RESUMO

Immunohistochemistry has been used to evaluate the relationship between the rate of CD44-H antigen expression in breast invasive intracanalicuar and cervical epidermoïd carcinomas and the presence of node metastasis. A total of 36 cases of breast carcinoma and 20 cases of cervical carcinoma was examined. For the two types of carcinoma, the CD44-H molecule was highly expressed in cases with node metastasis; with 50% and 75% median value of CD44-H positive tumor cells respectively for the breast (p < 0.02) and for the uterine cervix (p < 0.05) compared to respectively 10% and 37.5% in the cases without node metastasis. The expression of this molecule increases with invasive potential of tumor cells and the use of this antigen for prognostic ends may be possible.

Chlamydia trachomatis infection is recognised as the most common asymptomatic sexually transmitted disease, and this may lead to severe complication including infertility. The purpose of this study was to evaluate the part that this pathology takes in the female hypofertility, using serologic, cell culture, and histopathologic tests. Some of the women had undergone biopsies during coelioscopic exam, the others during salpingectomy. Cervical specimens were carried from other women. They had as clinical signs: primary or secondary infertility, ectopic pregnancy, syndrome of synechie, hydrosalpinx, or pelvic pains. 128 of these women had undergone serologic exam, 57 a cell culture, and 47 an histopathologic test. The results showed that 26% had anti Chlamydia trachomatis antibodies and 46% from them with tubal problems confirmed, had anti Chlamydia trachomatis antibodies as well, only 7% had cell culture positive from cervix specimens, none from the biopsies, and 73% of them had inflammatory responses. All women with inflammatory responses had a serologic and/or cell culture positive tests. Our results allow us to conclude that this infection takes a good part in female hypofertility, there is a correlation between a previous Chlamydia trachomatis infection and a tubal histopathology. In front of the difficulties of isolation by cell culture the detection of the microorganism by molecular biology assays may resolve a lot of problems.

RESUMO

The authors report two cases of inflammatory pseudotumour of the lung, one discovered by chance in a 15 year old boy and the other following a hemoptysis in a 48 year old man. In both cases, the granuloma was mainly fibroblastic but also included lymphocytes and plasma cells, macrophages and mast cells. The histology, age of onset, the sex and the pulmonary localisations are different for plasma cell granulomas and fibrosing hemangiomas.

Report of a series of 26 cases of cylindrical epithelioma of the nasal cavities and accessory nasal sinuses. These neoplasms constitute a remarkably homogenous group the main characteristics of which are following: -- uniform histological pattern, similar to that of the epitheliomas of the colon, -- ethmoidal location in almost all cases, -- preferential occurrence in wood-workers, for instance in the Bas-Rhin. The prognosis depends on the extension of the epithelioma verified in 16 patients during the intervention. Recurrences caused death in 13 patients of this series. Ganglionic or distant metastases are rare. Six patients, i.e. 24 p. 100 had a favourable evolution of at least 3 years. Discussion about the therapeutics of the primary tumor and of its recurrences.

Benign voluntary muscle tumours are exceedingly rare. Only 9 fetal and 35 adult type rhabdomyomata have been reported in the literature, former usually in children, the latter in adults. Both types affect the musculature of the head and neck almost exclusively. In this ultrastructural study of a fetal rhabdomyoma in a girl aged eleven years, we have found that the tumour is composed of cells showing myoblastic or myotubular differentiation although the myofilaments are poorly arranged and the myofibrils irregularly distributed. Motor-end plates have not been observed. These observations, together with the light microscopic findings and a review of previously described cellular characteristics, suggest that fetal and adult rhabdomyoma are different stages in the life history of the same tumour. Whether the lesion should be considered to be a hamartoma or a neoplasm is uncertain.

RESUMO

There are histological and experimental evidence for bone marrow microcirculation to play a role in histogenesis of myelosclerosis. Human sternal bone marrow microcirculation was studied after death using ink injection in two cases of primary myelofibrosis, two cases of secondary meylofibrosis (Hodgkin's disease) an one case of osteomyelosclerosis. Control material consisted of 30 previously studied normal human sternums. In all cases interadipocytic sinusoidal network has completely disappeared. Medullary vascularization is anarchic with a vascular and hypervascular areas. The significance of these modifications is discussed.

The study of the vascularization of the sternum allows to individualise the vascular pedicles, the anterior and posterior peripheral sternal networks and the intrasternal vascularization. 1. The numerous vascular pedicles of the sternum all come from the internal mammary arteries. The postero-superior and postero-inferior sternal arteries of one simple intercostal space or of contiguous spaces are anastomosed by systems of latero-sternal, retroarticular and retro-sternal arches. From these arches come vessels which go directly to the sternum or which form the posterior peripheral sternal network. The anterior sternal branches of perforating arteries form arches or go directly to the anterior peripheral sternal network or to the sternum. The veins are less anastomosed than the arteries and therefore their arches are less distinct. 2. The anterior and posterior peripheral sternal networks are especially developed with respect to the lower half of the body of the sternum at the level of the 4th and 5th intercostal spaces. The posterior network is more developed than the anterior network. The arteries often have a spiral course, their anastomoses occur with their contralateral and homolateral homologues. The veins, which are of larger calibre than the arteries, design a denser and more irregular network. 3. The intra-skeletal vascularization, tributary of the peripheral networks, arches and branches of the internal mammary vessels, includes a network of the compact bone and a medullary network. The network of the compact bone, situated in one plane, is composed of polyhedral elongated meshes, according to the large axe of the sternum. The medullary network may be divided into a sub-cortical and a centro-medullary part. The sub-cortical part is composed of a peripheral zone and a more central zone. The totality of the intramedullary network includes a few fine arteries, an interadipocytary network, which is very developed and is variable in form. It also include sinuses of increasing order connected into the venous system, into the network of the compact bone, or into the large trans-cortical veins, which are drained by the peripheral sternal networks or by the collateral veins or the internal mammary system.
