RESUMO

This study assessed the validity of insulated skin temperature (Tis) to predict rectal temperature (Tre) for use as a non-invasive measurement of thermal strain to reduce the risk of heat illness for emergency service personnel. Volunteers from the Police, Fire and Rescue, and Ambulance Services performed role-related tasks in hot (30 °C) and neutral (18 °C) conditions, wearing service specific personal protective equipment. Insulated skin temperature and micro climate temperature (Tmc) predicted Tre with an adjusted r(2) = 0.87 and standard error of the estimate (SEE) of 0.19 °C. A bootstrap validation of the equation resulted in an adjusted r(2) = 0.85 and SEE = 0.20 °C. Taking into account the 0.20 °C error, the prediction of Tre resulted in a sensitivity and specificity of 100% and 91%, respectively. Insulated skin temperature and Tmc can be used in a model to predict Tre in emergency service personnel wearing CBRN protective clothing with an SEE of 0.2 °C. However, the model is only valid for Tis over 36.5 °C, above which thermal stability is reached between the core and the skin.

OBJECTIVE: This study aimed to assess the validity and reliability of the 3DNX triaxial accelerometer in a laboratory and mechanical setting. METHODS: Experiment 1: A total of 10 units were fixed to a Multi-axis shaker table. A schedule comprising a range of accelerations was repeated on two occasions along each of the three measurement axes. Experiment 2: A total of 11 recreationally active individuals completed a treadmill protocol (4-20 km hr(-1)) on two occasions. Two 3DNX accelerometers were secured to the lower back, logging data every 5 s. Oxygen uptake was measured using the Douglas Bag technique. Reliability inter- and intra-units were assessed using a combination of limits of agreement (LoA), coefficient of variation (CV) and intra-class correlation coefficient (ICC). Validity was assessed using simple linear regression. All data are expressed in counts per 5 s (counts 5 s(-1)). RESULTS: Experiment 1: CV(intra) ranged from 0.0 to 8.9% in all axes. The absolute bias±95% LoA values were all below four counts 5 s(-1). CV(inter) did not rise above 4.5%. ICCs were 1.0 for all stages in all axes. The relationship between 3DNX counts and acceleration yielded an R(2) value of 0.99 and a standard error of the estimate (SEE) of six counts 5 s(-1). Experiment 2: CV(inter) ranged from 7.7 to 16.0% for trial 1 and from 7.7 to 16.2% for trial 2. ICCs between units ranged from 0.95 to 1.00 for trial 1 and from 0.90 to 0.99 for trial 2. Mean ratio bias±95% LoA values for intra-unit and inter-unit reliability were -0.7% (P>0.05)±12.4% and 0.4% (P>0.05)±5.6%, respectively. The relationship between VO(2) and 3DNX counts for walking and running was linear (R(2)=0.65, SEE=1.42 ml kg(-1) min(-1); R(2)=0.62, SEE=3.63 ml kg(-1) min(-1)). CONCLUSION: The 3DNX accelerometer is a reliable and valid device for measuring acceleration in a mechanical setting and during human treadmill exercise.

Over two decades ago it was predicted that nonlinear interactions between thermally driven fluctuations in dissipative nonlinear nonequilibrium systems lead to deviations from mean-field theory. Here we report experimental observations of such deviations as a supercritical primary bifurcation is approached. We measured the mean-square director-angle fluctuations below the bifurcation to electroconvection of two different nematic liquid crystals. For epsilon(mf) identical withV2/V(2)(c,mf)-1 less, similar-0.1 ( V is the applied voltage) we find approximately |epsilon(mf)|(-gamma) with gamma given by linear theory (LT). Closer to the bifurcation there are deviations from LT with a smaller gamma and with V(2)(c)>V(2)(c,mf).

Herpes virus was recovered from a throat swab, nasopharyngeal washings as well as from brain tissue from a six-month-old infant, who presented with fever, left focal seizures and an enhancing right frontal CT-scan lesion. Cytopathic effect (CPE) as seen with genital herpetic infection was seen, suggesting HSV-2. Immunofluorescent typing of the virus isolate confirmed HSV-2. Early IgM positivity preceding a CF and SN titer rise was observed. The patient received a course of ARA-A and recovered with a left sided hemiparesis. HSV-2 encephalitis occurs beyond the newborn period as a primary infection. In adults however HSV-2 encephalitis occurs predominantly in the immunocompromised host. HSV-1 encephalitis probably is due to reactivation of a latent herpetic infection in previous virus exposed juvenile or adult immunocompetent hosts.

Cavum vergae is a cavity posterior to the septum pellucidum whose clinical significance has previously been unclear. Ten children are described who presented with neurologic findings and were found to have a cavum vergae or cavum vergae and cavum septum pellucidum by brain computed tomographic (CT) scan. Of the ten patients, five had delayed development, four had macrocephaly, two had learning disabilities, two had abnormal electroencephalograms, and one had Apert's syndrome. No cases of cavum vergae were found in 50 children who had brain CT scans for head trauma. These observations suggest that cavum vergae is a nonspecific brain CT scan finding that can be associated with neurologic abnormality. The results of magnetic resonance imaging of one patient with cavum vergae are also described.

Investigations of the energy-dependent accumulation of orthophosphate by the blue-green alga Anacystis nidulans have established: 1. The transport through the cell membrane is the rate-limiting step in the incorporation of phosphate.-2. This transport is facilitated by a "carrier" that can be activated by Ca(2+) and Mg(2+) and inhibited by EDTA.-3. The activation of the carrier in the light is associated with changes of the cytoplasmic Mg(2+) content.-4. Intracellular phosphate is shown to be present in bound form.-5. The energy-dependent accumulation of orthophosphate within the cell depends strictly on the cytoplasmic pH and not on the energy conversion at the thylakoid membrane which is responsible for the energy supply. The cytoplasmic pH is different in the light, in the dark, and in the presence of the uncoupler carbonyl cyanide m-chlorophenylhydrazone (CCCP). Orthophosphate accumulation can most readily be explained in terms of a pH dependent precipitation into a complex with bivalent cations rather than by an active transport against a concentration gradient.

RESUMO

Elevated serum gamma-glutamyl transferase (GGT) levels were found in 29% of 155 chronic alcoholics undergoing detoxification treatment. Alkaline phosphatase (AP) was increased in 16%, alanine aminotransferase--SGPT (ALT) in 12% of the patients, while 23% had elevations of either AP or ALT or both. Of the foregoing parameters, GGT was the best single indicator of liver involvement. In course of the follow-ups GGT/ALT correlation improved, but GGT/AP correlation deteriorated. In 9 patients, abstinence during follow-up was associated with progressive decrease in previously elevated serum GGT. Because of its unique sensitivity, GGT may be useful as a screening and/or monitoring aid in alcoholism.

RESUMO

A 9-month-old boy developed encephalopathy associated with the noset of erythema infectiosum, which resulted in permanent neurologic sequelae. This is the second reported case of encephalitis following erythema infectiosum and the first, to our knowledge with permanent sequelae.

The pH in the cytoplasmic and thylakoid spaces of the blue-green alga, Anacystis nidulans, has been determined in the light and in the dark by uptake of 5,5-dimethyloxazolidine-2,4-dione and methylamine into the sucrose-impermeable (3)H-H(2)O space, as measured by silicon layer filtering centrifugation.Illumination causes an alkalinization in the cytoplasm which is accompanied by an acidification in the thylakoid space, reflecting light-dependent proton transport across the thylakoid membrane. Under light conditions, a pH gradient of approximately 2.8 between the cytoplasmic and thylakoid spaces has been measured that can be abolished almost completely by addition of the uncoupler, 3-chlorocarbonyl cyanide phenylhydrazone. The pH in the cytoplasm is independent of the pH in the medium.

A case of a rare acquired form of aqueductal stenosis and hydrocephalus developing as a late sequela of mumps virus infection of the central nervous system is presented. The experimental studies of mumps induced aqueductal stenosis in hamsters and mice and the four previously reported cases in the literature are reviewed.

RESUMO

Malignant atrophic papulosis (Kohlmeier-Degos disease) is reported for the first time with pathologic verification of central nervous system involvement in a child. The disease began in infancy with rare recurring skin lesions; the child died at the age of 7, after progressive neurologic deterioration. Diagnosis was suspected clinically and confirmed by biopsy of a typical skin lesion. Recognition of this disorder by its dermatologic manifestations may obviate invasive diagnostic procedures.
