In 75% of the patients with malignant melanoma of the choroid useful vision can be retained by beta-radiation with ruthenium-106/rhodium. Tumors with a prominence of more than 6 mm should be treated only for very special reasons. There is a statistically proven correlation between the initial tumor volume and the occurrence of residual tissue volume. Histopathological investigations have shown that residuals of the tumor do not permit any conclusions concerning the dignity or vitality of tumor cells. The risk to life is not increased by the conservative procedure, and it may even be reduced.

A 57-year-old man with a myocardial infarction was subjected to cardiac catheterization as a follow-up study for his heart problem. The catheter, made for one time use only, was reautoclaved and used sometime before catheterizing this man. A small fragment of this catheter dislodged while guiding the parent catheter into coronary vessels with a metallic guide-wire. The fragment traveled getting lodged in the central retinal artery of the right eye leaving the eye with only light perception vision. The catheter fragment could be seen in the central retinal artery at the optic nerve head.

A patient with a malignant melanoma of the choroid was treated at first successfully with the 106Ru beta ray applicator. Because new tumor growth could not be ruled out, the eye was enucleated 2 1/2 years after radiotherapy. The histopathologic examination revealed, however, a complete tumor cell necrosis.

22 patients with extending intraocular lesions are examined by serial computed tomography after a bolus injection of contrast medium. The changes of X-ray density of the tumor areas are compared with those of the vitreum and of brain tissue. Choroidal melanomas exceeding 2 mm in prominence are identified as highly vascularized areas in which contrast enhancement according to certain brain tumors takes place. A differentiation from large pseudotumors seems to be possible. The value of this method for the differential diagnosis of choroidal melanomas is discussed.

A history is presented of a 40-year-old male patient suffering from bilateral choroidal melanoma. Both eyes were treated with 106Ru/106Rh beta-irradiation. 2 years later one eye was enucleated because of radiogenic central vein thrombosis with secondary glaucoma. Histologically the tumor cells showed radiogenic damages and there were no signs of a new tumor growth. The other eye, however, is still functioning 6 years later with a visual acuity of 1.0.

Depending on prominence and localization of malign melanomas of the choroid, there are - besides enucleation - other clinical treatments possible. Irradiation therapy with beta applicators (106Ru/106Rh) gives also radiogenous complications. But contrarily to a treatment with cobalt-60 applicators, these complications are less important.

Recently in nuclear medicine two diagnostic methods have been developed: the topographical diagnosis utilizing mobile probes or scanners and the advanced diagnosis of function with the scintillation gamma camera. A similar trend is encountered in ophthalmic nuclear medicine.
