There is an increasing interest in the microbiota of the dairy value chain, from field to fork. Studies to understand the effects of environmental, feed and management factors on the raw milk microbiota have been performed to elucidate the origin of the bacteria and find ways to control the presence or absence of specific bacteria. In this study, we explored the microbiota in feedstuff, bedding material and milk on a Swedish dairy farm to investigate the effects of feeding different silages on the bacterial compositions throughout the dairy value chain. Three ensiling treatments were evaluated: without additive, with acid treatment, and with inoculation of starter culture. The silage treatments were fed as partial mixed rations to 67 dairy cows for 3 weeks each, with one treatment fed twice to evaluate if a potential change in milk microbiota could be repeated. The highest average total bacteria counts were found in the used bedding material (9.6 log10 cfu/g), while milk showed the lowest (3.5 log10 cfu/g). Principal coordinate analysis of the weighted UniFrac distance matrix showed clear separation between 3 clusters of materials: 1) herbage, 2) silage and partial mixed ration, and 3) used bedding material and milk. Surprisingly, the expected effect of the ensiling treatments on silage microbiota was not clear. Transfer of major bacteria from the silages and resulting partial mixed rations to the used bedding material was observed, but rarely to milk. The milk microbiota showed most resemblance to that of the used bedding material. Lactobacillus was a major genus in both feed and milk, but investigations at amplicon sequence variant level showed that in most cases the sequences differed between materials. However, low total bacteria count in the milk in combination with a high diversity suggests that results may be biased due to environmental contamination of the milk samples. Considering that the study was performed on a research farm, strict hygienic measures during the feeding experiment may have contributed to the low transfer of bacteria from feed to milk.

The objective of the current study was to evaluate feed intake prediction models of varying complexity using individual observations of lactating cows subjected to experimental dietary treatments in periodic sequences (i.e., change-over trials). Observed or previous period animal data were combined with the current period feed data in the evaluations of the different feed intake prediction models. This would illustrate the situation and amount of available data when formulating rations for dairy cows in practice and test the robustness of the models when milk yield is used in feed intake predictions. The models to be evaluated in the current study were chosen based on the input data required in the models and the applicability to Nordic conditions. A data set comprising 2,161 total individual observations was constructed from 24 trials conducted at research barns in Denmark, Finland, Norway, and Sweden. Prediction models were evaluated by residual analysis using mixed and simple model regression. Great variation in animal and feed factors was observed in the data set, with ranges in total dry matter intake (DMI) from 10.4 to 30.8kg/d, forage DMI from 4.1 to 23.0kg/d, and milk yield from 8.4 to 51.1kg/d. The mean biases of DMI predictions for the National Research Council, the Cornell Net Carbohydrate and Protein System, the British, Finnish, and Scandinavian models were -1.71, 0.67, 2.80, 0.83, -0.60kg/d with prediction errors of 2.33, 1.71, 3.19, 1.62, and 2.03kg/d, respectively, when observed milk yield was used in the predictions. The performance of the models were ranked the same, using either mixed or simple model regression analysis, but generally the random contribution to the prediction error increased with simple rather than mixed model regression analysis. The prediction error of all models was generally greater when using previous period data compared with the observed milk yield. When the average milk yield over all periods was used in the predictions of feed intake, the increase in prediction error of all models was generally less than when compared with previous period animal data combined with current feed data. Milk yield as a model input in intake predictions can be substantially affected by current dietary factors. Milk yield can be used as model input when formulating rations aiming to sustain a given milk yield, but can generate large errors in estimates of future feed intake and milk production if the economically optimal diet deviates from the current diet.

The aim of this study was to assess the effects of a novel antidepressant, vortioxetine 10 mg, on driving, cognitive, and psychomotor performance in 24 healthy subjects in a double-blind, placebo-controlled, three-way crossover design. Mirtazapine 30 mg was included as an active comparator. Drugs were administered in the evening of 15 consecutive days. Performance was measured in the morning of days 2 and 16, using standardized tests measuring on-the-road driving, memory, tracking, divided attention, and vigilance. The statistical analysis on the primary measure of driving, i.e., SD of lateral position showed noninferiority of vortioxetine on days 2 and 16, and inferiority for mirtazapine on day 2. Vortioxetine did not cause cognitive or psychomotor impairment. Mirtazapine, however, impaired cognitive and psychomotor performance on day 2. Most of these effects disappeared after multiple doses of mirtazapine. To conclude, vortioxetine did not impair driving, cognitive, or psychomotor performance after single or multiple doses.

Phytoestrogens are hormone-like substances in plants that can substantially influence human health (positively or negatively), and when fed to dairy cows are partly transferred to their milk. The aim of this study was to investigate the effects of varying the botanical composition and regrowth interval of legume-grass silage on phytoestrogen intake and milk phytoestrogen concentrations. In one experiment, 15 Swedish Red dairy cows were fed 2- or 3-cut red clover-grass silage, or 2-cut birdsfoot trefoil-grass silage. In a second experiment, 16 Norwegian Red dairy cows were fed short-term ley silage with red clover or long-term ley silage with white clover, and the effects of supplementation with α-tocopherol were also tested. High concentrations of formononetin and biochanin A were found in all silage mixtures with red clover. The milk concentration of equol was highest for cows on the 2-cut red clover-grass silage diet (1,494 µg/kg of milk). Because of the metabolism of biochanin A, genistein, and prunetin, their concentrations in milk and the apparent recovery were low. Coumestrol was detected in only short-term and long-term ley silage mixtures, and its milk concentration was low. Concentrations of secoisolariciresinol and matairesinol were higher in 2-cut birdsfoot trefoil-grass and long-term ley silage mixtures, those with legume species other than red clover, and the highest grass proportions. The 2-cut birdsfoot trefoil-grass silage diet also resulted in higher enterolactone concentration than the other diets (226 µg/kg of milk). Lengthening the regrowth interval increased the intake of secoisolariciresinol and decreased the recovery of lignans. Feeding long-term ley silage resulted in higher milk lignan concentrations but lower milk isoflavone concentrations than feeding short-term ley silage. The apparent recovery of all phytoestrogens except prunetin was highest on the 2-cut birdsfoot trefoil-grass silage diet. No effect of α-tocopherol supplementation was observed on milk concentrations of any of the measured phytoestrogens. Variations were observed in milk concentrations of phytoestrogens, especially of equol, among cows, which could not be explained by variations in diet composition or phytoestrogen intake. The results show that milk phytoestrogen concentration is strongly influenced by silage botanical composition, but questions regarding phytoestrogen metabolism remain to be answered.

INTRODUCTION: The aim of this study was to assess the extent to which GI endoscopic treatment of bleeding gastroduodenal ulcers is effective and whether it could reduce the need for surgery. METHODS: A retrospective evaluation was carried out. Over a period of three years (1995-1997), 122 patients with bleeding caused by a gastroduodenal ulcer, defined as fresh haematemesis with or without melaena, old haematemesis with melaena or melaena accompanied by a systolic blood pressure below 80 mmHg, underwent emergency GI endoscopy. Endoscopic therapy consisted of either injection therapy or electrocoagulation. RESULTS: Altogether 72 patients received endoscopic treatment. Primary haemostasis was achieved in 94%. Definitive haemostasis after one endoscopic treatment with either injection therapy or electrocoagulation was achieved in 63% and 62%, respectively. Including repeated endoscopic treatment, definitive haemostasis was achieved in 79%. No direct complications of the treatment were observed. DISCUSSION: Endoscopic treatment of bleeding gastroduodenal ulcers is the only alternative to an operation. Endoscopic treatment can reduce the need for surgery, as it was effective in about 80% of the patients.

The aim of the study was to estimate the value of CT in the diagnosis of abdominal wall hernias and at the same time to create a standard for this CT investigation. Twenty-four patients with suspected hernia of the abdominal wall were examined. All were operated on. The CT scans were assessed by two radiologists to estimate the interobserver variation. The CT diagnoses made by the two radiologists were correct in 83 % and 79 % of cases, respectively. The sensitivity was 0.83 in both CT evaluations and the specificity was 0.83 and 0.67, respectively. The predictive value of a positive CT finding was 0.94 and 0.88, while the predictive value of a negative CT finding was 0.63 and 0.57, respectively. The interobserver variation (kappa) was 0.87. The study therefore indicates that a positive CT finding of abdominal wall hernia is reliable, while a negative finding does not exclude the diagnosis. The interobserver variation of the CT diagnoses is acceptable. To achieve the highest diagnostic accuracy, it is recommended to always use the Valsalva manoeuvre, oral intake of contrast and 10/10 mm CT slices.
