RESUMO

A new automatic blood-pressure (BP) measuring device TONOPORT V was evaluated according to the International Protocol for Validation of Blood Pressure Measuring Devices in adults by the European Society of Hypertension. BP values measured by the TONOPORT V were compared to BP readings from two independent observers. A total of 33 patients (20 males, 13 females) provided systolic and diastolic BP readings in the normotensive, borderline hypertensive, and hypertensive range. Their age varied between 30 and 83 years, and their arm circumference between 23 and 36 cm. The device showed a mean (+/-s.d.) deviation from observer measurements of -0.7 (4.6) mmHg for systolic and -0.8 (4.4) mmHg for diastolic BP. The accuracy of the device did not vary according to BP values or other patient characteristics. The device passed all phases of the protocol and can be recommended following the regulation rules of the European Society of Hypertension.

The pathogenesis of endometriosis is dependent on implantation. Therefore, angiogenesis of the ectopic endometrium is discussed as a crucial step in the development of an endometriotic lesion. It is not only the main condition for implantation, but also a parameter for the clinical activity of endometriosis. Pigmented and non-pigmented endometriotic lesions differ according to their clinical activity and angiogenic potential. Using a model system of endometriosis, the chorioallantoic membrane assay, mRNA expression of VEGF and matrix metalloproteinases 1 and 2 was analysed. As the most representative angiogenic factors, they were correlated to the angiogenic reaction on the CAM by the vessel index. We assume that angiogenic activity of ectopic endometrium is a condition sine qua non for the pathogenesis of endometriosis, but is regulated by different other factors.

The prevalence, clinical presentation, and risk factors for hyperlactatemia among patients receiving antiretroviral therapy was determined during a 1-month period for patients in the Swiss HIV Cohort Study. Overall, 73 (8.3%) of 880 patients presented an increase in serum lactate of >1.1 times the upper normal limit (UNL). For 9 patients (1%), lactate elevation was moderate or severe (>2.2 times the UNL). Patients who presented with hyperlactatemia were more likely to be receiving stavudine with or without didanosine (odds ratio, 2.7; 95% confidence interval, 1.5-4.8), as compared with patients who received zidovudine-based regimens. The risk increased with increasing time receiving stavudine with or without didanosine. The association between hyperlactatemia and stavudine with or without didanosine was not biased by these medications being more recently available and, therefore, being given preferentially to patients who had prolonged use of nucleoside analog reverse-transcriptase inhibitors. Hyperlactatemia was associated with lipoatrophy, hyperlipidemia, and hyperglycemia. Age, sex, or stage of infection with human immunodeficiency virus were not predictive of hyperlactatemia. Determination of lactate levels may prove useful in the screening for mitochondrial toxicity.
