RESUMO

Objective:To investigate the clinical characteristics and therapeutic effect of adult primary hemophagocytic syndrome (HPS).Methods:The clinical data of a patient with primary HPS in the Third Affiliated Hospital of Southern Medical University in July 2017 were retrospectively analyzed, and the related literature was reviewed.Results:The patient was a 53-year-old female without history of basic disease, presenting as repeated high fever, with mutation of STXBP2 (FHL5), and was diagnosed as HPS according to hemophagocytic lymphohistiocytosis (HLH)-2004 criteria. The patient was treated with HLH-2004 regimen, and the efficacy was good. The patient was followed up until May 2021, and the overall survival time was 45 months.Conclusions:The atypical primary HPS and delayed primary HPS are rare, with mild clinical symptoms and only manifested by repeated high fever. Therefore, the gene mutations associated with HPS should be detected as soon as possible to confirm the diagnosis and to treat the disease early.

Objective To evaluate an indwelling anal tube in the prevention of anastomotic leakage after laparoscopic Dixon procedure for rectal cancer.Methods From June 2015 to June 2017,71 rectal cancer patients undergoing laparoscopic Dixon procedure with the anastomotic margin to dentate line < 4 cm were randomly divided into the study group (39 cases) to have an anal tube dranage and the control group (32 cases) without tube dranage.Within a week after surgery,the postoperative pressure changes in the rectum,defecation,anastomotic leakeage were monitored and observed.Results In study group postoperative intra rectal pressure at 2 h,and on days 1,2,3,4,5,6,7 were (13 ± 3),(8 ± 3),(11 ±2),(14 ±4),(16 ±3),(19 ±2),(21 ±3),(22±3) cmH2O,while in control group were (17 ±2),(11 ±3),(15 ±3),(17 ±3),(20 ±2),(22±3),(25 ±4),(26 ±2)cmH2O (all P< 0.05).In the study group the postoperative discharge and defecation were 1-2 days earlier than the control group.No anastomotic leakage occurred in study group,while in control group,there were 4 cases with the incidence rate of 12%,and the difference between the two groups was statistically significant (all P < 0.05).Conclusion In Dixon procedure,routinely placed anal tube effectively prevent anastomotic leakage from occurring.

BACKGROUND: Fibular fixation and tantalum rod implantation are two commonly used methods for the treatment of early osteonecrosis of the femoral head (ONFH), both of which can effectively delay or even reverse the progress of ONFH. However, further comparative evaluation on their mechanical properties and therapeutic efficacy is required.OBJECTIVE: To compare the clinical efficacy of fibular fixation and tantalum rod implantation on ONFH at early stage.METHODS: Fifty-eight patients (81 hips) suffered from ONFH with ARCO stage 1 and stage 2, and underwent fibular fixation (30 cases, 41 hips) or tantalum rod implantation (28 cases, 40 hips). Postoperatively, both groups were followed up for over 2 years. The Harris scores of the hip were compared between two groups before and after treatment. With femoral head collapse and the collapse distance > 4 mm as observation points, the survival rate of the femoral head was compared between two groups.RESULTS AND CONCLUSION: The postoperative Harris scores of the two groups were significantly improved than before (P 4 mm collapse as the observation point, the Kaplan-Meier survival curve showed that overall survival rate of the hip was 95% in the fibular fixation group and 83% in the tantalum rod implantation group. After examined by log-rank (Mantel-Cox), there was a significant difference in the survival rate of the hip at Stage IIC between two groups (P=0.0418). To conclude, both fibular fixation and tantalum rods implantation applied to ONFH at early stage can effectively improve the hip function, and the survival rate of the hip at ARCO Stage IIC is better in patients undergoing fibular fixation than tantalum rod implantation.

UNLABELLED: Fungal cell walls are predominantly composed of glucans, mannans, and chitin. Recognition of these glycans by the innate immune system is a critical component of host defenses against the mycoses. Complement, an important arm of innate immunity, plays a significant role in fungal pathogenesis, especially the alternative pathway (AP). Here we determine that the glycan monosaccharide composition and glycosidic linkages affect AP activation and C3 deposition. Furthermore, properdin, a positive regulator of the AP, contributes to these functions. AP activation by glycan particles that varied in composition and linkage was measured by C3a generation in serum treated with 10 mM EGTA and 10 mM Mg(2+) (Mg-EGTA-treated serum) (AP specific; properdin functional) or Mg-EGTA-treated serum that lacked functional properdin. Particles that contained either ß1→3 or ß1→6 glucans or both generated large and similar amounts of C3a when the AP was intact. Blocking properdin function resulted in 5- to 10-fold-less C3a production by particulate ß1→3 glucans. However, particulate ß1→6 glucans generated C3a via the AP only in the presence of intact properdin. Interestingly, zymosan and glucan-mannan particles (GMP), which contain both ß-glucans and mannans, also required properdin to generate C3a. The ß1→4 glycans chitin and chitosan minimally activated C3 even when properdin was functional. Finally, properdin binding to glucan particles (GP) and zymosan in serum required active C3. Properdin colocalized with bound C3, suggesting that in the presence of serum, properdin bound indirectly to glycans through C3 convertases. These findings provide a better understanding of how properdin facilitates AP activation by fungi through interaction with the cell wall components. IMPORTANCE: Invasive fungal infections have increased in incidence with the widespread use of immunosuppressive therapy and invasive procedures. Activation of the complement system contributes to innate immunity against fungi by generating chemoattractants that recruit white blood cells and by coating the pathogen with complement fragments that "mark" them for phagocytosis. The fungal cell wall activates complement in an antibody-independent manner through the alternative pathway (AP). Properdin is a positive regulator of the AP. This study elucidates how the specificity of cell wall glycan linkages affects AP activation and the role properdin plays in this process. Particulate ß1→3 glucans activated the AP even in the absence of properdin, while ß1→6 glucans required properdin for AP activation. In contrast, the ß1→4 glycans chitin and chitosan failed to activate the AP. These findings enhance our mechanistic understanding of how fungi activate complement and have implications for the use of glycans in biomedical applications.

Objective To observe the effect of sensory reeducation on the motor function and activities of daily living (ADL) in stroke patients with proprioceptive disturbance. Methods Sixty patients with proprioceptive disturbance after stroke were randomly divided into the treatment group and observation group, and 30 stroke patients with no proprioception disturbance were recruited as the controls. Three groups were treated with routine rehabilitation interventions, but the treatment group were given sensory reeducation in addition to the regular treatment. Fugl-Meyer motor assessment scale (FMA) and sensory assessment scale, and with Barthel index (MBI) for activities of daily living (ADL) were used to evaluate the patients before and after treatment. Results FMA and MBI scores of patients in three groups were increased significantly (P<0.01) after treatment. The scores of patients in observation group were significantly lower than those of the control group and treatment group (P<0.01). Those of the treatment group and the control group were with no significant difference (P>0.05). Conclusion The proprioception disturbance significantly affects the rehabilitation process, and sensory reeducation can improve the sensory function, motor function and activities of daily living (ADL).

In order to test validity and fea si bility of the impedance spectroscopy analysis method,a series of blood experime n ts have been carried out to study influences of the hematocrit,the plasma conc e ntration and the dextran concentration in suspend medium on the blood impedance and the RBC aggregation.The results show that the blood impedance of lower freq uency R_0 increases with increase of Hctc,the time constant T has little chang e when Hctc hange from 28.9 to 48.9;with the increase of plasma concentration ,t he RBC aggregation radius increases;When the dextran concentration is about 4g/ 100ml,the RBC aggregate reaches its maximum.It suggests that the impedance spe ctroscopy analysis method is a new,effective method to study RBC aggregation.

RESUMO

Objective To stady the clinical effects of repairing soft tissue defects on body surface by grafting free anterolateral thigh flaps. Methods 11 patients were treated for soft tissue defects on body surface by grafting free anterolateral thigh flaps from October,1986 to January,2002. Results all flaps in the 11 patients survived.Follow-up survey in the (6~18) months after the operation showed satisfactory results. Conclusions Free anterolateral thigh flap is well supplied with blood,and it is in a concealed area of the body,where flaps of large size can be cut off.It is the ideal flap to repair large areas of soft tissue defects on body surface.

According to the three-parameter expression of blood , the dual-frequency complex impedance information is selected as the known quantity. In this paper , the particular solution process of three-parameter is introduced. A calculational method to obtain basic quantity and pulsatile quantity of three-parameter of blood electrical impedance is established.
