RESUMO

Este trabajo explora las posibilidades de la Carpeta Personal de Salud e identifica la brecha entre el potencial de esta herramienta y las aplicaciones que ofrece a través de Internet. Se presenta la Carpeta Personal de Salud, un proyecto vinculado a la Historia Clínica Compartida de Cataluña, que ofrece un punto de acceso a la información sobre la propia salud que es seguro, personalizado y soportado por las tecnologías de la información y la comunicación. La experiencia, realizada íntegramente por la Consejería de Salud de la Generalitat de Cataluña, escogió una metodología mediante encuesta anónima. Los resultados han sido de gran relevancia para obtener información sobre la idoneidad de los datos publicados y las expectativas de una herramienta dirigida a la población en general (AU)

RESUMO

The aims of this study were to explore the possibilities of the Personal Health Folder and to identify the gap between the potential applications of this tool and what it offers through the Internet. The Personal Health Folder is presented, a project linked to the Shared Medical Record of Catalonia (Spain), which provides citizens with an access point to information about their health insurance, customized and supported by information and communication technologies. The project was carried out by the Ministry of Health of the Government of Catalonia (Generalitat de Catalunya) and data were gathered through an anonymous survey. The results were critical to obtain information on the suitability of the published data and on the expectations of a tool aimed at the general population.

RESUMO

La información es un recurso abundante en sanidad. Las tecnologías de la información y las comunicaciones (TIC) han incrementado las posibilidades de capturar y almacenar información, aunque no siempre ha supuesto la construcción de conocimiento o de un valorañadido sobre situaciones anteriores. Los usuarios y los usos de la información disponible son diversos: personas preocupadas por el cuidado de su salud, profesionales en su práctica asistencial diaria o en labores de investigación, planificadores y gestores en procesos relacionados con los centros o con el conjunto del sistema de salud o incluso con otros agentes ajenos al sector que requieren de información. Es importante tener presente que en todos los casos el uso final que los usuarios esperan de la información es que les resulte útil para la toma de decisiones de forma más eficiente y segura o que, en un ámbito más operativo, facilite o simplifique una tarea. De ese modo entendemos que las TIC deben tener como objetivo final contribuir a la sostenibilidad y calidad del sistema de salud. De esta manera, observar, generar, captar, codificar, almacenar, transferir, interpretar y analizar la información para aplicarla en beneficio de nuestro sistema son ya procesos habituales dentro de la cadena de valor de la información (AU)


The application of the TIC in health they have ahead big challenges that will be drawing the future of the sanitary attention: the capacity of the citizens to take responsibility of his health, the possibilities of the biomedical investigation, the changes in the way of being employed of the professionals and, especially, the improvement of the quality at the assistance (AU)

RESUMO

Under the law adopted by its Parliament, the Government of Catalonia has developed an electronic medical record system for its National Health System (NHS). The model is governed by the following principles: 1) The citizen as owner of the data: direct access to his data and right to exercise his opposition's privileges; 2) Generate confidence in the system: security and confidentiality strength; 3) Shared model of information management: publishing system and access to organized and structured information, keeping in mind that the NHS of Catalonia is formally an "Integrated system of healthcare public use" (catalan acronym: SISCAT) with a wide variety of legal structures within its healthcare institutions; 4) Use of communication standards and catalogs as a need for technological and functional integration. In summary: single system of medical records shared between different actors, using interoperability tools and whose development is according to the legislation applicable in Catalonia and within its healthcare system. The result has been the establishment of a set of components and relation rules among which we highlight the following: 1) Display of information that collects sociodemographic data of the citizen, documents or reports (radiology, laboratory, therapeutic procedures, hospital release, emergency room), diagnostic health, prescription and immunization plus a summary screen with the most recent and relevant references; 2) Set of tools helping the user and direct messaging between professionals to facilitate their cooperation; 3) Model designed for supranational connections which will allow adding later, with ad hoc rules, clinical data provided by the private health sector or the proper citizen.

RESUMO

The use of Information Communication and Technology (ICT) in medical image and telemedicine, can help improve the quality of life and well-being of citizens (patients and professionals) and overcome the challenges facing the health system, benefiting all parties involved in the health system (patients, professionals, administration, health providers, insurance and industry); ICT will not be the solution by itself, but certainly the solution will pass through ICT. It needs a strong political and clinical directing flexible strategies, aiming to contribute to the realization of care of higher quality and human care leadership.

RESUMO

Personal Health Records shared between patients and their health team can improve the capacity of the patients to become active partners in the care of their own health. PHR can offer the patients access to their medical information, services, such as resources programming, or communication between the members of the health team and the patient. They could also allow patients themselves to directly register certain data, be picked up from home monitoring devices, thus becoming a very useful tool in the management of chronic disease and for the prevention and promotion of health. The Government of Catalonia is developing the Personal Health Folder, a space where the Health Department will give the people of Catalonia secure and confidential access to their health information. The Personal Health Folder is within the framework of the Catalonian Shared Clinical History project, which is based on a index system of medical records shared between different personnel, using interoperability tools and is in accordance with the legislation applicable in Catalonia and within its healthcare system. Within the framework of this project and these principles, the Personal Health Folder has the aim to give people responsibility for their own health, allowing them to take part in the management of the preventive actions and care, and to improve the quality of care, and coordination between the different health areas and professionals.

RESUMO

Por mandato legal de su Parlamento, el Gobierno de Cataluña ha desarrollado un sistema de historia clínica electrónica para su Sistema Nacional de Salud (SNS). Los principios que rigen el modelo son: a) el ciudadano como titular de los datos: acceso a su información y ejercicio del derecho a oposición; b)generar confianza en el sistema: solidez en seguridad y confidencialidad; c) modelo común de gestión de la información: sistema de publicación y acceso a información organizada y estructurada, teniendo presente que nuestro SNS formalmente es un “Sistema sanitario integral de utilización pública de Cataluña (SISCAT)” con diversidad de formas jurídicas en sus instituciones prestadoras de servicios asistenciales, yd) utilización de estándares de comunicación y de catálogos como necesidad de integración tecnológica y funcional. En síntesis: sistema único de historia clínica compartida entre diversidad de agentes, con instrumentos de interoperabilidad y conforme a la legislación aplicable en Cataluña y a su sistema sanitario. El resultado ha sido el establecimiento de un conjunto de componentes y de reglas de relación entre los que destacamos: a) visor de información que reúne datos sociodemográficos del ciudadano, documentos o informes (radiología, laboratorio, procedimientos terapéuticos, alta hospitalaria, urgencias), datos de diagnósticos de salud, prescripción e inmunización más una pantalla de resumen con los datos y referencias más recientes o relevantes; b) conjunto de instrumentos de ayuda y mensajería directa entre profesionales para facilitar su cooperación, y c) modelo apto para su conexión supranacional e incorporar posteriormente, con reglas ad hoc, datos clínicos aportados por sanidad privada o el propio ciudadano (AU)

El uso de las tecnologías de la información y la comunicación (TIC), tanto en imagen médica como en telemedicina, puede ayudar a mejorar la calidad de vida y bienestar de los ciudadanos (pacientes y profesionales) y a superar los nuevos desafíos a los que se enfrenta el sistema de salud, beneficiando a todas las partes implicadas en el sistema sanitario (pacientes, profesionales de salud, administración, proveedores de salud, aseguradoras e industria); no serán por sí mismas la solución, pero sin lugar a dudas la solución pasará por las TIC. Es necesario un fuerte liderazgo político y clínico dirigiendo estrategias flexibles, con el objetivo de contribuir a la realización de una atención asistencial de mayor calidad y humana (AU)

RESUMO

La creación de Registros Personales de Salud compartida entre pacientes y su equipo de atención de salud puede mejorar la capacidad de los pacientes a convertirse en socios activos en el cuidado de su propia salud. Pueden ofrecer a los pacientes acceso a su información médica, servicios como la programación de citas, o comunicación entre los miembros del equipo de atención de salud y el paciente/ciudadano. También podrían permitir al propio paciente registrar determinados datos escribiéndolos directamente o recogidos desde aparatos de vigilancia y monitorización domésticos, constituyendo por tanto un instrumento de gran utilidad para la gestión de la enfermedad crónica y para la prevención y promoción de la salud. El Gobierno de Catalunya está desarrollando la Carpeta Personal de Salud, un espacio donde el Departament de Salut pondrá a disposición de cada uno de los ciudadanos de Catalunya datos referentes a su salud de forma segura y garantizando su confidencialidad. La Carpeta Personal de Salud se sitúa en el marco del proyecto de Historia Clínica Compartida de Cataluña, proyecto basado en un sistema índice de historia clínica compartida entre diversidad de agentes de salud, obtenido mediante instrumentos de interoperabilidad y conforme a la legislación aplicable en Catalunya y a su sistema sanitario. Bajo el marco de este proyecto y de estos principios, la Carpeta Personal de Salud tiene el objetivo de favorecer la responsabilización del ciudadano de su propia salud, facilitando su participación en la gestión de las actuaciones preventivas y de su cuidado, para mejorar la calidad de la asistencia, la coordinación entre las diferentes líneas asistenciales y entre los profesionales (AU)
