RESUMO

The examination of 123 former prisoners with pulmonary tuberculosis treated in the Republican Tuberculous Hospital in 1998-2000 versus 37 control patients demonstrated that the specific pulmonary process is revealed at younger age (20-39 years). The treatment effect in former prisoners by percentage of cavern closure is 1.8 times worse than in the controls. This can be explained by significantly higher occurrence of cases with drug-resistant M. tuberculosis and associated diseases.

RESUMO

The outcomes of surgical treatment were analyzed in 264 new cases of restrictive pulmonary tuberculosis subjected to pulmonectomy without pretreatment with drugs in the preoperative period. Sixty one patients who had refused to have surgery or operated on after treatment were as a control. Indications for such policy were defined. These included limited tuberculomas and cavernous tuberculosis without local seeding, perifocal infiltration, exacerbations and preserved cellular and humoral immunity and non-specific protection factors. The outcomes of surgical interventions in the experimental group were slightly better. They had fewer complications, rapid recovery of working capacity, low per cent of disability than patients operated on after preoperative chemotherapy. There was no increase in the incidence of relapses.

RESUMO

Changes of hemocoagulation were studied in three groups of patients in different periods of surgical intervention for various forms of pulmonary tuberculosis. Mini-doses of heparin were included in the complex of preventive measures applied during the operation in group 3. It was found out that changes of blood coagulation during operation on phthisiosurgical patients develop according to the mechanism of the thrombohemorrhagic syndrome (THS). The more injurious the operation, the more manifest are the changes in the system of blood coagulation and fibrinolysis. Heparin prevents intravascular coagulation of blood, arrest the development of THS and, in this manner, the occurrence of thrombohemorrhagic complications.
