OBJECTIVE: To assess the validity of ultrasound (US) in the early detection of arthritis and enthesitis, with assessment of the validity of magnetic resonance imaging (MRI) in the early detection of sacroiliitis and spondylitis in patients with psoriasis and to compare the findings of clinical examination and conventional radiography. MATERIAL AND METHODS: The study included 50 patients with psoriasis and 20 healthy controls. All patients and controls underwent US and power Doppler analyses for the joints of both hands and feet and the entheseal sites. MRI of the lumbosacral spine and sacroiliac joints was performed. RESULTS: Abnormal US findings of arthritis were present in 18% patients, whereas only 6% patients had X-ray abnormalities, the enthesopathy represent 74%, at a higher percentage than clinical and radiological assessment (46, 26% respectively). MRI and radiological study demonstrated evidence of inflammation in the spine in 44% and 16% patients, respectively, and evidence of sacroiliitis in 10% and 6% patients, respectively. CONCLUSION: Use of newer imaging modalities allows early diagnosis and early initiation of therapy.

The incidence of hepatitis C virus (HCV) genotype 4 infection in Egypt provides a unique opportunity to study the innate immune response to symptomatic acute HCV infection. We investigated whether plasmacytoid dendritic cells (pDCs) are activated as a result of HCV infection. We demonstrate that, even during symptomatic acute infection, circulating pDCs maintained a similar precursor frequency and resting phenotype, compared with pDCs in healthy individuals. Moreover, stimulation with a Toll-like receptor 9 agonist resulted in an intact inflammatory response. These data support the growing consensus that pDCs are not directly activated by HCV and therefore are viable targets for immunotherapy throughout HCV infection.

Adipocytokines are hormone and cytokine like substances produced mainly from white adipose tissue. A relation between plasma adipocytokines and many inflammatory disorders including rheumatoid arthritis (RA) had been investigated. This work was done to investigate the systemic behavior of the main adipocytokines at the plasma level as well as its local behavior at the synovial fluid level in patients with RA and osteoarthritis (OA). The study had been conducted on 32 patients with RA and 18 patients with OA. Paired blood and synovial fluid samples had been collected from all patients and level of plasma and synovial fluid (SF) leptin, adiponectin and resistin had been quantitated by enzyme linked immunosorbent assay (ELISA). Results had been compared between RA group and OA group. Adipocytokines had also been compared in patients with erosive and non-erosive disease and had been related to clinical and laboratory markers of activity. Plasma resistin and BMI-corrected plasma leptin were significantly higher in RA group. Female patients showed significantly higher plasma leptin, even after correction to BMI. Studied SF adipocytokines were significantly higher in RA group and correlated positively with synovial fluid WBCs. Comparing plasma and SF results showed a significant increase in SF resistin especially in RA group and a significant drop of SF adiponectin especially in OA group. In conclusion, Adipocytokines are probably involved in inflammatory and degenerative articular disease. The different behavior between plasma and SF would suggest a pro-inflammatory role for resistin and chondro-protective role for adiponectin.

Unstable coronary syndromes usually involve platelet activation and thrombus formation at the site of atherosclerotic plaque. P-selectin in platelets and endothelial cells mediates adhesive interaction with leucocytes to form thrombi. The aim of this study was to asses the level of soluble P-selectin (CD62P) as a non invasive marker of coronary plaque destabilization in unstable angina (U.A.). Serum samples were collected from 23 male patients with UA, 20 male patients with stable angina (SA) and 13 healthy controls. Soluble P-selectin (sP-selectin) level was measured by ELISA technique. The mean sP-selectin level was significantly higher in patients with UA (87.6+/-30.10 ng/ml) than SA (36.2 +/-13.8 ng/ml) and control subjects (16.7+/-8.6 ng/ml). In conclusion, s-Pslectin level could be used as a marker of plaque destabilization in unstable angina.
