RESUMO

A 37-year-old man developed persistent hemoptysis after sustaining a gunshot wound to the right shoulder and lung. A right upper lobectomy was performed, in which Aspergillus species microorganisms were identified within retained bullet fragments. The role of infected bullet fragments in the pathogenesis of hemoptysis in this patient is discussed.

RESUMO

Microcystic meningioma is an unusual variant, which has recently been proposed for inclusion in the WHO Classification of Central Nervous System Tumors. Its unique structure produces findings that may be confusing to radiologist and pathologist alike. A case is reported and the English language literature reviewed.

Hyoscine butylbromide (Buscopan) is an intravenously administered hypotonic agent that significantly reduces pressure in the lower esophageal sphincter and may therefore artificially induce gastroesophageal reflux during barium examination of the upper gastrointestinal tract. This study was performed to test this hypothesis. The presence or absence and severity of gastroesophageal reflux before and after intravenous injection of 20 mg Buscopan were evaluated in 112 consecutive patients undergoing biphasic upper gastrointestinal examination. Gastroesophageal reflux was seen in 49 (44%) of the patients. Reflux was evident both before and after the injection of Buscopan in 35 (31%) of the patients, before injection only in 10 (9%) and after injection only in 4 (4%). The magnitude of reflux after injection of Buscopan was reduced or the same in 108 of the patients (96%) and increased in 4 (4%). There was no significant difference in the overall occurrence (p = 0.41) or degree (p = 0.81) of gastroesophageal reflux before and after injection of Buscopan (chi 2 test). The authors conclude that the routine use of Buscopan is unlikely to spuriously increase the frequency or degree of gastroesophageal reflux observed on upper gastrointestinal barium studies.

The chest radiographs and computed tomographic (CT) scans obtained in eight patients with pathologically proved cases of thoracic actinomycosis were independently reviewed by two observers. All patients were alcoholic men aged 42-62 years. Air-space consolidation, seen on the radiograph and CT scan in each case, was present in the lower lobes in seven patients (88%) and upper lobes in three (38%). Pleural effusion was present in five (62%). Pleural thickening adjacent to the air-space consolidation was identified on the radiograph in four (50%) and CT scan in all eight. Cavitation or central areas of low attenuation not apparent on the radiograph were seen on the CT scan in five cases (62%). Hilar or mediastinal lymphadenopathy was identified on the radiograph in three cases (38%) and on the CT scan in six (75%). Chest-wall invasion occurred in only one case (12%). Thoracic actinomycosis is characterized on CT scans by air-space consolidation with adjacent pleural thickening. CT is superior to radiography in assessing the thoracic manifestations of actinomycosis.

To assess the accuracy of computed tomography (CT) in the diagnosis of emphysema, we performed CT on 32 patients prior to surgery for removal of suspected tumors. The CT scans were assessed visually for emphysema by 2 independent radiologists and 1 chest physician. Intraobserver and interobserver variation were assessed. Pathologic emphysema was graded on the resected lung specimens. These grades were correlated with the CT scores and with pulmonary function tests obtained prior to surgery. Significant correlation was found between the pathologic grade on resected lung specimens and the preoperative CT score of both the resected lobe (r greater than or equal to 0.57, p less than 0.001) and the whole lung (r greater than or equal to 0.63, p less than 0.001). Compared with pulmonary function tests, CT was a better predictor of emphysema and distinguished patients with moderate emphysema from patients with normal lungs. We conclude that CT is a useful adjunct in assessing the presence and severity of emphysema.
