RESUMO

A clinical case of a man 66 y.o. who was diagnosed with hormone-inactive pituitary macroadenoma complicated by corneal erosion and partial atrophy of the optic nerve of the left eye due to exophthalmos. The increase in prolactin level was regarded due to a «stalk-effect¼. The patient underwent a transnasal pituitary adenomectomy with subsequent regression of symptoms. After 4 years, against the background of a new coronavirus infection, increasing general weakness, headaches, a crisis increase in blood pressure and tachycardia attacks appeared. Computed tomography (CT) accidentally revealed an adrenal incidentaloma, in laboratory tests - hypercortisolism, elevated ACTH levels, hypokalemia, hyperglycemia, increased levels of metanephrine and normetanephrine. The patient developed acute steroid psychosis, after which an adrenalectomy with a tumor was performed, a pheochromocytoma was histologically confirmed. After surgery, there was a regression of symptoms, the development of adrenal insufficiency with reduced levels of ACTH and cortisol. Upon further examination, a polynodose euthyroid goiter was established, the biopsy of the nodes - Hashimoto's thyroiditis (Bethesda II). Meanwhile, primary hyperparathyroidism was detected. According to ultrasound, scintigraphy with Ts99m-Technetril and CT revealed an increase of left parathyroid gland. A bilateral revision of the neck, removal of the right upper and left upper parathyroid adenomas were performed. In the postoperative period, the levels of calcium and parathyroid hormone were normalized. Given the presence of a combination of multiple tumors of the endocrine system (primary hyperparathyroidism, corticotropin-producing pheochromocytoma, hormone-inactive pituitary macroadenoma, polynodose euthyroid goiter), the MEN1 syndrome was clinically established. The study of 2 and 10 exons of the MEN1 gene revealed no mutations, which does not exclude the presence of a hereditary syndrome. The patient continues observation. In the available literature in Russian and English languages the case of ACTH pheochromocytoma as part of the MEN type 1 syndrome have not been found. Therefore, we consider the presented case to be the first one.

The purpose of the present work is to investigate the effect of dietary-supplemented artichoke (Cynara scolymus L.) on the mRNA expression of calbindin 1 (Calb1), osteopontin (Spp1), albumin (Alb) and CALB1 protein in the eggshell gland (ESG) of laying hens. A total of 80 ISA Brown hens (each at 40 weeks of age) were randomly divided into two groups: a control and a treated group. All poultry received 130 g/day of compound feed for laying hens but the treated hens' diet was also supplemented with 3g/kg of dried and milled artichoke (Cynara scolymus L.). The increase of the Ca content in blood of the treated hens was established. Significantly decrease of Spp1 mRNA transcripts was found in the eggshell gland of the treated hens, while the mRNA level of Alb was increased. The relative expression of Calb1 mRNA tended to increase in the treated group. The expression of calbindin protein in the cytoplasm of glandular cells of the shell gland was defined by immunohistochemical method. Very strong signals of calbindin were observed in the treated group. The supplementation of the laying hens' diet with dried artichoke (C. scolymus L.) led to a significant increase of Ca content in blood that was reflected in the changes of expression of the eggshell gland genes involved in the mineralization of eggshell.

The aim of this study was to assess the biological effect of the product Jodis Concentrate (JC) on the rabbit ovaries by evaluating the folliculogenesis and expression of oocyte-specific growth differentiation factor 9 (GDF9).The experiment was conducted with 30 female two month old New Zealand rabbits that were the F1 offspring born to mothers differently treated with Jodis concentrate. The control group (n=10), consisted of F1 offspring born to mothers without iodine treatment, and was not supple- mented with JC. The first experimental group (n=10), consisted of F1 offspring born to mothers treated with JC during pregnancy and the suckling period, and was supplemented with JC daily at a dose of 2 ml/L drinking. The second experimental group (n=10), consisted of F1 offspring born to mothers without iodine treatment, and was also supplemented daily with the same dose of JC - 2 ml/L drinking. All groups were fed with total mixed ration for growing rabbits. The trial lasted 48 days. The ovaries were weighed and prepared for histological examination. The GDF9 protein expression in the ovary was determined by immunohistochemical analysis. The addition of JC to the drinking water of female rabbits led to more active development of the ovarian follicles from primordial to tertiary stage in both experimental groups. More intensive GDF9 protein expression in the oocytes and cumulus cells of rabbits, supplemented with JC was observed.

The use of up-to-date diagnostic methods for the examination of 72 patients presenting with grade II-III arterial hypertension and high risk of cardiovascular complications made it possible to estimate the state of brachiocephalic arteries before and after combined antihypertensive therapy that continued during 14 days. The following variables were measured: blood flow rate, peripheral vascular resistance indices at the extra- and intracranial levels, metabolic indices of cerebrovascular responsiveness, and coefficient of variability as an indicator of the cerebrovascular reserve capacity under conditions of antihypertensive treatment. Three antihypertensive therapeutic regimens were employed, viz. lisinopril plus indapamide, bisoprolol plus indapamide, and amlodipine plus indapamide. It was shown that all the three regimen resulted in positive changes in the parameters of blood flow and peripheral vascular resistance. At the same time, the use of amlodipine plus indapamide ensured a more gradual reduction of peripheral vascular resistance and the most pronounced increase of cerebrovascular reserve capacity compared with the two remaining regimens. It is concluded that the parameters of peripheral vascular resistance and metabolic indices of cerebrovascular responsiveness are the most sensitive and informative end points for the pharmacotherapeutic treatment of cerebral hemodynamics in the patients presenting with grade II-III arterial hypertension and high risk of development of cardiovascular complications.

According to epidemiological researches data, in 75-80% of coronary heart disease (CHD) cases, the cause of sudden death is ventricular rhythm disturbances. Degree of coronary arteries lesion plays an important role in development of these disturbances. Study of Q-T interval dispersion is one of prospective methods of noninvasive assessment of risk of occurrence of dangerous for life ventricular arrhythmias. The authors studied indices of Q-T interval end its parts dispersion in patients with coronary atherosclerosis, and also estimated influence of angioplastic myocardial revasculization on electrical instability markers during 6 months after intracoronary intervention. It was proved that coronary atherosclerosis in patients with CHD contributes to increase of degree of myocardial depolarization and repolarization nonhomogeneity. Value of Q-T interval and its parts dispersion does not depend on number of affected coronary arteries. Revascularization of myocardium contributes to reliable decrease of Q-T interval and its parts dispersion. More marked its decrease is registered in 6 months after transcutaneous intracoronary intervention. Development of restenosis is accompanied by increase of indices of Q-T interval and its parts dispersion.

RESUMO

A social-psychological study was carried out on 96 pregnant women divided into three groups: I (control)-20-25 year-old, II-26-34 year-old and III (experimental)-over 35 year-old, the so called elder primiparous women. Giessen test was performed. It was established a positive social resonance in a correspondence with better contact possibilities and a good social potency in the control group. The third group has normal role interrelations in slightly increased intraphysical control and positive social resonance leading to some obedience and depression which explains the embarrassment for the establishment of stable relation and marriage. It is drown a conclusion that elder primiparous women are high risk group from psychological and social-psychological point of view.

Irradiation flavour in apple juice and concentrate which is mainly attributable, according to our previous studies, to tetrahydrofuran (THF) that may be quantitatively determined by means of the so-called mercury acetate method. This is based on the fact that the amount of a 4% solution of mercury acetate needed for removing the THF odour is proportional to the THF concentration. Using this method, the amounts of THF produced by various gamma radiation doses (varying from 0.64 to 2.55 Mrad) could be determined titrimetrically. Calculations showed that the THF concentration increases linearly with the radiation dose. Thus, the minimum radiation doses at which the irradiation flavour is first perceived (threshold of perception) may be calculated readily. The mercury acetate method is also a model case of the use of subjective olfactometry for the quantitative determination of flavour key substances in foods so far as they may be quantitatively determined by means of chemical reactions since the endpoint of titration is controlled by the sense of smell.
