RESUMO

Keratins form intermediate filaments of the cytoskeleton in keratinocytes and have roles in cell structure, signaling, intracellular transport, and cell death. Oral lichen planus (OLP) is an oral inflammatory disease with derangements in basal keratinocytes and disruption of the basal membrane. Here, we focused on epithelial expression of keratins 8, 18, and 19 because these proteins are known to modulate cell death. Biopsies were taken from buccal oral mucosa of persons with normal oral mucosa (n = 10) or atrophic OLP (n = 10). Cultured normal oral keratinocytes (n = 4) showed expression of mRNA and protein for keratins 8, 18, and 19. Immunohistochemistry showed consistent staining for keratins 8 and 18 in basal keratinocytes of normal oral mucosa. In OLP, staining for keratin (K)8 was mostly negative and staining for K18 was weak. Keratin 19 was expressed irregularly in most biopsies of normal oral mucosa and not at all in OLP. Several mononuclear leukocytes in the cellular infiltrate showed membrane staining for K8 and K18. Positive staining for K16 confirmed partial collapse of the basal cell layer in OLP. The basal cell niche in OLP therefore appeared to be partly populated with keratinocytes demonstrating a higher degree of differentiation (K8-  K18-  K19-  K16+ ); consequently, such areas may be more susceptible to the action of cell death factors released from the cell infiltrate as a result of lacking the protective, normal keratin present in the basal epithelial cell layer of normal oral mucosa.

OBJECTIVE: To describe and analyze recurrent pattern of KOTs according to time. STUDY DESIGN: This retrospective chart study evaluated histologically confirmed nonsyndromic keratocystic odontogenic tumors (KOT) treated at our institutions between 1975 and 2012. RESULTS: Mean age at time of diagnosis was 48 years, and male/female ratio was 1.8:1. The typical nonsyndromic KOT lesion was most often found in the posterior mandible, had radiographically smooth borders, and was associated with symptoms and clinical findings, perforated bone, with a thin capsule. Anterior jaw KOT were found more frequently in patients 50 years or older, and maxillary KOTs were more frequently infected. Treatment was enucleation (n = 70) or marsupialization with subsequent enucleation (n = 22). Observation time ranged from 0.5 months to 25 years (mean 53 months). Crude recurrence rate was 29%. Risk of recurrence increased until 7 years after treatment and then remained stable. Patients with impaired general health had a significantly increased risk of recurrence. Tumors in the posterior mandible that had bone perforation and a thin capsule and contained keratin showed increased recurrence rates. Although recurrence rates were similar for both treatment groups, the groups differed in size and tumor site; thus, the results of this study need to be confirmed in a prospective study. CONCLUSIONS: Time-dependent recurrence pattern of KOTs is described and analyzed. The following follow-up protocol is suggested for patients with impaired health or large lesions: clinical and radiographic examinations at 2, 4, and 7 years, followed by intervals of 3 to 4 years; for other patients: clinical and radiographic examination at 4, 6, and 8 years, followed by intervals of 3 to 4 years.

AIM: To investigate the indications for, and the outcome of auto-transplantation of teeth to the anterior maxillary region. MATERIAL AND METHODS: From 1978 to 1994, 41 teeth in 31 subjects were transplanted to anterior maxillary sites at the Department of Oral Surgery, Stavanger University Hospital, Norway. All transplantations were performed by one oral surgeon (B.G.). Relevant information was collected from patients' files, including radiographs of the tooth graft, the recipient site and follow-up radiographs. RESULTS: The mean observation period was 55.1 months (range 1-158 months). The age of the patients at the time of the auto-transplantation ranged from 10 to 30 years (mean 14.8 year). The most common indications for auto-transplantation were aplasia (41.5%), sequelae of trauma (36.6%) and impacted or ectopic teeth (17.1%). Eight teeth were judged to be failures; five had been extracted because of severe root resorptions and periodontal infection and three were judged as failures owing to severe ongoing root resorption but remained in the alveolus. CONCLUSION: Trauma is as common indication as aplasia for transplantation. From a biological point of view, dental auto-transplantation to the anterior maxillary region has a high success rate. Hence, auto-transplantation is an important treatment option for missing or lost maxillary anterior teeth where preservation of the alveolar bone is important during growth and development in adolescents. The major reason for failure was various types of root resorptions, some of which were detected late.

BACKGROUND: Recent reports have questioned if metal sensitivity may arise from exposure to titanium. The objective of this study was to histologically evaluate non-perforated mucosa covering submerged maxillary titanium implants with regard to induced tissue reactions. METHODS: Thirteen patients, 21 to 69 years of age, without previous implants were included. After initial examination, the bone crest areas destined for dental implant placement were exposed, and threaded external hex dental implants were inserted. Prior to wound closure, a full mucosal tissue slice was biopsied from the edge of the mucoperiosteal flap (baseline). The patients were monitored monthly for 6 months. At the abutment connection, biopsies were taken by a 6-mm punch, altogether yielding 26 specimens. Tissue reactions were analyzed by coded histometric analysis at four defined areas at increasing distance from the oral epithelium, including ratios of inflammatory cells (IC)/epithelial cells, IC/fibroblasts, and number of dense particles. RESULTS: The stained sections portrayed gingival tissue with intact oral epithelium and connective tissue with variable accumulation of IC. Experimental biopsies demonstrated mineralized areas and dense particles of different sizes. Analysis of variance revealed a higher IC/fibroblast ratio for level 3 at baseline compared to level 3 at 6 months (P<0.01). Furthermore, a significant decrease in IC/fibroblast ratio was observed between levels 2 and 3 and 2 and 4 at 6 months (P<0.001). The connective tissue level facing the cover screw contained the highest number of dense particles (P<0.01). CONCLUSIONS: Tissue sensitivity reactions to titanium implants were not disclosed. All 6-month biopsies contained dense particles that were most likely metals.
