Enhancement on infectious diseases nursing plan information system.

Based on researches, the most time-consuming nursing activities, in teaching hospital, are: room patrols, the blood pressure survey, the body temperature pulse breath survey, the nursing record maintenance. The nursing record is one way to communicate data. It can allow the medical service team to understand what measures the nursing staff once did for sickness, as well as responses from sickness. Nevertheless, it is the key component to utilize the record with a clinical nursing plan, so as to provide a proficient health management. Since the maintenance of nursing plan is costly and time-consuming, therefore, it is essential to establish the nursing plan information system, which can effectively promote the nursing quality. This research main body comes from one infectious disease division nursing plan information system, which was developed in 1992, and its data base covers entire courtyard compatibility and various faculties characteristic nursing plan. The nursing staff often complained that this system is not user-friendly, its contents are not comprehensive, and sometimes it does not let staff choose the right diagnosis. Therefore this research is based on history analysis and the questionnaire survey procedure first, the infectious disease nursing plan use number of times, the frequency and the project content, then by the literature scientific theory and result of the improvement group discussion together. The original 38 infectious disease division nursing plan will be expanded to 45 nursing plans. Moreover, the common 38 infectious disease code (ICD-9), and its corresponding diagnosis items, shall automatically appear in the disease diagnose code field, so it would be better off for the nursing staff to set up the nursing plan efficiently. Infectious disease division nursing plan information system utilization ratio is promoted 9.6-folds, according to research outcome. Each task consumes 3.68 minutes beforehand-including computer program operation, the printing of nursing plan, and now it turned out that it shall not take more than 3.08 minutes. The time efficiency is raised by 17% (0.6 minute). Yet only 17% of the users are satisfied with it before, and now 74% nursing staffs are happy with the new tool.Therefore, it shall not only improve our service quality, but also save labor cost by using this information supplementary system.

[Nursing terminology standards in taiwan: current situation and trends].

Nursing terminology standards are critical to effective nursing communications in clinical practice, education, administration, and research. Recently, the national Department of Health has promoted implementation of a standardized Taiwan electronic medical record template (TMT). The TMT provides a specific need for nursing terminology standards used in all medical facilities. In this article, we survey and compare the development of nursing terminology standards in Taiwan with that of the international community. The two principal recommendations made in this paper include 1) link Taiwan terminology standards at the education and research levels to international standards and 2) integrate localized nursing terminology into Taiwan terminology standards for use in nursing information system relational databases and electronic nursing records. We hope that the government, medical institutions, and academic organizations will pay greater attention to the standardization of local nursing terminology in order to advance nursing quality in Taiwan.

Building a portable data and information interoperability infrastructure-framework for a standard Taiwan Electronic Medical Record Template.

Traditional electronic health record (EHR) data are produced from various hospital information systems. They could not have existed independently without an information system until the incarnation of XML technology. The interoperability of a healthcare system can be divided into two dimensions: functional interoperability and semantic interoperability. Currently, no single EHR standard exists that provides complete EHR interoperability. In order to establish a national EHR standard, we developed a set of local EHR templates. The Taiwan Electronic Medical Record Template (TMT) is a standard that aims to achieve semantic interoperability in EHR exchanges nationally. The TMT architecture is basically composed of forms, components, sections, and elements. Data stored in the elements which can be referenced by the code set, data type, and narrative block. The TMT was established with the following requirements in mind: (1) transformable to international standards; (2) having a minimal impact on the existing healthcare system; (3) easy to implement and deploy, and (4) compliant with Taiwan's current laws and regulations. The TMT provides a basis for building a portable, interoperable information infrastructure for EHR exchange in Taiwan.
