correlation coefficient between the difference and the average of each pair of values. Using simulated data and real data from the city of Rome (1998-2001) and Milan (1996-2002) we identify three conditions responsible of exposure misclassification when averaged concentrations are used in the analysis.
