RESUMO

A rare case of myocardial infarction developing in the course of congestive cardiomyopathy has been reported. The author describes diagnostic difficulties associated with the presence of combined heart diseases. The role of newer diagnostic methods (determination of CK-MB activity, Tl201 scintigraphy) has been pointed to in establishing the correct diagnosis. Marked worsening of the prognosis in congestive cardiomyopathy complicated by the infarction has been noted.

RESUMO

Authors discussed the most frequent CT signs of aortic dissection, such as: double- and multichannel aorta, intimal flap, displacement of calcifications into the aortic lumen, distortion of true aortic lumen, aortic dilatation and aortic wall thickening. Authors describe also the CT pictures of complications of aortic dissection: hematomas, rupture of aneurysm, false aneurysm, organs ischaemia and infection. CT is very useful modality in detection of aortic dissection and its complications.

RESUMO

A group of 23 patients with aortic dissection submitted for surgical operation was studied by CT (type I-10, type II-7, Type III-6 patients, according to DeBakey classification). In 9 (56.3%) patients with aortic dissection, type I and III, followed up for 3,5-10 years after surgical intervention the signs of aortic dissection were still visible, and 7 (43.7%) patients after 3-9 years developed thrombotic changes of various intensity in the false aortic lumen. In 2 cases a small external leakage and aneurysmal dilatation of the aorta around the implanted prosthesis were found. Five patients with type II dissection showed no signs of the dissection and other complications, whereas two patients developed aneurysmal dilatation of the aorta around the prosthesis.

RESUMO

A group of 22 patients with aortic dissection (type I, II and III acc. to De Bakey's classification) and surgically implanted aortic prosthetic grafts was studied, CT scanning is useful in visualizing aortic prosthetic grafts. Dacron vascular prostheses, initially clearly visible give weaker shadows with time, whereas homografts with time, contrast with increasing intensity, with the background on CT scans, due to calcifications.

The author lists the most frequent causes of incorrect diagnosis of aortic dissection by CT scanning. False-positive and false-negative diagnoses are rare owing to the availability of sophisticated scanners and growing experience of physicians.

RESUMO

Based upon literature and personal experience the authors review radiological techniques of evaluating colonic transit time in functional disorders of the large intestine. The distribution of given per os markers in the whole colon indicates prolonged transit time, which takes place in constipation of colonic inertia type. In the case of markers accumulation in the distal segment of the large intestine we encounter functional constipation of outlet obstruction type. The authors believe that colonic markers studies, easy to perform and non invasive for the patient should be carried out in order to obtain full information about defecation disorders.

RESUMO

Based upon personal experience and literature the authors briefly discuss the usefulness of arteriography, ultrasonography, CT and MR in the diagnosis of aortic dissecting aneurysms. In the diagnostic algorithm concerning aortic dissection type I and II acc. DeBakey, the initial diagnostic method should be echocardiography, followed by CT or MR or eventually aortography. In the case of type III aortic dissection the authors recommend transesophageal echocardiography and CT or MR or eventually aortography as the last examination.

RESUMO

The present study has been undertaken to analyse variability in the supra-acetabular sclerotic zone during a follow-up of 2-10 years after a total hip replacement. The material comprises 124 cases of Mc Kee Farrar and Weller total hip prosthesis. The criteria of clinical and radiological evaluation were based on literature data. The size of the supra-acetabular sclerotic zone was assessed planimetrically. A relationship between the size of the supra-acetabular sclerotic zone, the acetabular cup inclination and its fixation was found. In cases of excellent and good clinical results the size of the supra-acetabular zone had been decreasing. In cases of poor and fair results, in our study, the size of the supra-acetabular sclerotic zone increased.

RESUMO

We made an attempt to evaluate the width of the joint space of the normal hip joint in men aged 30-60 years. We analysed 164 roentgenograms and 68 CT scans of the pelvis measuring the width of the right and left hip joint in four segments of the frontal plane (roentgenograms) and in three segments of the horizontal plane (CT scans). In each segment we determined the mean width of the joint space and mutual relation between the measurements by an analysis of variance and correlation coefficient. It was shown that with advancing age of the patient the joint space increased bilaterally in the inferoposterior part (segment IV on the roentgenograms and segment III on the CT scans). The width of the joint space did not change significantly with age in the remaining segments of the hip joint.

RESUMO

The subtle morphologic structures of the normal and the cirrhotic liver preparations visualized by the use of computer tomography have been presented; moreover, the decreasing radiation has been evaluated in the consecutive layers of the organ. The material comprised 10 preparations of the normal liver and 15 preparations of the liver with histopathologic diagnosis of micronodular cirrhosis. The subtle morphologic structures corresponding to final vascular branches, liver plates and regeneration nodules has been visualized which up to now has been known only from pathomorphologic images. In the liver preparations have been detected the higher value of the radiation decrease equal to 12 +/- 5 Hounsfield units.

Computerized tomography (CT) of the sella turcica was performed in 106 women with hypothalamic-hypophyseal-ovarian hypofunction, aged from 15 to 50 years. In each patient we evaluated tomographically hypophyseal volume and subarachnoid space cistern invaginated into the sella turcica. We analysed statistically the incidence of amenorrhea, infertility, obesity and arterial hypertension in five groups of patients classified according to hypophyseal volume i.e. from below 100 to over 400 mm3. Is was found that hypophyseal volume of women in the so-called empty sella turcica that could correlate significantly with obesity and arterial hypertension was up to about 200 mm3, and in case of amenorrhea and infertility below 100 mm3. In case of empty sella turcica in women with amenorrhea mean hypophyseal volume was significantly lower (about 194 mm3) than in menstruating women (about 248 mm3). Invagination of cisterns of the cerebral basis into the sella turcica equal to or exceeding three time hypophyseal volume in about 82% was associated with its small size i.e. below 150 mm3. Basing on our analysis it seem justified to assume that hypophyseal volume up to 150 mm3 is a CT criterion of diagnosing completely empty sella turcica.

Based upon the results of microangiographic (MAR) and computed tomography (CT) studies of thin slices of the normal liver preparations the author reveals subtle morphological structures of the liver, namely, terminal branches of the portal vein and lobular sinusoids shown in MAR examinations, and hepatic laminae arranged in the so-called muralium simplex visualized in CT scanning.

RESUMO

Patterns of angiographic images of neoplastic metastases in the liver have been evaluated in 41 patients the cancer of various organs. It has been demonstrated that metastatic focuses showed vascularization independent from the histological tumor type. Metastases of the pancreatic adenocarcinoma were characterized by extremely poor vascularization. However, criteria enabling the determination of the primary neoplastic focus localization or of the cancer histological type based on the angiographic imaging of metastases in the liver have not been found.

RESUMO

The paper presents the most important features of the macro- and microangiographic picture of the liver primary carcinomata. Arterial ramifications of the liver were injected with a contrast medium - Micropaque, the liver was cut into 0.5 cm thick slices in the vertical and horizontal plane. The angiograms revealed radiological pictures of macroscopic arterial vascularization of the liver. In order to make a microangiographic estimation, the author cut the cubes about 20 X 20 X 5 mm in sizes out of 0.5 cm thick slices. The cubes were cut frozen in a microtome into sections of a thickness 1500 mu. Microangiographic pictures were taken and assessed in 8 X magnification. According to the author, the obtained results present a great cognitive importance and will help in better assessment of angiograms performed in patients with liver carcinomata.
