INTRODUCTION: Osteoarthritis is a disabling pathology characterised by joint pain and stiffness. A prevalence of coxarthrosis of 7.4% is reported in our country. Total hip joint replacement is indicated in advanced stages, a procedure that is not free of complications, the most frequent being prosthetic dislocation, which can be prevented with dual mobility systems. The following study aims to determine the rate of complications and clinical outcomes in dual mobility systems in primary coxarthrosis. MATERIALS AND METHODS: A retrospective study included 120 cases in 114 patients diagnosed with grade III coxarthrosis, mean age was 62.43 years, with a mean follow-up of 4.5 years. Joint replacement was performed by Hardinge approach. All cases were assessed clinically using the Harris Hip Score (HHS) and radiologically to demonstrate mid-term results. RESULTS: The preoperative value on the HHS scale had a mean of 56.45, postoperative at one month 74.23; 6 months 85.40; 1 year 94.01 and at 5 years 94.84 points, representing a functional improvement of 17.78 postoperative month; 28.95 at 6 months postoperative; 37.56 at one year postoperative and 38.39 points at 5 years postoperative. A complication rate of 3.44%; 0.86% of complications were associated with the prosthetic components. CONCLUSION: The dual mobility system should be considered as a therapeutic option in primary hip joint replacement due to excellent functional results and low complication rates. EVIDENCE LEVEL: IV. INTRODUCTION: Mucopolysaccharidosis type III (MPS III), also known as Sanfilippo syndrome, is a lysosomal storage disease with progressive neurodegenerative features, predominantly affecting the central nervous system. Diagnosis is based on clinical features, with neurodevelopmental and neuropsychiatric alterations taking precedence, including over phenotype alterations. The disease is confirmed by biochemical analysis to identify the type of glycosaminoglycans present, enzyme assay and molecular genetic studies. CASE REPORTS: A clinical description was performed for eight patients diagnosed with MPS III in Colombia. Their initial symptoms were related to developmental delay and behavioural disorders presenting between 3 and 8 years of age, associated in all cases with coarse facial features, thick eyebrows, hepatomegaly and progressive hearing loss. One of the patients presented cardiac anomalies; two presented focal epilepsy; and one presented optic atrophy. They all presented neuroimaging alterations, with evidence of parenchymal volume loss, corpus callosum atrophy and cortical thinning; the diagnosis was performed by biochemical glycosaminoglycan chromatography studies, and all patients have a confirmatory genetic study. CONCLUSIONS: MPS III is a challenge for diagnosis, particularly in its early stages and in patients in which the course of the disease is attenuated. This is due to its variable course, non-specific early neuropsychiatric symptoms, and the absence of obvious somatic features compared to other types of MPS. After a definitive diagnosis has been made, interdisciplinary care must be provided for the patient and their family, and support given for the treatment of physical symptoms, ensuring the best possible care and quality of life for the patient and their family, as the condition is neurodegenerative.


TITLE: Historia natural de la mucopolisacaridosis III en una serie de pacientes colombianos.Introducción. La mucopolisacaridosis de tipo III (MPS III), o síndrome de Sanfilippo, es un trastorno de almacenamiento lisosómico con características neurodegenerativas progresivas, predominante del sistema nervioso central. Su diagnóstico se basa en el cuadro clínico, y priman alteraciones en el neurodesarrollo y neuropsiquiátricas, incluso antes de la presencia de alteraciones fenotípicas. El análisis bioquímico para identificar el tipo de glucosaminoglucanos presente, la determinación enzimática y el estudio de genética molecular confirman la enfermedad. Casos clínicos. Se realiza la descripción clínica de ocho pacientes con diagnóstico de MPS III en Colombia, con síntomas iniciales en relación con retraso del desarrollo y trastornos comportamentales evidenciados entre los 3 y 8 años, asociado a facies toscas, cejas pobladas, hepatomegalia y pérdida auditiva progresiva en todos los casos. Uno de los pacientes presentó anomalías cardíacas; dos de ellos, epilepsia focal; y en uno se evidenció atrofia óptica. Todos presentaron alteraciones en las neuroimágenes con evidencia de pérdida del volumen parenquimatoso, atrofia del cuerpo calloso y adelgazamiento cortical; el diagnostico se realizó a través de estudios bioquímicos de cromatografía de glucosaminoglucanos y todos cuentan con un estudio genético confirmatorio. Conclusiones. La MPS III es un desafío diagnóstico, particularmente en pacientes con un curso atenuado de la enfermedad, debido al curso variable, síntomas neuropsiquiátricos tempranos inespecíficos y falta de características somáticas evidentes en comparación con otros tipos de MPS. Cuando se tiene el diagnóstico definitivo, es fundamental brindar atención interdisciplinaria para el paciente y la familia, y apoyar el tratamiento de los síntomas físicos, garantizando ofrecer el mejor cuidado posible y la mejor calidad de vida para el paciente y su familia, al tratarse de una condición neurodegenerativa.

En este capítulo describimos los tipos más frecuentes de metástasis raquídeas, sus localizaciones más habituales dentro de la columna vertebral, así como su comportamiento clínico. Analizamos también los cuadros neurológicos más comúnmente asociados a las metástasis de columna: compresión radicular, compresión medular, cauda equina y afectación medular.(AU)

El presente trabajo tiene como propósito presentar 23 casos operados en distintas prácticas privadas, donde se utilizaron adhesivos de fibrina para: cirugía ortognática bimaxilar, expansión palatina rápida asistida quirúrgicamente (SARPE), y reconstrucciones de rebordes atróficos maxilares y mandibulares empleando injertos e implantes. El uso del adhesivo de fibrina del sistema Vivostat® permitió no solo sellar los abordajes quirúrgicos, sino también fue un excelente complemento en estas cirugías para la cobertura de la mucosa de fosas nasales y mucosa antral que se dañan durante las osteotomías y accesos quirúrgicos. Junto con adquirir ventajas, como variabilidad de factores de crecimiento y protección de los injertos subyacentes en las regiones operadas, se obtienen asimismo una posible disminución de la incidencia en los sangramientos postoperatorios, principalmente epistaxis; con lo cual se brinda al paciente una mejor recuperación en el postoperatorio y evita la posibilidad de dehiscencias y, o aperturas de heridas operatorias con exposición de injertos y de elementos de osteosíntesis subyacentes.


The purpose of this paper is to present 23 operative cases in different private practices, where fibrin adhesives were used for: bimaxillary orthognathic surgery, surgically assisted rapid palatal expansion (SARPE), and maxillary and mandibular atrophic ridge reconstructions using grafts and implants. The use of the fibrin adhesive of the Vivostat® system allowed not only sealing the surgical approaches, but was also an excellent complement in these surgeries for covering the mucosa of the nasal passages and antral mucosa that are damaged during osteotomies and surgical accesses. Along with acquiring advantages, as well as growth factors and protection of the underlying grafts in the operated regions, a possible decrease in the incidence of postoperative bleeding was obtained, mainly epistaxis; with which the patient is given a better recovery in the postoperative period and avoids the possibility of dehiscences and/or openings of surgical wounds with exposure of grafts and underlying osteosynthesis elements.

La poliuria es una condición clínica frecuente caracterizada por un volumen de orina inapropiadamente alto para los niveles de presión arterial y sodio plasmático del paciente (volumen de orina >3L/24h). Desde el punto de vista fisiopatológico se clasifica en 2 tipos: debido a una mayor excreción de solutos (osmolaridad urinaria >300mOsm/L) o debido a una incapacidad de aumentar la concentración de solutos (osmolaridad urinaria <150mOsm/L). En ocasiones pueden coexistir ambos mecanismos (osmolaridad urinaria 150-300mOsm/L). La poliuria supone un reto diagnóstico y su tratamiento correcto exige una evaluación de la historia clínica, la determinación de la osmolaridad urinaria, la estimación del aclaramiento de agua libre, el uso de pruebas de deprivación hídrica en la poliuria acuosa y la medición de electrólitos en sangre y orina en el caso de la poliuria osmótica (AU)

Polyuria is a common clinical condition characterized by a urine output that is inappropriately high (more than 3 L in 24 h) for the patient's blood pressure and plasma sodium levels. From a pathophysiological point of view, it is classified into two types: polyuria due to a greater excretion of solutes (urine osmolality >300 mOsm/L) or due to an inability to increase solute concentration (urine osmolality <150 mOsm/L). Resumen Introducción: PIPAC (Pressurized Intraperitoneal Aerosol Chemotherapy) Es una técnica que, vía laparoscopía, permite administrar quimioterapia en aerosol intraperitoneal, para el tratamiento de la carcinomatosis, ya sea para disminuir masa tumoral y aumentar la resecabilidad, o como paliación sintomática. Objetivo: Presentar los dos primeros casos de PIPAC en Chile, las consideraciones técnicas y revisión de la literatura. Pacientes y Método: Se describe la forma en que un programa PIPAC fue implementado en Clínica Las Condes. Se describe la técnica. Este procedimiento se realizó en dos pacientes, ambas portadoras de carcinomatosis con ascitis refractaria. Resultados: No hubo complicaciones. Alta a las 24 h. Ambas pacientes presentaron disminución de la ascitis, la que se ha mantenido a los seis meses de seguimiento. Discusión: PIPAC es una técnica emergente, que ha demostrado ser segura, con escasas complicaciones, cuya indicación incluye carcinomatosis por cáncer de colon y ovario y que se está extendiendo a páncreas, vía biliar y estómago. Su rol exacto está por definirse. Conclusiones: PIPAC es una técnica factible de realizar en nuestro país; sus resultados preliminares son alentadores y exentos de complicaciones.


Introduction: PIPAC (Pressurized Intraperitoneal Aerosol Chemotherapy is a technique that allows laparoscopic administration of aerosol chemotherapy in the peritoneum. This procedure is utilized for treatment of carcinomatosis, for debulk abdominal tumors, increasing resectability, or for palliation of abdominal symptoms. Aim: To present the first two cases of PIPAC performed in Chile, technical considerations and review of the literature. Patients and Method: The way this program was started at Clínica Las Condes is presented. The technique is described. This procedure was performed in two females, both with refractory ascites due to carcinomatosis. Results: The procedure was uneventfully and patients were discharged 24 hours later. Both patients showed important reduction of ascites, maintained at 6 months of followup. Discussion: PIPAC is a safe emerging technique, with low complication rate. It is indicated in carcinomatosis of colonic and ovarían origin and in selected cases of pancreatic, bile duct and gastric carcinomatosis. More prospective, randomized studies should be done to stablish its exact role. Conclusions: PIPAC is a feasible technique to perform in our country. Preliminary results are encouraging and no complications were observed.

RESUMEN La asociación y coexistencia de psoriasis en placas y penfigoide ampollar (Psoriasis-PA) es una variante clínica poco frecuente. Predomina en el sexo masculino, después de los 60 años de edad. En general, los pacientes ya cuentan con el antecedente de psoriasis y posteriormente se agrega la enfermedad ampollar, con un intervalo promedio de 20 años. Aunque la etiopatogenia aún no se encuentra del todo establecida, se identifican factores desencadenantes tales comoPUVA, fármacos e infecciones. Las opciones terapéuticas consisten en esquemas de monoterapia obiterapia con asociación de corticoesteroides orales e inmunosupresores como el metotrexato (MTX) con evolución favorable en la mayoría de los casos. Se presenta el caso de un paciente masculino, de 52 años de edad con psoriasis vulgar de tres años de evolución y lareciente aparición de penfigoide ampollar.


ABSTRACT The association and coexistence of plaque psoriasis and bullous pemphigoid is a rare clinical variant. It predominates in the male sex, after 60 years old. In general, patients already have a history of psoriasis and then bullous disease is added, with an average interval of 20 years. Although the pathogenesis is not yet fully established, it identifies triggers such as PUVA, drugs, and infections. Therapeutic options consist of monotherapy or combination regimens of systemic corticosteroids and other immunosuppressants such as methotrexate, with favorable evolution in most cases. We present the case of a 52-year-old male patient with 3-year history of vulgar psoriasis and recent appearance of bullous pemphigoid.

Pancreatic ductal adenocarcinoma (PDAC) is one of the most intractable and devastating malignant tumors. Epigenetic modifications such as DNA methylation and histone modification regulate tumor initiation and progression. However, the contribution of histone variants in PDAC is unknown. Here, we demonstrated that the histone variant H2A.Z is highly expressed in PDAC cell lines and PDAC patients and that its overexpression correlates with poor prognosis. Moreover, all three H2A.Z isoforms (H2A.Z.1, H2A.Z.2.1, and H2A.Z.2.2) are highly expressed in PDAC cell lines and PDAC patients. Knockdown of these H2A.Z isoforms in PDAC cell lines induces a senescent phenotype, cell cycle arrest in phase G2/M, increased expression of cyclin-dependent kinase inhibitor CDKN2A/p16, SA-ß-galactosidase activity and interleukin 8 production. Transcriptome analysis of H2A.Z-depleted PDAC cells showed altered gene expression in fatty acid biosynthesis pathways and those that regulate cell cycle and DNA damage repair. Importantly, depletion of H2A.Z isoforms reduces the tumor size in a mouse xenograft model in vivo and sensitizes PDAC cells to gemcitabine. Overexpression of H2A.Z.1 and H2A.Z.2.1 more than H2A.Z.2.2 partially restores the oncogenic phenotype. Therefore, our data suggest that overexpression of H2A.Z isoforms enables cells to overcome the oncoprotective barrier associated with senescence, favoring PDAC tumor grow and chemoresistance. These results make H2A.Z a potential candidate as a diagnostic biomarker and therapeutic target for PDAC.

In this work we provide a detailed study on grafting reactions of various dialkylphosphonate-based ILs. Special attention has been devoted to a comprehensive investigation on how the nature of the anion and the organic spacer composition (hydrophilic or hydrophobic groups) could impact the grafting densities and bonding modes of phosphonate-based ILs anchored to γ-alumina (γ-Al2O3) powders. For the first time, the bonding of phosphonate-based ILs with only surface hexacoordinated aluminum nuclei was established using both solid-state 31P-27Al D-HMQC and 31P NMR experiments. It has been demonstrated that the grafting of dialkylphosphonate-based ILs is competing with a hydrolysis and/or precipitation process which could be attractively hindered by changing the anion nature: bis(trifluoromethane)sulfonylimide anion instead of bromide. In additon, independently of the chosen spacer, similar reaction conditions led to equivalent grafting densities with different bonding mode configurations. The CO2 physisorption analysis on both pure ILs and grafted ILs on alumina powders confirmed that the initial sorption properties of ILs do not change upon grafting, thus confirming the attractive potential of as-grafted ILs for the preparation of hybrid materials in a form of selective adsorbers or membranes for CO2 separation applications.

The role of type I IFNs in the pathogenesis and control of mycobacterial infection is still controversial. It has been reported that type I IFNs exacerbated M. tuberculosis infection through hampering Th1 type cellular immune response. However, under certain conditions they can act as natural immune adjuvants for commercial vaccines. At this point, we have reported recently that successive IFN-alpha boosting of Mycobacterium bovis Bacillus Calmette Güerin (BCG) vaccinated mice protected adult mice from intradermal M. lepraemurium infection and a difference in iNOS was observed. In the present work, we have found that intramuscular IFN-α boosting of Mycobacterium bovis Bacillus Calmette Güerin (BCG) vaccine, either in vitro (human cell line or macrophages derived from PBMC) or in vivo (aerosol mouse model of MTb infection), promoted mostly the development of specific anti-antimycobacterial Th1 type cytokines (IFN-γ; IL-12, TNF-alpha, and IL-17; IL1ß) while bacterial load reduction (0.9 logs versus PBS or BCG vaccine) was observed. These findings indicate that, under the experimental settings reported here, interferon alpha can drive or affect the TH cellular immune response in favour of BCG-inducing immunity against M. tuberculosis infection.

Se realizó un estudio cualitativo con el objetivo de describir las percepciones de profesionales en enfermería sobre la violencia\r\nde la pareja íntima contra adolescentes gestantes atendidas en el Hospital de Usaquén (Bogotá, Colombia). Este parte del papel\r\nque desempeña enfermería en la detección, prevención y atención de las gestantes víctimas de violencia de sus parejas y surge\r\nde una investigación anterior en ese centro de atención, que determinó que las gestantes más jóvenes son las más vulnerables.\r\nEl estudio se desarrolló mediante dos grupos focales, con participación de 14 enfermeras que laboran en el hospital. El análisis, de\r\ntipo fenomenológico, se realizó con apoyo del software Atlas ti 7. Entre los principales resultados se establece que, según la percepción de las enfermeras, la forma más frecuente de violencia de la pareja íntima contra la adolescente gestante es la psicológica, seguida del abandono y la violencia económica. En cuanto a las necesidades de detección resulta importante ampliar los tiempos de consulta, generar un ambiente de confianza, implementar protocolos de detección inicial y realizar tamizajes. En lo relativo a la prevención, se recomienda iniciar desde la infancia la formación en género y derechos, además de trabajar conjuntamente con la escuela. Para la atención, se considera imperioso el trabajo interdisciplinario. Como barreras se mencionan la negación de las gestantes a denunciar y la amenaza de sus parejas al equipo de enfermería ante la notificación, así como la falta de capacitación del personal de salud.


A qualitative study was conducted with the objective of describing the perceptions of nursing professionals regarding intimate partner violence (IPV) against pregnant adolescents attended at the Hospital de Usaquén (Bogotá, Colombia). This study is based on the role that nurses play in the detection, prevention and care of pregnant victims of violence from their partners and emerges from an earlier study in this center of attention that established that the youngest pregnant women are the most vulnerable. The study was developed through two focus groups, with the participation of 14 nurses that work at the hospital. A phenomenological analysis was carried out with the support of the Atlas ti 7 software. Among the main results, it is established that according to the perception of nurses, the most frequent form of violence form the intimate partner towards the pregnant adolescent is psychological, followed by abandonment and economic violence. Concerning the need for detection, it is considered important to extend consultation times, generate an atmosphere of trust, implement initial detection protocols, and perform screenings. In regard to prevention, it is recommended to initiate training in gender and rights from childhood, in addition to working together with the school. For the medical attention, interdisciplinary work is considered imperative. As barriers, mentioned were the refusal of the pregnant women to denounce, and the threats of their partners to the nursing staff after notification, as well as the lack of training of the medical personnel.


Um estudo qualitativo foi realizado com o objetivo de descrever a percepção de um grupo de enfermeiras sobre a violência por parceiro íntimo na gestação que afeta adolescentes gestantes atendidas no Hospital de Usaquén (Bogotá, Colômbia). Este estudo parte da importância da enfermagem na detecção, prevenção e cuidados de mulheres grávidas vítimas de violência por seus parceiros e é baseado em um estudo anterior em que o centro de atenção estabeleceu que as mulheres grávidas mais jovens são as mais vulneráveis. O estudo foi desenvolvido por meio de dois grupos de foco, com a participação de 14 enfermeiras vinculadas ao hospital. A análise, tipo fenomenológica, foi realizada com o apoio do software Atlas ti 7. Entre os principais resultados, define-se que, de acordo com a percepção das enfermeiras, a forma mais frequente de violência por parceiro íntimo é a psicológica, seguida do abandono e da violência econômica. No que diz respeito às necessidades de detecção é considerada importante para expandir o tempo de consulta, gerar um ambiente de confiança, implementar protocolos de detecção inicial e realizar triagens; no que diz respeito à prevenção, recomenda-se iniciar desde a infância a formação no domínio da igualdade entre os gêneros e direitos humanos, além de trabalhar em conjunto com a escola; já a atenção, é imperativa para o trabalho interdisciplinar. Como barreiras, foram mencionadas a negação por parte das gestantes em denunciar e a ameaça de seus parceiros à equipe de enfermagem, assim como a falta de capacitação dos profissionais de saúde.

Si bien es cierto que la lepra es la menos contagiosa de las enfermedades transmisibles, no deja de ser una enfermedad infecto-contagiosa y como tal debe manejarse. Se debe subrayar que el daño del nervio periférico se presenta en todos los individuos infectados con el Mycobacterium leprae y que en términos cuantitativos se ha reportado que más del 60% de los casos hay daño del nervio periférico al momento del diagnóstico


Although leprosy is the least contagious of the communicable diseases, it is nevertheless an infectious disease and must be managed as such. It should be stressed that peripheral nerve damage is present in all individuals infected with Mycobacterium leprae and that in quantitative terms it has been reported that more than 60% of cases have peripheral nerve damage at the time of diagnosis.

Calophyllum brasiliense (Calophyllaceae) is a tropical rain forest tree, mainly distributed in South and Central America. It is an important source of bioactive natural products like, for instance soulatrolide, and mammea type coumarins. Soulatrolide is a tetracyclic dipyranocoumarins and a potent inhibitor of HIV-1 reverse transcriptase and Mycobacterium tuberculosis. Mammea A/BA and A/BB coumarins, pure or as a mixture, are highly active against several leukemia cell lines, Trypanosoma cruzi and Leishmania amazonensis. In the present work, a toxicogenomic analysis of Soulatrolide and Mammea A/BA + A/BB (3:1) mixture was performed in order to validate the toxicological potential of this type of compounds. Soulatrolide or mixture of mammea A/BA + A/BB (3:1) was administered orally to male mice (CD-1) at dose of 100 mg/kg/daily, for 1 week. After this time, mice were sacrificed, and RNA extracted from the liver of treated animals. Transcriptomic analysis was performed using Affymetrix Mouse Gene 1.0 ST Array. Robust microarray analysis (RMA) and two way ANOVA test revealed for mammea mixture treatment 46 genes upregulated and 72 downregulated genes; meanwhile, for soulatrolide 665 were upregulated and 1077 downregulated genes. Enrichment analysis for such genes revealed that in both type of treatments genetic expression were mainly involved in drug metabolism. Overall results indicate a safety profile. The microarray data complies with MIAME guidelines and are deposited in GEO under accession number GSE72755.
