INTRODUCTION AND IMPORTANCE: Degloving injuries of the hand or fingers have a devastating presentation which challenges a surgeon to conduct reconstruction in order to resurface the naked finger and recover its function. The gold standard treatment for degloving injuries is using skin grafting and flap. The pedicle of groin flap is based on superficial circumflex iliac artery. It is one of standard flaps used in reconstruction of degloving fingers. In this study, we use groin flap for reconstruction of traumatic total degloving little finger. PRESENTATION OF CASE: This is a case of 34-years old man with total degloving of his left little finger because stuck inside running cutting machine in a clothes factory. The patient was then brought to the Hasan Sadikin General Hospital. The patient underwent thorough debridement, preparation of the donor site, and groin flap. After a week, the wound was in good condition with no signs of infection. CLINICAL DISCUSSION: The groin or skin flap is pedicled and vascularized by superficial circumflex artery. It can be considered as an option for treatment of single finger degloving wound because of its compliant nature and vascularization reliability. Despite this, it often results in bulky appearance which needs to be reconstructed later. THE CONCLUSION: Groin flaps are an appropriate method in managing degloving little fingers and are still cosmetically acceptable.

INTRODUCTION: Treatment of choice for posttraumatic arthritis in proximal interphalangeal (PIP) joints has remained a controversial subject. There is a lack of consensus regarding the optimal approach and treatment options for PIP joint arthritis. In this study, we report our experience in management of PIP joint arthritis using soft-tissue interpositional arthroplasty. CASE PRESENTATION: A 12-year-old female, right-handed, complains of stiffness on the left ring finger. There was a flexion deformity accompanied by limitation on both active and passive flexion-extension range of motion (ROM) of the PIP joint. Soft tissue interpositional arthroplasty was then performed using middle phalanx periosteum as interpositional material. Postoperative follow-up showed improvement in both active and passive ROM of PIP joint, improvement in joint function, and a good pain relief. Overall, the patient was satisfied with the outcomes of the procedure. CLINICAL DISCUSSION: Arthrodesis, implant arthroplasty, and interpositional arthroplasty are the treatments of choice for stiff fingers that are caused by bone problems. In this study, we report a case of PIP joint arthritis treated with interpositional arthroplasty. It was able to provide ROM improvement and good functional results. Periosteum is used as an interposition material because of its in-situ nature thereby reducing donor site related morbidity and postoperative complications. In addition, the use of the periosteum makes this procedure relatively inexpensive and makes it a suitable treatment of choice in limited resources areas. CONCLUSION: Soft tissue interpositional arthroplasty shows promising results as a treatment option for arthritis of PIP joint, especially in limited resource areas.

BACKGROUND: Human papillomavirus (HPV) infections cause a spectrum of clinical disease states, depending on the causative HPV and the characteristics of the infected host, especially the status of cell-mediated immunity. Generalized verrucosis is an unusual clinical presentation of a disseminated HPV infection associated with severe immunodeficiency status. OBSERVATIONS: We present a case of extreme disfigurement associated with an HPV-2 (common wart virus) infection. Virologic studies, immune status of the patient, and treatment(s) are summarized. CONCLUSIONS: The severe disfigurement of this patient was a result of an underlying severe immunodeficiency, permissive for a disseminated HPV-2 infection that was allowed to progress for many years before the initiation of therapy. Such a rare case illustrates the natural history of generalized verrucosis in the setting of severe immunodeficiency in the absence of sustained medical interventions. Medical and surgical treatments resulted in marked improvement in the general health of this patient, as well as improvement of the disfigurement that resulted from the generalized verrucosis.
