RESUMO

Case summary: A 7-year-old female neutered domestic shorthair cat was presented with weight loss, abdominal distension, lethargy, pyrexia and hyporexia. Haematology and biochemistry were unremarkable. Abdominal ultrasound revealed a cystic mass within the left cranial abdominal quadrant, adjacent to the pancreas, stomach and spleen. CT revealed a 10.5 cm, rounded, well-encapsulated, hypodense mass arising from the left pancreatic lobe. The remaining organs of the abdominal and thoracic cavity were unremarkable. A partial left lobe pancreatectomy and splenic lymph node removal were performed. Histopathology of the pancreatic mass reported a proliferating population of neoplastic spindle cells most consistent with a preliminary diagnosis of soft tissue sarcoma. Immunohistochemistry characteristics, together with the morphological findings, were consistent with a diagnosis of leiomyosarcoma. The patient recovered without any concerns after receiving routine postoperative care. Clinical examination and abdominal ultrasound 5 months postoperatively revealed no abnormalities. Relevance and novel information: To our knowledge, this is the first report of pancreatic leiomyosarcoma in a cat. Pancreatic leiomyosarcoma should be considered as one of the possible differential diagnoses for cats presenting with a pancreatic mass.

The objective of this study was to assess the variables associated with complications of total hip replacement (THR) and report owner-assessed outcomes. Entries into the British Veterinary Orthopaedic Association-Canine Hip Registry (BVOA-CHR) between September 2011 and December 2012 were reviewed separately and in conjunction with previous data (January 2010-August 2011). An outcomes assessment questionnaire was used to collect data from owners. Incidences of surgeon-reported and owner-reported complications were 8.2 per cent and 4.3 per cent, respectively. THR using the BioMedtrix BFX cup/stem prosthesis had a greater incidence of complications compared with THR using the BioMedtrix CFX cup/stem prosthesis (P=0.002); complications were 4.48 times more likely when using the BioMedtrix BFX cup/stem prosthesis versus the BioMedtrix CFX cup/stem prosthesis. THR using the BioMedtrix BFX cup/stem prosthesis had a higher incidence of complications compared with THR using a hybrid prosthesis (BioMedtrix BFX cup/CFX stem, BioMedtrix CFX cup/BFX stem) (P=0.046); complications were 2.85 times more likely when using the BioMedtrix BFX cup/stem prosthesis versus a hybrid prosthesis. In 95 per cent of cases, owner satisfaction with the outcome of THR was 'very good' or 'good'. Complication rates from the BVOA-CHR are similar to previous studies. The data suggest that prosthesis type is associated with complication rate, with BioMedtrix BFX (circa 2012) having a high short-term complication rate.

OBJECTIVES: To compare the suitability of thoracolumbar fascia (TLF) and fascia lata (FL) for body wall defect repair in dogs, by examining their biomechanical properties and useable surface area. STUDY DESIGN: Experimental. ANIMALS: Dogs (n = 8). METHODS: Fresh TLF and FL grafts were obtained, surface area was calculated before testing to failure in 2 different modes: tensile testing and resistance to suture pullout, in 2 perpendicular orientations. RESULTS: Useable TLF surface area was significantly greater than for FL. Maximum load, energy to break, and elastic modulus of FL was significantly greater than that of TLF in tensile testing, but no apparent difference in the ultimate stress or strain was identified. There was no overall difference in suture pullout load between TLF and FL. During tensile testing, tissue orientation had a significant influence on ultimate load, stress, and elastic modulus for both tissue types, with strain and energy to break only having significant effects for TLF and FL, respectively. CONCLUSIONS: The greater tensile strength and stiffness of FL compared to TLF was not reflected in its material properties, implying any difference was a consequence of greater thickness. Suture pullout was not significantly different between the 2 tissues, perhaps limiting the clinical significance of the tissue mechanics. Tissues were anisotropic with respect to mechanical properties, thus orientation may be an important factor.

INTRODUCTION: Hearing loss is common and, in young persons, can compromise social development and educational achievement. Exposure to heavy metals has been proposed as an important risk factor for hearing loss. METHODS: We evaluated the cross-sectional associations between blood lead, blood mercury, and urinary cadmium and arsenic levels and audiometrically determined hearing loss in participants aged 12 to 19 years in the 2005-2008 National Health and Nutrition Examination Survey after accounting for the complex survey design. There were 2535 individuals available for analysis of blood lead and mercury levels, 878 for urinary cadmium levels, and 875 for urinary arsenic levels. Multivariate logistic regression was used to calculate adjusted odds ratios (ORs) and 95% CIs. RESULTS: A blood lead level greater than or equal to 2 µg/dL (to convert to micromoles per liter, multiply by 0.0483) compared with less than 1 µg/dL was associated with increased odds of high-frequency hearing loss (OR, 2.22; 95% CI, 1.39-3.56). Individuals in the highest quartile of urinary cadmium levels had significantly higher odds of low-frequency hearing loss than those in the lowest quartile (OR, 3.08; 95% CI, 1.02-9.25). There was no overall association between quartiles of blood mercury or urinary arsenic levels and hearing loss. CONCLUSION: Blood lead levels well below the current recommended action level are associated with substantially increased odds of high-frequency hearing loss.

OBJECTIVE: Establish normative histopathologic data on the dimensions of the cochlear nerve canal (CNC). BACKGROUND: Evidence suggests that when the CNC is stenotic, the cochlear nerve may be hypoplastic. There is clear agreement in the literature that an internal auditory canal less than 2 mm in diameter is a relative contraindication to cochlear implantation in children. However, there has only been recent recognition in research that a narrowed CNC may lead to diminished ability to interpolate and use auditory information delivered through a cochlear implant. However, there is no consensus in the literature on the normal diameter of the CNC and what parameters should be used to determine stenosis. In addition, no normative histopathologic data is available for CNCs. METHODS: This study evaluated histopathologic axial sections from normal human temporal bones to measure the cochlear nerve canal in 110 individuals, aged 0-100 years. The maximum CNC diameter in each normal patient was identified and measured. RESULTS: The mean CNC diameter was 2.26 mm with a standard deviation of 0.25 mm. There were no differences in the CNC diameters between males and females or with increasing age. CONCLUSION: These measurements should provide a normative reference for comparison in histopathologic and radiographic assessment of any patient with suspected cochlear nerve canal stenosis.

OBJECTIVE: We investigated trends in noise-induced threshold shifts (NITSs), high-frequency hearing loss (HFHL), and low-frequency hearing loss (LFHL). METHODS: A total of 4310 adolescents 12 to 19 years of age completed audiometric testing during National Health and Nutrition Examination Surveys in 1988-1994 and 2005-2006. NITS criteria were audiometric patterns of decreased 3- to 6-kHz thresholds but preserved 0.5- to 1-kHz and 8-kHz thresholds; HFHL and LFHL criteria were high and low pure-tone averages, respectively, of >15 dB HL. RESULTS: There were no significant increases in NITSs (odds ratio [OR]: 0.81 [95% confidence interval [CI]: 0.53-1.22]; P = .29), HFHL (OR: 1.21 [95% CI: 0.87-1.69]; P = .25), or LFHL (OR: 1.37 [95% CI: 0.77-2.45]; P = .28) between survey periods. However, a significant increase in the prevalence of NITSs occurred among female youths (11.6% [95% CI: 9.0%-14.1%] vs 16.7% [95% CI: 13.2%-20.3%]; P < .0001). The overall prevalence of exposure to loud noise or listening to music through headphones in the previous 24 hours increased from 19.8% (95% CI: 17.6%-22.1%) to 34.8% (95% CI: 31.0%-38.5%; P < .0001). In 2005-2006, female youths had a similar prevalence of exposure to recreational noise (23.6% [95% CI: 19.6%-27.6%] vs 27.7% [95% CI: 23.6%-31.8%]; P < .0001) and a lower prevalence of hearing-protection use (3.4% [95% CI: 1.6%-5.3%] vs 10.3% [95% CI: 7.3%-13.2%]; P < .0001) compared with male youths. CONCLUSIONS: Increased exposure to recreational noise and minimal use of hearing protection might have lead to an increase in NITS prevalence among female youths.
