RESUMO

Aging is generally associated with a decline in important cognitive functions that can be observed in EEG. Physical activity in older adults should be considered one of the main strategies to promote health and prevent disease in the elderly. The present study aimed to systematically review studies of EEG activity and cognitive function changes associated with physical activity in older adults. Records from PubMed, Scopus, and EBSCO databases were searched and, following the PRISMA guidelines, nine studies were included in the present systematic review. A risk of bias assessment was performed using the National Institute of Health Quality Assessment Tool for Case-control Studies instrument. The studies analyzed used two main strategies to determine the effects of physical activity on cognition and EEG: (1) multiscale entropy and power frequencies; and (2) event-related potentials. In terms of EEG activity, it can be concluded that exercise-induced neuroplasticity underlies improvements in cognitive function in healthy older adults.

The CB2 cannabinoid receptor has been found in brain areas that are part of the reward system and has been shown to play a role in food intake regulation. Herein, we conducted a systematic review of studies assessing the role of the CB2 receptor in food intake regulation. Records from the PubMed, Scopus, and EBSCO databases were screened, resulting in 13 studies that were used in the present systematic review, following the PRISMA guidelines. A risk of bias assessment was carried out using the tool of the Systematic Review Center for Laboratory Animal Experimentation (SYRCLE). The studies analyzed used two main strategies: (1) the intraperitoneal or intracerebroventricular administration of a CB2 agonist/antagonist; and (2) depletion of CB2 receptors via knockout in mice. Both strategies are useful in identifying the role of the CB2 receptor in food intake in standard and palatable diets. The conclusions derived from animal models showed that CB2 receptors are necessary for modulating food intake and mediating energy balance.

La miopía constituye un problema de salud pública a nivel mundial, y se espera que afecte a un número cada vez mayor de personas en las próximas décadas. El control de la progresión de este defecto refractivo se ha convertido en una parte importante de la práctica clínica, ante la ausencia de un tratamiento protocolizado con el objetivo de disminuirla, se han valorado varias opciones terapéuticas. El objetivo del estudio fue proporcionar información actualizada sobre el tratamiento óptico en niños miopes. Myopia constitutes a worldwide public health problem, and is expected to affect an increasing number of people in the coming decades. The control of the progression of this refractive defect has become an important part of clinical practice, and in the absence of a protocolized treatment with the aim of reducing it, several therapeutic options have been evaluated. Epsilon-near-zero (ENZ) media have been very actively investigated due to their unconventional wave phenomena and strengthened nonlinear response. However, the technological impact of ENZ media will be determined by the quality of realistic ENZ materials, including material loss and surface roughness. Here, we provide a comprehensive experimental study of the impact of surface roughness on ENZ substrates. Using silicon carbide (SiC) substrates with artificially induced roughness, we analyze samples whose roughness ranges from a few to hundreds of nanometer size scales. It is concluded that ENZ substrates with roughness in the few nanometer scale are negatively affected by coupling to longitudinal phonons and strong ENZ fields normal to the surface. On the other hand, when the roughness is in the hundreds of nanometers scale, the ENZ band is found to be more robust than dielectric and surface phonon polariton (SPhP) bands.

Intervention programs based on self-compassion have demonstrated their efficacy both in reducing psychological distress and increasing well-being. The goal of this study was to test the efficacy of an online intervention to increase mindfulness and self-compassion levels in a non-clinical sample in a highly stressful context: the ten weeks of lockdown imposed in the early stages of the COVID-19 pandemic. The intervention sessions consisted of thirty-minute guided meditations followed by thirty minutes of inquiry. Sixty-one participants completed two thirds of the sessions or more, and 65 individuals participated in a waiting-list (WL) control group. Self-compassion, anxiety, depression and stress levels were assessed. The analysis of pre-post results suggests that the interventions increased self-compassion levels and decreased anxiety, depression and stress levels, whereas the WL group did not show any significant changes. The emotional changes in the intervention group were associated with the increase in self-compassion. However, at follow-up, the scores of emotional distress variables returned to the initial pre-intervention scores. These data can be interpreted in line with previous results that have shown the efficacy of self-compassion-based intervention programs. Given that this efficacy was not maintained at follow-up, data are discussed according to the pervasive role of a highly stressful context and-as described in other studies-the need for regular practice to maintain the benefits obtained.

BACKGROUND: Major depressive disorder (MDD) is the leading cause of global disability; however, the existing treatments do not always address cognitive dysfunction-a core feature of MDD. Immersive virtual reality (VR) has emerged as a promising modality to enhance the real-world effectiveness of cognitive remediation. OBJECTIVE: This study aimed to develop the first prototype VR cognitive remediation program for MDD ("bWell-D"). This study gathered qualitative data from end users early in the design process to enhance its efficacy and feasibility in clinical settings. METHODS: Semistructured end-user interviews were conducted remotely (n=15 patients and n=12 clinicians), assessing the participants' perceptions and goals for a VR cognitive remediation program. Video samples of bWell-D were also shared to obtain feedback regarding the program. The interviews were transcribed, coded, and analyzed via thematic analysis. RESULTS: End users showed an optimistic outlook toward VR as a treatment modality, and perceived it as a novel approach with the potential of having multiple applications. The participants expressed a need for an engaging VR treatment that included realistic and multisensorial settings and activities, as well as customizable features. Some skepticism regarding its effectiveness was also reported, especially when the real-world applications of the practiced skills were not made explicit, as well as some concerns regarding equipment accessibility. A home-based or hybrid (ie, home and clinic) treatment modality was preferred. CONCLUSIONS: Patients and clinicians considered bWell-D interesting, acceptable, and potentially feasible, and provided suggestions to enhance its real-world applicability. The inclusion of end-user feedback is encouraged when developing future VR programs for clinical purposes.

Among the different species of mammals, the expression of maternal behavior varies considerably, although the end points of nurturance and protection are the same. Females may display passive or active responses of acceptance, recognition, rejection/fear, or motivation to care for the offspring. Each type of response may indicate different levels of neural activation. Different natural stimuli can trigger the expression of maternal and paternal behavior in both pregnant or virgin females and males, such as hormone priming during pregnancy, vagino-cervical stimulation during parturition, mating, exposure to pups, previous experience, or environmental enrichment. Herein, we discuss how the olfactory pathways and the interconnections of the medial preoptic area (mPOA) with structures such as nucleus accumbens, ventral tegmental area, amygdala, and bed nucleus of stria terminalis mediate maternal behavior. We also discuss how the triggering stimuli activate oxytocin, vasopressin, dopamine, galanin, and opioids in neurocircuitries that mediate acceptance, recognition, maternal motivation, and rejection/fear.

Several studies provide evidence that obesity is a significant risk factor for adverse outcomes in coronavirus disease 2019 (COVID-19). Altered renal function and disturbances in magnesium levels have been reported to play important pathophysiological roles in COVID-19. However, the relationship between obesity, renal function, circulating magnesium levels, and mortality in patients with COVID-19 remains unclear. In this retrospective cohort study, we characterized 390 hospitalized patients with COVID-19 that were categorized according to their body mass index (BMI). Patients were clinically characterized and biochemical parameters, renal function, and electrolyte markers measured upon admission. We found that in patients who died, BMI was associated with reduced estimated glomerular filtration rate (eGFR, Rho: −0.251, p = 0.001) and serum magnesium levels (Rho: −0.308, p < 0.0001). Multiple linear regression analyses showed that death was significantly associated with obesity (p = 0.001). The Cox model for obese patients showed that magnesium levels were associated with increased risk of death (hazard ratio: 0.213, 95% confidence interval: 0.077 to 0.586, p = 0.003). Thus, reduced renal function and lower magnesium levels were associated with increased mortality in obese COVID-19 patients. These results suggest that assessment of kidney function, including magnesium levels, may assist in developing effective treatment strategies to reduce mortality among obese COVID-19 patients.

Oxidative stress plays an important role in vascular complications observed in patients with obesity and Type 2 Diabetes (T2D). Xanthine oxidase (XO) breaks down purine nucleotides into uric acid and contributes to the production of reactive oxygen species (ROS). However, the relationship between XO activity and glucose homeostasis in T2D subjects with obesity is unclear. We hypothesized that disordered glucose levels are associated with serum XO activity in overweight women and men with T2D and without hyperuricemia. We studied serum XO activity in women and men with and without T2D. Our results show that serum XO activity was greater in T2D patients with body mass index (BMI) ≥ 25 kg/m2 than in those with BMI < 25 kg/m2 (p < 0.0001). Sex-based comparative analyses of overweight T2D patients showed that serum XO activity correlated with homeostasis model assessment of ß-cell function (HOMA-ß), fasting plasma glucose (FPG), and hemoglobin A1C in overweight T2D women but not in overweight T2D men. In addition, as compared to overweight T2D men, women had higher high-sensitivity C-reactive protein (hs-CRP) levels. However, overweight T2D men had higher XO activity and uric acid levels than women. Our results suggest that XO activity is higher in overweight T2D patients, especially in men, but is more sensitive to disordered glucose levels in overweight women with T2D.

Cardiovascular disease (CVD) is a common comorbidity observed in patients with coronavirus disease 2019 (COVID-19), which is associated with increased severity and mortality. However, the effects of biological sex on CVD-associated mortality in patients with COVID-19 are poorly established, particularly among Hispanic/Latin Americans. We examined the association of preexisting CVD with COVID-19 mortality in hospitalized Latin American men and women. This multicenter study included Mexican patients hospitalized with a positive diagnosis of COVID-19. The main outcome was in-hospital mortality. Multivariable regression analyses were used to calculate the adjusted odds ratio with 95% confidence interval for mortality in women and men. Of 81,400 patients with a positive diagnosis for SARS-CoV-2 infection, 28,929 (35.54%) hospitalized patients were evaluated. Of these, 35.41% (10,243) were women. In-hospital death was higher in men than in women. In relation to CVD between the sexes, women had a higher incidence of CVD than men (4.69 vs. 3.93%, P = 0.0023). The adjusted logistic regression analyses showed that CVD was significantly associated with COVID-19 mortality in women but not men. We then stratified by sex according to age <52 and ≥52 yr old. Similar significant association was also found in prespecified analysis in women ≥52 yr old but not in men of similar age. We conclude that CVD's effect on mortality among patients hospitalized with COVID-19 is dependent on biological sex and age in this Latin American cohort. These results suggest that therapeutic strategies for Latin American women with CVD and COVID-19 should include particular attention to their cardiovascular health.NEW & NOTEWORTHY CVD's effect on COVID-19 mortality is dependent on biological sex and age. CVD in women but not men with COVID-19 is associated with significantly unfavorable outcomes.

RESUMEN Fundamento Para el manejo quirúrgico del paciente con deformidad facial resulta vital el uso del examen clínico y radiográfico de cada caso, pues en dependencia de los hallazgos se decide entre las diversas alternativas terapéuticas disponibles en este campo. Objetivo describir el comportamiento imagenológico de las deformidades faciales en pacientes adultos. Métodos se realizó un estudio descriptivo de corte transversal, en 46 pacientes mayores de 18 años, con diagnóstico de deformidad facial, los cuales acudieron al Servicio de Cirugía Maxilofacial, del Hospital Calixto García, en el período comprendido desde noviembre de 2017 a noviembre de 2019. Se analizaron las variables: tipo de tejido, magnitud, y plano en el que se evidenciaron las alteraciones. Se utilizó la prueba exacta de Fisher, con un nivel de confianza al 95 %, para determinar las relaciones analíticas entre las variables; así como los índices tau b de Kendall y el Rho de Spearman. Resultados el 45,7 % de las deformidades se presentó en ambos planos; el 50 % de ellas con magnitudes entre 11-30 mm; y en el 41,3 % de los pacientes afectaron al tejido blando. Las correlaciones entre la distribución de pacientes y los conjuntos de variables plano/tipo de tejido y plano/magnitud resultaron negativas débiles; mientras que tipo de tejido/magnitud presentó una correlación positiva moderada muy significativa, con valores de significación entre 0,001 a 0,003. Conclusión predominaron las deformidades de tejido blando, en ambos planos y con magnitudes entre 11-30 mm. Las variables imagenológicas estudiadas mostraron un comportamiento interdependiente.


ABSTRACT Background For the surgical management of the patient with facial deformity, the use of the clinical and radiographic examination of each case is vital, since depending on the findings, a decision is made between the various therapeutic alternatives available in this field. Objective to describe the imaging behavior of facial deformities in adult patients. Methods a descriptive cross-sectional study was carried out in 46 patients older than 18 years old, with a diagnosis of facial deformity, who attended the Maxillofacial Surgery Service, Calixto García Hospital, from November 2017 to November 2019. The analyzed variables were: type of tissue, magnitude, and plane in which the alterations were evidenced. Fisher's exact test was used, with a confidence level of 95 %, to determine the analytical relationships between the variables; as well as Kendall's tau b and Spearman's Rho rates. Results The 45.7 % of the deformities presented in both planes; 50 % of them with magnitudes between 11-30 mm; and in 41.3 % of the patients they affected the soft tissue. The correlations between the distribution of patients and the sets of variables plane/tissue type and plane/magnitude were weakly negative; while tissue type/magnitude presented a very significant moderate positive correlation, with significance values between 0.001 and 0.003. Conclusion soft tissue deformities predominated, in both planes and with magnitudes between 11-30 mm. The imaging variables studied showed an interdependent behavior.

RESUMEN Introducción: El rinofima es una enfermedad de la cual se han descrito 4 clasificaciones clínicas asociadas a la gravedad de la afectación dérmica. Aun así, existen casos en quienes no son clasificables por ninguna de las alternativas. Objetivo: Mostrar un paciente no clasificable, según las clasificaciones preexistentes del rinofima. Caso clínico: Se presenta un paciente masculino de 68 años de edad, de piel blanca, con diagnóstico de rinofima, que en el examen físico se observa engrosamiento nasal, nodular, asimétrico, con piel normocoloreada, presencia de telangiectasias en toda su superficie y compromiso de más de una subunidad anatómica nasal. Conclusiones: El caso presentado no se puede enmarcar en ninguno de los ítems de las clasificaciones existentes, lo cual muestra la necesidad del diseño de una nueva clasificación del rinofima más exhaustiva e inclusiva para pacientes con este diagnóstico.


ABSTRACT Introduction: Rhinophyma is a disease of which four clinical classifications associated with the severity of dermal involvement have been described. Even so, there are cases in which they are not classifiable by any of the alternatives. Objective: To show an unclassifiable patient, according to the preexisting classifications of rhinophyma. Clinical case: A 68-year-old male patient is presented, with white skin, with a diagnosis of rhinophyma, who in the physical examination shows nasal, nodular, asymmetric thickening, with normal-colored skin, presence of telangiectasias on all its surface and compromise of more than one nasal anatomical subunit. Conclusions: The case presented cannot be framed in any of the items of the existing classifications, which shows the need to design a new, more exhaustive and inclusive rhinophyma classification for patients with this diagnosis.

A new method is proposed to tune the interferometric response of wavelength-based optical fiber sensors. Using the nanoparticle in situ synthesis (ISS) technique, it is possible to synthesize gold nanoparticles (AuNPs) within a pre-existing polymeric thin film deposited at the end-face of an optical fiber. This post-process technique allows us to adjust the optical response of the device. The effect of the progressive synthesis of AuNPs upon polymeric film contributed to a remarkable optical contrast enhancement and a very high tuning capability of the reflection spectra in the visible and near-infrared region. The spectral response of the sensor to relative humidity (RH) variations was studied as a proof of concept. These results suggest that the ISS technique can be a useful tool for fiber optic sensor manufacturing.

BACKGROUND AND OBJECTIVES: Clinicians often fail to deliver the best psychological treatments available, especially if they perceive their patients as fragile or vulnerable. This fragility might be interpreted by clinicians through their internalised gender stereotypes (e.g. female patients are less resilient to a demanding treatment) or according to their patients' emotional state (e.g. the patient is too delicate to endure the most stress-inducing aspects of therapy). The aim of this study was to test experimentally whether patients' characteristics influenced therapy delivery. Some clinician characteristics were also considered. METHODS: This was an experimental, vignette-based study that evaluated clinicians' likelihood of utilizing several techniques commonly used in CBT by manipulating patients' mood and gender. Clinicians' personality traits were also included as covariates. RESULTS: Anxious patients were the most likely to receive the techniques, especially exposure and other behavioural techniques. Therapists delivered more techniques to male patients, while angry and calm female patients were the least likely to receive the techniques. Therapists were more likely to deliver talking techniques to female patients. Clinicians' firmness and empathy had an effect on CBT delivery. LIMITATIONS: Future vignette-based studies should validate and pilot the vignettes. Technique clustering should also be based in factor analysis or similar methods. Direct observational methods might be more reliable than self-report. CONCLUSION: The findings suggest that clinicians treat their patients differently, either consciously or inadvertently. These differences are likely to be related to clinicians' own concerns and gender stereotypes about their patients.

Objective: To know the nurses' perception of the factors that influence the safe administration ofmedications in the work context of the General University Hospital of Elda (Alicante), Spain. Method:This is a qualitative phenomenological study through interviewssemi-structured in which 15 nurses chosen by deliberate sampling participated. Results: The participantsexpressed concern about the performance of the medication administration procedures, which made itpossible to perceive safety as the greatest concern in the study unit. A main category called “knowing”was established, which encompasses all the reflections about the nurse's performance during themedication administration process. The following subcategories are developed from this main category:work overload, insufficient training, distractions, and communication and information failures.Conclusion: Knowing the nurses' perception of the factors that influence the safe administration ofmedications could facilitate the use of instruments to standardize and reduce variability in the safeadministration of medications. As well as improving conditions in work environments and not favoringsocial, personal and professional stigmatization due to care error. (AU)


Objetivo: Conocer la percepción de las enfermeras sobre los factores que influyen en laadministración segura de medicamentos en el contexto laboral del Hospital General Universitario de Elda(Alicante), España. Método: Se trata de un estudio cualitativo fenomenológico mediante entrevistas semiestructuradas en donde participaron 15 enfermeras elegidas por muestreo intencionado. Resultados: Lasparticipantes manifestaron preocupación ante la realización de los procedimientos de administración demedicamentos, lo que permitió percibir la seguridad como la mayor preocupación en la unidad deestudio. Se estableció una principal categoría denominada “conocer” que engloba todas las reflexionesacerca de la actuación de la enfermera durante el proceso administración de medicamentos. A partir deesta categoría principal se desarrollan las siguientes subcategorías: sobrecarga de trabajo, formacióninsuficiente, distracciones y fallos de comunicación e información. Conclusión: Conocer la percepción delas enfermeras sobre los factores que influyen en la administración segura de medicamentos nos podríafacilitar el uso de instrumentos para estandarizar y disminuir la variabilidad en la administración segurade medicamentos. Así como mejorar las condiciones en los entornos laborales y no favorecer laestigmatización social, personal y profesional por el error asistencial. (AU)

Método: trata-se de um estudo qualitativo fenomenológico por meio de entrevistas.semiestruturado no qual participaram 15 enfermeiras escolhidas por amostragem deliberada. Resultados:Os participantes expressaram preocupação com a realização dos procedimentos de administração demedicamentos, o que possibilitou perceber a segurança como a maior preocupação na unidade de estudo.Foi estabelecida uma categoria principal denominada “saber”, que engloba todas as reflexões sobre aatuação do enfermeiro durante o processo de administração de medicamentos. As seguintes subcategoriassão desenvolvidas a partir desta categoria principal: sobrecarga de trabalho, treinamento insuficiente,distrações e falhas de comunicação e informação. Conclusão: Conhecer a percepção dos enfermeirossobre os fatores que influenciam na administração segura de medicamentos pode facilitar a utilização deinstrumentos para padronizar e reduzir a variabilidade na administração segura de medicamentos. Alémde melhorar as condições nos ambientes de trabalho e não favorecer a estigmatização social, pessoal eprofissional por erro assistencial. (AU)

RESUMEN Fundamento El estudio de los costos de la hospitalización por gastroenteritis por rotavirus es un tema estratégico ante la disyuntiva de introducir o no una vacuna contra rotavirus en Cuba. Objetivo estimar el costo institucional de hospitalización de pacientes menores de cinco años de edad por gastroenteritis a causa de rotavirus. Métodos estudio descriptivo, específicamente una evaluación económica parcial del tipo descripción de costos. Se utilizó el costo modelado por protocolo de atención ajustado a la práctica habitual. Fueron revisados los documentos normativos y se aplicó un ejercicio de ajuste a la práctica de rutina mediante consulta a expertos. El costo fue expresado en CUP de 2018. Resultados se estimó el ingreso en sala de Gastroenterología para el 93,1 % de los pacientes, de los cuales el 99,4 % resolvería el problema de salud. El salario resultó la partida de mayor aporte al gasto, aunque en la sala de Gastroenterología lo fueron los exámenes complementarios. El costo de hospitalización promedio modelado para la gastroenteritis por rotavirus fue de 435,13 CUP; este varió según la condición clínica con que arribe el paciente, entre 415,72 CUP y 1057,60 CUP. El costo fue sensible a la variación de la estadía en los servicios. Conclusión La hospitalización de un paciente menor de cinco años por gastroenteritis a causa de rotavirus representa altos gastos para el hospital. El servicio de atención a pacientes críticos y el ingreso previo en una sala clínica distinta a la de gastroenterología incrementan el costo de la atención.


ABSTRACT Background The study of the hospitalization costs for rotavirus gastroenteritis is a strategic issue given the dilemma of introducing or not a rotavirus vaccine in Cuba. Objective to estimate the institutional cost of hospitalization of patients younger than five years of age because of gastroenteritis due to rotavirus. Methods descriptive study, specifically a partial economic evaluation of the costs description type. Modeled cost per care protocol adjusted to standard practice was used. The normative documents were reviewed and an adjustment exercise was applied to routine practice by consulting experts. The cost was expressed in 2018 Cuban pesos. Results admission to the Gastroenterology ward was estimated for 93.1% of the patients, of which 99.4% would solve the health problem. The salary was the item with the highest contribution to spending, although in the Gastroenterology ward it was the complementary examinations. The average cost of hospitalization modeled for rotavirus gastroenteritis was 435.13 Cuban pesos; this varied according to the clinical condition with which the patient arrives, between 415.72 pesos and 1057.60 pesos. The cost was sensitive to the variation of the stay in the services. Conclusion Hospitalization of a patient younger than five years of age for gastroenteritis due to rotavirus represents high costs for the hospital. The attention service to critical patients and the previous admission to a clinical room other than that of Gastroenterology, increase the cost of care.

RESUMEN Fundamento :existen ocasiones en las que ante un mismo problema se actúa diferente y una forma de paliar este efecto es incorporar instrumentos que faciliten la toma de decisiones. Objetivo : evaluaren la década del 2010 al 2020 los protocolos existentes en el manejo del paciente con bruxismo en Cuba y el mundo. Métodos: se realizó un estudio observacional transversal para la protocolización del bruxismo desde la perspectiva clínica en Cuba y el mundo desde febrero de 2019 a febrero de 2020. El universo documental y de investigadores lo conformaron: cinco protocolos (cuatro extranjeros y un protocolo de carácter nacional) y un grupo evaluadorintegrado por cuatro profesionales de Estomatología. Se confeccionó una base de datos con la puntuación otorgada a el instrumento evaluativo con la escala cubana conformado por 15 ítems, según cinco categorías que establecían una evaluación del uno al cinco de los ítems, según opinión de cada integrante del grupo evaluador. Resultados: ninguno de los protocolos en existencia evaluados logró la puntuación máxima (300 puntos), tampoco la puntuación mínima (60 puntos). Los protocolos cuatro y cinco alcanzaron los mayores valores con 178 y 184 puntos respectivamente e igual IDT P de 0,53. Conclusiones : los protocolos para la atención de pacientes con bruxismo no alcanzaron la máxima puntuación posible, pero constituyen aciertos científicos e incorporan de una manera u otra, principios o funciones para desarrollar aspectos válidos en las buenas prácticas estomatológicas.


ABSTRACT Background: there are times in which facing the same problem, the performance is different and a way of reducing this effect is to incorporate instruments that facilitate decision-making. Objective: to evaluatein the last decade (from 2010 to 2020) the existing protocol in the management of the patient with bruxism in Cuba and the world. Methods: a cross sectional observational study was carried out for the protocolization of bruxism from a clinical perspective in Cuba and the world from February 2019 to February 2020.The universe of documents and researchers made it up: five protocols (four foreignersprotocols and a national one) and an evaluation group made up of four dental professionals. A database was drawn up with the grade given to the assessment instrument with the Cuban scale formed by 15 items, divided into five categories that established an evaluation from one to five of the items, according to the opinion of each member of the assessment group. Results: noneof the existing assessed protocol got neither the maximum grade (300 points)nor the minimum one (60 points). The fourth and fifth protocols reached the highest grades with 178 and 184 points respectively as well as the same IDT P of 0,53. Conclusions: these protocols the assistance of patients with bruxism did not reach the maximum possible grade, but they constitute scientific good decisions and incorporate, from one way or another, principles or functions to develop valid aspects in the good practice of dentistry.

The generation of new neurons in the adult brain is a currently accepted phenomenon. Over the past few decades, the subventricular zone and the hippocampal dentate gyrus have been described as the two main neurogenic niches. Neurogenic niches generate new neurons through an asymmetric division process involving several developmental steps. This process occurs throughout life in several species, including humans. These new neurons possess unique properties that contribute to the local circuitry. Despite several efforts, no other neurogenic zones have been observed in many years; the lack of observation is probably due to technical issues. However, in recent years, more brain niches have been described, once again breaking the current paradigms. Currently, a debate in the scientific community about new neurogenic areas of the brain, namely, human adult neurogenesis, is ongoing. Thus, several open questions regarding new neurogenic niches, as well as this phenomenon in adult humans, their functional relevance, and their mechanisms, remain to be answered. In this review, we discuss the literature and provide a compressive overview of the known neurogenic zones, traditional zones, and newly described zones. Additionally, we will review the regulatory roles of some molecular mechanisms, such as miRNAs, neurotrophic factors, and neurotrophins. We also join the debate on human adult neurogenesis, and we will identify similarities and differences in the literature and summarize the knowledge regarding these interesting topics.

Fundamento: el enfoque científico del bruxismo resulta, en no pocas ocasiones, incongruente y obedece en lo particular a tres aspectos pendientes para la ciencia en el siglo XXI. Primero la ausencia de esquemas diagnósticos y terapéuticos definitivos, segundo la necesidad del empleo de específicos y depurados métodos de evaluación epidemiológica y clínica. Por último la integración tangente y activa de los profesionales de las Ciencias Médicas para desarrollar en un orden secuencial documentos científicos. Objetivo: diseñar un protocolo de procedimientos clínicos interdisciplinarios para un nuevo enfoque del bruxismo desde la integración médica en Cuba. Métodos: se realizó una investigación de desarrollo en el período de marzo de 2019 a marzo de 2020. Se conformó el universo del grupo de trabajo por nueve estomatólogos de alta especialización y el universo de investigaciones lo integraron 41, de ellas con una subsiguiente muestra de 19 investigaciones, destacadas por sus aportes y fortalezas con relación a la integración interdisciplinaria, seleccionadas por muestreo intencional. Se estructuró el documento en tres acápites que desglosan momentos, acciones específicas y condicionantes conformes a la valoración del grupo de trabajo; a través de la técnica de consenso del grupo nominal. Resultados:se establecieron consultas de evolución periódicas, valoración por etapas y procedimientos terapéuticos. Además procedimientos clínicos interdisciplinarios, según el momento de integración médica que se precisa. Se asumió el diagnóstico integral del bruxismo desde un concepto sistémico-neurológico. Conclusiones: se diseñó un documento científico que protocoliza procedimientos clínicos interdisciplinarios para un nuevo enfoque del bruxismo desde la integración médica en Cuba (AU)

Methods: a development research was carried out in the period from March 2019 to March 2020. The universe of the work group was formed by nine highly specialized dentists and the universe of investigations was made up of 41 of them with a subsequent sample of 19 researches, highlighted by its contributions and strengths in relation to interdisciplinary integration, selected by intentional sampling. The document was structured into three sections that break down moments, specific actions and conditioning factors in accordance with the assessment of the working group; through the nominal group consensus technique.Results: periodic evolution consultations were established, assessment by stages and therapeutic procedures. Besides interdisciplinary clinical procedures, according to the time of medical integration that is required. It was assumed the comprehensive diagnosis of bruxism from a systemic-neurological concept.
