RESUMO

Ectoine is an amino acid derivative and an important natural product in halophilic microorganisms. It plays an important role in protecting cells and stabilizing biological macromolecules, and can be widely used in biomedical fields such as drug preparation adjuvants, organ transplantation and preservation, skin wound repair and cosmetics. Due to the medical value and commercial market demand of ectoine, this article summarized the recent advances in the microbial production of ectoine, including the mutation and breeding of hyper-producing strains, construction of genetically and metabolically engineered strains, optimization of fermentation processes, and extraction and purification processes. The application of multi-omics technologies and computational biology to develop an ectoine producing cell factory was prospected, with the aim to provide a reference for ectoine overproduction.

RESUMO

The fixed number package management of medical consumables is an important content of SPD management mode. In the process of medical low-value consumables management, hospitals are generally faced with the problems of frequent access and low matching between push quantity and actual use quantity, resulting in complicated access operations of medical staff, shortage or hoarding of consumables and so on. In order to reduce the times of taking and using consumables and improve the comfort of medical staff, the authors dynamically adjusted the content and push quantity of fixed number packages according to the historical use data. The results showed that the adjusted fixed number setting could significantly reduce the average daily access times of medical staff, and reduce the probability of out of stock and hoarding.

RESUMO

Airway humidification is an important treatment for tracheotomy patients. At present, the commonly used methods of humidification are atomization inhalation, intra-tracheal drip, etc., but most of them have the disadvantages of interrupted humidification, inadequate humidification, repeated exposure of airway, increased nursing workload, etc. An improved disposable atomizer was designed by the emergency department of Jiaxing First Hospital in Zhejiang Province, which solved the above problems and obtained the National Utility Model Patent of China (ZL 2014 2 0406688.9). In the traditional atomizer, a make-up pipeline is added to run through the liquid container. The replenishing pipe is connected with an external infusion device. At the end of the pipeline inside the liquid container, a buoy with a guide rod is designed to continuously add liquid and automatically control the make-up speed. The device is driven by oxygen to perform airway humidification. The design can keep sufficient airway humidification, avoid frequent addition of humidification fluid, achieve the effect of increasing humidification, reducing the occurrence of complications, increasing the comfort of patients, and reducing the workload of nursing, and has a certain clinical value.

Objective@#To understand the current situation of human cytomegalovirus(HCMV) infection in local pregnant women and its impact on pregnancy outcomes, so as to provide reference for strengthening health care during pregnancy and promoting the policy of "fewer and better children"@*Methods@#A total of 2 000 pregnant women underwent prenatal examination who had delivery in the Maternal and Child Health Hospital of Huzhou from July 2017 to December 2018. HCMV IgM, IgG and IgG- avidity index (AI) were detected by enzyme-linked immunosorbent assay (ELISA), and HCMV DNA was detected by real-time polymerase chain reaction (PCR) -fluorescent probe. We also analyzed the positive status of HCMV and its relationship with pregnancy outcome.@*Results@#Among the 2 000 pregnant women, 1 941 cases (97.05%) were positive for HCMV IgG, 26 cases (1.30%) had active infection with HCMV, of whom 5 cases (0.25%) were serum HCMV IgM/IgG dual positive, 24 cases (1.20%) were urine HCMV DNA positive. HCMV IgG AI test result showed that 3 cases of AI ≤30%, which means that they had primary infections. The detection rates of HCMV DNA positive and HCMV IgM/IgG dual positive were 12.12%, 4.54% in 66 pregnant women with adverse pregnancy and 0.83%, 0.10% in 1 934 pregnant women with normal pregnancy. The difference was significant (χ2=68.663, 50.499, P<0.01). Logistic regression analysis showed that HCMV IgM/IgG dual positive (OR=12.743, 95%CI: 1.202-135.100, P=0.035), HCMV-DNA positive (OR=10.426, 95%CI: 3.635-29.909, P<0.01) were independent risk factors for adverse pregnancy outcomes. In addition, low education level, living in rural areas and having a history of adverse pregnancy also increased the incidence of adverse pregnancy outcomes.@*Conclusions@#HCMV infection is prevalent in pregnant women in this area, but only a few of them had active infection. HCMV active infection is a risk factor for adverse pregnancy outcomes.

In daily medical work, most of the critically ill patients who cannot move by themselves are pulled and lifted by manpower, often relying on the cooperation of many doctors and nurses, which not only increases the risk of transfer and patients' discomfort, but also causes certain skeletal and muscle damage to the porters. The emergency department of the First Hospital of Jiaxing City, Zhejiang Province designed a kind of patient transfer device, and obtained the National Utility Model Patent (ZL 2018 2 0579844.X). The transfer device is composed of upper frame, lower frame and base. The upper frame and the lower frame are rectangular and in a horizontal position. The upper frame can slide laterally through the circular tubes which are fixed on the lower frame. The lower part of the base is provided with four universal foot brake wheels. During the usage, the booster frame facilitates the transfer of patients by the rolling and two sliding tracks of the circular tube, which can make patients move smoothly and comfortably, and reduce the working intensity of the transporter. This device has good practical value.

Objective To investigate the distribution of methyleneterahydrofolate reductase (MTHFR) C677T gene polymorphisms and serum homocysteine (Hcy) levels in pregnant women. Method A total of 2 066 women with singleton pregnancies undergoing prenatal examinations in the Maternity and Child Health Care Hospital of Huzhou from January 2017 to October 2017 were recruited for the study.The MTHFR C677T genotype was detected by PCR-fluorescence probing, and the serum Hcy levels were detected by the cyclic enzyme method. According to the MTHFR C677T genotype detection analysis, the results were divided into the CC-type, CT-type, and TT-type groups. Statistical analyses were performed using the Hardy-Weinberg genetic equilibrium test, chi-square test, variance analysis, and t test. Result Among the 2066 pregnant women, the CC, CT, and TT genotype frequencies for the MTHFR C677T gene were 39.35%,46.52%,and 14.13%,respectively.The Cand T allele frequencies were 62.61% and 37.39%, respectively. The statistical significance values for the Shanxi, Zibo, Shanghai, Suzhou, Shangzhi, Xi'an, Huizhou, and Nanning areas were as follows: χ2=161.999, 166.083; 111.005, 112.517; 416.146, 441.245;14.262,14.23;36.368,35.871;199.498,204.771;19.641,16.377;and 66.79,61.593,respectively;P<0.05. The level of serum Hcy was(7.48±2.20)μmol/L,and the abnormal rate was 3.53%.The level of serum Hcy and the abnormal rate among the 3 genotypes were distributed as follows: TT>CT>CC. The difference was statistically significant(F=120.968, χ2=52.572, P=0.000). Conclusion The distribution of genotype frequencies for MTHFR C667T in 2 066 pregnant women was different in various geographical regions.The level of serum Hcy was associated with the MTHFR C677T gene. Observing MTHFR C677T gene polymorphisms and serum Hcy levels is helpful for monitoring and guiding folic acid supplementation during pregnancy,and establishing reasonable individual health preventive measures.It is an important method to improve the quality of life of the offspring and the family happiness index.

Objective To investigate the prevalence of thyroid diseases in women within 20-week gestation,and to discuss necessity of thyroid diseases screening in women within 20-week gestation.Methods 4986 singleton pregnant women performing prenatal examination in the Maternity and Child Health Care Hospital of Huzhou were recruited into this study.TSH,FT4,and TPOAb levels were detected with ABBOTT ARCHITECT i4000SR.Results ① The median value of TSH rised,the median value of FT4 went down with increase of the gestational weeks.There was significant difference of median value of TSH and FT4 between early and second trimester of pregnancy(P<0.01).② Rate of abnormal thyroid function was 12.88%,which was higher in the sec ond trimester of pregnancy than that in early pregnancy,and the difference had statistical significance (P<0.05).③ Positive rate of TPOAb in pregnant women was 11.41%.TPOAb levels were positively correlated with TSH,and no correlation with FT.TSH levels were negatively correlated with FT4.Prevalance of clinical hypothyroidism and subclinical hypothyroidism in TPOAb positive group was significantly higher than that in TPOAb negative group,and the difference had significant difference (P<0.05).Conclusions Prevalence of thyroid diseases during pregnancy is high.TPOAb is a risk factor and important predictor for hypothyroidism during pregnancy.It is very necessary to perform thyroid diseases screening for pregnant women as early as possible to ensure safety of pregnant women and their babies.

RESUMO

[Objective]To compare and contrast the dosimetry between Tomo planning and BrainLab planning for lung metasta-ses in stereotactic body radiation therapy(SBRT).[Methods]Four Patients with one,two,three and four metastases were selected. The PTV is 2.89 ± 1.15 cm3. Two plannings with total dose of 50 Gy to cover 95% of PTV ,5 Gy/Fraction and 10 fractions were designed using Tomo planning system and BrainLab planning system respectively. The DVH curves of spinal cord ,both lungs and normal tissue were compared. The conformity index andhomogeneityindex were analyzed as well.[Results]The homogeneity index (HI)and conformity index(CI)of the targets in Tomo planning system were 1.0314 ± 0.0700 and 0.687 ± 0.075,respectively. In BrainLab planning system the HI and CI of the targets were 1.0764 ± 0.1241 and 0.571 ± 0.042,respectively. To HI the P value in T test was less than 0.01 and the HI was better in Tomo than BrainLab and so was CI. The dose to spinal cord was higher in BrainLab planning system than that in Tomo. The dose to nomal tissue and both lungs were not different in the two planning systems and V20 of lung is as small as 10%.[Conclusions]For small volume lung metastases which longest diameter were less than 4 cm,the tomotherapy should be better choice.

Objective To investigate vitamin D nutrition of children aging 0-6 years who received routine physical checkup in outpatient clinic,and to provide evidence for supplementation of vitamin D.Methods A total of 3 724 children were enrolled in children healthcare outpatient clinic in the Maternity and Child Health Care Hospital of Huzhou from January 2013 to March 2014.Serum 25-hydroxy vitamin D [25-(OH) D] levels of the children were measured using electrochemiluminescence to compare serum 25-(OH) D levels and vitamin D deficiency in children of different ages and in seasons.Results The mean 25-(OH) D level in these children was (37.80 ± 11.67) ng/ml,and the prevalence of vitamin D deficiency was 7.33%,neither showing significant difference between boys and girls (t =-1.57,P =0.117 ; x2 = 0.37,P =0.543).The serum 25-(OH) D level in >3-and-≤6 years group was remarkably lower than the levels in ≤1 year,> 1-and-≤2 years and >2-and-≤3 years groups [(27.36 ±8.90) ng/ml vs.(38.64 ± 12.10) ng/ml,t =18.60,P=0.000;vs.(41.63±10.31) ng/ml,t=26.07,P=0.000;vs.(36.85±10.01) ng/ml,t=16.42,P =0.000],and the prevalence of vitamin D deficiency significanl higher than those in the other 3 age groups (104/466 vs.122/1 487,x2 =69.06,P =0.000; vs.18/1 106,x2 =196.06,P =0.000; vs.29/665,x2 =85.14,P =0.000).The serum 25-(OH) D level in children in winter was significantly lower than in spring,summer,and autumn [(35.16 ± 11.30) ng/ml vs.(40.03 ± 12.57) ng/ml,t =9.15,P =0.000;vs.(36.86±10.60) ng/ml,t=3.34,P=0.001; vs.(39.99±11.36) ng/ml,t=9.65,P=0.000],and the prevalence of vitamin D deficiency significantly higher than in the other 3 seasons (127/1 189 vs.56/ 849,x2 =10.11,P =0.001; vs.54/787,x2 =28.30,P=0.004; vs.36/899,x2 =31.71,P=0.000).Conclusions Vitamin D nutrition status of infants at 0-6 years in our child healthcare outpatient clinic is generally good,except for lower vitamin D level and higher prevalence of vitamin D deficiency in winter and >3-and-≤6 years age group.Therefore,vitamin D supplementation in winter and in children above 3 years old is especially important.Recommendations include more outdoor activities and public education for scientific prevention of vitamin D deficiency.

RESUMO

[Objective]To discuss the difference of mechanical stability in using different fixation methods in treating radius and ulna fracture,in order to provide a theoretical basis for clinical applications.[Method]In the experiment,15 pieces of fresh radius and ulna were used to produce middle-part transverse fracture models(A3.2)which were fixed by three different methods(double plate fixation group,double-intramedullary nail fixation group,ulnar intramedullary nailing + radial plate group).Through biomechanical tests,axial stiffness,bending stiffness,torsional stiffness and the equivalent stiffness were compared.[Result]There were no difference between double-plate group and ulnar intramedullary nailing + radial plate group,the stability in these two groups were higher than that in double-intramedullary nail fixation group.[Conclusion]Ulnar intramedullary nailing + radial plate has a good biomechanical property,it can provide firm fixation and with less trauma.So it is a good fixation.
