RESUMO

Besides the various operative procedures, which nowadays have come into use in hemorrhoidal disorders, drug therapy as well continues to keep an important place. Its main conditions of application are inflammatory processes before, between and after hemorrhoidal sclerosis or proctological operations. In a clinical study suppositories with the corticoid fluocortolone-21-pivalate and the local anesthetic lidocaine hydrochloride as well as a cream with the same substances and in addition, chlorquinaldol were tested in 92 patients with hemorrhoidal diseases and their concomitant conditions. In 92% of the cases efficacy of the preparations proved to be good or very good, and in all cases they were well tolerated.

In 3 controlled multi-center studies, 372 patients suffering from dermatomycoses were treated with isoconazole as cream, solution, and spray either once or twice daily. Neither with regard to the duration of healing nor the healing rates were observed statistically significant differences between the two modes of treatment. As to the forms of preparation, there were no differences of efficacy either. At the end of treatment, the cure rates amounted to 90-94%. Follow-up examinations carried out two weeks later gave no indications of relapses.

The discussion on ethical problems in reproductive medicine that has been going on for quite a decade, is of an exemplary nature; it is mainly focussed on extracorporeal fertilisation, prenatal diagnosis with a view to abortion, and post partum diagnosis and treatment of genetic diseases. The fundamental problems of medical ethics in general come to the fore particularly clearly in this discussion. The following article deals with these fundamental problems, whereas a subsequent article will be concerned in greater detail with the three special disciplines I have just mentioned.


PIP: The discussion on ethical problems in reproductive medicine which has been going on for more than a decade has mainly focused on external fertilization, prenatal diagnosis with a view toward abortion and postpartum diagnosis and treatment of genetic diseases. In order to discuss the implications of medical ethics in each of these three special cases, it is necessary to develop the fundamental ideas of medical ethics in some detail. The present article deals with medical fundamentals; a subsequent article will discuss the three specific applications of ethics in more detail. The author develops the main outlines of modern philosophical and theological treatment of ethics on the basis of Christian theology considering the nature and destiny of humanity. He points out that each ethical decision is a decision which is based upon the ethical criterion of preferability and that this criterion has as its bench mark an appreciation of the nature and the destiny of humanity. A further attribute of an ethical decision is that it must be made by an individual and therefore cannot be delegated to anyone else. Although the criterion upon which an ethical decision is based remains constant, the content of the guiding principles changes from case to case. Finally, an ethical decision exists in the tension between morality and legality; neither consideration can be a complete guide to the full implications of an ethical decision.
