RESUMO

We present a prospective follow-up study of 114 patient infected by Candida and HIV, in order to assess the role of oral candidiasis (OC) as a predictive marker of tuberculosis and opportunistic infections (OI) which define the Acquired Immunodeficiency Syndrome (AIDS). We compared the study group with an homogeneous control group of 152 patients infected by HIV without OC during the same period. TBC findings in the first group were discriminant with respect to the control group (p less than 0.0001) with and average period of 2.5 months between the diagnosis of OC and the development of TBC. These findings suggest that OC is an early evolutive marker of TBC and a late marker of AIDS.

RESUMO

Of a total of 41 VIH positive patients, parenteral drug addicts, with persistent generalized lymphadenopathy syndrome, we performed 43 lymph-node biopsies. In two sequential biopsies, 2 and 12 months respectively after the first biopsy, were carried-out. After a clinical follow-up (ranging between 4-28 months) 12 patients (24.3%) were qualified as SIDA cases according to CDC-1987 criteria. Lymph-node biopsy was of diagnostic or prognostic value in 16.27% and in 6.97% respectively. Upon comparing histological types and peripheral lymphocyte subsets it was demonstrated how the forms of major morphological deterioration tend to associate with a lower number of OKT4+ lymphocytes and a lower relation OKT4+/OKT8+.

In 24 cases of thrombopenic purpura associated with human immunodeficiency virus infection the clinical, immunological and therapeutic features were evaluated. Thrombopenia resulted in clinical manifestations in 20 patients. Splenomegaly was found in only one fourth of patients. Antiplatelet antibodies were found in 9 patients, and thrombopenia was associated with anemia in 37% of cases and with leukopenia in 21%. Bone marrow examination showed megakaryocyte hyperplasia in two thirds of the patients. The major immunological abnormalities were an inverted helper/suppressor T lymphocytes ratio, a reduction in the number of helper T lymphocytes, polyclonal hypergammaglobulinemia, and increased serum concentrations of circulating immunocomplexes. The different therapeutic modalities, steroids, vincristine, danatrol and plasma exchange, resulted in short responses; only two patients had normal platelet counts. The median follow-up was 14 months; during this time three patients fulfilled the criteria of acquired immunodeficiency syndrome.
