RESUMO

The arterial wall (abdominal aorta, splenic artery) has been investigated after irradiation with 3000 rad (3 X 1000 rad within 3 weeks) in rabbits. In addition to light microscopic techniques semi-thin sections, electron microscopic sections, permeability staining and autoradiographic techniques have been applied. These studies revealed typical lesions in the media (elastic degeneration, cellular proliferation of the smooth muscle cells) and lesions of the endothelial cells using ultrastructural techniques especially. The intima showed an increasing permeability in the irradiated area, while autoradiographically a higher mitotic activity in the media was demonstrable. The results are discussed in the light of similar atherosclerotic and X-ray induced lesions of human blood vessels.

RESUMO

The immunological reactivity of 19 operated cases of breast cancer (14 T1 2, 5T3) was investigated before and at 3-weekly intervals up to 12 weeks after radiotherapy by means of a large panel of techniques. The results were compared with two groups of identically-investigated patients, one consisting of 11 cases of cervical cancer (stage III), treated solely by irradiation and the other of 12 cases of cervical cancer (Stages I a and II b), treated by operation only. Before irradiation and in the non-irradiated group cellular immunity appeared to be more disturbed in cases of breast cancer than in cases of cervical carcinoma, as shown by the lowest number of lymphocytes, the lowest number of spontaneously-rosetting lymphocytes, the lowest percentage of DNCB sensitization in patients and the lowest percentage of Tuberculin-positive skin tests. In both types of carcinoma the humoral immunity appeared to be impaired, as apparent from the lowered immune response to tetanus vaccination. Other parameters of humoral immunity such as the immune globulin concentration, iso- and heteroagglutinine, the titre of measles antibodies or membrane fluorescence of lymphocytes showed no dinstinct trends during the investigation period. After irradiation a) inhibiting and b) stimulating influences were observed: a) The incidence and extent of the immune response to tetanus vaccination was further reduced and a distinct lymphopenic effect was observed. b) The incidence of positive skin tests with varidase, as well as the number of spontaneously-rosetting lymphocytes increased after the commencement of irradiation. Apart from the known sensitization with DNCB and skin tests with tuberculin, determination of the antitoxin titre before and after tetanus vaccination provided the most reliable results in regard to immunological reactivity in the investigated tumour patients.

Eleven patients with stage-III cancer of the cervix were investigated before, during and after radio-therapy in regard to their state of humoral immunity on the basis of determinations of the serum IgG, IgM and IgA concentration, of hetero- and isoagglutinins, of tetanus antitoxin before and after vaccination with toxoid, of measles antibodies and of the percentage of lymphocyte membrane fluorescence. The cellular immunity of the same patients was investigated by determination of the percentage of spontaneously-rosetting lymphocytes, of skin-test reactivity with DNCB before and after sensitization, of skin-test reactivity with candida, trichophyton, varidase, OT and staphylo antigen. The function of polymorpho-nuclear leucocytes was investigated by means of the NVT test and St. aureus, E. coli and latex particles. All investigations were performed both before, and 3, 6, 9 and 12 weeks after the commencement of radiotherapy and the results were compared with those of an operated, non-irradiated group (stages I b and II a). Two types of noteworthy results were observed: 1. A decrease in immunological reactivity, probably in connection with cancer, since this reaction was observed both in irradiated and in non-irradiated cases, characterized by lowered or absent immune answer to tetanus toxoid, lymphopenia, decrease in sensitization to DNCB and less positive skin tests to old tuberculin and varidase. 2. An additional inhibition (although in one investigation stimulation of the immune answer was also seen), probably in connection with radiotherapy, characterized by an additional decrease in immune answer to tetanus toxoid, in skin sensitivity to DNCB sensitization and in tests with old tuberculin, and an augmented lymphopenia, as well as an increase in positive skin tests with varidase. No significant changes were observed with any other method.

RESUMO

A short survey is given of the historical development and the physical basis of ozone therapy. Ozone is used in medicine as well as in other spheres. Papers reporting good results of ozone treatment in carcinoma seemed of particular interest. The efficacy of ozone as an adjuvant to the irradiation of carcinosarcomas of rats was confirmed by us. On account of this fact ozone was introduced by us as an adjuvant to the irradiation of women with gynaecological cancer and appeared to give good results. The mechanism of action of ozone is not yet fully clarified and several theories are discussed. Investigations are currently being undertaken in respect to the behaviour of several substances in the organism during ozone therapy.

RESUMO

The introduction of a pollen diet as an adjuvant in the reduction of side effects during radiotherapy of patients with gynaecological cancer is described and its efficacy evaluated. 15 women with carcinoma of the cervix received a pollen diet during irradiation, whilst ten further patients receiving irradiation served as controls without pollen added to the diet. Serum enzymes, proteins, vitamins and blood count were analysed before and after irradiation. It appears, that pollen favourably influences the efficacy of irradiation and reduces the frequency of side effects, both subjectively and objectively.

12 patients were investigated in regard to their state of humoral immunity by means of the following determinations: IgG, IgM, and IgA concentrations in the serum, hetero- and isoagglutinins, tetanus antitoxin before and after immunization with toxoid measles antibodies and the percentage of lymphocytes with membrane fluorescence. The cellular immunity was investigated in some patients with determination of the percentage of spontaneously-rosetting lymphocytes, of the DNCB skin-test reactivity before and after sensitization with DNCB and of skin-test reactivity to candida, trichophyton, varidase, OT and staphylo antigen. The functional efficiency of polymorphonuclear leucocytes was investigated by means of the NBT test and Staph. aureus, E. coli and Latex particles. All investigations were performed before, and 3, 6, 9 and 12 weeks after surgery. 4 methods only amongst the entire spectrum employed in the investigation proved to give sufficient evidence of signs of decreased efficiency of the immune response in the investigated patients: determination of tetanus antitoxin before and after immunization, a distinct increase in titre being observed only in 5 cases, determination of the sensitization to DNCB, which was positive only in 4 cases and determination of skin-test reactivity to varidase and OT, which was found to be less frequently positive than in controls. However, all these reactions showed no evidence of any correlation to surgical management and appeared merely to reflect characteristic responses of these cancer patients. Moreover, with the exception of the decrease in lymphocytes, no changes were detected in the data obtained by the other methods used in this study which might show some reflection of an alteration in response following surgery.

This is a report about 442 patients with a carcinoma of the cervix uteri, who underwent a primary radiotherapeutic treatment in the period from 1966 to 1968. 342 (77,4%) were at the age between fifty and seventy years. 226 (60%) of the women were in the stages III and IV. A five-years-healing could be obtained with 234 patients (45%). Nobody died in consequence of the treatment, i.e. primaryly. 34 (7,6%) died intercurrently. A rectovaginal fistula appeared on 10 women (2,2%). No vesicovaginal fistula was observed. 58 women (13%) suffered from an alimentary diabetes. It is determined that this grouping represents a negative selection of patients as the stages I and II have been operated according to Wertheim, if no contraindication for the operation was existent; for that reason, the intercurrent rate of mortality in the stage I was relatively high.

RESUMO

A report is given of 448 cases of corpus carcinoma, treated by primary radiotherapy in the I-st Gynecological University Hospital in the years 1960 to 1968. The 5-years-healing rate in this period was 50.4 percent. It is emphasized that the cases represent a negative selection, as only inbetween the ages of 61 and 90 years. Two women died primarily from pulmonary embolism (0.4 percent). In two patients a rectovaginal fistula developed. The increasing number of patients with diabetes mellitus (23.2 percent) is striking.

From January 1st 1966 until December 31st 1968, 593 women with carcinoma of the cervix uteri were admitted to and treated at the I. Universitäts-Frauenklinik of Vienna. 151 cases underwent a radical operation (Wertheim) resulting in a relative 5-years-healing of 85.43% and 442 women were exclusively treated by a combined radium-X-ray-therapy. From the 442 inoperable cases in 234 cases--which is 53%--a 5-years healing was reached. Of all the 593 cases together, 363 are living without relapse after 5 years, which is equal to 61.21% absolute healing.

RESUMO

The parenterally application of oxygen-ozone gas mixture gives good resultats in the treatment of various deseases. Ozone seems to influence the metabolic process of fat, so it was of interest to analyse this influence especially to phospholipids. 40 women with gynaecological cancer got 10 ml oxygen-ozone gas mixture with a content of 450 gamma ozone into the cubital vene. Venous blood was removed before and 10 minutes after application and the level of lecithin, lysolecithin, cephalin and spingomyelin was determined by the method of Randerath. A decrease of all four substances was obvious, although all values remained in normal range.

RESUMO

The numerous anatomical and radiological nomenclatures of hepatic arteries are compared and discussed. Our own material for the assessment of accessory hepatic arteries consisted of 220 anatomically prepared individuals and 74 angiograms. The individual branches in the normal case as well as all variations and their frequency are discussed. The present, often confusing nomenclature is critically reviewed and an attempt is made to establish a simple and clinically acceptable nomenclature. We propose to name the additional branches supplying the liver as 'arteria hepatica accessoria' with reference to the vessel from which they originate. Additional branches in the terminal branching (ramus accessorius) and vessels of the liver capsula should not be referred to as accessory hepatic arteries.

211 female patients with proved uterus myomatosus have been mammographed. With more than 50% chronic mastopathies could be identified; i.e. twice as much than with non-selected radiographically examined patients. The pattern showed fibrocytic changes as well as fibroplastic ones, fibrous involutions and--especially frequently--cystic degenerations. Fibrocytic degeneration occurred mostly with women who had already given birth and lactated. In glandular-cystic hyperplasias of the endometrium, mastophathies occurred twice as often than in hysteromyomas with normal mucose. An at least relative hyperoestrinism is made responsible for the myoma, but especially for the mucosa hyperplasia. Therefore, the same hormonal situation has to be responsible for the a.m., mainly fibrotic changes of the mammary gland. Of course no characteristic effect of the oestrogens can be derived from this, as it certainly is connected with the development of the total hormonal situation of the female. According to various investigators, most forms of chronic mastopathy bear a higher risk of carcinomas. For this reason, all structural transformations which appear one-sided in radiological case control (course observation) have to be considered cases of risk.

DCS and ACTH causes an increasing cellular proliferation in the arterial wall of the rabbit. The combination of both shows only a small further increasing. The importance of these experiments is thought as an additional cause in the atherosclerotic vessel wall alteration.
