Coronary artery disease (CAD) is one of the important causes of mortality worldwide. South Asians, notably Indians are unduly prone to develop CAD with its incidence being doubled in the last three decades among both rural and urban settlers. CAD prevalence of in Bangladesh is not known. There are merely a limited number of small-scale epidemiological studies are existing. Recent data indicates CAD prevalence in our country to lie between 1.85-3.4% in rural and 19.6% in an urban sample of working professionals. Despite marked disparity in values, the disease seems to be in rising trend. Patients with concomitant CAD and carotid artery disease are at increased risk of developing peri-operative neurological events including stroke. By far, the prevalence of carotid artery disease in candidates of CABG has not yet been determined in our country. There is a lack of pre-operative guidelines as well for the necessary vascular investigations that should be performed on CABG candidates before they go to the operation table. Pre-operative non-invasive carotid Doppler ultrasonography is a useful screening tool for carotid artery disease in all patients undergoing CABG. This was a cross-sectional observational study, was conducted in the Radiology & Imaging department of Ibrahim Cardiac Hospital & Research Institute, Dhaka, Bangladesh from January 2017 to June 2017. The present cross-sectional study was intended to determine the prevalence of concurrent occurrence of carotid and coronary artery disease in elderly patients undergoing CABG. Total 210 elderly (from 60 & above) patients scheduled for CABG taken as study population. There was bilateral carotid atherosclerotic plaque in 15(12.2%) patients. Right carotid plaque was in 69(56.0%) patients, left carotid plaque in 54(43.9%) patients. Carotid stenosis grading was done in percentage (%). There was significant (>50.0%) stenosis of right carotid system in 12 patients (17.4%) and significant stenosis of left carotid system in 18 patients (33.3%). Right carotid system & bulb was the most common site of plaque formation. We can conclude from this study that a substantial proportion of patients after a particular age possess carotid artery disease simultaneously with coronary artery disease, routine evaluation of carotid arteries of the elderly patients scheduled for CABG is strongly suggested.

The study sought to compare procedural outcomes for patients undergoing percutaneous coronary intervention (PCI) of a chronic total coronary artery occlusion (CTO) with a matched non-CTO cohort. Percutaneous coronary intervention of a CTO is a common occurrence, and the outcome for patients with successful PCI of a CTO has not been clearly defined. Between November 2006 and December 2010, a total of 2,000 consecutive patients consecutively underwent PCI for a CTO. Utilizing propensity scoring methods, a matched non-CTO cohort of 2,000 patients was identified and compared to the CTO group. The cohorts were stratified as successful and failed procedures in United Hospital Limited Dhaka. The in-hospital major adverse cardiac event (MACE) rate was 3.8% in the CTO cohort. Technical success has improved over the last 10 years (overall 74.4%, slope 1.0%/year, p=0.02, R²=49.9%) as did procedural success (overall 69.9%, slope 1.2%/year, p=0.02, R²=51.5%) without a concomitant increase in in-hospital MACE rates (slope 0.1%/year, p=0.7). There was a distinct advantage for successful CTO treatment compared with failed CTO treatment (73.5% vs. 65.1%, p=0.001). The CTO versus non-CTO survival was the same (71.2% vs. 71.4%, p=0.9). Diabetics in the CTO cohort had a lower survival compared with non-diabetics (58.3% vs. 74.3%, p=0.0001). These data represent outcome of PCI for a CTO. The 10-year survival rates for matched non-CTO and the CTO cohorts were similar. Success rates have continued to improve without an accompanying increase in MACE rates. A successfully revascularized CTO confers a significant survival advantage compared with failed revascularization.

The present studies found that microalbuminuria is predictive, independent of classical risk factor of cardiovascular diseases and all causes of mortality in diabetes or hypertension patient groups and in the general population. Coronary angiograms for extent of severe CAD (luminal narrowing 50%) in patients with Diabetes Mellitus (DM) and general population were examined. The study comprised 150 patients undergoing coronary angiography at United Hospital Limited, Dhaka, Bangladesh, (M/F 80/70, mean age 57±11 years). Urine albumin excretion was measured in 24 hours urine samples employing immune precipitation technique. Age-gender distribution of coronary risk factors and microalbuminuria were compared between patient with and without coronary artery disease. As many as 70.5% (106) of patient had coronary artery disease and 29.4%(44) had no coronary lesion. Microalbuminuria was detected at 62.9% in patients with CAD and 8.8% in those without coronary artery lesion (p<0.001). The presence of 1 or 2 vessel CAD showed a linear increase between the groups without microalbuminuria. Patients with microalbuminuria have more severe angiographically detected coronary artery disease than those without microalbuminuria, thus a link can be established independent of other risk factors.

Soft tissue stiffening is a common mechanical observation reported in foot pathologies including diabetes mellitus and gout. These material changes influence the spatial distribution of stress and affect blood flow, which is essential to nutrient entry and waste removal. An anatomically-based subject-specific foot model was developed to explore the influence of tissue stiffening on plantar pressure and internal von Mises stress at heel-strike, midstance and toe-off. This work draws on the model database developed for the Physiome project consisting of muscles, bones, soft tissue and other structures such as sensory nerves. The anisotropic structure of soft tissue was embedded in a single continuum as an efficient model for finite soft tissue deformation, and customisation methods were used to capture the unique foot profile. The model was informed by kinetics from an instrumented treadmill and kinematics from motion capture, synchronised together. Foot sole pressure predictions were evaluated against a commercial pressure platform. Key outcomes showed that internal stress can be up to 1.6 times the surface pressure with implications for internal soft tissue damage not observed at the surface. The main nerve branch stimulated during gait was the lateral plantar nerve. This subject-specific modelling framework can play an integral part in therapeutic treatments by informing assistive strategies such as mechanical noise stimulation and orthotics.

RESUMO

Aneurysmal bone cysts of the skull are rare and involvement of sphenoid bone is even less frequent. We present X-ray, CT, MR imaging and histopathologic findings of an aneurismal bone cyst of the sphenoid in a 15-years old female adolescent. Radiological findings of the aneurysmal bone cyst of the skull were highly suggestive of the diagnosis and that were confirmed by histopathologic analysis.

The epicardial application of lactic acid induced a biphasic rectal motility response in lightly anaesthetised, open-chested and artificially ventilated cats. This rectal biphasic response is reflexogenic in nature as epicardial lignocaine abolished such response. This rectal biphasic response is abolished by cardiac sympathectomy and reprecipitated by left inferior cardiac afferent nerve stimulation. Such response is also abolished by sacral ventral rhizotomy and reproduced by stimulation of the peripheral cut end of split sacral ventral roots. This indicates that the afferent and efferent pathways for such reflex are lying in the cardiac sympathetic and sacral pelvic nerves, respectively. The higher centers involved for such reflex are lying above the mid-collicular level of the brain as decerebration at the mid-collicular level completely abolished such type of rectal response. Furthermore, the relaxation phase and contraction phase of such rectal response are mediated through nitric oxide release and cholinergic neurones, respectively, as NG-nitro-L-arginine and atropine abolished relaxation and contraction phase of the rectal response, respectively.

We describe the fine needle aspiration cytologic study of 210 splenic aspirates from patients clinically diagnosed as having visceral leishmaniasis. Of these, 147 aspirates were found to be positive for Leishmania donovani bodies. We highlighted the cytomorphology of these positive smears. A majority of the cases showed an increase in the number and size of reticuloendothelial cells, with altered shapes, variations in the cytoplasm and decreased white pulp elements. In some of these cases (10%), immature erythroid cells were also found.

This paper describes the practical implementation of a tomographic reconstruction algorithm on different types of computers (micro to mainframe) and in different programming languages (BASIC to OCCAM) and performance studies were made.
