Background: Heavy strength (HS) and short-sprint (SS) are commonly used training methods for competitive road cyclists, with the aim to improve the anaerobic power and short time cycling performance. Knowledge of how such training methods affects biochemical as well as molecular factors, are particularly important for determining individual recovery and long-term adaptations. The primary aim of the current study was to investigate the expression levels of small non-coding RNAs in response to HS and SS training in elite cyclists as potential biomarkers for individual optimal restitution time. Methods: Eleven well trained cyclists performed one session of HS training and one session of SS training on separate days. Blood samples were taken at baseline and 5 min, 1 h and 21 h post training. Along with physiological measurements and biochemical factors (serum creatine kinase, myoglobin, human growth hormone and plasma lactate), real-time quantitative PCR was used to explore whether HS and/or SS training influenced the abundance of 24 circulating miRNAs, in serum, associated with muscle development, angiogenesis, and/or inflammation. Results: Based on complete miRNA profiles from nine cyclists, the miRNAs showing most altered expression after both training sessions included the three striated muscle-specific miRNAs (myomiRs) miR-1-3p, 133a-3p and 133b-3p. While all three miRNAs showed significantly highest expression at 1 h post HS session, the acute effect of the SS session included a significantly higher level of miR-1-3p alone, at 5 min (highest), as well as at 1 h and 21 h post session. Correlation (negative) with biochemical markers was only shown for miR-133a-3p and CK (r = -0.786, p = 0.041) and between miR-133b-3p and [La-] (r = -0.711, p = .032), at 21 h post SS session. Conclusion: Our findings support that unique myomiRs are regulated by HS and SS training. Such knowledge may be important for individually adjusted restitution times.

Little is known about the placebo effects when comparing training interventions. Consequently, we investigated whether subjects being told they are in the intervention group get better training results compared to subjects being told they are in a control group. Forty athletes (male: n = 31, female: n = 9) completed a 10-week training intervention (age: 22 ± 4 years, height: 183 ± 10 cm, and body mass: 84 ± 15 kg). After randomization, the participants were either told that the training program they got was individualized based on their force-velocity profile (Placebo), or that they were in the control group (Control). However, both groups were doing the same workouts. Measurements included countermovement jump (CMJ), 20-m sprint, one-repetition maximum (1RM) back-squat, a leg-press test, ultrasonography of muscle-thickness (m. rectus femoris), and a questionnaire (Stanford Expectations of Treatment Scale) (Younger et al. in Clin Trials 9(6):767-776, 2012). Placebo increased 1RM squat more than Control (5.7 ± 6.4% vs 0.9 ± 6.9%, [0.26 vs 0.02 Effect Size], Bayes Factor: 5.1 [BF10], p = 0.025). Placebo had slightly higher adherence compared to control (82 ± 18% vs 72 ± 13%, BF10: 2.0, p = 0.08). Importantly, the difference in the 1RM squat was significant after controlling for adherence (p = 0.013). No significant differences were observed in the other measurements. The results suggest that the placebo effect may be meaningful in sports and exercise training interventions. It is possible that ineffective training interventions will go unquestioned in the absence of placebo-controlled trials.

INTRODUCTION: Earlier studies have demonstrated that the oldest in a competition class are more likely to succeed than the youngest, a phenomenon called relative age effect (RAE). Track and field give us an opportunity to investigate the advantage of being born early in the year based upon actual performance, since objective criteria are the performance indicators. Hence, the aim of the present study was to investigate the occurrence of RAE in Norwegian track and field athletes in events where physical capacity is important for success. METHODS: All individual season best results from the register of The Norwegian Athletics Federation (n = 28 999) obtained in all competition classes from the age of 10 years to senior in both sexes on 60m and 600m from 2011 to 2020 were downloaded. One-way ANOVA and LSD post hoc analyses were used to analyze performance differences according to birth quartiles between athletes. Further, odds ratios (OR) were used to calculate the odds of being among the top-100 for athletes for those born in the first quartile of the year compared to the last. RESULTS: The RAE was present in several of the competition classes in sprint compared to middle-distance running, and in more male than female competition classes. Overall, the OR of being among the top-100 in one of the competition classes on 60m sprint when born in first quartile compared to last quartile was 2.88 [2.30-3.62] for males and 1.54 [1.26-1.89] for females. CONCLUSION: Being born early in the year in events with high demand for specific physical capacities is an advantage in both sexes in most of the youngest competition classes. In males, the advantage of being born early in the year lasted longer in sprint than in middle-distance running, indicating that puberty affects performance in sprint and middle-distance running differently.

INTRODUCTION: Biological maturity level has shown to affect sport performance in youths. However, most previous studies have used noninvasive methods to estimate maturity level. Thus, the main aim of the present study was to investigate the association between skeletal age (SA) as a measure of biological maturation level, match locomotion, and physical capacity in male youth soccer players. METHOD: Thirty-eight Norwegian players were followed during two consecutive seasons (U14 and U15). Match locomotion was assessed with GPS-tracking in matches. SA, assessed by x-ray, physical capacities (speed, strength and endurance) and anthropometrics were measured in the middle of each season. Analysis of associations between SA, match locomotion, and physical capacities were adjusted for the potential confounding effect of body height and weight. RESULTS: In matches, positive associations were found between SA and maximal speed and running distance in the highest speed zones. Further, SA was associated with 40 m sprint time and countermovement jump (CMJ) height, and with intermittent-endurance capacity after adjusting for body height (U14). Associations between SA and leg strength and power, and between SA and absolute VO2max were not significant after adjusting for body weight. There was no association between SA and total distance covered in matches. CONCLUSION: Biological maturity level influence match locomotion and performance on physical capacity tests. It is important that players, parents and coaches are aware of the advantages more mature players have during puberty, and that less mature players also are given attention, appropriate training and match competition to ensure proper development.

The purpose of this study was to investigate the degree of relative age effect (RAE) among the best Norwegian track and field athletes of all time, aged 13 years to senior, as well as to investigate the differences between athletes in events that impose different demands on their physical characteristics, categorised in endurance and explosiveness. The degree of RAE was investigated by examining the difference between the sample's (N = 21,711) quarterly birth distribution and the quarterly distribution of birth of the Norwegian population as a whole from 1966 to 2019. To determine whether or not an RAE was present, chi-square tests (χ2) were conducted against an even distribution, with Cramer's V (phi or ɸ) as a measure of effect size. The study's results show a strong RAE in the two youngest age groups in both genders. RAE decreases with increasing age, but the effect is still present at the senior level in both men and women. Furthermore, the degree of RAE was strongest in explosive events in both boys (ɸ = 0.46) and girls (ɸ = 0.30), while in endurance events it was strong in boys (ɸ = 0.38) but not in girls (ɸ = 0.13). Prominent effect of RAE in the 13- and 14-year-old classes can be explained by the fact that in the youngest age groups impose the highest relative age difference. In addition, this is an age group where there are large differences in growth spurts, physical characteristics and training experience. Elimination of RAE with increasing age may be due to the fact that after puberty inherent physical advantages as a result of the month of birth are evened out. The prominent RAE in explosive events and in boys may be due to the fact that puberty and growth spurts make boys faster, stronger and larger, while puberty and growth spurts in girls are not always beneficial for girls in track and field events. The practical significance of the results relates to athletes developmental opportunities. Irrespective of whether young track and field athletes are relatively older or younger they should be met with patience and dedication from coaches. Superficial short-term categorization of young athletes potential do more harm than good.

This study aimed to investigate match running performance in U14 male soccer players in Norway, and the influence of position, competitive level and contextual factors on running performance. Locomotion was monitored in 64 different U14 players during 23 official matches. Matches were played at two different competitive levels: U14 elite level (n = 7) and U14 sub-elite level (n = 16). The inclusion criterion was completed match halves played in the same playing position. The variables' influence on match running performance was assessed using mixed-effect models, pairwise comparisons with Bonferroni correction, and effect size. The results showed that the U14 players, on average, moved 7645 ± 840 m during a match, of which 1730 ± 681 m (22.6%) included high-intensity running (HIR, 13.5-18.5 km·h-1) and sprinting (>18.5 km·h-1). Wide midfielders (WM) and fullbacks (FB) covered the greatest sprint distance (569 ± 40 m) and, in addition to the centre midfield position (CM), also covered the greatest total distance (TD) (8014 ± 140 m) and HIR distance (1446 ± 64 m). Centre forwards (CF) performed significantly more accelerations (49.5 ± 3.8) compared other positions. TD (7952 ± 120 m vs. 7590 ± 94 m) and HIR (1432 ± 57 m vs. 1236 ± 43 m) were greater in U14 elite-level matches compared with sub-elite matches. Greater TD and sprint distances were performed in home matches, but, on the other hand, more accelerations and decelerations were performed in matches played away or in neutral locations. Significantly higher TD, HIR and sprinting distances were also found in lost or drawn matches. In conclusion, physical performance during matches is highly related to playing position, and wide positions seem to be the most physically demanding. Further, competitive level and contextual match variables are associated with players' running performance.

In laboratory studies, imposed sleep restriction consistently reduces cognitive performance. However, the association between objectively measured, free-living sleep and cognitive function has not been studied in older adolescents. To address this gap, we measured one week of sleep with a wrist-worn GT3X+ actigraph in 160 adolescents (96 girls, 17.7 ± 0.3 years) followed by assessment of working memory with an n-back task and visual attention with a Posner cue-target task. Over the week, participants spent 7.1 ± 0.8 h/night in bed and slept 6.2 ± 0.8 h/night with 88.5 ± 4.8% efficiency and considerable intra-participant night-to-night variation, with a standard deviation in sleep duration of 1.2 ± 0.7 h. Sleep measures the night before cognitive testing were similar to weekly averages. Time in bed the night before cognitive testing was negatively associated with response times during the most challenging memory task (3-back; p = 0.005). However, sleep measures the night before did not correlate with performance on the attention task and weekly sleep parameters were not associated with either cognitive task. Our data suggests shorter acute free-living sleep may negatively impact difficult memory tasks, however the relationship between free-living sleep and cognitive task performance in healthy adolescents is less clear than that of laboratory findings, perhaps due to high night-to-night sleep variation.

OBJECTIVE: To evaluate changes in pulmonary function and feasibility of portable continuous laryngoscopy during maximal uphill running. METHODS: Healthy volunteers participated in an uphill race. Forced expiratory volume in 1 s (FEV1) and forced vital capacity (FVC) were obtained before and 5 and 10 min after finishing the race. Capillary blood lactate concentration ([BLa-]) and Borg score for perceived exertion were registered immediately after the race. One participant wore a portable video-laryngoscope during the race, and the video was assessed for technical performance. RESULTS: Twenty adult subjects participated with a mean (SD) age of 40.2 (9.7) years. Mean (SD) race duration and post-exercise [BLa-] was 13.9 (2.3) min and 10.7 (2.1) mmol/L, respectively, and the median (range) Borg score for perceived exertion was 9 (5-10). Mean percentage change (95% CI) 5 and 10 min post-exercise in FEV1 were 6.9 (3.7 to 10.2) % and 5.9 (2.7 to 9.0) %, respectively, and in FVC 5.2 (2.3 to 8.1) % and 4.7 (1.6 to 7.9) %, respectively. The recorded video of the larynx was of good quality. CONCLUSIONS: Maximal aerobic field exercise induced bronchodilatation in the majority of the healthy non-asthmatic participants. It is feasible to perform continuous video-laryngoscopy during heavy uphill exercise.

The main aim of the present study was to compare skeletal maturity level and physical capacities between male Norwegian soccer players playing at elite, sub-elite and non-elite level. Secondary, we aimed to investigate the association between skeletal maturity level and physical capacities. One hundred and two U14 soccer players (12.8-14.5 years old) recruited from four local clubs, and a regional team were tested for bone age and physical capacities. Bone age was estimated with x-ray of their left hand and used to indicate maturation of the skeleton. Players went through a comprehensive test battery to assess their physical capacities. Between-groups analysis revealed no difference in chronological age, skeletal maturity level, leg strength, body weight, or stature. However, elite players were superior to sub-elite and non-elite players on important functional characteristics as intermittent-endurance capacity (running distance: 1664 m ± 367 vs 1197 m ± 338 vs 693 m ± 235) and running speed (fastest 10 m split time: 1.27 seconds ± 0.06 vs 1.33 seconds ± 0.10 vs 1.39 seconds ± 0.11), in addition to maximal oxygen uptake ( V ˙ O 2 m a x ), standing long jump, and upper body strength (P < .05 for all comparisons). Medium-to-large correlations were found between skeletal maturity level and peak force (r = 695, P < .01), power (r = 684, P < .01), sprint (r = -.471, P<.001), and jump performance (r = .359, P < .01), but no correlation with upper body strength, V ˙ O 2 m a x , or intermittent-endurance capacity. These findings imply that skeletal maturity level does not bias the selection of players, although well-developed physical capacity clearly distinguishes competitive levels. The superior physical performance of the highest-ranked players seems related to an appropriate training environment.

The aim of the study was to compare running performance of three competitive standards and to examine the effects of being promoted to a higher league in Norwegian football. One club's first and second team were included. The first team consisted of professional soccer players playing at Level 2 (2015 season) and Level 1 (2016 season). The second team consisted of amateurs playing at Level 4. A fully automatic tracking system was used to examine running performance, divided into different running-speed categories and playing position. Forty-one matches were included containing 278 observations. Level 1 performed 61 and 51% sprinting compared to Level 2 and Level 4 but similar high-speed running. Similar high-speed running distances were observed only for the different playing positions at Level 1 compared to Level 2 and 4. The sprinting distance was greater for the central defender and attacker, and the number of accelerations was greater for central midfielders and wide midfielders' playing at Level 1 compared to lower competitive standards. In conclusion, better competitive standards resulted in greater high-intensity actions than lower leagues in Norwegian soccer. Furthermore, only central defenders and attackers increased their high-intensity locomotions when the team was promoted.

PURPOSE: To compare the effects of short-sprint training (SST) and heavy-strength training (HST) following a 4-week strength-training period on sprint and endurance capacities in well-trained cyclists. METHODS: Twenty-eight competitive cyclists (age 29 ± 6 years) with maximal oxygen uptake () of 61.1 ± 5.9 mL⋅min-1⋅kg-1 participated. After a 4-weeks preparation strength-training period, the participants were randomized to add either HST or SST to their usual endurance training for the subsequent 6 weeks. Body composition, and power output at blood lactate concentration ([La-]) of 4 mmol⋅L-1, as well as a 100 min cycling test including 6 and 30-s sprints, 60 min cycling at [La-] of 2 mmol⋅L-1 and 5-min all-out cycling were performed before the 4-week preparation strength-training period, and before and after the 6-week intervention period. In addition, 1 repetition maximum (RM) in half-squat and 55-m maximal sprints on the cyclists' own bikes were measured before and after the 6-week intervention. RESULTS: SST was superior to HST in 6-s sprint performance, both in a fresh state (4.7 ± 2.6% vs. 1.1 ± 3.5%) and after prolong cycling (6.1 ± 1.8% vs. 1.8 ± 4.2%), in 30-s sprint (3.7 ± 2.8% vs. 1.3 ± 2.5%) and in 55-m seated sprint on own bike (4.3 ± 2.1% vs. 0.2 ± 1.8%) (all p < 0.002). HST induced a larger 1RM improvement in the half-squat test than SST (9.3 ± 3.6% vs. -3.9 ± 3.8%; p < 0.001). No group differences were revealed in the 5-min all-out test, , power output at 4 mmol⋅L-1 [La-], or in gross efficiency. CONCLUSION: SST led to a greater increase in average and peak power output on all sprint tests compared to HST, whereas HST led to a greater increase in maximal strength. No group differences were found in relative changes in endurance capacities. Altogether, our results show a high degree of specificity in the adaptations of both SST and HST.

Purpose To examine the effects of shift work and extended working hours on sleepiness among pilots and Helicopter Emergency Medical Service (HEMS) crew members in the Norwegian Air Ambulance. METHODS: This field study investigated sleepiness during 3 consecutive weeks: the week before work, the work week, and the week after work. The pilots and HEMS crew members (N = 50) kept a wake diary during all 3 weeks and completed reaction time tests during the work week. RESULTS: The overall sleepiness scores were low during all 3 weeks. When comparing the 3 weeks, the lowest sleepiness levels were found for the work week. There was a small difference across work days, in which subjective sleepiness scores were highest the first duty day. No change in the reaction time tests was evident during the work week. The crew members reported being most sleepy at midnight, compared to all the other timepoints over the course of a duty day. Regarding workload and total work time, having larger workload was associated with lower sleepiness scores, while having higher total work time was associated with higher sleepiness score, both compared to the medium category. CONCLUSIONS: The findings indicate that the work schedules and setting for this distinct occupational group do not seem to negatively affect the sleepiness levels.

Purpose: Although strength and sprint training are widely used methods in competitive cycling, no previous studies have compared the acute responses and recovery rates following such sessions among highly trained cyclists. The primary aim of the current study was to compare power production and biochemical markers of metabolic stress and muscle damage following a session of heavy strength (HS) and short-sprint training (SS). Methods: Eleven well-trained male cyclists (18 ± 2 years with maximal oxygen uptake of 67.2 ± 5.0 mL·kg-1·min-1) completed one HS session and one SS session in a randomized order, separated by 48 h. Power production and biochemical variables were measured at baseline and at different time points during the first 45 h post exercise. Results: Lactate and human growth hormone were higher 5 min, 30 min and 1 h post the SS compared to the HS session (all p ≤ 0.019). Myoglobin was higher following the HS than the SS session 5 min, 30 min and 1 h post exercise (all p ≤ 0.005), while creatine kinase (CK) was higher following the HS session 21 and 45 h post exercise (p ≤ 0.038). Counter movement jump and power production during 4 sec sprint returned to baseline levels at 23 and 47 h with no difference between the HS and SS session, whereas the delayed muscle soreness score was higher 45 h following the HS compared to the SS session (p = 0.010). Conclusion: Our findings indicate that SS training provides greater metabolic stress than HS training, whereas HS training leads to more muscle damage compared to that caused by SS training. The ability to produce power remained back to baseline already 23 h after both training sessions, indicating maintained performance levels although higher CK level and muscle soreness were present 45 h post the HS training session.

BACKGROUND: Opioid induced cerebral changes may contribute to neuropsychological difficulties, like attention problems, frequently reported in prenatally opioid-exposed children. Reduced regional brain volumes have been shown after prenatal opioid exposure, but no study to date has explored the possible impact of prenatal opioids on brain activation patterns. MATERIALS AND METHODS: A hospital-based sample of prenatally opioid-exposed school-aged children (n = 11) and unexposed controls (n = 12) underwent functional magnetic resonance imaging (fMRI) during a combined working memory-selective attention task. Within-group- and between-group analyses of blood-oxygen-level-dependent (BOLD) activation were performed using the SPM12 software package and group differences in task performance were analyzed using Cox proportional hazards modeling. RESULTS: Overall, similar patterns of task related parietal and prefrontal BOLD activations were found in both groups. The opioid-exposed group showed impaired task performance, and during the most cognitive demanding versions of the working memory-selective attention task, increased activation in prefrontal cortical areas was found in the opioid-exposed group compared to controls. CONCLUSION: Our findings suggest that prenatal opioids affect later brain function, visible through changes in BOLD activation patterns. However, results should be considered preliminary until replicated in larger samples better suited to control for potential confounding factors.

Background: Previous studies have shown associations of physical fitness and cognition in children and in younger and older adults. However, knowledge about associations in high-school adolescents and young adults is sparse. Thus, the aim of this study was to evaluate the association of physical fitness, measured as maximal oxygen uptake ([Formula: see text]), muscle mass, weekly training, and cognitive function in the executive domains of selective attention and inhibitory control, in healthy male high-school students. Methods: Fifty-four males (17.9 ± 0.9 years, 72 ± 11 kg and 182 ± 7 cm) completed a [Formula: see text] test, a body composition test and a visual cognitive task based on the Posner cue paradigm with three types of stimuli with different attentional demands (i.e., stimuli presentation following no cue, valid cue or invalid cue presentations). The task consisted of 336 target stimuli, where 56 (17%) of the target stimuli appeared without a cue (no cue), 224 (67%) appeared in the same rectangle as the cue (valid cue) and 56 (17%) appeared in the rectangle opposite to the cue (invalid cue). Mean reaction time (RT) and corresponding errors was calculated for each stimuli type. Total task duration was 9 min and 20 s In addition, relevant background information was obtained in a questionnaire. Results: Linear mixed model analyses showed that higher [Formula: see text] was associated with faster RT for stimuli following invalid cue (Estimate = -2.69, SE = 1.03, p = 0.011), and for stimuli following valid cue (Estimate = -2.08, SE = 1.03, p = 0.048). There was no association of muscle mass and stimuli (F = 1.01, p = 0.397) or of weekly training and stimuli (F = 0.99, p = 0.405). Conclusion: The results suggest that cardiorespiratory fitness is associated with cognitive performance in healthy male high-school students in the executive domains of selective attention.

Many athletes sleep poorly due to stress, travel, and competition anxiety. In the present study, we investigated the effects of sleep deprivation on soccer skills (juggling, dribbling, ball control, continuous kicking, 20 and 40 m sprint, and 30 m sprint with changes of direction). In all, 19 male junior soccer players (14-19 years old) were recruited and participated in a cross-balanced experimental study comprising two conditions; habitual sleep and 24 hours sleep deprivation. In both conditions, testing took place between 8 a.m. and 10 a.m. Order of tests was counterbalanced. Each test was conducted once or twice in a sequence repeated three times. The results revealed a negative effect of sleep deprivation on the continuous kicking test. On one test, 30 meter sprint with directional changes, a significant condition × test repetition interaction was found, indicating a steeper learning curve in the sleep deprived condition from Test 1 to Test 2 and a steeper learning curve in the rested condition from Test 2 to Test 3. The results are discussed in terms of limitations and strengths, and recommendations for future studies are outlined.

BACKGROUND: Sleep and mood have seldom been compared between elite athletes and nonelite athletes, although potential differences suggest that physical activity may affect these parameters. This study aims to explore whether adolescent elite athletes differ from controls in terms of sleep, positive affect (PA) and negative affect (NA). METHODS: Forty-eight elite athletes and 26 controls participating in organized and nonorganized sport completed a questionnaire, and a 7-day sleep diary. RESULTS: On school days, the athletes and the controls who participated in organized and nonorganized sport differed in bedtime (22:46, 23:14, 23:42, P < .01), sleep onset (23:03, 23:27, 00:12, P < .01), and total sleep time (7:52, 8:00, 6:50, P < 01). During weekend, the athletes, the controls who participated in organized and nonorganized sport differed in bedtime (23:30, 00:04, 00:49, P < .01), sleep onset (23.42, 00:18, 01:13, P < .01), rise time (9:15, 9:47, 10:55, P < .01), sleep efficiency (95.0%, 94.2%, 90.0%, P < 05), and sleep onset latency (11.8, 18.0, 28.0 minutes, P < .01). Furthermore, the athletes reported less social jetlag (0:53) and higher score for PA (34.3) compared with the controls who participated in nonorganized sport (jetlag: 1:25, P < .05, PA: 29.8, P < .05). CONCLUSIONS: An almost dose-response association was found between weekly training hours, sleep, social jetlag and mood in adolescents.

Aim: Hamstring strain injury is common in soccer. The aim of this study was to evaluate the physical capacity of players who have and have not suffered from hamstring strain injury in a sample of semi-professional and professional Norwegian soccer players in order to evaluate characteristics and to identify possible indications of insufficient rehabilitation. Method: Seventy-five semi-professional and professional soccer players (19 ± 3 years) playing at the second and third level in the Norwegian league participated in the study. All players answered a questionnaire, including one question about hamstring strain injury (yes/no) during the previous 2 years. They also performed a 40 m maximal sprint test, a repeated sprint test (8 × 20 m), a countermovement jump, a maximal oxygen consumption (VO2max) test, strength tests and flexibility tests. Independent sample t-tests were used to evaluate differences in the physical capacity of the players who had suffered from hamstring strain injury and those who had not. Mixed between-within subject's analyses of variance was used to compare changes in speed during the repeated sprint test between groups. Results: Players who reported hamstring strain injury during the previous two years (16%) had a significantly higher drop in speed (0.07 vs. 0.02 s, p = 0.007) during the repeated sprint test, compared to players reporting no previous hamstring strain injury. In addition, there was a significant interaction (groups × time) (F = 3.22, p = 0.002), showing that speed in the two groups changed differently during the repeated sprint test. There were no significant differences in relations to age, weight, height, body fat, linear speed, countermovement jump height, leg strength, VO2max, or hamstring flexibility between the groups. Conclusion: Soccer players who reported hamstring strain injury during the previous 2 years showed significant higher drop in speed during the repeated sprint test compared to players with no hamstring strain injury. The maximal speed, leg strength, ability to produce maximal power, endurance capacity, and hamstring flexibility was similar for both groups. Thus, a repeated sprint test consisting of 8 × 20 m could be used as a field-based diagnostic tool to identify players in need of reconditioning programs to ensure complete post-injury rehabilitation.

Our objective is to present the case of an uncommon but probably under-recognized cause of stroke: Non-bacterial thrombotic endocarditis (NBTE). A 59-year-old man presented to our hospital with multiple bihemispheric infarcts despite taking rivaroxaban for pulmonary emboli diagnosed 2 weeks earlier. The patient's symptoms progressed quickly and he died within a week of his initial presentation despite attempts at neuroradiologically guided clot retrieval and early recognition and treatment of disseminated intravascular coagulation. On postmortem examination it was discovered that he had an undiagnosed squamous cell adenocarcinoma of the lung and NBTE. NBTE is difficult to diagnose and difficult to treat. It is associated with a mortality rate and is often not diagnosed until autopsy. However there are case reports in the literature where NBTE has been successfully treated. Early recognition and prompt treatment of the underlying disease process is the essential first step.

Prior research shows that work on board vessels of the Royal Norwegian Navy (RNoN) is associated with noise exposure levels above recommended standards. Further, noise exposure has been found to impair cognitive performance in environmental, occupational, and experimental settings, although prior research in naval and maritime settings is sparse. The aim of this study was to evaluate cognitive performance after exposure to noise among personnel working on board vessels in the RNoN. Altogether 87 Navy personnel (80 men, 7 women; 31 ± 9 years) from 24 RNoN vessels were included. Noise exposure was recorded by personal noise dosimeters at a minimum of 4 h prior to testing, and categorized into 4 groups for the analysis: <72.6 dB(A), 72.6-77.0 dB(A), 77.1-85.2 dB(A), and >85.2 dB(A). The participants performed a visual attention test based on the Posner cue-target paradigm. Multivariable general linear model (GLM) analyses were performed to analyze whether noise exposure was associated with response time (RT) when adjusting for the covariates age, alertness, workload, noise exposure in test location, sleep the night before testing, use of hearing protection device (HPD), and percentage of errors. When adjusting for covariates, RT was significantly increased among personnel exposed to >85.2 dB(A) and 77.1-85.2 dB(A) compared to personnel exposed to <72.6 dB(A).
