RESUMO

BACKGROUND: Inadequate or poor sleep quality is common problems in adolescent that affect on their learning, memory and school performance. The present study aimed to determine the association between sleep hours and academic performance in young adults. METHODS: This cross-sectional study was designed as a descriptive-analytic study. Samples of adolescents of 14-18 years old in Qazvin city were enrolled. The Pediatric sleep questionnaire and BEARS questionnaire used for all students to screen comprehensively major sleeps problems in them. Chi-square test, t-test, analysis of variance (ANOVA), and correlation were performed to determine the relationship between the data (P < 0.001). RESULTS: Between 653 adolescents, 40% were male and 60% were female. Sleep duration, sleep onset delay, sleep insufficient, rate of oversleeping and academic performance had a direct relationship with gender (P < 0.001). The sleep duration, rate of oversleeping and academic performance were significantly higher in boys, sleep onset delay and sleep insufficient was significantly higher in girls. Time of falling sleep at weekend nights and weekday nights have positively correlation with age (P < 0.001). Also, a significant relationship between students' sleep hours with academic performance was shown (P < 0.001). CONCLUSIONS: The overall result was that sleep duration, sleep onset delay, sleep insufficient and rate of oversleeping of students in this study had a significant influence on academic performance. Students without difficulty in falling asleep had good academic performance in compared to students with difficulty in falling asleep.

RESUMO

BACKGROUND: Organizational learning has been identified as necessary for different organizations to improve their performance in the changing and competitive environment. PURPOSE: The main purpose of this research was to specify the learning organization profile of educational and health centers of Tehran and Qazvin Universities of Medical Sciences in Iran. METHODOLOGY: The present research was conducted using a cross-sectional method in the academic year of 2013-2014. A staff of 530 from educational hospitals subordinated to Tehran and Qazvin universities of medical sciences participated in the research. The participants were selected using stratified random sampling. That is to say, a random sample of a proportionate size was selected from each hospital. The instrument for data collection was a Likert-scale questionnaire involving 50 items. The statistical techniques of ANOVA, t-test, Chi-square, correlation coefficients (Pearson and Spearman), and regression were utilized to analyze the data. All of them were performed using the Statistical Package for Social Sciences (SPSS) 16.0 for windows. RESULT: the results indicated that 449 of participants (84.7%) had a B.S. degree and 78 of them (14.7%) had an M.S. or a Ph.D. degree. Among the fivefold dimensions of "Learning Organization" model (Learning, Organization, People, Knowledge, and Technology) in comparison of the two universities, the "people" dimension was the highest-rated dimension with the mean rating of 25.71±8.36 and the "learning" dimension was the lowest-rated dimension with the mean of 25.35±8.04. Comparison between the two universities yielded the result that educational hospitals in Tehran University of medical sciences with the rating of 126.56 had a more complete profile than that of educational hospitals in Qazvin university of medical sciences with the rating of 122.23. CONCLUSION: The hospitals of the two above-mentioned universities were, to a great extent, far from the characteristics of Learning Organization. In light of the massive mission of these centers to maintain and improve the community health and to train the skilled labor force, the centers should embark on updating the data and institutionalizing learning. Furthermore, to modify staff's behavior and performance and to achieve their goals, they should accentuate the importance of acquiring, creating, and transferring knowledge.

RESUMO

Rapid diagnosis of acute pyelonephritis is important because of its association with long-standing complications. This study was conducted to compare the reliability of serum procalcitonin (PCT) and interleukin-1 beta (IL-1ß) with conventional laboratory parameters for diagnosis of acute pyelonephritis in children. Seventy nine children with urinary tract infection were divided into two groups based on the result of Tc-99m dimercaptosuccinic acid renal scan: acute pyelonephritis (n=33) and lower UTI (urinary tract infection) (n=46) groups. White blood cell (WBC) count, neutrophil count, erythrocyte sedimentation rate (ESR), serum C-reactive protein (CRP), PCT and IL-1ß concentrations of both groups were measured and compared. WBC count, neutrophil count, ESR, serum CRP, PCT and IL-1ß concentrations were higher in acute pyelonephritis patients than in the lower UTI group (P<0.05). The sensitivity and specificity of serum PCT and IL-1ß for diagnosis of acute pyelonephritis were 31, 84.7% and 27.2, 90% respectively (using a cut-point value of 0.5 ng/ml for PCT and 6.9 pg/ml for IL-1ß). The sensitivity of PCT and IL-1ß for diagnosis of acute pyelonephritis was less than that of conventional markers such as ESR and CRP. This study revealed that serum PCT and IL-1ß are not good biologic markers for differentiating acute pyelonephritis from lower UTI. It seems that conventional inflammatory markers such as ESR and CRP besides the clinical findings are more reliable for the diagnosis of acute pyelonephritis in children.

RESUMO

PURPOSE: Early diagnosis and treatment of acute pyelonephritis in children is of special importance in order to prevent serious complications. This study was conducted to determine the diagnostic value of serum interleukin (IL)-6 and IL-8 in children with acute pyelonephritis. METHODS: Eighty-seven patients between 1 month to 12 years old with urinary tract infection (UTI) were divided into 2 groups based on the result of 99m-technetium dimercapto-succinic acid renal scan: acute pyelonephritis (n=37) and lower UTI (n=50) groups. White blood cell (WBC) count, neutrophil (Neutl) count, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP) concentration, platelet count, and serum IL-6 and IL-8 concentrations of both groups were measured and compared. RESULTS: There was a significant difference between two groups regarding WBC count, Neutl count, ESR, and CRP concentration (P<0.05). In addition, the difference between the two groups regarding serum IL-6 and IL-8 concentrations was not significant (IL-6, 60 and 35.4 pg/mL and IL-8, 404 and 617 pg/mL, respectively). The sensitivity and specificity of serum IL-6 and IL-8 for diagnosis of acute pyelonephritis were 73%, 42% and 78%, 32%, respectively. Sensitivity, specificity, negative and positive predictive values of serum IL-6 and IL-8 were less than those of acute phase serum reactants such as CRP. CONCLUSION: This study showed that there was no significant difference between acute pyelonephritis and lower UTI groups regarding serum IL-6 and IL-8 levels. Therefore, despite confirming results of previous studies, it seems that IL-6 and IL-8 are not suitable markers for differentiating between acute pyelonephritis and lower UTI.

RESUMO

OBJECTIVES: The emergence of magnetic Resonance Imaging (MRI) technique has made it possible to answer many questions in the field of anatomy more precisely while observing different anatomic parts of living persons at different ages. This study was conducted to determine the dimensions of different parts of pons and also comparing the data in accordance with age and sex by MRI. METHODS: The present study carried out on 300 healthy individuals referred to imaging center at Imam Reza hospital in the city of Kermanshah, Iran. The precondition for people to be included in the study was the approval made by in-house physician based on person's state of health and the lack of any pathological lesion in brain on the basis of images obtained by MRI. Following MRI procedure, the dimensions of target parts in pons were calculated by the MRI-associated measuring system and recorded along with the age and sex of the patients. Measurements were recorded in mm and based on selecting the largest height, width and length of the organ during the examination of all axial, sagittal, and T(1) coronal profiles. Data analysis was performed using t-student test and Pearson's correlation. The significance level was set at P=0.05. RESULTS: The mean size of different parts of pons recorded in mm was as follows: The height of tegmentum of pons in men and women were 21.10+/-1.99 and 20.35+/-1.95, respectively. The height of ventricle of pons was 24.96+/-1.45 in men and 23.72+/-1.41 in women. The length of pons in men was 21.40+/-1.55 and in women 21.10+/-1.28. The length of ventricle in men and women were 16.85+/-1.21 and 16.77+/-1.36, respectively. The length of tegmentum of pons was 4.57+/-0.50 in men and 4.40+/-0.58 in women. Lastly, the transverse length of the ventricle of pons was 27.30+/-1.68 in men and 26.52+/-1.63 in women. DISCUSSION: The values obtained for the transverse length, length of tegmentum of ventricle, height of ventricle and height of tegmentum in men were larger than those found for women. There were significant relationships between the transverse length, length of tegmentum of pons, height of ventricle of pons and the age in men. There was no significant relationship between dimension of pons and age in women.
