Business writing: a foolproof system for getting started.

Writing well is a valuable skill, but one often lacking among higher and lower level employees. Most people recognize good writing when they see it, but are at a loss to define exactly what it is. I define it as writing that's easy to follow and understand.

[Management of hallux varus]. / A hallux varus és kezelése.

Authors describe the congenital and acquired forms of the rarely occurring hallux varus on the basis of the material of 14 patients, treated in 1951-87 on the Orthopaedic Department of the Semmelweis University Budapest. In 11 cases the deformity was congenital and in the majority bilateral, multiple, together with other foot deformities.

Authors have evaluated the results in patients operated for coxe saltans in 1982-1986. The etio-pathogenesis, the clinical features, the therapy, the results of operation are reported and shortly reviewed, based on the data of their 42 patients.

Regulation of serum antibodies induced in syngeneic rats after administration of monoclonal antibody MT10:21.

Monoclonal antibody (mAb) MT10:21, a rat IgG2a, reacts with antigens expressed on the metastatic subclone MTLn3 of the 13762NF rat mammary adrenocarcinoma (North, Steck & Nicolson, 1986). The clearance of mAb MT10:21 from circulation was monitored 15 days after s.c. injection of MTLn3 tumour cells into the mammary fat pad of syngeneic rats. At this point, when tumour-burden was small (< 1 cm average diameter), mAb MT10:21 was injected i.v. and serum samples were taken over the 7 days following injection. It was observed that when mAb MT10:21 was injected i.v. into syngeneic rats it induced a humoral immune response. Four days after injection of mAb, IgM serum antibodies which bound to the MTLn3 cell line were detected in both tumour and non-tumour bearing rats. Using a binding assay, tumour-bearer sera showed a steady increase in MTLn3-reactive antibodies over the 7-day assay period. These MTLn3-reactive antibodies were detectable in non-tumour-bearer sera, but reactivity was not as pronounced. Tumour-bearing rats also had serum IgM antibodies which bound to mAb MT10:21 in vitro, but not to a non-specific, isotype-identical control mAb, MC9:13. These serum antibodies were able to partially inhibit (up to 47%) the binding of 125I-labelled mAb MT10:21 to MTLn3 cells. This anti-idiotypic immune response was not observed in the non-tumour bearers. When the tumour was allowed to grow for a period of 30 days in vivo (average tumour diameter 2 cm), these serum antibodies were not readily detectable, suggesting that tumour burden had a significant effect on suppressing the humoral immune responsiveness of these tumour-bearing rats. In a standard delayed-type hypersensitivity (DTH) assay specific immunity to mAb MT10:21 was induced in vivo.

Results of our operations for carpal ganglion.

Authors surveyed the material of patients operated for carpal ganglion at the Department of Orthopaedics of Semmelweis University Medical School, in 1952-1986. A brief survey is given of the aetiology, treatment of the disease, of their own material and of the results of surgery of 112 patients. It was found that operation was performed in 5-6% of the cases for recurrences which seems to be an acceptable ratio. Only minimal residual complaints occurred, the patients working capacity was not reduced, they had good wrist function. Patients were satisfied with the operation.

Joint occurrence of aseptic necrosis of the head of the third metacarpal and Freiberg's disease.

Freiberg's disease of the second metatarsal was found together with the aseptic necrosis of the head of the third metacarpal in a 54-year-old female patient. No similar case was found in the available literature. The deformity of the second metatarsophalangeal joint was corrected with an operation, the alteration of the third metacarpal did not need operative correction.

The pathologic effect of ureteral ligation, with clinical implications.

Inadvertent, ureteral ligation is a frequent occurrence with pelvic surgery. We determined the effect of the ureteral ligation of varying lengths of time in an animal model in order to provide a rational basis for the clinical management of such injuries in humans. In the rat, simple deligation of a ureter that has been ligated for as long as 24 hours results in complete resolution of the injury with no gross or microscopic evidence of the trauma. After 48 and 72 hours of ligation, hydroureter was apparent.

Associated anomalies of mullerian and wolffian duct structures.

Over a ten-year period, we identified 457 patients with renal or uterine anomalies. Only 12 of 55 patients with anomalies of mullerian development had an intravenous pyelogram, which revealed an associated renal anomaly in five. Conversely, only two of 21 women with renal anomalies had a gynecologic evaluation; one of them had a bicornuate uterus. Although the association between anomalies of mullerian and wolffian development has been known for some time, the appropriate studies to demonstrate such an association are not routinely done in clinical practice. All female patients with congenital renal anomalies of size, position, or number should have either ultrasonography (in children) or hysterosalpingography (in adults). All patients with congenital uterine or vaginal anomalies should have an intravenous pyelogram.

[Evaluation of operations on discoid meniscus]. / Discoid meniscus miatt végzett mütéteink értékelése.

A short survey of literature is given in the article. 1480 meniscectomies were performed at the Orthopedic Clinic of Semmelweis Medical University in the interval between 1952--75; 29 of these cases were carried out for discoid menisci. Symptoms are introduced and results of operative treatment are evaluated. The author makes a few suggestions based on his own experiences.
