RESUMO

Four hundred and ninety-five patients (390 and 105 grafted from unrelated and sibling (SIB) donors, respectively) and their donors were analyzed for the impact of interleukin-10 (IL-10) promoter genotype [rs18000896 (-1082 G/A), rs18000871 (-819 C/T) and rs18000872 (-592 C/A)] on the outcome of hematopoietic stem cell transplantation (HSCT). Patients having ACC haplotype were at a lower risk of acute graft versus host disease (aGvHD, grade > I) if transplanted from human leukocyte antigen (HLA) well-matched (10/10) unrelated donors (20/135 vs 39/117, P < 0.001, Pcorr = 0.002), which was not seen if patients were transplanted from either sibling (SIB) or poorly matched (<10/10) unrelated donors (MUD). In addition, GCC haplotype positive recipients of unrelated donor transplants tended to be more susceptible to aGvHD (68/199 vs 39/169, P = 0.019, Pcorr = 0.057). Multivariate logistic regression analysis in the MUD transplanted group showed that donor-recipient human leukocyte antigen (HLA) mismatch [odds ratio (OR) = 3.937, P = 0.001] and a lack of ACC haplotype in recipients (OR = 0.417, P = 0.013) played a significant role as independent risk factors of aGvHD grade > I. ACC carriers had higher proportions of FoxP3+ lymphocytes gated in CD4+ lymphocytes as compared with patients with other IL-10 haplotypes. It was seen at the time of hematological recovery (mean ± SEM: 3.80 ± 0.91% vs 2.06 ± 0.98%, P = 0.012) and 2 weeks later (5.32 ± 0.87% vs 2.50 ± 0.83%, P = 0.013); -592 C/A polymorphism was separately analyzed and it was found that AA homozygotes tended to have a higher incidence of aGvHD (8/15 vs 116/456, P = 0.034) and low proportions of FoxP3 CD4+ lymphocytes in blood (0.43 ± 0.22% vs 4.32 ± 0.71%, P = 0.051) measured 2 weeks after hematological recovery. Functional IL-10 polymorphism associated features influenced the risk of aGvHD with a positive effect of ACC on the pool of Treg in blood.

Donor and recipient are usually regarded as well matched for bone marrow transplantation when they are compatible in HLA class I and II antigens and in mixed lymphocyte culture (MLC). However, results of serological typing of class II antigens may be unreliable. Hence, polymerase chain reaction fingerprinting of HLA DRB (PCR FP) was introduced for the screening of related donors for 29 patients awaiting bone marrow transplantation. In addition, the sequence-specific oligonucleotide (SSO) typing of DQA alleles was performed. In 18 pairs the results of DNA analysis methods were compared with the results of MLC. 72% of pairs were HLA DQA compatible and 59% showed compatibility in PCR FP. MLC compatibility was found in 61%. A higher correlation of PCR FP and MLC results was observed.

RESUMO

Functional assays intended to reflect the haemolysis in vivo have been performed with peripheral monocytes, occasionally only with peritoneal macrophages or monocyte-like cell lines. Although the splenic macrophages presumably play the major role in immune red cell destruction, the functional assays have not been done with these cells because of their relative inaccessibility. In this study, the activity of peripheral monocytes and splenic macrophages was compared using the monocyte monolayer assay (MMA), and no substantial differences were found.

RESUMO

The experience in organizing the HLA class I typed donors registry and practical application for platelet transfusions in immunized patients is presented. It appears that the registry containing 3461 donors gives quite a high possibility of finding a compatible or partly compatible HLA donor for a patient with relatively high frequency antigens. A discrepancy is, however, observed between finding a donor in the registry and the transfusion of the matched platelets. Better collaboration between physicians and persons organizing the registry is required to improve transfusions of the matched platelets in immunized patients. Preliminary experience is also described concerning the usefulness of the same registry for finding the HLA class I compatible donors for bone marrow transplantation.

RESUMO

Two cases of neonatal alloimmune granulocytopenia due to fetomaternal incompatibility with granulocyte-specific antigen NA1 are presented. The diagnosis was based on the detection of anti-NA1 alloantibodies in maternal and neonates sera in leukoaglutination and immunofluorescence tests with granulocytes. Both children (twins) had severe granulocytopenia and unfortunately died.

RESUMO

The hemostatic effectiveness of platelet concentrates cryopreserved in a vapor phase of liquid nitrogen were evaluated after transfusion of 14 concentrates to 6 patients. Transfusion of AB0 and Rh-compatible platelet concentrates elevated the number of circulated platelets in donors by about 24 G/L. The mean corrected count increment (C.I.) after 001 and 24 hours post transfusion was 14.694 and 7.735/microliters/m2/10(11) respectively. Decrease of haemorrhagic diathesis, and correction of bleeding time in all cases have been observed.

RESUMO

A case of neonatal alloimmune thrombocytopenia due to fetomaternal incompatibility against the platelet specific antigen PlA2 (Zwb) is described. Anti-PlA2 antibodies were present in the maternal serum together with anti-HLA antibodies. Initial identification of the platelet-specific antibodies was achieved by using chloroquine-treated PlA1(+) and PlA1(-) platelets. Antibody specificity was later confirmed using a panel of platelets of known phenotype.

RESUMO

Results of the phagocytosis assay with monocytes and erythrocytes sensitized with subclass specific anti-Rh(D) antibodies (IgG1, IgG3, IgG1 + 3) are dependent on the selection of sera used for sensitization. Phagocytic abilities expressed as the percentage of active monocytes as well as the number of interacting red cells requires a certain degree of the sensitization of erythrocytes and is higher if serum contains IgG3 antibodies.
