RESUMO

Patellar components come in onset and inset designs. Kinematic differences between these designs were studied in a cadaver model of closed kinetic chain knee extension. Seven frozen knees were implanted with a standard posterior cruciate-retaining design. Each knee first was tested with the inset design, followed by the onset design in the Oxford Knee Rig. Three-dimensional tracking of the femur, tibia, and patella was performed using an electromagnetic system during active knee extension under load. No statistically significant differences were seen in knee kinematics between the 2 designs. The inset patella tended to shift laterally and tilt laterally more than the onset patella. This difference may be clinically significant.

RESUMO

A patient with post-traumatic mechanical instability associated with a significant posterior acetabular deficiency in the presence of an otherwise good articular surface was treated with an allograft reconstruction. The short-term result was good; at 10-month follow-up there was full range of motion with no clinical evidence of instability. This procedure may be indicated in rare instances of post-traumatic mechanical instability where insufficiency of the posterior acetabular wall is felt to be a significant factor.

Original Gianturco expandable stents and their modifications were used to create an experimental intrahepatic portacaval anastomosis (EIPCA) in 30 young domestic swine without portal hypertension. The study focused on the design of a suitable stent, the technique of its application, and the evaluation of short-term patency of the EIPCA. A stent with a 2.5-cm-long body and wire skirts on both ends was most suitable for EIPCA creation. Well-positioned stents shunted most of the portal blood in the inferior vena cava circulation and remained patent for 4-6 weeks. Ingrowth of liver parenchyma and abundant proliferation of the intima and connective tissue inside the stent lumen in these rapidly growing animals gradually decreased EIPCA patency, and thrombus formation with diminished blood flow closed them completely.
