RESUMO

One case of histiocytic sarcoma of the spleen is reported. The patient, a 54-year-old female, presented with a huge soft tissue mass with a clear border and pseudo capsule in the splenic parenchyma on CT. The density of the mass was uneven, multifocal cystic necrosis and irregular bleeding were observed inside, but no calcification could be seen. On contrast-enhancement scan, the solid component of tumor exhibited moderate progressive enhancement, and area of cystic necrosis exhibited no enhancement. In MRI, T1WI showed mixed low signal, and T2WI showed mixed slightly high signal. The pathological diagnosis was histiocytic sarcoma.

RESUMO

BACKGROUND: Liver cancer is one of the most common malignant tumors, and ranks as the fourth leading cause of cancer death worldwide. Microvascular invasion (MVI) is considered one of the most important factors for recurrence and poor prognosis of liver cancer. Thus, accurately identifying MVI before surgery is of great importance in making treatment strategies and predicting the prognosis of patients with hepatocellular carcinoma (HCC). Radiomics as an emerging field, aims to utilize artificial intelligence software to develop methods that may contribute to cancer diagnosis, treatment improvement and evaluation, and better prediction. AIM: To investigate the predictive value of computed tomography radiomics for MVI in solitary HCC ≤ 5 cm. METHODS: A total of 185 HCC patients, including 122 MVI negative and 63 MVI positive patients, were retrospectively analyzed. All patients were randomly assigned to the training group (n = 124) and validation group (n = 61). A total of 1351 radiomic features were extracted based on three-dimensional images. The diagnostic performance of the radiomics model was verified in the validation group, and the Delong test was applied to compare the radiomics and MVI-related imaging features (two-trait predictor of venous invasion and radiogenomic invasion). RESULTS: A total of ten radiomics features were finally obtained after screening 1531 features. According to the weighting coefficient that corresponded to the features, the radiomics score (RS) calculation formula was obtained, and the RS score of each patient was calculated. The radiomics model exhibited a better correction and identification ability in the training and validation groups [area under the curve: 0.72 (95% confidence interval: 0.58-0.86) and 0.74 (95% confidence interval: 0.66-0.83), respectively]. Its prediction performance was significantly higher than that of the image features (P < 0.05). CONCLUSION: Computed tomography radiomics has certain predictive value for MVI in solitary HCC ≤ 5 cm, and the predictive ability is higher than that of image features.

RESUMO

OBJECTIVE: To establish a safe and effective clinical transplantation strategy for determining when to prioritize frozen embryo transfers (FET) or fresh embryo transfers (ET) we conducted a retrospective analysis study, examining several key clinical outcomes. STUDY DESIGN: In a retrospective cohort study, 1423 patients (age<40) were categorized into four groups according to the number of oocytes retrieved (15-18, 19-21, 22-24 and ≥25 oocytes). The clinical outcomes of 1423 in vitro fertilization (IVF) cycles (896 fresh ET and 527 FET) were reviewed for each group. Data demonstrated that the clinical pregnancy rates (PR) and live birth rate (LBR) of the FET group was higher than those of the fresh ET group. RESULTS: Our study further indicates that the clinical benefits of FET become most meaningful when the number of oocytes retrieved ≥19. When the number of oocytes retrieved ≥20,this difference was more significant, with benefits in PR(odds ratio [OR]=2.46, 95% confidence interval [CI]: 1.74-3.46, P<0.001) and LBR (OR=2.27, 95% CI: 1.60-3.22, P<0.001). CONCLUSION: 20 oocytes retrieved may be the optimal cut-off point number for FET in order to achieve both a successful pregnancy and a live birth.

RESUMO

OBJECTIVE: To investigate the effects of hysteroscopic treatment of women with previous cesarean scar defect (PCSD). METHODS: From May 2006 to October 2008, 12 patients with PCSD were diagnosed and treated hysteroscopically in our hospital, all of them were successful followed-up for one year postoperatively, and their clinical data were analyzed. RESULTS: All 12 hysteroscopic procedures were completed successfully, and there were no surgical complications. Nine patients with longer periods and 1 patient with intermenstrual spotting preoperatively remained asymptomatic after hysteroscopic surgery, and 1 patient with longer periods and infertility experienced normal periods, while remained infertility, and the remaining 1 patient complaining postcoital bleeding preoperatively had recurrence of the bleeding. CONCLUSION: Hysteroscopic surgery of women with PCSD was minimally invasive and effective.
