A proton exchange ionomer is one of the most important components in membrane electrode assemblies (MEAs) of polymer electrolyte membrane fuel cells (PEMFCs). It acts as both a proton conductor and a binder for nanocatalysts and carbon supports. The structure and the wetting conditions of the MEAs have a great impact on the microenvironment at the three-phase interphases in the MEAs, which can significantly influence the electrode kinetics such as the oxygen reduction reaction (ORR) at the cathode. Herein, by using the Pt(111)|X ionomer interface as a model system (X = Nafion, Aciplex, D72), we find that higher drying temperature lowers the onset potential for sulfonate adsorption and reduces apparent ORR current, while the current wave for OHad formation drops and shifts positively. Surprisingly, the intrinsic ORR activity is higher after properly correcting the blocking effect of Pt active sites by sulfonate adsorption and the poly(tetrafluoroethylene) (PTFE) skeleton. These results are well explained by the reduced water activity at the interfaces induced by the ionomer/PTFE, according to the mixed potential effect. Implications for how to prepare MEAs with improved ORR activity are provided.

Knee osteoarthritis (KOA) is one of the main problems of an aging society in terms of incidence, impairment to the quality of daily living (QOL), and economics. The main aim of this study was to verify the usefulness, in practical terms, of applying the existing diagnostic criteria of early knee osteoarthritis (EKOA). The secondary objective of this project was to evaluate the clinical progression of healthy subjects (HS) at risk of osteoarthritis and of patients with diagnosed EKOA. A cross-sectional longitudinal pilot study was carried out, in which 105 participants were classified as EKOA patients or HS according to the diagnostic criteria. Measures of disability, pain, and self-reported variables were assessed. Two follow-ups were performed in order to assess the diagnoses and radiological progression, and the clinical progression was evaluated using self-reported measures. Following the current diagnostic criteria, the participants were divided into EKOA and HS. Most of the participants did not present changes in their classification, although some subjects were reclassified as EKOA or HS in the follow-ups which were performed. The current classification criteria for EKOA based on self-reported measures, radiological findings, and clinical conditions such as pain could lead to a misdiagnosis of this process, as fluctuations in the classifications of patients according to their conditions were found during follow up.

Introduction: Osteoarthritis (OA) is a common joint condition and one of the greatest causes of disability worldwide. The role of serum lipid and inflammatory biomarkers in the origin and development of the disease is not clear, although it could have important implications for diagnosis and treatment. The primary aim of this study was to evaluate differences of serum lipid and inflammatory biomarkers with knee EOA in comparison with matched controls, in order to determine the role of these factors in the origin of EOA. Methods: For this proposal, a cross-sectional study with a non-randomized sample was performed. 48 subjects with early osteoarthritis (EOA) and 48 matched controls were selected and serum lipid levels (total cholesterol, LDL, HDL) and inflammatory biomarkers C-reactive protein (CRP), uric acid (UA) were analyzed. In addition, clinical (pain, disability) and functional (gait speed, sit-to-stand) variables were measured to establish their relationship to serum lipid levels and inflammatory biomarkers. Results: Patients with EOA showed higher levels of total cholesterol LDL, UA, and CRP. Higher levels of total cholesterol, LDL and CRP were correlated with higher levels of pain intensity and higher disability (p < 0.05). In addition, UA and CRP were inversely correlated with gait speed and sit-to-stand tests (r = -0.038 to -0.5, p < 0.05). Conclusion: These results highlight the relevance of metabolic and proinflammatory aspects in the early stages of knee OA and could be key to developing early diagnoses to prevent the onset and development of the disease.

The potential dependence of the rate of dehydration of formic acid to adsorbed CO (COad) on Pt at pH 1 has been studied on a polycrystalline Pt surface by time-resolved surface-enhanced infrared absorption spectroscopy in the attenuated total reflection mode (ATR-SEIRAS) with simultaneous recording of current transients after a potential step. A range of formic acid concentrations has been used to obtain a deeper insight into the mechanism of the reaction. The experiments have allowed us to confirm that the potential dependence of the rate of dehydration has a bell shape, going through a maximum around the potential of zero total charge (pztc) of the most active site. The analysis of the integrated intensity and frequency of the bands corresponding to COL and COB/M shows a progressive population of the active sites on the surface. The observed potential dependence of the rate of formation of COad is consistent with a mechanism in which the reversible electroadsorption of HCOOad is followed by its rate-determining reduction to COad.

Although the oxygen reduction reaction (ORR) involves multiple proton-coupled electron transfer processes, early studies reported the absence of kinetic isotope effects (KIEs) on polycrystalline platinum, probably due to the use of unpurified D2O. Here we developed a methodology to prepare ultra-pure D2O, which is indispensable for reliably investigating extremely surface-sensitive platinum single crystals. We find that Pt(111) exhibits much higher ORR activity in D2O than in H2O, with potential-dependent inverse KIEs of ~0.5, whereas Pt(100) and Pt(110) exhibit potential-independent inverse KIEs of ~0.8. Such inverse KIEs are closely correlated to the lower *OD coverage and weakened *OD binding strength relative to *OH, which, based on theoretical calculations, are attributed to the differences in their zero-point energies. This study suggests that the competing adsorption between *OH/*OD and *O2 probably plays an important role in the ORR rate-determining steps that involve a chemical step preceding an electrochemical step (CE mechanism).

Background: The main objective was to evaluate differences in the clinical, motor, or functional variables in patients with Early Osteoarthritis (EOA) and individuals at risk of developing osteoarthritis (OA). Methods: A cross-sectional study was performed. All the participants were divided into two groups: EOA patients and healthy subjects (HS) at risk of developing OA. The main outcomes were clinical tests, such as those of knee morphology, instability, or proprioception; motor and functional variables, such as knee strength, range of motion, walking speed, and the sit-to-stand test; pain and disability, assessed through the Western Ontario McMaster Universities Osteoarthritis Index (WOMAC) and Knee injury and Osteoarthritis Outcome Score (KOOS) scales; and knee alignment and leg length inequality, assessed via X-ray images. Results: A total of 97 participants were included (54 EOA and 43 HS). Patients with EOA showed a greater presence of knee pain (p < 0.01). In addition, more EOA patients showed instability both in the left (p < 0.01) and right legs (p < 0.05). Regarding the knee alignment variable, significant differences were found (p < 0.04), with more patients with EOA diagnosed as possessing a varus alignment. In addition, EOA patients showed lower knee strength, since statistically significant differences were found regarding flexion and extension strength in the left leg (Mean Difference (MD): 12.92; p = 0.03; d = −0.46 and MD:7.81; p = 0.04; d = −0.39). Differences were found for the sit-to-stand test scores, showing lower results for the EOA group (MD: −1.91; p < 0.01; d = 0.54). Conclusions: The results of this research show statistically significant differences between patients with EOA and HS at risk of developing OA with respect to pain, disability, instability, knee strength, and the sit-to-stand test. Our results suggest that the evaluation of clinical, motor, and functional features could contribute to an early management of knee OA.

The study of the OH adsorption process on Pt single crystals is of paramount importance since this adsorbed species is considered the main intermediate in many electrochemical reactions of interest, in particular, those oxidation reactions that require a source of oxygen. So far, it is frequently assumed that the OH adsorption on Pt only takes place at potentials higher than 0.55 V (versus the reversible hydrogen electrode), regardless of the Pt surface structure. However, by CO displacement experiments, alternating current voltammetry, and Raman spectroscopy, we demonstrate here that OH is adsorbed at more negative potentials on the low coordinated Pt atoms, the Pt steps. This finding opens a new door in the mechanistic study of many relevant electrochemical reactions, leading to a better understanding that, ultimately, can be essential to reach the final goal of obtaining improved catalysts for electrochemical applications of technological interest.

BACKGROUND: Specific serum biomarkers of cartilage metabolism such as cartilage oligomeric matrix protein (sCOMP) and procollagen type II C-terminal propeptide (sPIICP) as well as hyaluronan (sHA), a biomarker of synovitis, have been implicated in the pathophysiology of knee osteoarthritis (OA). However, the associations of these biomarkers with the severity of the disease and OA risk factors, including age and obesity remain inconclusive. This analysis examines the associations between these serum biomarkers and the radiographic severity of OA and knee pain, as wells as obesity, the age and gender of the participants, and other OA risk factors. METHODS: From 44 patients with early knee OA and 130 patients with late knee OA we analyzed the radiographic severity of the disease using the Kellgren and Lawrence (KL) grading system. Moreover, 38 overweight healthy individuals were used as a control group. Specific information was collected from all participants during their recruitment. The levels of the three serum biomarkers were quantified using commercially available ELISA kits. Serum biomarkers were analyzed for associations with the average KL scores and pain in both knees, as well as with specific OA risk factors. RESULTS: The levels of sCOMP were elevated in patients with severe late OA and knee pain and correlated weakly with OA severity. A weakly correlation of sHA levels and OA severity OA was observed. We demonstrated that only sPIICP levels were markedly decreased in patients with late knee OA suggesting the alterations of cartilage metabolism in this arthritic disease. Moreover, we found that sPIICP has the strongest correlation with obesity and the severity of OA, as well as with the knee pain at rest and during walking regardless of the severity of the disease. ROC analysis showed that the area under the ROC curve (AUC) was 0.980 (95% CI: 0.945-0.995; p < 0.0001), suggesting high diagnostic accuracy of sPIICP. Interestingly, gender and age had also an effect on the levels of sPIICP. CONCLUSION: This study revealed the potential of serum PIICP to be used as a biomarker to monitor the progression of knee OA, however, further studies are warranted to elucidate its clinical implication.

Hydrogen energy-based electrochemical energy conversion technologies offer the promise of enabling a transition of the global energy landscape from fossil fuels to renewable energy. Here, we present a comprehensive review of the fundamentals of electrocatalysis in alkaline media and applications in alkaline-based energy technologies, particularly alkaline fuel cells and water electrolyzers. Anion exchange (alkaline) membrane fuel cells (AEMFCs) enable the use of nonprecious electrocatalysts for the sluggish oxygen reduction reaction (ORR), relative to proton exchange membrane fuel cells (PEMFCs), which require Pt-based electrocatalysts. However, the hydrogen oxidation reaction (HOR) kinetics is significantly slower in alkaline media than in acidic media. Understanding these phenomena requires applying theoretical and experimental methods to unravel molecular-level thermodynamics and kinetics of hydrogen and oxygen electrocatalysis and, particularly, the proton-coupled electron transfer (PCET) process that takes place in a proton-deficient alkaline media. Extensive electrochemical and spectroscopic studies, on single-crystal Pt and metal oxides, have contributed to the development of activity descriptors, as well as the identification of the nature of active sites, and the rate-determining steps of the HOR and ORR. Among these, the structure and reactivity of interfacial water serve as key potential and pH-dependent kinetic factors that are helping elucidate the origins of the HOR and ORR activity differences in acids and bases. Additionally, deliberately modulating and controlling catalyst-support interactions have provided valuable insights for enhancing catalyst accessibility and durability during operation. The design and synthesis of highly conductive and durable alkaline membranes/ionomers have enabled AEMFCs to reach initial performance metrics equal to or higher than those of PEMFCs. We emphasize the importance of using membrane electrode assemblies (MEAs) to integrate the often separately pursued/optimized electrocatalyst/support and membranes/ionomer components. Operando/in situ methods, at multiscales, and ab initio simulations provide a mechanistic understanding of electron, ion, and mass transport at catalyst/ionomer/membrane interfaces and the necessary guidance to achieve fuel cell operation in air over thousands of hours. We hope that this Review will serve as a roadmap for advancing the scientific understanding of the fundamental factors governing electrochemical energy conversion in alkaline media with the ultimate goal of achieving ultralow Pt or precious-metal-free high-performance and durable alkaline fuel cells and related technologies.

Osteoarthritis (OA) is a common joint condition and one of the greatest causes of disability worldwide. The role of vitamin D in the origin and development of the disease is not clear, although it could have important implications for diagnosis and treatment. For this proposal, a cross-sectional study with a non-probabilistic sample was performed. In total, 48 with early osteoarthritis (EOA) and 48 matched controls were selected, and serum 25(OH)D and parathyroid hormone (PTH) levels were analyzed. In addition, physical and psychological variables were measured to establish their relationship with vitamin D levels. Patients with EOA showed lower levels (22.3 ± 7.3 ng/mL) in comparison to matched controls (29.31 ± 9.2 ng/mL). A statistically significant higher number (Chi-squared = 8.525; p = 0.004) of patients with EOA had deficiency levels (<20 ng/mL) compared to the control group. Patients with lower vitamin D levels showed higher levels of pain intensity, disability, and anxiety, as well as poorer values for sit-to-stand, walking speed, and social participation. Correlation analysis showed a relationship between serum 25(OH)D, PTH and pain intensity, and social participation. These results highlight the relevance of vitamin D in the early diagnosis and prevention of EOA.

The main objective of this research is to determine the psychosocial differences between patients with knee pain or early osteoarthritis (EOA) and healthy subjects at risk of developing osteoarthritis. The secondary objective is to determine how psychosocial factors might influence pain and social participation in patients with EOA. A cross-sectional study was performed. Participants were divided according to the presence of pain or EOA. Pain intensity both at rest and walking, psychological variables such as anxiety and depression, and social participation were evaluated. A total of 105 participants were included (64 with knee pain and 41 without pain), with a mean age of 51.42 ± 5.92 (35 men and 70 women). Patients with knee pain had higher levels of anxiety (MD = -2.35; p < 0.01; d = 0.66) and depression (MD = -2.45; p < 0.01; d = 0.87), regardless of the presence of EOA. In addition, patients with higher depression levels had lower levels of social participation. The results revealed a relationship between the psychological variables, anxiety and depression, with knee pain and the onset of symptomatic OA, as well as an influence of depression levels on social participation. Improving these psychological characteristics may be useful in delaying the onset of symptomatic OA and enhancing social participation.

The interface between the Pt(111) surface and several MeF/HClO4 (Me+ = Li+, Na+, or Cs+) aqueous electrolytes is investigated by means of cyclic voltammetry and laser-induced temperature jump experiments. Results point out that the effect of the electrolyte on the interfacial water structure is different depending on the nature of the metal alkali cation, with the values of the potential of maximum entropy (pme) following the order pme (Li+) < pme (Na+) < pme (Cs+). In addition, the hydrogen peroxide reduction reaction is studied under these conditions. This reaction is inhibited at low potentials as a consequence of the build up of negative charges on the electrode surface. The potential where this inhibition takes place (E inhibition) follows the same trend as the pme. These results evidence that the activity of an electrocatalytic reaction can depend to great extent on the structure of the interfacial water adlayer and that the latter can be modulated by the nature of the alkali metal cation.

The site geometry preference of CO binding on stepped Pt single crystals in alkaline solution was investigated by in situ FTIR spectroscopy. The surfaces of the Pt single crystals consisted of different width (111) terraces, interrupted by (110) or (100) monatomic steps. Experiments carried out with CO adsorbed exclusively on the top of the steps revealed that only linearly bonded CO formed on the (110) steps, while two CO binding geometries (linear and bridge) were observed on the (100) steps. On one hand, for CO adsorbed only on the steps, the positions of the bands corresponding to linearly bonded CO were similar, regardless of the density of steps, suggesting the existence of an interaction between COads only along the line of the steps. On the other hand, for full CO coverage, the CO stretching frequencies and the geometry of bound CO were sensitive to the width of the (111) terraces and the step orientations. Consequently, the CO binding sites favored linearly bonded CO for surfaces consisting of shorter (111) terraces and (110) steps. Bridge-bonded CO was favored on surfaces consisting of shorter (111) terraces interrupted by (100) steps. In order to understand the origin of the preference of CO binding sites, the results were compared to the corresponding behavior in acid media, which revealed that, in addition to the effect inherent to the Pt surface, the charge on the metal side in an aqueous environment should be taken into consideration. The analysis suggested that the CO adlayers formed at full coverage in acidic and alkaline media had different structures. On the other hand, the structure of the layer of CO adsorbed only at the steps was independent of pH.

The catalytic effect of Pd on gold electrodes for glycerol oxidation is evaluated for Pd-Au surfaces prepared using three different methods: irreversible adsorption of palladium by a simple immersion of a gold electrode in palladium solution, the deposition of palladium on the gold substrate by a step potential from 1 to 0.75 V, and the forced deposition of palladium on the gold electrode with the help of a reducing hydrogen atmosphere. Voltammetry has been used for the electrochemical characterization of the Pd-Au deposits and to determine its reactivity towards glycerol oxidation, whereas FTIR experiments have allowed detecting adsorbed species and products formed during the oxidation reaction. Pd-Au surfaces prepared by irreversible adsorption are the electrodes that show the highest activity for the glycerol complete oxidation to carbonate, whereas Pd-Au surfaces made by the step potential are the catalyst that exhibits the highest rate for the formation and adsorption CO before carbonate production, poisoning the surface and diminishing their electrocatalytic properties. In addition to carbonate, glycerate, glycolate, and formate are detected as oxidation products. The integrated bands of the spectra are used to give quantitative information for comparing the product distribution of the different Pd-Au deposits prepared.

BACKGROUND: A major goal in stroke rehabilitation is the establishment of more effective physical therapy techniques to recover postural stability. Functional Principal Component Analysis provides greater insight into recovery trends. However, when missing values exist, obtaining functional data presents some difficulties. The purpose of this study was to reveal an alternative technique for obtaining the Functional Principal Components without requiring the conversion to functional data beforehand and to investigate this methodology to determine the effect of specific physical therapy techniques in balance recovery trends in elderly subjects with hemiplegia post-stroke. METHODS: A randomized controlled pilot trial was developed. Thirty inpatients post-stroke were included. Control and target groups were treated with the same conventional physical therapy protocol based on functional criteria, but specific techniques were added to the target group depending on the subjects' functional level. Postural stability during standing was quantified by posturography. The assessments were performed once a month from the moment the participants were able to stand up to six months post-stroke. FINDINGS: The target group showed a significant improvement in postural control recovery trend six months after stroke that was not present in the control group. Some of the assessed parameters revealed significant differences between treatment groups (P < 0.05). INTERPRETATION: The proposed methodology allows Functional Principal Component Analysis to be performed when data is scarce. Moreover, it allowed the dynamics of recovery of two different treatment groups to be determined, showing that the techniques added in the target group increased postural stability compared to the base protocol.

BACKGROUND: The study aim was to assess the hemodynamic results and implantation technique for the latest-generation St. Jude Medical aortic valve bioprosthesis, the Trifecta™ GT, which was first marketed in 2016. METHODS: The first 100 patients (mean age 74.59 ± 7.41 years) undergoing aortic valve replacement (AVR) with the Trifecta GT, whether associated or not with other procedures, were included and assessed. All patients underwent a baseline ultrasound scan prior to hospital discharge to monitor postoperative gradients and the presence of periprosthetic leakage. RESULTS: The predominant valvular heart disease was aortic stenosis (85%). An isolated AVR was required in 43% of patients. The prosthesis sizes used were 19, 21, 23, 25, and 27 mm. The overall hospital mortality was 5%; all deaths occurred in patients with associated surgeries. Peak gradients measured prior to hospital discharge ranged from 17.95 mmHg to 10.95 mmHg for 19 mm and 27 mm prostheses, respectively; mean gradients were 9.94 and 6.18 mmHg for 19 mm and 27 mm prostheses, respectively. Neither implant-related complications nor significant periprosthetic leakages were recorded. CONCLUSIONS: Based on experience with patients, the Trifecta GT demonstrated an excellent hemodynamic performance after implantation, which involved a simple and safe technique. Further long-term studies to determine the durability of the prosthesis are required.

The yeast Saccharomyces cerevisiae is employed as a model to study the cellular mechanisms of toxicity and defense against selenite, the most frequent environmental selenium form. We show that yeast cells lacking Aft2, a transcription factor that together with Aft1 regulates iron homeostasis, are highly sensitive to selenite but, in contrast to aft1 mutants, this is not rescued by iron supplementation. The absence of Aft2 strongly potentiates the transcriptional responses to selenite, particularly for DNA damage- and oxidative stress-responsive genes, and results in intracellular hyperaccumulation of selenium. Overexpression of PHO4, the transcriptional activator of the PHO regulon under low phosphate conditions, partially reverses sensitivity and hyperaccumulation of selenite in a way that requires the presence of Spl2, a Pho4-controlled protein responsible for post-transcriptional downregulation of the low-affinity phosphate transporters Pho87 and Pho90. SPL2 expression is strongly downregulated in aft2 cells, especially upon selenite treatment. Selenite hypersensitivity of aft2 cells is fully rescued by deletion of PHO90, suggesting a major role for Pho90 in selenite uptake. We propose that the absence of Aft2 leads to enhanced Pho90 function, involving both Spl2-dependent and independent events and resulting in selenite hyperaccumulation and toxicity.

Cth2 is an mRNA-binding protein that participates in remodeling yeast cell metabolism in iron starvation conditions by promoting decay of the targeted molecules, in order to avoid excess iron consumption. This study shows that in the absence of Cth2 immediate upregulation of expression of several of the iron regulon genes (involved in high affinity iron uptake and intracellular iron redistribution) upon oxidative stress by hydroperoxide is more intense than in wild type conditions where Cth2 is present. The oxidative stress provokes a temporary increase in the levels of Cth2 (itself a member of the iron regulon). In such conditions Cth2 molecules accumulate at P bodies-like structures when the constitutive mRNA decay machinery is compromised. In addition, a null Δcth2 mutant shows defects, in comparison to CTH2 wild type cells, in exit from α factor-induced arrest at the G1 stage of the cell cycle when hydroperoxide treatment is applied. The cell cycle defects are rescued in conditions that compromise uptake of external iron into the cytosol. The observations support a role of Cth2 in modulating expression of diverse iron regulon genes, excluding those specifically involved in the reductive branch of the high-affinity transport. This would result in immediate adaptation of the yeast cells to an oxidative stress, by controlling uptake of oxidant-promoting iron cations.
