BACKGROUND: From the point of view of the pandemic officer of a 300-bed specialty care hospital, the first year of the SARS-CoV­2 (Severe Acute Respiratory Syndrome Coronavirus Type 2) pandemic with all the challenges for daily clinical routine was reviewed on the basis of the data collected with the aim of making future pandemic management easier to plan. METHODS: The Caritas Hospital St. Josef in Regensburg employs around 1330 employees and treats almost 20,000 inpatients annually. All records of the pandemic officer between 1 March 2020 and 28 February 2021 were included in the present descriptive study. RESULTS: In all, 280 SARS-CoV-2-positive patients (mean age 68 ± 19 years) were treated during the observation period; 81% (n = 227) of these were treated as inpatients between November 2020 and February 2021. In addition to the analysis of these patients, an analysis of the test concept was carried out, both for the patients (53% of the tests in the so-called 2nd wave) and for the employees; 1633 PCR tests were carried out, with 5.6% of all employees being infected with SARS-CoV­2 in the course of the pandemic. In addition, other measures such as reducing the surgical program and other general measures were evaluated. CONCLUSION: Many established processes had to be changed in the hospital during the pandemic. Both staff shortages due to SARS-CoV-2-positive employees and the quarantine measures of contact persons led to further tension in the already tight staffing levels, especially in the nursing care sector, and made patient care more difficult. The test concept in the hospital was very complex and asymptomatic tests in particular showed a low hit rate, while being associated with high personnel costs.

BACKGROUND: Abdominal pain is one of the most common symptoms of patients who present to the emergency department (ED). The aim of this study was to collect current epidemiological data, the frequency of these findings, and the measures that derived from them. METHODS: We performed a retrospective analysis in the period between January 1 and June 30, 2016, including all patients who presented to the ED of the Caritas Hospital St. Josef in Regensburg (teaching hospital with an academic urology and gynecology department, about 32,000 patients in the ED per year) and were categorized as "abdominal pain in adults" using the Manchester Triage System (MTS). RESULTS: The study population consisted of 1417 patients (9.8% of all ED patients). The admission rate was 48.2%. Vomiting and fever as concomitant symptoms made hospitalization more likely (p = 0.00). Almost half of the patients had nonspecific abdominal pain (28.2%), gynecological causes (13.2%), or suspected acute appendicitis (6.7%). In all, 10% of patients received an abdominal CT investigation; 73% of the patients presented in the time from 08:00-20:00 h, and more frequently on weekdays (74.2%). Of these patients, 6.4% returned after discharge because of persisting or worsening symptoms. Finally, 58.6% of outpatients and 77.5% of inpatients received further treatment recommendations for new pharmacologic therapy (e.g., analgesics, proton pump inhibitors, antibiotics). CONCLUSION: A variety of epidemiological data of our collective could be analyzed, which should be transferable to many other German EDs. The MTS shows a high reliability in terms of conversion rate in abdominal pain; despite comprehensive emergency diagnostics, a relevant proportion of complaints remain unclear.

BACKGROUND: The clinical manifestation of COVID-19 is nonspecific and varies greatly, which makes it more difficult to discriminate from other (virus) infections. Neither individual findings nor combinations of findings are specific enough to be able to diagnose COVID-19 with a high degree of certainty. The goal was to identify patients in the emergency department, who are at risk for COVID-19 disease, early by using a score, so that they could be isolated pre-emptively. METHOD: Development and implementation of a symptom-based COVID-19 score based on a multicentric retrospective evaluation in three German emergency departments from 9 March until 30 April 2020 of patients suspected of having COVID-19 and subsequent SARS-CoV­2 PCR testing. RESULTS: The study population included 697 patients and 9.4% of these patients were diagnosed with COVID-19 infection. A COVID-19 score of ≥5 points was associated with a significantly increased likelihood of illness. The sensitivity of the score was 98.4% with a moderate specificity of 48.3%. DISCUSSION: The score, which is easy to obtain during the initial assessment, supports the assessment of the pretest probability for a COVID-19 infection as part of the risk stratification and can influence the treatment pathway in terms of pre-emptive isolation, PCR testing and other treatment options at an early stage. Due to the nonspecific symptoms of the disease; however, it must be accepted that the goal of high sensitivity results in a relatively low specificity of the score.

RESUMO

The topic of acute dyspnea is presented in two separate articles. This first part deals with the definition and pathophysiology of dyspnea as well as important considerations on the history of the present illness, physical examination, initial therapy and differential diagnostic considerations. The second part covers relevant diagnostic investigations and principles for the initial management. The causes, consequences and perception of acute dyspnea can be very different. The adult patient with acute dyspnea presents difficult challenges in the diagnosis and management. The emergency clinician must work through a wide range of differential diagnostic considerations while providing appropriate initial treatment for a potentially life-threatening disease.

RESUMO

This article presents the relevant diagnostic examinations and principles for the initial management of acute dyspnea in detail. The emergency physician must work through broad differential diagnostic considerations while providing appropriate initial treatment for a potentially life-threatening disease. The airway, breathing and circulation are the primary focus for the emergency physician when beginning emergency management. As soon as these are stabilized, further clinical investigations and treatment can be continued. The appropriate place for further treatment is determined by risk stratification.

OBJECTIVES: Viral blips are thought to represent random biological variations around a steady state of residual HIV viraemia and to lack clinical significance. We aimed to assess the association of immune activation and the occurrence of blips. METHODS: HIV-infected patients from our out-patient cohort who developed a blip after having been on fully suppressive highly active antiretroviral therapy (HAART) for at least 180 days were matched with patients without blips according to duration of complete viral suppression (CVS), age, sex and Centers for Disease Control and Prevention (CDC) stage. Frequencies of CD3(+), CD3(+) CD4(+), CD3(+) CD8(+), CD3(+) HLA-DR(+), CD4(+) CD45RA(+), CD16(+) CD56(+) CD3(-) and CD19(+) cells, as well as C-reactive protein (CRP) levels and clinical parameters, were included in conditional logistic regression models. Adherence to HAART was assessed by measuring prescribed nonnucleoside reverse transcriptase inhibitor (NNRTI) or protease inhibitor (PI) plasma levels in a sample of 57 patients. RESULTS: Eighty-two patients with viral blip were matched with 82 controls from the same cohort. The mean age was 47.2 years [standard deviation (SD) 12.1 years], 80.5% of patients were male and 42.7% had CDC stage C disease. Viral blips occurred after a median of 14 months [interquartile range (IQR) 8-34 months] of CVS. In the logistic regression, activated CD3(+) HLA-DR(+) lymphocytes [odds ratio (OR) 1.25 per 100 cells/µL; 95% confidence interval (CI) 1.02-1.54; P = 0.03] were significantly associated with blips and there was a trend for an association of longer time on HAART with blips (OR 1.31 per year; 95% CI 0.96-1.78; P = 0.09). No between-group difference regarding subtherapeutic drug levels was found (P = 0.46). CONCLUSIONS: The occurrence of viral blips after suppressive HAART was associated with elevated markers of T-cell activation. Blips may identify a subset of patients with higher immune activation and increased risk for HIV disease progression.

The detailed investigation of a methanolic extract of aerial parts of Achillea pannonica SCHEELE. within a chemotaxonomic study led to the isolation of 6 flavonoid glycosides. Besides rutin, apigenin-7-O-glucopyranoside, luteolin-7-O-glucopyranoside, apigenin-7-O-rutinoside and acacetin-7-O-rutinoside, an unusual flavondiglucoside was isolated. Its structure was established by UV, 1H NMR and 13C NMR spectroscopic methods including 2D-NMR techniques and ESI-MS as luteolin-7,4'-O-beta-diglucoside. This substance is reported for the first time in the genus Achillea. Chemotaxonomic aspects are discussed briefly.
