BACKGROUND: The purpose of this study was to identify the species of artiodactyl host related to the fecal matter collected in a forest area in Rio de Janeiro state and carry out a parasitological investigation. METHODS: Artiodactyl feces were collected between 2020 and 2021. The fecal samples were examined to identify the host through macroscopic and molecular analysis. The remaining samples were subjected to a fecal parasite analysis using microscopic techniques, and feces containing cysts of the phylum Ciliophora underwent a molecular analysis. RESULTS: Of the 101 samples collected, 71.3% were found in Pavão Valley, the most anthropized area of the park. In the molecular analysis, 79 samples were identified as belonging to Sus scrofa and 2 as Mazama gouazoubira. The most frequently detected forms were cysts of the phylum Ciliophora (39.6%), followed by eggs of Ascaris spp. (11.8%) and Metastrongylus spp. (5.9%). Nucleotide sequences of Balantioides coli were characterized in 26 samples, and in 13 samples variants of type B0 and in 11 type A0 were identified. CONCLUSIONS: It should be noted that this is the first study in the Americas that has identified B. coli in free-living S. scrofa feces, citing this bioinvader as one of the reservoirs of this parasite.

The aim of the present study was to use an integrated approach for the identification of risk areas for Schistosoma mansoni transmission in an area of low endemicity in Minas Gerais, Brazil. For that, areas of distribution of Biomphalaria glabrata were identified and were related to environmental variables and communities with reported schistosomiasis cases, in order to determine the risk of infection by spatial analyses with predictive models. The research was carried out in the municipality of Alvorada de Minas, with data obtained between the years 2017 and 2019 inclusive. The Google Earth Engine was used to obtain geo-climatic variables (temperature, precipitation, vegetation index and digital elevation model), R software to determine Pearson's correlation and MaxEnt software to obtain an ecological model. ArcGis Software was used to create maps with data spatialization and risk maps, using buffer models (diameters: 500, 1,000 and 1,500 m) and CoKriging. Throughout the municipality, 46 collection points were evaluated. Of these, 14 presented snails of the genus Biomphalaria. Molecular analyses identified the presence of different species of Biomphalaria, including B. glabrata. None of the snails eliminated S. mansoni cercariae. The distribution of B. glabrata was more abundant in areas of natural vegetation (forest and cerrado) and, for spatial analysis (Buffer), the main risk areas were identified especially in the main urban area and toward the northern and eastern extensions of the municipality. The distribution of snails correlated with temperature and precipitation, with the latter being the main variable for the ecological model. In addition, the integration of data from malacological surveys, environmental characterization, fecal contamination, and data from communities with confirmed human cases, revealed areas of potential risk for infection in the northern and eastern regions of the municipality. In the present study, information was integrated on epidemiological aspects, transmission and risk areas for schistosomiasis in a small, rural municipality with low endemicity. Such integrated methods have been proposed as important tools for the creation of schistosomiasis transmission risk maps, serve as an example for other communities and can be used for control actions by local health authorities, e.g., indicate priority sectors for sanitation measures.

Rabies is an important zoonosis to public health associated with lethal encephalitis and economic losses. Analysis of its spatial distribution is a meaningful tool in understanding its dispersion, which may contribute to the control and prophylaxis of the disease. This study analyzed the spatial-temporal distribution of rabies outbreaks in livestock in Pará state, Brazil, from 2004 to 2013. We used records of neurological syndromes obtained from the state's livestock authority (Adepará). The analysis recorded 711 neurological syndromes reports in livestock, of which 32.8% were positive for rabies. In 8% of the neurological syndromes (n=57) was not possible to perform the analysis because of bad-packaging conditions of the samples sent. Outbreaks involved at least 1,179 animals and cattle were the most affected animal species (76.8%). The numbers of reported neurological syndromes and of rabies outbreak shad strong positive correlation and exhibited decreasing linear trend. Spatially, most outbreaks occurred in two mesoregions in Pará (Northeast and Southeast). One of the justifications for this spatial distribution may be related with the distribution of the animals in the state, since these mesoregions are the largest cattle producers in Pará and have most of their territory deforested for pasture implementation.(AU)


A raiva é uma zoonose importante para a saúde pública associada à encefalite letal e às perdas econômicas. A análise de sua distribuição espacial é uma ferramenta importante no entendimento de sua dispersão, o que pode contribuir para o controle e a profilaxia da doença. Este estudo analisou a distribuição espaço-temporal do surto de raiva em rebanhos no estado do Pará, Brasil, entre 2004 e 2013. Foram utilizados registros de síndromes neurológicas obtidas junto à agência de defesa agropecuária do estado (Adepará). A análise revelou 711 notificações de síndromes neurológicas em herbívoros, das quais 32,8% foram positivas para raiva. Em 8% das síndromes neurológicas (n=57) não foi possível realizar as análises devido às más condições das amostras enviadas. Surtos envolveram pelo menos 1.179 animais e os bovinos foram a espécie animal mais afetada (76,8%). Os números de síndromes neurológicas relatadas e de surtos de raiva apresentam forte correlação positiva e exibem tendência linear decrescente. Espacialmente, a maioria dos surtos ocorreu em duas mesorregiões no Pará (Nordeste e Sudeste). Uma das justificativas para essa distribuição espacial pode estar relacionada à distribuição dos animais no estado, uma vez que essas mesorregiões são os maiores produtores de gado do Pará e possuem grande parte do seu território desflorestado para implantação de pastagens.(AU)

Dirofilaria immitis, a parasite that mainly infects domestic or wild canids, but can infect felines or humans as well, is frequent in many Brazilian areas. The main objective of this research was to determine the prevalence of natural canine infection at the Algodoal-Maiandeua Island complex, in the coastal region of the state of Pará, Brazil. A total of 67 dogs were sampled for blood microfilariae detection and for D. immitis DNA detection. Microfilaria and D. immitis DNA could be detected in 35.8% (24/67) of the animals. In one dog's sample no microfilariae were detected, but the PCR was positive, suggesting that either larvae recently were eliminated or adults died shortly before sample collecting. Therefore, it can be concluded that the occurrence of D. immitis is a health threat for domestic and wild canids at the Island of Algodoal, as well as for feline or human health.(AU)


Dirofilaria immitis, um parasito que infecta principalmente canídeos domésticos ou selvagens, embora também possa infectar felinos e humanos, é frequente em muitas áreas do Brasil. O objetivo deste estudo foi determinar a ocorrência da infecção natural em cães provenientes do complexo da Ilha de Algodoal-Maiandeua, região litorânea do estado do Pará, Brasil. Um total de 67 cães tiveram o sangue coletado para detecção de microfilárias de D. immitis e seu DNA. Microfilárias e o DNA de D. immitis foram detectados em 35,8% (24/67) dos animais. Na amostra de um animal, não foram observadas microfilárias, mas o seu DNA foi detectado, sugerindo que as larvas tenham sido recentemente eliminadas ou os adultos tenham morrido antes da coleta da amostra. Portanto, pode-se concluir que a ocorrência de D. immitis é uma ameaça à saúde de canídeos domésticos no complexo da Ilha de Algodoal-Maiandeua, bem como para felinos e seres humanos.(AU)

Leprosy is endemic in large part of Brazil with 28,761 new patients in 2015, the second largest number worldwide and reaches 9/10.000 in highly endemic regions and 2.7/10.000 in the city of Fortaleza, Ceará, Northeast Brazil. For better understanding of risk factors for leprosy transmission, we conducted an epidemiologic study supplemented by 17 locus VNTR and SNP 1-4 typing of Mycobacterium leprae in skin biopsy samples from new multibacillary (MB) patients diagnosed at a reference center in 2009 and 2010. Among the 1,519 new patients detected during the study period, 998 (65.7%) were MB and we performed DNA extraction and genotyping on 160 skin biopsy samples, resulting in 159 (16%) good multilocus VNTR types. Thirty-eight of these patients also provided VNTR types from M. leprae in nasal swabs. The SNP-Type was obtained for 157 patients and 87% were of type 4. Upon consideration all VNTR markers, 156 different genotypes and three pairs with identical genotypes were observed; no epidemiologic relation could be observed between individuals in these pairs. Considerable variability in differentiating index (DI) was observed between the different markers and the four with highest DI [(AT)15, (TA)18, (AT)17 and (GAA)21] frequently demonstrated differences in copy number when comparing genotypes from both type of samples. Excluding these markers from analysis resulted in 83 genotypes, 20 of which included 96 of the patients (60.3%). These clusters were composed of two (n = 8), three (n = 6), four (n = 1), five (n = 2), six (n = 1), 19 (n = 1) and 23 (n = 23) individuals and suggests that recent transmission is contributing to the maintenance of leprosy in Fortaleza. When comparing epidemiological and clinical variables among patients within clustered or with unique M. leprae genotypes, a positive bacterial index in skin biopsies and knowledge of working with someone with the disease were significantly associated with clustering. A tendency to belong to a cluster was observed with later notification of disease (mean value of 3.4 months) and having disability grade 2. A tendency for lack of clustering was observed for patients who reported to have lived with another leprosy case but this might be due to lack of inclusion of household contacts in the study. Although clusters were spread over the city, kernel analysis revealed that some of the patients belonging to the two major clusters were spatially related to some neighborhoods that report poverty and high disease incidence in children. Finally, inclusion of genotypes from nasal swabs might be warranted. A major limitation of the study is that sample size of 160 patients from a two year period represents only 15% of the new patients and this could have weakened statistical outcomes. This is the first molecular epidemiology study of leprosy in Brazil and although the high clustering level suggests that recent transmission is the major cause of disease in Fortaleza; the existence of two large clusters needs further investigation.

As a signatory to World Health Assembly Resolution WHA65.21 on eliminating schistosomiasis, the Brazilian Ministry of Health (MoH) recommends early identification and timely treatment of the infection carriers for morbidity control, plus complementary preventive measures, such as health education, for transmission control. This study reports infection and awareness of schistosomiasis among schoolchildren before the implementation of school-based educational actions in an endemic municipality with persisting moderate prevalence levels despite successive control campaigns since the late 1990s. A questionnaire was applied in April 2013 to schoolchildren in the middle years of schooling (6th to 8th year) of Malacacheta municipality to assess baseline knowledge and risk behaviour related to schistosomiasis. A stool survey was conducted in May/June 2013 in 2519 schoolchildren from all years of fundamental education (first to 9th year) to identify the infection carriers, as well as to assess baseline prevalence and intensity of infection using the Kato-Katz method (one sample, two slides). The infected schoolchildren were treated promptly with single-dose praziquantel 60mg/kg and followed up after 45days for treatment efficacy. Relevant outcomes from baseline stool survey, treatment and follow-up were statistically evaluated in relation to area of residence (rural/urban), gender, age group (<11/≥years) and infection. Adherence to baseline survey was 81.2%, and prevalence of infection was 21.4%. Of the 539 positives, 60 (11.1%) had ≥400 eggs per gram of faeces (heavy-intensity infection). Prevalence of infection was significantly higher among rural residents and≥11year olds, whereas intensity of infection was higher among rural residents,≥11year olds and boys. Adherence by the positives to treatment was 93.3% and adherence by the treated children to 45-day follow-up was 72.2%. At 45days after treatment, 97.0% of the 363 children surveyed were egg-negative; the egg reduction rate was 99.4%. Of the 924 children who responded to the questionnaire, 95.5% showed awareness of schistosomiasis, although 76.2% reported contact with natural, unsafe bodies of water. Reported contact with water was significantly more frequent among infected than non-infected, and boys than girls. The results show persisting infection and risk behaviour among schoolchildren, regardless of their basic knowledge about schistosomiasis. These are grounds for implementing specific educational actions to improve awareness and behavioural change, jointly with other control measures, to attain the MoH goals.

BACKGROUND: Among the neglected tropical diseases (NTDs), schistosomiasis and the three main soil-transmitted helminthiases (STHs), i.e., ascariasis, trichuriasis and hookworm infection, represent the most common infections in developing countries. In Brazil, there is a lack of epidemiological data in many parts of the country, which favors the unawareness of the real situation concerning these diseases. Due to this, we investigated the occurrence of schistosomiasis and STHs in a region of Minas Gerais State, Brazil. METHODS: One stool sample was collected from 503 individuals, whose ages ranged from 0.1 to 91.2 years, and screened using both the Kato-Katz and the Formol-Ether methods. In parallel, a malacological survey was carried out in the main water bodies of the district, and Biomphalaria susceptibility assays and kernel-based techniques were also performed. RESULTS: No individual was found infected with Ascaris lumbricoides or hookworm. Schistosoma mansoni was the most common parasite found (1.6%). The prevalence was higher in males and the chance of acquiring the disease increased by 43.35 times with contact with a body of water. None of the Biomphalaria tenagophila and B. glabrata specimens were found naturally infected, but B. glabrata was highly susceptible to infection with Schistosoma mansoni. Using kernel-based techniques, clusters of Biomphalaria were found near the households where the infected individuals lived. CONCLUSIONS: Schistosomiasis was the most prevalent parasitic infection found. Our findings show that the occurrence of this disease has been underestimated by the local health care service, and highlight the importance of epidemiological surveillance in areas of low prevalence for schistosomiasis.

A expansão da esquistossomose para o litoral de Pernambuco vem sendo registrada desde 1992 coma detecção de casos agudos da doença em indivíduos de classe média/alta. Para diagnosticar estenovo cenário de transmissão da endemia em localidades turísticas na orla marítima do estado é defundamental importância o conhecimento prévio da distribuição georreferenciada das espécies demoluscos vetores, por meio de mapas que apontem sua exata localização, assim como é necessárioespacializar as situações de risco biológico e as condições ambientais insalubres. Nos períodos de 15a 18 de setembro de 2008 e 10 a 13 de setembro de 2013 foram realizadas caravanas expedicionárias,por epidemiologistas e parasitologistas por 11 municípios do litoral do estado, percorrendo todasas coleções de água doce distantes até 2 km da beira-mar. Os dados foram georreferenciados paraposterior condução de análises espaciais. Para identificação da positividade dos exemplares deBiomphalaria glabrata, foi utilizada a técnica de exposição à luz. Os espécimes de B. stramineaque permaneceram negativos até o 15º dia foram submetidos à técnica de diagnóstico molecular(PCR-single tube). Nas coletas de 2008 foram capturados 3.392 caramujos B. glabrata e 725 B.straminea, cujas taxas de infecção variaram entre 0,9 por cento a 22,2 por cento. Em 2013, foram coletados 948B. glabrata e 504 B. straminea e somente foram encontrados caramujos eliminando cercarias nalocalidade Porto de Galinhas com taxa de infectividade de 81,4 por cento, permanecendo como localidadede maior risco para transmissão da esquistossomose.


The expansion of schistosomiasis to the coast of Pernambuco has been registered since 1992 withthe detection of acute cases of the disease in individuals of middle/upper class. To diagnose thisnew scenario of transmission of schistosomiasis in tourist areas on the waterfront of the state,knowledge of georeferenced species distribution of vector snails through maps that show theirexact location seems essential, as it is necessary to localize situations with biohazard and unhealthyenvironmental conditions. From 15 to 18 September 2008 and 10 to 13 September 2013, expeditionswere conducted by groups of epidemiologists and parasitologists for 11 coastal municipalities ofthe state, checking all the collections of fresh water up to 2 km away from the seaside. Data weregeoreferenced for a later conduction of spatial analysis. For positive identification of Biomphalariaglabrata, the technique of light exposure was used. Specimens of B. straminea that remainednegative until the 15th day underwent the technique of molecular diagnosis (PCR-single tube). Inthe 2008 expedition, 3,392 B. glabrata snails and 725 B. straminea snails were captured, whose infection rates ranged from 0.9 percent to 22.2 percent. In the 2013 expedition, 948 B. glabrata and 504 B. straminea snails were collected and only in Porto de Galinhas city were B. glabrata snails found releasing S. mansoni cercariae, with an infectivity rate of 81.4 percent, confirming that in this locality the risk of schistosomiasis transmission is still present.

Geographic Information Systems (GISs) are composed of useful tools to map and to model the spatial distribution of events that have geographic importance as schistosomiasis. This paper is a review of the use the indicator kriging, implemented on the Georeferenced Information Processing System (SPRING) to make inferences about the prevalence of schistosomiasis and the presence of the species of Biomphalaria, intermediate hosts of Schistosoma mansoni, in areas without this information, in the Minas Gerais State, Brazil. The results were two maps. The first one was a map of Biomphalaria species, and the second was a new map of estimated prevalence of schistosomiasis. The obtained results showed that the indicator kriging can be used to better allocate resources for study and control of schistosomiasis in areas with transmission or the possibility of disease transmission.

The impact of intestinal helminths on human health is well known among the population and health authorities because of their wide geographic distribution and the serious problems they cause. Geohelminths are highly prevalent and have a big impact on public health, mainly in underdeveloped and developing countries. Geohelminths are responsible for the high levels of debility found in the younger population and are often related to cases of chronic diarrhea and malnutrition, which put the physical and intellectual development of children at risk. These geohelminths have not been sufficiently studied. One obstacle in implementing a control program is the lack of knowledge of the prevalence and geographical distribution. Geographical information systems (GIS) and remote sensing (RS) have been utilized to improve understanding of infectious disease distribution and climatic patterns. In this study, GIS and RS technologies, as well as meteorological, social, and environmental variables were utilized for the modeling and prediction of ascariasis and trichuriasis. The GIS and RS technologies specifically used were those produced by orbital sensing including the Moderate Resolution Imaging Spectroradiometer (MODIS) and the Shuttle Radar Topography Mission (SRTM). The results of this study demonstrated important factors related to the transmission of ascariasis and trichuriasis and confirmed the key association between environmental variables and the poverty index, which enabled us to identify priority areas for intervention planning in the state of Minas Gerais in Brazil.

This paper analyses the associations between Normalized Difference Vegetation Index (NDVI) and Enhanced Vegetation Index (EVI) on the prevalence of schistosomiasis and the presence of Biomphalaria glabrata in the state of Minas Gerais (MG), Brazil. Additionally, vegetation, soil and shade fraction images were created using a Linear Spectral Mixture Model (LSMM) from the blue, red and infrared channels of the Moderate Resolution Imaging Spectroradiometer spaceborne sensor and the relationship between these images and the prevalence of schistosomiasis and the presence of B. glabrata was analysed. First, we found a high correlation between the vegetation fraction image and EVI and second, a high correlation between soil fraction image and NDVI. The results also indicate that there was a positive correlation between prevalence and the vegetation fraction image (July 2002), a negative correlation between prevalence and the soil fraction image (July 2002) and a positive correlation between B. glabrata and the shade fraction image (July 2002). This paper demonstrates that the LSMM variables can be used as a substitute for the standard vegetation indices (EVI and NDVI) to determine and delimit risk areas for B. glabrata and schistosomiasis in MG, which can be used to improve the allocation of resources for disease control.

Geographical Information System (GIS) is a tool that has recently been applied to better understand spatial disease distributions. Using meteorological, social, sanitation, mollusc distribution data and remote sensing variables, this study aimed to further develop the GIS technology by creating a model for the spatial distribution of schistosomiasis and to apply this model to an area with rural tourism in the Brazilian state of Minas Gerais (MG). The Estrada Real, covering about 1,400 km, is the largest and most important Brazilian tourism project, involving 163 cities in MG with different schistosomiasis prevalence rates. The model with three variables showed a R(2) = 0.34, with a standard deviation of risk estimated adequate for public health needs. The main variables selected for modelling were summer vegetation, summer minimal temperature and winter minimal temperature. The results confirmed the importance of Remote Sensing data and the valuable contribution of GIS in identifying priority areas for intervention in tourism regions which are endemic to schistosomiasis.

Schistosomiasis mansoni is not just a physical disease, but is related to social and behavioural factors as well. Snails of the Biomphalaria genus are an intermediate host for Schistosoma mansoni and infect humans through water. The objective of this study is to classify the risk of schistosomiasis in the state of Minas Gerais (MG). We focus on socioeconomic and demographic features, basic sanitation features, the presence of accumulated water bodies, dense vegetation in the summer and winter seasons and related terrain characteristics. We draw on the decision tree approach to infection risk modelling and mapping. The model robustness was properly verified. The main variables that were selected by the procedure included the terrain's water accumulation capacity, temperature extremes and the Human Development Index. In addition, the model was used to generate two maps, one that included risk classification for the entire of MG and another that included classification errors. The resulting map was 62.9% accurate.

Praziquantel chemotherapy has been the focus of the Schistosomiasis Control Program in Brazil for the past two decades. Nevertheless, information on the impact of selective chemotherapy against Schistosoma mansoni infection under the conditions confronted by the health teams in endemic municipalities remains scarce. This paper compares the spatial pattern of infection before and after treatment with either a 40 mg/kg or 60 mg/kg dose of praziquantel by determining the intensity of spatial cluster among patients at 180 and 360 days after treatment. The spatial-temporal distribution of egg-positive patients was analysed in a Geographic Information System using the kernel smoothing technique. While all patients became egg-negative after 21 days, 17.9% and 30.9% reverted to an egg-positive condition after 180 and 360 days, respectively. Both the prevalence and intensity of infection after treatment were significantly lower in the 60 mg/kg than in the 40 mg/kg treatment group. The higher intensity of the kernel in the 40 mg/kg group compared to the 60 mg/kg group, at both 180 and 360 days, reflects the higher number of reverted cases in the lower dose group. Auxiliary, preventive measures to control transmission should be integrated with chemotherapy to achieve a more enduring impact.

Geostatistics is used in this work to make inferences about the presence of the species of Biomphalaria (B. glabrata, B. tenagophila and/or B. straminea), intermediate hosts of Schistosoma mansoni, at the São Francisco River Basin, in Minas Gerais, Brazil. One of these geostatistical procedures, known as indicator kriging, allows the classification of categorical data, in areas where the data are not available, using a punctual sample set. The result is a map of species and risk area definition. More than a single map of the categorical attribute, the procedure also permits the association of uncertainties of the stochastic model, which can be used to qualify the inferences. In order to validate the estimated data of the risk map, a fieldwork in five municipalities was carried out. The obtained results showed that indicator kriging is a rather robust tool since it presented a very good agreement with the field findings. The obtained risk map can be thought as an auxiliary tool to formulate proper public health strategies, and to guide other fieldwork, considering the places with higher occurrence probability of the most important snail species. Also, the risk map will enable better resource distribution and adequate policies for the mollusk control. This methodology will be applied to other river basins to generate a predictive map for Biomphalaria species distribution for the entire state of Minas Gerais.

The influence of climate and environmental variables to the distribution of schistosomiasis has been assessed in several previous studies. Also Geographical Information System (GIS), is a tool that has been recently tested for better understanding the spatial disease distribution. The objective of this paper is to further develop the GIS technology for modeling and control of schistosomiasis using meteorological and social variables and introducing new potential environmental-related variables, particularly those produced by recently launched orbital sensors like the Moderate Resolution Imaging Spectroradiometer (MODIS) and the Shuttle Radar Topography Mission (SRTM). Three different scenarios have been analyzed, and despite of not quite large determination factor, the standard deviation of risk estimates was considered adequate for public health needs. The main variables selected as important for modeling purposes was topographic elevation, summer minimum temperature, the NDVI vegetation index, and the social index HDI91.
