RESUMO

A complete transverse rupture of the kidney diagnosed preoperatively by IV pyelogram and selective renal angiography is reported. It could be conserved by tissue gluing with highly concentrated human fibrinogen. An IV pyelogram should always be performed, especially since immediate aortography and renal arteriography is indicated by visible extravasation of contrast medium in every case. The lesion is exactly determined in the early stage and an organ-conserving operation becomes possible. Without the availability of angiography, nephrectomy was most frequently performed.

In groups of 10 specimens the testicles of 100 Wistar rats were half unilaterally half bilaterally rotated 720 degrees and retorted 1, 2, 4, 6 and 8 hours after torsion. The fertility rate was investigated by copulation experiments following experimental torsion of the spermatic cord. Fertility was demonstrated by pregancy. Independently of torsion period the fertility rate of unilateral contorted specinens was 60% -- like in both control groups. The bilaterally contorted animals were fertile in 40% from the 1 h -- group and in 30% from the 2 hours -- group. If the torsion period was longer than 2 hrs. bilaterally conterted animals were infertile in all cases. There is no influence of unilateral conterted testicle on contralateral uncontorted testicle.

RESUMO

This prospective study shows, that, neither an antibiotic prophylaxis of abacteriuric patients nor postoperative treatment of an asymptomatic urinary tract infection after transurethral prostatectomy have a significant influence on the clinical complications or the microbiologic results. After suprapubic prostatectomy complications in the control group were not significantly more frequent than in the group with antimicrobial prophylaxis. Patients with asymptomatic urinary tract infections before suprapubic prostatectomy had better clinical results, if they were treated with antibiotics as compared to a control group, although the difference control group was not significant. The correlation between consumption of antibiotics and development of resistant in pathogenic organisms forced us to use a more critical approach in the use of antimicrobial prophylaxis and therapy. This study shows clearly, that aseptic operations do not need an antibiotic protection and that faulty of asepsis cannot be corrected by antibiotics.

In three experimental groups and a control group unilateral or bilateral torsion of the testicle were carried out in 195 mature rats. The testicles were retorted at various intervals up to 24 hours. Four weeks and three months following the torsion the retorted as well as the contralateral normal testicles were examined morphologically and histologically. A total testicular torsion is fully reversable after one hour in all cases and after 2 hours in 40% of the cases. The Sertoli cells are destroyed after 6 hours and the Leydig cells after 10 hours. The time causing irreversable tissue damage therefore appears to be shorter than generally assumed, and the germinative epithelium is reacting faster than the testosterone producing Leydig cells. No changes were noted in the contralateral normal testis.

A bilateral testicular torsion war carried out in 65 mature rats. The timing of testicular tissue changes was investigated. A complete human testicular torsion corresponds to a 270 degrees torsion in the rat. The earliest changes were found in the semineferous tubules. Interstitial edema with vasodilatation and hyperemia already occurred one hour following complete torsion. Degenerative changes in the nuclei of the spermatogonia and spermatocytes were found after one hour. After two to four hours necrosis of the epithelium was found. After six hours these changes appeared irreversible. The Leydig cells appear to be damaged 2--3 hours and the Sertoli cells 4--6 hours later than the spermatogenic epithelium. Chronic experiments should be carried out to determine the reversability of these early changes.

Spermiograms of 46 patients were examined 8 days, 4 weeks, 3 months, 6 months and 1 year after acute unilateral non-specific epididymitis. The treatment consisted in 24 cases besides antibiotic treatment with doxycycline in local infiltrations of the spermatic cord with 1% Scandicain solution. Primarily there was found an inhibiting effect on spermatogenesis. In more than 2/3 of the cases a varying oligoasthenospermia was shown 8 days after beginning of the infection. In the further course this disturbance of fertility improves; if local infiltration of the spermatic cord with 1% Scandicain solution is performed, the improvement is more evident. Still, there remain about 20% of the cases in which the fertility disturbance is so severe, that sterility results. One year after the beginning of the infection spermagglutinating antibodies could be shown only in 1 case. The seminal fluid fructose was controlled over 1 year following the beginning of infection and found to be normal in all cases.

RESUMO

The effect of insulin, pentagastrin, carbachol and feeding on the electrical and mechanical activity of the stomach was examined in the conscious dog. In these experiments vagal stimulation, a gastric hormone, a parasympathetic agent and distention elecited a similar stimulation of electrical and mechanical activity. The activation occurred in case of insulin only after sufficient lowering of the blood glucose level, in case of the other tested stimulants after a few minutes.
